JULY 21

Hawk’s BBQ & Seafood, 8191-B Brook Road – The following violations were reported during a follow-up inspection: facility couldn’t provide food protection manager certification or written procedures for cleaning up vomit and diarrhea; dried food debris found on can opener, collard greens crock pot, exteriors of spice shakers and bulk bins, interior of breading bin, and in microwave; temperature of collard greens is too low; temperature of raw fish on counter on ice is elevated (not submerged in ice water); bulk bins of flour, cornmeal, etc. are unlabeled; observed live and dead roaches in walk-in storage area, under three-compartment sink, on wall in kitchen and on light fixtures; observed rusted wire racks and torn gaskets on cookline freezer and white reach-in cooler; ice plate on ice machine is rusted; long cutting board has deep scratches and staining; cooling unit gaskets are dirty; observed dried food debris on wire racks and exteriors/interiors of cooling and cooking equipment; there is a direct plumbing connection under three-compartment sink; ceiling tiles are heavily soiled; mop isn’t hung or inverted to air dry; flooring throughout kitchen is in disrepair; wall in center of kitchen is in disrepair and bulging around the base/coving due to water damage; there is heavy grease on hood vents and filters.

Dunya Kabob RVA, 9039 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; raw meats are stored above bottles of ready-to-eat sauce; microwave is dirty; employee didn’t sanitize knives or blenders after washing with soap; temperatures of potato and lentils in two-door reach-in cooler are elevated (there are several hot foods cooling in this cooler); there are no date marks on meats, potatoes or lentils that are cooked and cooled; small bin of chemicals is stored on top of clean dish shelf above dishes; glass cleaner and pest spray are stored next to foods on front counter shelves; two spray bottles of cleaner are unlabeled; tea bags are removed from packaging and not protected from contamination on front counter; facility doesn’t have chlorine sanitizer test strips; reach-in cooler and lowboy cooler gaskets are dirty.


JULY 22

Bubba’s Bar & Grill, 4000 Williamsburg Road – The following violations were reported during a routine inspection: mildew found on soda nozzles and ice machine; observed stained and missing ceiling tiles.

Terra Nova Bar & Grille, 4700 South Laburnum Avenue – The following violations were reported during a routine inspection: there is ice buildup in walk-in freezer; walk-in #1 is holding at a slightly elevated temperature because there is heavy ice buildup on fans; fryer baskets and walk-in racks need cleaning; there is a leak at the prep sink; observed low grouting throughout back kitchen; holes, patches and tape found on wall of back cookline; floor, ceiling and light in storage room are in disrepair; observed hole in wall by mops; there are missing and stained wall tiles under dish/three-compartment sink; ceiling is leaking and there are multiple catch buckets.

Lulabelle’s Café & Catering, 2012 Staples Mill Road – The following violations were reported during a routine inspection: food staff aren’t wearing hair restraints; cutting boards are scored; there is no irreversible temperature indicator (thermo plate, thermo strips) on site; facility doesn’t have sanitizer test strips; reach-in cooler door gaskets throughout kitchen have dried food debris on/in them; there is grease residue on sides of hot line cooking equipment; deli-style display cooler sliding door indentations are dirty; there is a direct plumbing connection underneath kitchen and bar area three-compartment sinks; there is a missing floor tile in back corridor near doorway; ceiling vent in kitchen is dusty; floor, wall and pipes underneath three-compartment sink are dirty.

Sushi Hibachi Bistro, 10490 Ridgefield Parkway – The following violations were reported during a routine inspection: prep and sushi bar cutting boards are heavily scratched and stained; observed buildup of food and grease on kitchen walls.

Canastas Chicken, 11424 West Broad Street – The following violations were reported during a follow-up inspection: container of noodles in walk-in cooler is nine days past its expiration date; menu still doesn’t contain an asterisk next to the Canasta Burger or Marinated Salmon linking the undercooked food items back to the consumer advisory statement; gasket liner on walk-in cooler door is still torn; drying rack across from three-compartment sink is still dirty; there is still a y-valve attached to mop sink faucet without the appropriate backflow prevention device installed.


