SB's Lakeside Loveshack, 6935-37 Lakeside Avenue - During a routine inspection on Aug. 11, inspectors observed no handwashing sign at the bar sink; sanitizer at the three-compartment sink at less than 150 ppm; dried food debris on a can-opener blade; a vegetable slicer and pans stored as clean with food debris or stickers remaining on them; hollandaise above the grill at 103 degrees; ham, egg salad, cut celery and cooked grits held past seven days; many large flies throughout the establishment; unwrapped carry-out containers stored on a dusty bottom shelf; several stained and deeply cut cutting boards; a reach-in freezer not maintaining freezing temperatures, with food at 34 degrees, and a bar mini-refrigerator with an ambient temperature of 59 degrees; leaks at and beneath the three-compartment sink and at a hand sink in the front coffee area; an open restroom door that was not self-closing; flooring, walls and tile in disrepair throughout the establishment; a grease spill behind the cookline and soiled ceiling tiles and vents; and grease accumulation on hood vents and filters.
Empanadas Market, 1110 Westbriar Drive - During a routine inspection on Aug. 11, inspectors observed an employee eating at a prep table where food also was being prepared; black residue buildup inside the ice maker; two spray bottles containing sanitizer without identifying labels; ice scoops stored on a dirty surface on top of the ice maker; forks at the front counter stored with their food-contact surfaces facing upward; heavily stained and scratched cutting boards; dust and grease buildup on wire racks; and dust buildup on kitchen ceiling vents and panels.
Starbucks inside Target, 4521 South Laburnum Avenue - During a routine inspection on Aug. 11, inspectors observed no paper towels at the back hand sink; and a dish machine reaching only 150 degrees.
Tous Les Jours Bakery Co., 7801 West Broad Street - During a routine inspection on Aug. 11, inspectors observed no employee health policy; no written procedures for cleaning up vomit or diarrhea; self-service tongs being sanitized and wiped dry rather than properly washed, rinsed and sanitized; heavy pink slime buildup on an interior ice-machine plate; cleaned utensils and cutting boards stored with food debris remaining on them; dried food debris inside a microwave; cream cheese cold foam on a counter at 62 degrees; no times marked for savory pastries containing cheese, egg and hot dog that were being held using time control; a sanitizer bucket and two cleaner spray bottles without identifying labels; several containers of white powdered substances without identifying labels; many small flies in the back kitchen around heavily soiled linen bags and a hand sink; a bulk container of flour stored on the floor beneath a hand sink with its lid unsecured; in-use spoons stored in water at 74 degrees; three knives with chipped and dented edges; no chlorine test strips; dried food debris on gaskets of all cooling units; grime on kitchen and walk-in floors and food debris buildup in floor drains; and dust accumulation on hood vents and filters above ovens.
Nash & Smashed, 10174 West Broad Street - During a follow-up inspection on Aug. 11, inspectors observed an open container of mayonnaise outside refrigeration at 70 degrees; heavily bent fryer baskets; quaternary sanitizer being used without the correct test strips; and a leak at the front hand sink.
Arby's, 5220 Williamsburg Road - During a routine inspection on Aug. 11, inspectors observed two bags of chicken with an Aug. 8 discard date; grime buildup on refrigeration unit gaskets and food debris in the bottoms of units; and low grout along the prep line floor and discolored ceiling tiles.
Juicebar Plant-Based Health, 1366 Gaskins Road - During a routine inspection on Aug. 10, inspectors reported no violations.
VFW Post 6364, 6502 Dickens Place - During a routine inspection on Aug. 10, inspectors observed raw meat and shell eggs stored above ready-to-eat food in a reach-in cooler; dried food debris inside a kitchen microwave; beef stew in the walk-in cooler without a date mark; several spray bottles containing cleaner without identifying labels; food staff without hair restraints; stacks of paper plates and carry-out containers stored food-contact-side up and outside their original packaging; dust on walk-in-cooler fan guards and dirty door tracks and wire racks in a reach-in cooler near the bar; a direct plumbing connection beneath the three-compartment sink; food stains and debris on wall beneath the three-compartment sink; holes in the wall behind the three-compartment sink and deteriorating caulk; and dust and grease accumulation on hood filters.
