Johnson Elementary School, 5600 Bethlehem Road – During a routine inspection on Sept. 15, inspectors observed sliced turkey in a reach-in cooler dated Sept. 8; two dead cockroaches beneath the dish machine; no temperature measuring strips available; black stains, grime and/or mold on reach-in cooler door gaskets throughout the kitchen and walk-in freezer door gaskets; direct plumbing connections beneath both two-compartment sinks; and an unsealed floor beneath the front three-door reach-in cooler.
Laburnum Elementary School, 500 Meriwether Avenue – During a routine inspection on Sept. 15, inspectors observed black stains and grime on reach-in cooler door gaskets; direct plumbing connections beneath the three-compartment and prep sinks; and dust on kitchen ceiling tiles and vents.
Highland Springs High School, 200 South Airport Drive – During a routine inspection on Sept. 16, inspectors found no paper towels at the dish area hand sink and a nonfunctioning towel dispenser near the deep fryers; observed dried food debris inside a food processor and on its base and removable parts; scored cutting boards; dirty door gaskets on three reach-in coolers, food debris on serving line shelves, grease residue on the sides of deep fryers, dirty walk-in cooler and freezer door gaskets, dusty walk-in cooler fan guards and moldy wire storage racks; and a plumbing leak beneath the dish machine spray sink.
Donahoe Elementary School, 1801 Graves Road – During a routine inspection on Sept. 16, inspectors observed milk at 51 degrees in a milk cooler; dried food debris on wire storage racks in the walk-in cooler, dirty and dusty walk-in cooler fan guards, dried food debris on a blue wall beneath the serving line and black stains on reach-in cooler door gaskets; direct plumbing connections beneath the two-compartment and three-compartment sinks; and dust on ceiling tiles near ventilation.
Civita Italia, 3401 Haydenpark Lane – During a follow-up inspection on Sept. 16, inspectors observed black and pink residue inside an ice machine; and leaks beneath the three-compartment sink.
MOD Pizza, 8671 Quioccasin Road – During a routine inspection on Sept. 16, inspectors found that the establishment did not have a certified food protection manager on staff; observed pizza sauce, salami and cheese at 44 degrees in the far-right serving line unit; an unlabeled spray bottle containing blue all-surface cleaner and an unlabeled bucket of sugar; the right side of a serving line refrigeration unit failing to maintain food at 41 degrees or below, a problem identified as a "historic" violation; a nonfunctioning make table on the back wall that reportedly had not worked for a year; rusted chemical storage racks; no irreversible temperature measuring device for the high-temperature dish machine; a green hose connected to the mop sink faucet and resting in the sink without an air gap; leaks from both hand sink faucets; and water dripping from a blue hose at the mop sink (origin unknown).
Starbucks Coffee Company, 8631 Quioccasin Road – During a routine inspection on Sept. 16, inspectors observed an empty soap dispenser at the back kitchen hand sink; trash on the ground around outdoor dumpsters and spilling from the enclosure; open dumpster lids; and pitted wall tiles throughout the front service line.
Oak Avenue Complex, 15 South Oak Avenue – During a routine inspection on Sept. 16, inspectors observed a staff restroom hand sink that provided water for only 11 seconds; one dead cockroach beneath the three-compartment sink; dirty reach-in cooler door gaskets and torn and dirty milk cooler door gaskets; and dust on ceiling tiles near ventilation and dirty floor drains throughout the kitchen.
KickBack Jacks, 9318 West Broad Street – During a follow-up inspection on Sept. 16, inspectors observed brown buildup inside an ice machine.
Highland Springs Elementary School, 600 Pleasant Street – During a routine inspection on Sept. 17, inspectors observed a staff member wearing a wristband; dirty reach-in cooler and produce walk-in cooler door gaskets; dried food stains and debris on milk cooler door gaskets; direct plumbing connections beneath the three-compartment sink and prep sinks; and dried food stains on red walls beneath the serving line.
Holladay Elementary School, 7300 Galaxie Road – During a routine inspection on Sept. 17, inspectors observed plastic utensils stored on the serving line with their lip contact surfaces facing upward.
Baker Elementary School, 6651 Willson Road – During a routine inspection on Sept. 17, inspectors reported no violations.
