Wood and Iron—Short Pump, 12288 West Broad Street – The following violations were reported during a routine inspection: open drink found on prep line; there are no paper towels at first hand sink on prep line; temperature of diced tomatoes in walk-in cooler is elevated and is one hour past cooling time parameters; two chemical spray bottles in ware wash area are unlabeled; boxes of frozen food are stored directly on walk-in freezer floor; two knives are stored in crevice of middle prep unit on the line; long cutting boards on prep line are scored and stained; gasket liners are torn on all three doors of middle prep unit on the line, freezer chest on cookline, middle door of salad prep unit, and on left-side door of sauce lowboy; mops are not hung or inverted to air dry.

Teppanyaki Grill & Buffet, 9043 West Broad Street – The following violations were reported during a routine inspection: employee picked broom off the floor and then sliced pizza without first washing hands; there are no paper towels at two hand sinks in kitchen; there is no hand soap at hibachi grill hand sink; raw chicken is stored over raw fish and raw beef in several coolers; can opener blade is dirty; brown buildup found in ice machine; observed seafood and chicken breading bins unsifted with visible clumps; boxes of crawfish are thawing on the kitchen floor; salmon in sushi station lowboy is thawing while still in reduced oxygen packaging; water is dripping onto the floor from walk-in freezer ceiling (some foods are uncovered and exposed to the dripping water); employee is cooking food while wearing a non-smooth ring; wires are bent on two fryer baskets; racks are rusty in both walk-in coolers; front walkway of walk-in freezer is coming loose from the floor; walk-in freezer floor is bumpy; there is a hole in interior side of walk-in cooler door; wall outside of walk-in cooler is peeling; grout is worn near dishwashing area.

Chick-fil-A, 1529 North Parham Road – The following violations were reported during a routine inspection: employee returned from a delivery, entered kitchen and started prepping drinks without first washing hands; bottle of spray Lysol and a bottle of sunscreen are stored on shelf next to coffee filters, lids and drink syrup; handle of garnish tong is touching lemons in drive-thru container; observed grime buildup on floor under ice machine.

Dunkin Donuts, 10600 Patterson Avenue – The following violations were reported during a routine inspection: stir spoon at drive-thru is stored in room temperature water; y-connector with hose and sprayer under pressure is connected at mop sink without the proper backflow prevention device installed; paint is peeling off the ceiling above Bunn coffeemakers.

Subway, 1216 North Concord Avenue – The following violations were reported during a follow-up inspection: didn’t observe an employee health policy; gasket is torn on walk-in cooler; dried food debris found on walk-in cooler gasket; wire racks are dusty; wall is crumbling in back left corner with black mold-like substance and ceiling exposed above walk-in cooler; mop isn’t hung or inverted to air dry.

McDonald's, 2310 Mechanicsville Turnpike – The following violations were reported during a routine inspection: food staff are wearing bracelets; there are torn door gaskets on French fry reach-in freezer; there is manufacturer’s tape on inside door of drive-thru reach-in cooler; observed dried food debris on reach-in cooler and freezer door gaskets in kitchen and on walk-in cooler door gasket; there is a direct plumbing connection underneath three-compartment sink.

McDonald’s, 607 East Laburnum Avenue – The following violations were reported during a routine inspection: food containers stored “clean” have food residue on them; staff are wearing watches and non-smooth rings; there is dried food debris on reach-in cooler/freezer door gaskets and drawer gaskets throughout kitchen; walk-in cooler door gaskets are dirty; dried food debris found on wire storage racks in walk-in cooler.

Maya Latin Kitchen, 353 East Williamsburg Road – The following violations were reported during a routine inspection: observed some food on stacked containers stored “clean” on dish rack; there are no date marks on cooked chicken, pickled onions or salsa.

Holy Burger, 8300 Brook Road – The following violations were reported during a routine inspection: didn’t observe an employee health policy; raw chicken is stored next to ready-to-eat sauces and above buttermilk; ice cream scoop is stored next to cleaners and a soapy water bucket; temperature of two large boxes of chicken in walk-in cooler is elevated because of improper thawing procedures; ice scoop handle is touching the ice; ice cream scoop is stored in room temperature water; there are torn gaskets on walk-in cooler and freezer; walk-in freezer has excessive ice at the door and ice is forming outside of the door where a small amount of air is escaping.

Chick-fil-A, 10176 Brook Road – The following violations were reported during a routine inspection: temperature of chicken wraps in three lowboy coolers is elevated; food debris buildup found on some cooling unit gaskets throughout kitchen; there is grease buildup on sides of equipment and racks in kitchen; observed food debris and grime buildup under stationary equipment and food debris buildup in floor drains.

