Lucky AF, 11800 West Broad Street – The following violations were reported during a routine inspection: hand sink in back of facility doesn’t have hand soap; raw minced chicken is stored over ready-to-eat soup in lowboy on prep line; temperatures of miso soup on service line and shrimp tempura on sushi line are too low; two spray bottles containing chemicals are unlabeled; fish is thawing while still in vacuum sealed packaging; rice spoon is stored in room temperature water on service line; gasket liner is torn on walk-in cooler door; y-valve attached to mop sink faucet doesn’t have the appropriate backflow prevention device installed.
JULY 14
Taco Bamba Taqueria, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: bar hand sink doesn’t have paper towels or hand soap; sanitizer bucket found in hand sink between bar and cookline; box of single-service spoons found in hand sink at end of cookline; observed what appears to be black mildew on bottom of ice guard in ice machine; there is brown buildup in left soda nozzle at the bar; temperatures of sliced tomatoes in right make table, raw shrimp and raw chicken in refrigerated drawer and salsas in an ice bath are elevated; quart containers of baja shrimp creama, yogurt and pimento cheese in walk-in cooler are past their expiration dates; facility is not utilizing a date marking system for packaged salsas; there is no date mark on watermelon garnish in bar lowboy; one bottle of stainless steel cleaner is stored next to in-use liquor bottles at the bar and another bottle is stored next to single-service containers; single-service containers not protected from contamination at bar area; right make table cutting board is scratched and stained; fryer baskets have bent wires; one light bulb is burned out in vent hood; floor fans are dirty; suppression nozzle above fryer is covered in grease; y-valve attached to mop sink with two hoses doesn’t have the appropriate dual check valve installed; observed low grout in kitchen in front of make table leading into dishwashing area.
Domino’s Pizza, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: person in charge couldn’t produce proof of food protection manager certification; there is a direct plumbing connection at three-compartment sink; sliding doors to both outdoor dumpsters were left open; wall behind/under three-compartment sink is dirty; mop isn’t hung or inverted to air dry.
Kabab House, 3411 Old Parham Road – The following violations were reported during a routine inspection: non-food grade plastic wicker drawers are being used for the storage of loose spices; inside and outside most refrigeration units are dirty; there is a direct plumbing connection at three-compartment sink; baseboards and wall blanks are peeling away from wall; observed stains on ceiling; floor under units is dirty.

Market Café, 10950 Nuckols Road – The following violations were reported during a routine inspection: bar hand sink doesn’t have hand soap or paper towels; employee filled water pitchers at bar hand sink; raw shell eggs are stored above ready-to-eat foods; fried chicken was placed directly on counter surface where raw fish and meat were recently prepared without cleaning and sanitizing first; raw peppers were placed directly on prep table where dirty containers are stored; observed dried food debris on pans at steam well, knives, serving utensils, and pans and containers on storage racks; observed inconsistency with date marking; many items are missing date marks (rice, sauces, paneer, etc.); temperature of coconut and ginger sauce made this morning is elevated because of improper cooling procedures (in a large container); temperature of white fish thawed under prep table is elevated; there is a large amount of fruit flies inside onion bags; observed evidence of many pests (dead and living cockroaches inside prep table, large flies throughout restaurant, and what appears to be mouse droppings at mop sink); many containers of unprotected foods are stored directly on walk-in cooler and walk-in freezer floors; many foods are uncovered in both walk-ins and freezer; many food containers, lids and utensils have chips and cracks; there is dried food debris on wire storage racks in kitchen, walk-ins and interior of prep units; observed grime and residue on handles and exteriors of prep units, sinks and prep tables; two lights are out in walk-in freezer; heavy grease found on outside of hood and adjacent ceiling tiles; observed dried food and grime on floors, especially under cookline and storage shelves; area between restaurant and neighboring convenience store is dirty; there is heavy grease and carbon buildup on hood and filters.
Burger King, 3720 Mechanicsville Turnpike – The following violations were reported during a routine inspection: there are no discard times marked on foods held on time control; hood filter is missing in ventilation hoods over broiler oven; grease residue found on French fry warmer, sides of deep fryers, top of deep fryer hoods and inside hood system; there is food debris on walk-in cooler storage racks and inside warmer units/shelves; surfaces underneath sandwich make table are dirty; fire suppression pipes above broiler oven are very dirty and sooty; walls behind/under three-compartment sink are mildewed; there are dried food stains on walls throughout kitchen; kitchen ceiling tile is stained and dusty near ventilation; floor under deep fryers is greasy; there is black grime forming on floor under front counter.
Angela’s Ristorante, 425 North Ridge Road – The following violations were reported during a routine inspection: temperature of large pot of marinara cooked yesterday in walk-in cooler is elevated because of improper cooling methods; various chemical spray bottles and stainless steel cleaner are stored near foods and/or food contact surfaces; racks throughout kitchen are rusted; observed dried food debris in crevices of deli slicer (not on surface that touches food); grease accumulation found on rack next to stove; floor tiles are cracked and chipped next to stove and in dish pit.