Family Secrets Restaurant, 7103 Brook Road – During a routine inspection on Sept. 17, inspectors observed chlorine sanitizer below 25 ppm in the three-compartment sink and 0 ppm in the dish machine; dried flour debris in bulk bins used for poultry and fish breading and another bin on a wire rack; pink and gray slime inside an ice machine and grime inside a soda gun; dried food debris in a microwave; rice dated Sept. 10 and potato salad dated Sept. 9, with the person in charge stating that those were old dates; macaroni and cheese, collard greens and mashed potatoes cooling in large aluminum pans covered with plastic wrap and aluminum lids and stacked in the walk-in cooler, with macaroni and cheese measuring 86 and 79 degrees about an hour after cooking; catfish listed on the menu while swai was stored in the walk-in cooler; dead cockroaches beneath the three-compartment sink, around the bar ice machine and service sink and in traps, as well as several large flies in the kitchen; a broken table in the dish area, a nonfunctioning two-door reach-in cooler, an unused but functional one-door reach-in cooler, a broken ice machine door, a glass-front bar cooler holding at 44 degrees, rusted wire racks and a rolling cart, and a leak beneath the dish machine; deeply cut and stained cutting boards; dried food debris and grease on a magnetic knife strip, the sides of cooking equipment, shelves of make tables, interiors and gaskets of cooling units, grease traps and pipes beneath the three-compartment sink and wheeled food carts; a dusty floor fan; a direct plumbing connection at the three-compartment sink; dried food debris and grease on floors beneath cooking equipment, the three-compartment sink and dish area; and grease accumulation on hood vents and filters.
Gayton Elementary School, 12481 Church Road – During a routine inspection on Sept. 17, inspectors observed direct plumbing connections beneath the three-compartment sinks and prep sink.
Applebee's Neighborhood Grill & Bar, 9601 West Broad Street – During a routine inspection on Sept. 17, inspectors observed a cook with a full beard who was not wearing a beard guard; a heavily rusted salad cooler rack; heavy carbon buildup on grills and dirty prep cooler condensers; heavy grease buildup behind fryers, dust on ceiling vents and limescale-like buildup on the floor beneath the dish machine; and worn floor grout along the cookline and dish machine.
Rolfe Middle School, 6901 Messer Road – During a routine inspection on Sept. 17, inspectors observed pasta with meat sauce at 120 to 138 degrees in hot holding units; lettuce and cheese at 47 degrees and sour cream at 52 degrees in the walk-in cooler; and sanitizer buckets containing 0 ppm sanitizer.
Twin Hickory Tavern, 1420 North Parham Road – During a follow-up inspection on Sept. 17, inspectors observed sanitizer at 0 ppm in the downstairs bar dish machine; and cut cherry tomatoes at 45 degrees, diced tomatoes at 44 degrees and portioned shredded cheese at 47 degrees in the right make table, with the products stocked above the fill line of the metal bins and the cherry tomatoes placed in the unit immediately after cutting.
Panera Bread, 1370 North Parham Road – During a follow-up inspection on Sept. 17, inspectors observed a housefly land on a pumpkin cinnamon roll in a display case; and several flies throughout the kitchen.
Harvie Elementary School, 3401 Harvie Road – During a routine inspection on Sept. 17, inspectors observed black staining on reach-in cooler and walk-in cooler door gaskets; moldy wire storage racks in the walk-in cooler; and dusty ceiling tiles near ventilation and dirty floor drains throughout the kitchen.
Arthur Ashe Elementary School, 1001 Cedar Fork Road – During a routine inspection on Sept. 17, inspectors observed dried food debris on food processor parts; multiple cracked lids on plastic food containers; black staining on reach-in cooler and walk-in cooler door gaskets; direct plumbing connections beneath both three-compartment sinks; and stained and dusty ceiling tiles.
Chennai Cafe, 200 Towne Center West Boulevard – During a routine inspection on Sept. 18, inspectors found no paper towels at the hand sink across from the dishwasher; recorded 0 ppm of sanitizer being dispensed from the dishwasher after equipment was washed within; rice batter prepared the previous day without a preparation date in the walk-in cooler; and unlabeled bulk bins of dry ingredients beneath a prep table across from the cookline.
Longdale Elementary School, 1351 Cole Boulevard – During a routine inspection on Sept. 18, inspectors observed milk at 46 degrees in a refrigerator to the right of the serving line; grime on reach-in freezer and walk-in cooler door gaskets; a T-connector with two hoses attached to the mop sink under continuous water pressure without appropriate backflow prevention; and a direct plumbing connection beneath the three-compartment sink.
Taqueria y Restaurant El Chido #2, 1412 Starling Drive, Suite C – During a routine inspection on Sept. 18, inspectors reported no violations.
Henry Ward Elementary School, 3400 Darbytown Road – During a routine inspection on Sept. 18, inspectors observed a milk cooler in disrepair.
Glen Allen Elementary School, 1101 Mill Road – During a routine inspection on Sept. 18, inspectors observed taco meat at 129 degrees in a portion cup on the top shelf of the serving line; buttermilk ranch at 54 degrees in an ice bath, spinach at 55 degrees and watermelon at 48 degrees on the serving line; and direct plumbing connections beneath the three-compartment and two-compartment sinks.
My Empanada, 1421 Blue Jay Lane – During a risk factor inspection on Sept. 18, inspectors found no paper towels at the back kitchen hand sink and an unplugged electric hand dryer; and no required warning label on unpasteurized juice packaged at the facility.
