Dugout Bar & Grille, 1510 Mountain Road – The following violations were reported during a routine inspection: did not observe employee health policy or written procedures for cleaning up vomit and diarrhea; raw shell eggs stored above pie in kitchen reach-in cooler; temperatures of chicken wings and raw hamburgers are elevated in kitchen reach-in cooler; ambient temperature in this unit is elevated; wood shelving and drawers are in need of resealing/painting; flooring throughout kitchen is in disrepair; there is ceiling damage by back door; ceiling in beer storage area is in disrepair; there is missing coving by cooking area/under hand sink in kitchen; observed grease buildup on hood vents and filters.
Domino’s Pizza, 8703 West Broad Street – The following violations were reported during a routine inspection: person in charge could not present or verify procedures for sauces; did not observe employee health policy or written procedures for cleaning up vomit and diarrhea; dust buildup found on fans and vents of pizza oven; restroom doesn’t have a covered trash can; grout is worn throughout kitchen.
El Torito Mexican Restaurant & Grill, 5600 Williamsburg Road – The following violations were reported during a routine inspection: hand sinks in server area and behind bar don’t have paper towels; back hand sink doesn’t have hand soap; mildew found on shield of ice machine and on drink nozzle; temperature of rice and beans on hot line is too low; there are no date marks on refried beans and salsas; multiple spray bottles throughout facility are unlabeled; ground beef is thawing in hand sink and not under running water; pitcher of a mix drink is being kept in the ice bin.
JULY 9
Golden Unicorn, 7023 West Broad Street – The following violations were reported during a routine inspection: temperature of beef in reach-in cooler is elevated; rice scoop is stored in room temperature water; forks, spoons and chopsticks on buffet line are not stored inverted to protect from contamination.
Supper Club, 417 North Ridge Road – The following violations were reported during a routine inspection: knife is stored in hand sink; broccoli sauce containing garlic in oil is past its expiration date; racks in walk-in cooler and walk-in freezer are rusted; one white cutting board is scratched and stained; observed what appears to be dark colored mildew on fan guards of walk-in cooler; floor throughout kitchen is chipped.
Qdoba, 11282 West Broad Street – The following violations were reported during a routine inspection: temperature of diced chicken in hot box is too low; drying racks next to three-compartment sink are rusty; ingredient racks are covered in old food debris.
JULY 10
Backwoodz BBQ, 400 West Williamsburg Road – The following violation was reported during a routine inspection: observed buildup on racks in units and on bottom of units.
Domino’s Pizza, 70 South Airport Drive – The following violations were reported during a routine inspection: manufacturer’s tape is still on a few pieces of equipment in the kitchen; there is dried food debris on rolling carts and on wire storage racks in walk-in cooler; reach-in cooler door gaskets are dirty and moldy; floor fan in kitchen is dusty; observed dried food stains on walls throughout the kitchen; ceiling tile is stained; ceiling tile is dusty near ventilation; staff beverages are stored amongst food for customers.
Domino’s Pizza, 550 East Laburnum Avenue – The following violations were reported during a routine inspection: observed mold and food debris on reach-in cooler doors and door gaskets; there is dust accumulating on tubes, cords, surfaces and pipes behind pizza oven; dried food debris found on side of pizza make table and wire storage racks in walk-in cooler; there are dried food stains on walls throughout the kitchen.
Intermission Beer Company, 10089-A Brook Road – The following violation was reported during a routine inspection: there is 0 ppm of chlorine sanitizer in dish machine.
Ginza Spot, 3402 Pump Road – The following violations were reported during a routine inspection: there are no date marks on two large pots of homemade chicken broth in walk-in cooler; menu doesn’t contain an advisory statement next to foods that can be ordered raw; spray bottle of bleach and water mixture behind front counter is unlabeled; raw chicken breasts in vacuum sealed packages are being cooked using a sous-vide process (packages don’t contain label with product name, date and time, which is required unless the facility has filed a HACCP plan for sous-vide cooking); two containers of spices stored underneath cookline are unlabeled; knife is stored in crevice of prep unit and prep table in kitchen; take-out containers stored above prep line are not inverted to protect from contamination; there are rust spots on wire racks in walk-in cooler; gasket liner is torn on walk-in freezer door; ingredients racks beside walk-in freezer are covered in old food debris and grease; mop isn’t hung or inverted to air dry.

Tiki Tiki, 8917 Patterson Avenue – The following violation was reported during a routine inspection: bottle of Tylenol is stored above sauce packets.
Nineveh Bakery & Special Food, 8026-C West Broad Street – The following violation was reported during a routine inspection: handles of utensils are stored inside rice cooker and a container of pickled vegetables.
Bowlero—West End, 8037 Shrader Road – The following violations were reported during a routine inspection: front hand sink doesn’t have hand soap; there is pink/brown residue on ice plate in ice maker; spray cleaner is stored next to pizza spice toppings and single-use cups; spray bottles of chemical cleaners are unlabeled; bulk container of white powdery substance and liquid squeeze bottles are unlabeled; observed small flies in bar area attracted to sticky/moist areas and spills; in-use knife is stored directly inside dirty prep unit; fryer baskets are heavily bent; bar hand sink faucet is loose; unit gaskets are torn; front hand sink isn’t working; observed grease and dried food debris buildup on sides of cookline equipment, prep shelves, beverage mats, unit vents, pizza ovens, prep unit gaskets, utensil containers, and inside/outside of all cold holding units; observed grease and dried food on floor and wall behind cookline equipment; perimeter of kitchen is dirty; there is heavy dust buildup on hood vents and ceiling vents.