Burger King, 1013 Parham Road – The following violations were reported during a routine inspection: observed black residue on ice guard of ice machine; dried food debris found on tomato and onion slicer stored on shelf; cook is wearing bracelets; sanitizer bucket is stored next to bin of baking oil; observed various single-service containers not protected from contamination throughout kitchen (sandwich boxes, drink cups and fry/hashbrown containers); fryer baskets have bent and broken wires; walk-in cooler door seal is torn; floor fan and walk-in cooler fan guards are dusty; there is grease accumulation on power cords behind toaster; hose with spray nozzle attached to y-valve at mop sink doesn’t have the proper dual check valve installed; mops are not hung or inverted to air dry; multiple ceiling tiles are damaged throughout kitchen and some tiles are unable to sit flush with brackets; observed low grout in front of hand sink; there are cracked wall tiles behind lowboy on cookline; observed dust and grease accumulation on ceiling throughout kitchen.

Jersey Mike’s Subs, 1420 North Parham Road – The following violations were reported during a routine inspection: spray bottle full of clear liquid is unlabeled (person in charge states it is water); there is a leak from spray hose at three-compartment sink; y-valve attached at mop sink faucet doesn’t have the proper dual check valve installed; observed standing water and grime on floor of walk-in cooler under racks.

Rose Marie Inn, 8923 Patterson Avenue – The following violations were reported during a routine inspection: didn’t observe a food protection manager certificate; both hand sinks are missing hand soap and bar hand sink doesn’t have paper towels; bar hand sink is blocked with a large pan and bag of napkins; raw chicken is stored over raw ground beef in reach-in cooler; microwave interior is dirty; heavy dried food debris found on slicer; temperatures of sliced provolone, sliced lettuce, sliced Swiss and shredded cheddar in prep cooler are elevated; there are no date marks on opened ham, hot dogs, cooked beans or cooked corn cobs; disinfectant spray is stored next to straws and potatoes; observed many live and dead ants near water heater; fly trap is installed above spice shelf and food prep counter; prep cooler cutting board is scratched; floor and gasket of black reach-in cooler are dirty; there is heavy food debris on shelves next to prep cooler; observed grease buildup at fryer and grill; pipes under bar three-compartment sink are dusty; women’s restroom doesn’t have a covered trash can; staff restroom and walkway to restroom are blocked with large miscellaneous items; large, sliding door reach-in cooler is turned off and empty; mop isn’t hung or inverted to air dry; ceiling panels are loose and insulation is exposed/patched with duct tape near back kitchen three-compartment sink; beer walk-in cooler gasket is torn; front bottom of walk-in cooler is peeling; back door and screen are torn; floors throughout kitchen are dirty.

Burger King, 4240 Pouncey Tract Road – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; observed person in charge chewing gum while working in the kitchen; hand sink next to three-compartment sink doesn’t have paper towels; temperature of onion rings in warmer is too low; observed associates replacing the four-hour time control policy stamp stickers (discard at 2:15 p.m.) on the sliced tomatoes, shredded lettuce and sliced cheese containers without discarding the old product that has been stored out on the line at room temperature (time of observation was 2:25 p.m.); box of chicken nuggets is stored directly on walk-in freezer floor; wire racks in walk-in cooler are peeling; ceiling vents throughout kitchen are surrounded by dust; mop isn’t hung or inverted to air dry.

Little Caesars, 5101 South Laburnum Avenue – The following violations were reported during a routine inspection: there are no dates on prepped chicken wings or cheese sauce; there isn’t a marked time on pizzas held on a time control policy; several items need cleaning (racks, rolling racks, gaskets, inside of units, containers holding utensils and outside of sauce bottles); observed buildup under units and sinks and in floor drains; there are missing coving tiles, ceiling tiles and trim; some ceiling tiles are stained.

Marco’s Pizza, 2284 John Rolfe Parkway – The following violations were reported during a routine inspection: dried food debris found on blade and holder of can opener; bottle of blue liquid at three-compartment sink is unlabeled; employee with long facial hair is handling food without wearing a beard guard; sandwich prep unit cutting board is scored and stained brown; observed buildup of dust and flour on surfaces of pizza oven, wire storage racks and portable fans.

Jason’s Deli, 1700 Willow Lawn Drive – The following violations were reported during a routine inspection: temperatures of portioned chicken in back left make table, mac and cheese in back right make table, and potato salads and strawberry shortcake in deli case are elevated; ambient temperature of deli case is elevated; single-service containers are being used as a scoop in sugar bin; make table cutting boards are scratched and stained.

Panera Bread, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: temperature of lettuce in salad drawer is elevated (bottom drawer is unable to fully close and ice has built up in the machine); y-valve attached at mop sink faucet doesn’t have the proper dual check valve installed; there is a leak from faucet at mop sink; observed standing liquid and grime along baseboard on left wall in right walk-in cooler; there is low grout around some tiles near the dish machine; there are two loose tiles in front of dish machine; baseboard tiles are missing in right walk-in cooler.