Greenwood Elementary School, 10960 Greenwood Road – During a routine inspection on Sept. 18, inspectors observed plastic utensils on the serving line with their lip contact surfaces facing upward; and dried food debris on the bottom shelves of the serving line and on a milk refrigerator door gasket.
Con Salsa Venezuelan, 3016 Mountain Road, Unit A – During a risk factor inspection on Sept. 19, inspectors observed sponges stored in a hand sink; shredded cheese and garlic sauce at 48 degrees in a reach-in cooler farthest from the door; and several prepared temperature-controlled foods without date labels, with the person in charge reporting that the foods had been prepared two days earlier.
Pelagos VR, 1404 North Parham Road, Suite 225 – During a routine inspection on Sept. 20, inspectors observed pink mildew accumulation on the back of an ice machine; a dish machine dispensing chlorine sanitizer at 0 ppm during two cycles and flooding the kitchen floor while operating, even after draining; dust accumulation on walk-in cooler fan guards; and a deteriorating stair step leading to the walk-in cooler.
Taco Bell, 7210 West Broad Street – During a routine inspection on Sept. 21, inspectors found no paper towels at the back hand sink; observed residue buildup on soda nozzles and the ice dispenser at the drive-thru; dried food debris inside cold units and on their gaskets and heavy ice buildup in a cold unit beneath the second prep side; and a Y nozzle with shut off valves attached at the mop sink.
Chadar Thai LLC, 8030 West Broad Street, Unit A – During a routine inspection on Sept. 21, inspectors observed an employee handling a raw egg and then touching clean equipment and beginning to cook without washing hands; a bar hand sink being used for storage, with the person in charge reporting that employees washed their hands in the three-compartment sink; raw chicken and shell eggs in a container resting on pineapple pieces, with an egg contacting the pineapple; a dish machine that was not dispensing chlorine sanitizer even after priming and appeared to have a low sanitizer supply; containers stored as "clean" with rice and dried food debris, with cockroaches crawling on clean dishes; tofu and pad Thai sauce at 45 degrees, sticky rice at 44 degrees and pho broth at 45 degrees in the walk-in cooler, as well as curry sauce at 55 degrees in an ice bath to which ice was added when the inspector arrived; missing date marks on noodles, chicken skewers, curry and broths prepared and held for more than 24 hours; Raid and Top Shot pest control chemicals stored for use in the kitchen; unlabeled containers of salt and bottles of water and oil; several live cockroaches near the sanitizer basin and dead cockroaches throughout the kitchen; a rice scoop stored in water at 79 degrees; cardboard and foil used to line kitchen shelves and unpainted wood in the kitchen; a walk-in cooler not maintaining proper temperature, heavily bent fry baskets and several cracked or broken food containers; grime and dried food debris inside and outside cold units, on microwaves and shelves; and grime and dried food debris on floors throughout the kitchen.
Longhorn Steakhouse, 11311 West Broad Street, Suite A – During a risk factor inspection on Sept. 21, inspectors observed rice at 43 degrees and sliced tomatoes at 47 degrees, above the required cold holding temperature of 41 degrees.
Chicken Salad Chick, 9631 West Broad Street – During a follow-up inspection on Sept. 21, inspectors observed a dirty interior of an ice machine; white buildup on walk-in cooler racks; and a Y-connector on the mop sink without proper backflow prevention.
Kura Revolving Sushi Bar, 11331 West Broad Street, Suite 125 – During a routine inspection on Sept. 21, inspectors reported no violations.
Chipotle Mexican Grill, 10501 West Broad Street – During a routine inspection on Sept. 21, inspectors recorded grilled chicken cooking temperatures of 145 degrees to at least 165 degrees; and observed a leak at the vegetable wash sink faucet.
Los Panchos Mexican Restaurant, 3532 Pump Road – During a risk factor inspection on Sept. 22, inspectors observed a blender stored in the bar hand sink, making the sink inaccessible; two large dented food cans in dry storage; and black and pink buildup along a crevice of the ice machine's interior shield associated with increased condensation.
Good Vices, 1321 East Nine Mile Road – During a routine inspection on Sept. 22, inspectors reported no violations.
Wendy's, 11274 Patterson Avenue – During a routine inspection on Sept. 22, inspectors observed an employee handling exposed food without wearing a proper hair restraint; an employee with artificial nails handling exposed fries and nuggets without gloves; and food debris and dust on a wire beverage rack, a cup holder at the drive-thru, walk-in cooler fan grates and bread racks.
Healthy Mixes Herb Spot, 5203 South Laburnum Avenue – During a routine inspection on Sept. 22, inspectors found no paper towels at a hand sink; no date marks on prepared green beans, jackfruit and coleslaw; buildup on racks and gaskets of the main prep unit; no backflow prevention device on a mop sink with a hose and nozzle attached; and food splatter on walls and soiled ceiling tiles above the prep area.