JULY 23

Dunkin Donuts, 7721 Brook Road – The following violations were reported during a routine inspection: hot water doesn’t reach the required temperature at hand sink in front drink area to the right; dried debris found on sure shot syrup dispenser nozzle; there are old stickers on metal milk frothing jugs; observed dried debris on milk frother on right side (person in charge states that frothers are cleaned every night but it should be every four hours); employees are wearing watches and/or bracelets; multiple in-use spoons are stored in room temperature water.

Chipotle Mexican Grill, 1070 Virginia Center Parkway – The following violations were reported during a routine inspection: mildewy substance found on public ice dispenser; chlorine dish machine is in disrepair and not in use (facility is using three-compartment sink for sanitizing); walk-in cooler fan guards are dusty; there is food debris on cooling unit gasket under service counter on left side; there is a leak at women’s restroom hand sink; hand sink in back of kitchen leaks unless tightened with a lot of force; observed dried food debris on floors under/around stationary equipment and in floor drain under food prep sink; ceiling and ceiling vent in dish area are dusty.

Pinch’s Soul Food & Catering, 3708 Meadowbridge Road – The following violations were reported during a routine inspection: dried food stains found in all kitchen microwaves; observed grease residue on sides of hot line cooking equipment; there is a direct plumbing connection underneath three-compartment sink; observed dried food stains on walls behind hot line cooking equipment.

Mushmouth, 2810 North Parham Road – The following violations were reported during a routine inspection: spray bottle of oven cleaner is stored next to deli slicer; two boxes of sweet potato fries are stored directly on walk-in freezer floor; single-service forks in box for self-service are laying in multiple directions, which doesn’t prevent contamination of lip contact surfaces; floor fan is dusty; food splatter found in microwave; there is a direct plumbing connection at three-compartment sink.

Sicilian Bakery, 9052 West Broad Street – The following violations were reported during a routine inspection: hot water isn’t working at back kitchen hand sink; front hand sink doesn’t have paper towels; quat sanitizer concentration in three-compartment sink isn’t correct; there is no time marked on cachito bread and croissant delux bread put out on display; several non-reusable scoops (jugs cut open, single-service containers), some without handles, are being used in bulk foods; ice scoop handle is touching ice.

Arby’s—Tuckernuck Square, 9080 West Broad Street – The following violations were reported during a routine inspection: observed an employee take the trash out and then wash hands for less than five seconds before putting on gloves and begin food prep; there is a box of potatoes with fuzzy grey buildup that is attracting a large number of flies; temperature of whipped cream is elevated because it is sitting out on the counter; gasket is torn in salad cooler; floors are cracked in both walk-ins.


JULY 24

Jersey Mike’s Subs, 11321-A Nuckols Road – The following violation was reported during a routine inspection: quat sanitizer solution in wiping cloth bucket is less than 150 ppm.

McDonald's, 8210 Brook Road – The following violations were reported during a routine inspection: observed drink residue in basin of hand sink near ware washing area; hot water at two hand sinks (women’s restroom and men’s restroom) doesn’t reach the required temperature; there is dried food debris in two microwaves at middle end of cookline; there is a gap below back door across from washing machine; observed a few small and large flies around kitchen; plastic dispenser nozzle on frappe/smoothie machine is chipped; ice buildup found on door and near fans of walk-in freezer and on above-counter freezer; observed dried food debris in gaskets on above-counter freezer and inside freezer at the end of the grill; fan guards in walk-in cooler and walk-in freezer are dusty; y-valve is attached at mop sink under continuous pressure without the proper backflow prevention device installed; there is a direct plumbing connection at three-compartment sink.

Subway, 3135 Mechanicsville Turnpike – The following violations were reported during a routine inspection: dried food debris found inside microwave and on reach-in cooler door gaskets; cutting boards are scored and stained; there are dried beverage stains and pest droppings in cabinet interior under soda machine; there is a direct plumbing connection underneath three-compartment sink; ceiling vent over oven is dusty; there are dried food stains on wall behind prep table and wall underneath three-compartment sink; wall behind three-compartment sink is mildewed; ceiling tile in kitchen is stained.