Red Robin Gourmet Burgers and Brews, 11784 West Broad Street – The following violations were reported during a routine inspection: take-out containers on top of microwaves aren’t stored inverted to protect from contamination; long cutting boards on prep and pizza units are scored and stained; wire drying rack across from prep sinks is beginning to peel; there is a direct plumbing connection underneath three-compartment sink.

Krispy Kreme Doughnuts, 4906 West Broad Street – The following violations were reported during a routine inspection: dishes and an empty cleaning bottle are stored in all three kitchen hand sinks; there is no handwash signage posted at front counter hand sink; there are no paper towels at hand sink near donut conveyor belt; employee is wearing a wristband; manufacturer’s tape is still on the exterior of the ice machine; door gaskets are torn on reach-in coolers in kitchen; there is dried food residue on front service counter donut display racks; observed dried food debris and crumbs on shelves throughout kitchen; in-use plastic storage container exteriors and lids are dirty; walk-in cooler door gasket is moldy; outdoor dumpster doors and lids were left open; there are unnecessary items and trash in outdoor dumpster area; ceiling vents are dusty and ceiling tile near ventilation are dusty; observed food debris, trash and coffee stains underneath beverage line behind front counter; dried food residue found on three-compartment sink walls; there are cracked and missing floor tiles throughout the kitchen.
Shake Shack, 5400 West Broad Street – The following violations were reported during a routine inspection: manufacturer’s tape is still on drive-thru ice chest; observed grease residue on sides of hot line cooking equipment; ceiling tiles near ventilation are dusty.
America’s Best Wings, 7536 West Broad Street – The following violations were reported during a routine inspection: staff took phone orders and then proceeded to prep sauces and handle single-service items without first washing their hands; dried food residue found on can opener blade; temperatures of two containers of cooked chicken made yesterday and raw chicken prepared today are elevated; cooked pasta noodles and containers of coleslaw made yesterday are not date marked; the date mark on bins of chicken cooked yesterday is incorrect; observed several flies in dining room and kitchen; several containers of food are uncovered; fryer baskets are heavily bent; there is dried food debris on wire racks, outside of food/spice storage bins and containers, and on handles and gaskets of prep units; observed grease buildup on hood and exterior of fryers.
Taqueria y Restaurante El Chido, 7568 West Broad Street – The following violations were reported during a routine inspection: there are new employees who have not been given the employee health policy to sign; person in charge couldn’t provide written procedures for cleaning up vomit and diarrhea; cleaning items and a calculator are stored in bar hand sink; there is no hand soap at front hand sink; several foods have elevated temperatures in prep units across from grill/fryers (sliced tomatoes, cut lettuce, shredded lettuce, shredded cheese, salsa and beef); facility doesn’t have a food thermometer; there are missing or broken thermometers in all cold units; containers of flour, oil and water are unlabeled; observed several flies and small cockroaches; rice is hot holding in a non-food grade “thank you” to-go bag in steam well; cold prep units across from cookline are not holding foods at the proper temperature; dish machine isn’t working; chest freezer lids are broken; fryer handle is burned; several gaskets are torn; facility doesn’t have test strips for bleach used to sanitize at three-compartment sink; observed grease and dried food on wire racks, prep tables, inside prep units, around fryer and grill, on handles of units and walk-in cooler doors, and on floor of outdoor storage area; there are missing and cracked floor tiles; there are holes in the walls; water and food found on floors, especially near sinks and under stationary equipment.
QPot Hot Pot & BBQ, 11740 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; open cup of soda found on top of dishwasher; several electrical wires and cords are stored in hand sink next to ware wash area and no paper towels are available; facility couldn’t provide parasite destruction letter from salmon supplier; carton of raw, shelled eggs are stored over ready-to-eat sauce in three-door prep unit; there is a buildup of pink and black slime on interior shield of ice machine in service station; menu doesn’t contain a consumer advisory statement to inform customers of the increased risk of eating raw and undercooked foods; box of chicken is stored directly on walk-in freezer floor; gasket liner is torn on right-side door of three-door prep unit and on both doors of lowboy unit on prep line; wire racks in dry ingredient storage area and in walk-in cooler are peeling and rusty; two of four fryer baskets have broken off metal pieces; long cutting boards on prep units are heavily stained and scored; cardboard is lining the bottom shelf of prep tables in back of kitchen; facility couldn’t provide sanitizer test strips.