Torchy's Tacos, 11120 West Broad Street - During a risk factor inspection on Aug. 10, inspectors found that no employee working in the establishment had a Food Protection Manager certificate available; an open-top drinking cup was stored on a rack with clean dishes in the warewashing area; an employee handling raw chicken did not properly wash his hands before putting on new gloves; a hand sink at the end of the dry storage area was blocked by a trash can and large table scale; no paper towels were available at the bar hand sink; a soda nozzle at the bar was encrusted with dried debris; refried beans placed into a grill-drawer refrigeration unit at 10:30 a.m. after cooking measured 76 degrees at 12:20 p.m., 10 minutes before the end of the two-hour cooling period to reach 70 degrees or below; tater tots in a fry warmer measured 106 degrees; and a spray bottle containing a yellowish chemical had no identifying label.
Five Guys, 12210 West Broad Street - During a routine inspection on Aug. 10, inspectors observed a fryer basket with broken-off metal pieces.
Tropical Smoothie Cafe, 1601 Willow Lawn Drive - During a routine inspection on Aug. 7, inspectors found that the person in charge did not have a current Certified Food Protection Manager credential; a can of pineapple had dents along the top seam; what appeared to be mildew has built up in blender lids, particularly orange lids; cut watermelon in a smoothie make table measured between 40 and 44 degrees and was piled above the bin's fill line; the person in charge wore bracelets while handling food; an ice scoop was stored with its handle contacting ice and a single-service cup was being used as a scoop in a bulk-storage bin; dry-storage racks were rusted and condensation was dripping from a walk-in-cooler unit into catch bins, with exposed food beneath the bins; grime had accumulated on refrigeration unit gaskets and door edges and dark grime had built up in the bottoms of blender cups; a dump sink faucet was leaking; a direct plumbing connection was observed at the three-compartment sink; kitchen air vents were dusty; a baseboard was peeling away from the wall beneath a hand sink; and overhead kitchen lights lacked shields and it could not be determined whether the bulbs were shatter-resistant.
Chop't Creative Salad, 1601 Willow Lawn Drive - During a routine inspection on Aug. 7, inspectors observed elevated temperatures of TCS foods in a serving-line unit, including steak between 40 and 47 degrees, chicken at 42 degrees, hard-boiled eggs between 40 and 57 degrees and chickpeas between 40 and 47 degrees, with most TCS foods piled above the metal bins' fill lines; grime on a lowboy cooler gasket and frame, and grease and food debris accumulation beneath a serving line unit; and open lids and doors on outdoor dumpsters.
Emmy Squared Pizza, 2225 Old Brick Road - During a routine inspection on Aug. 7, inspectors observed grime buildup inside soda guns at the bar; lettuce at 53 degrees in a prep unit; spray bottles containing cleaning chemicals without identifying labels; bent and breaking metal wires on fryer baskets; food debris buildup on wire racks, walk-in cooler fan systems, beer lines and portable fans; and food debris and grime inside floor drains and on floors beneath and behind equipment.
Yummy Yummy Bowl, 4501 South Laburnum Avenue - During a routine inspection on Aug. 7, inspectors observed an open drink on a prep table next to bowls; raw shrimp in a hand sink; no parasite destruction documentation for fish served raw at the sushi station; raw meats and seafood improperly stored together or in the wrong order in multiple refrigeration units; raw shrimp, chicken and fish at 47 to 50 degrees in a reach-in cooler with an air temperature of 50 degrees; noodles at 70 degrees and cabbage at 57 degrees that had been prepared two to three hours earlier; shrimp at 65 degrees thawing on a counter alongside imitation crab at 33 degrees; multiple scoops and tongs stored in room-temperature water and a knife stored between a prep table and freezer; and containers stacked wet on a clean storage shelf.
Happy Arepas, 10170 West Broad Street - During a follow-up inspection on Aug. 7, inspectors observed water at the hand sink at 83 degrees, with the person in charge reporting that water was being heated on the stove to fill the water tank because a hose needed replacement; a nonfunctional hot-water heater; and no quaternary sanitizer test strips for the three-compartment sink.
Pho So 1, 9135 West Broad Street - During a routine inspection on Aug. 7, inspectors observed no Food Protection Manager certificate; an employee handling raw beef and then ready-to-eat food without proper handwashing; no handwashing sink in the dishwashing area; no paper towels at the kitchen or staff-bathroom hand sinks; rice weevils in buckets of rice with the lids open; dried food debris on a slicer; a bowl of noodles on the counter measuring between 101 and 144 degrees; condensed milk at 49 degrees, half-and-half at 45 degrees, brisket at 47 to 53 degrees and shrimp rice-paper rolls on the counter at 78 degrees; no date marks on any foods; no time marks for sprouts, lettuce and cooked beef being held using time control; an unlabeled cleaner spray bottle; three buckets of tripe thawing on the counter; small flies on soiled sugar bags and a heavily soiled storage room floor with dead bugs that appeared to be weevils and ants; several foods stored on the walk-in freezer floor; a food employee wearing a watch; single-service plastic containers without handles being used as scoops in rice buckets; torn gaskets on a prep cooler and lowboy cabinet; no chlorine sanitizer test strips; black buildup on prep cooler gaskets and a greasy hood; an open staff bathroom door; two mops left in dirty mop water throughout the inspection; and worn floor grout in the dish and dry storage areas.
Weinstein JCC, 5403 Monument Avenue - During a risk factor inspection on Aug. 7, inspectors found an unpermitted caterer using the kitchen to prepare food for an event at another location and ordered the caterer to cease operations; pink and dark-colored spots on the ice-machine ice guard; several plastic spatulas with chipped edges; and dried food splatter on top of a microwave.
Chipotle Mexican Grill, 4071 Concord Creek Place - During a routine inspection on Aug. 6, inspectors observed multiple flies in the kitchen; a leak in pipes beneath the three-compartment sink; and dried food debris and stains on kitchen walls.
Verena at the Glen, 10286 Brook Road - During a routine inspection on Aug. 6, inspectors observed an in-use chlorine dish machine with 0 ppm sanitizer; black and pink buildup inside the ice machine, dried buildup on drink dispenser nozzles and dried food debris on a can opener blade; sliced cheese piled above a prep unit's chill line at 51 degrees; salsa, egg salad and sausage gravy held past seven days; deli turkey and ham, cut tomatoes and lettuce and chicken salad without date marks; a consumer advisory that lacked asterisks identifying sunny-side-up and over-easy eggs as undercooked items; no ambient thermometers in two reach-in coolers or a grill drawer, and broken exterior thermometers on the grill drawer and a reach-in cooler; gaps around the back door allowing light to enter; dried food debris on refrigeration equipment, shelving and cooking equipment, dust on several surfaces, and condensation pooling in the bottom of a reach-in cooler; food debris and trash around the dumpster area; and dried food debris on floors, in floor drains and along perimeter edges throughout the kitchen.
Chung Hing Chinese Restaurant, 3053 Lauderdale Drive - During a routine inspection on Aug. 6, inspectors observed the kitchen's only hand sink blocked by cooking equipment and a laptop; raw meats stored above ready-to-eat sauces in a prep unit; sweet and sour chicken at 50 degrees; two bulk bins containing unidentified white powdered ingredients; a knife stored in a dirty crevice of a prep unit; torn walk-in cooler and freezer door gaskets, rusted walk-in cooler and drying racks and a fryer basket with a broken-off metal piece; a scored cutting board; a dusty kitchen fan used to cool foods; and a direct plumbing connection beneath the three-compartment sink.
