JUNE 22

Roots Natural Kitchen, 4101 Dominion Boulevard – The following violations were reported during a routine inspection: no paper towels or hand soap are available at hand sink across from the grill; no paper towels are available in first public unisex restroom; recorded 0 ppm of quat sanitizer in three-compartment sink; recorded three surface temperatures below 160 degrees while testing dishwasher with the thermometer plate; spray bottle containing a blue chemical is unlabeled; y-valve attached to mop sink faucet doesn’t have the appropriate backflow prevention device installed.

Poke Sushi Bowl, 11628 West Broad Street – The following violations were reported during a routine inspection: cooking equipment found in hand sink in front of facility; temperature of wontons prepared yesterday in reach-in refrigeration unit is elevated; these wontons are not date marked; boba sitting out at room temperature on a four-hour time policy was not discarded after the allotted time frame; menu doesn’t contain a disclosure asterisk next to Sushi Bowl item with raw tuna, salmon and yellow tail; two spray bottles containing chemicals are unlabeled; raw fish is thawing in two-door refrigeration unit while still in reduced oxygen packaging; mop isn’t hung or inverted to air dry.

The Melting Pot, 9704 Gayton Road – The following violations were reported during a routine inspection: did not observe employee health policy or written procedures for cleaning up vomit and diarrhea; there are no paper towels at the bar; black buildup found in ice machine; observed dried lemon debris on three lemon juicers stored with clean utensils; a spray bottle containing a chemical is unlabeled; rack in reach-in freezer is heavily rusted; plastic shelves in both walk-in coolers are dirty; y-valve attached to mop sink doesn’t have the proper backflow prevention device installed; tiles are loose and there is a hole in the wall under pre-wash sink at dish machine; observed low grout throughout kitchen.

Tazza Kitchen, 3332 Pump Road – The following violations were reported during a routine inspection: recorded a 139 degree final sanitizing temperature from the dishwasher after two cycles with the temperature plate; spray bottle containing a red chemical is unlabeled; front shelf storing entrée plates and take-out containers is covered in a layer of black buildup; long cutting boards on prep units are scored and stained; gasket liner is torn on reach-in freezer at front of the prep line; shared employee bathroom in the kitchen doesn’t have a covered trash can; dumpster area is dirty and surrounded by loose trash and debris; mop isn’t hung or inverted to air dry.

Anant Cuisine of India, 4348 Pouncey Tract Road – The following violations were reported during a routine inspection: raw minced meat is stored over a gallon of ready-to-drink milk in first prep unit on the line; temperature of hot tomato soup cooked yesterday in walk-in cooler is elevated because of improper cooling methods (stored in a large, deep container); this container of hot tomato soup isn’t date marked; container of flour stored underneath front prep table is unlabeled; box of tilapia fillets is stored directly on walk-in freezer floor; long cutting boards on prep units are stained and scored; gasket liners are torn on both doors of front prep unit and on walk-in cooler door; wire racks in walk-in cooler and drying racks next to dishwasher are peeling and rusty; dumpster area is dirty and surrounded by loose trash and debris.

Sake & Sushi, 11391-A Nuckols Road – The following violations were reported during a routine inspection: owner could not provide documentation that verifies employees have been trained on illness reporting responsibilities; parasite destruction records from fish supplier is unavailable; several bags of tilapia and tuna are thawing while still in vacuum packaging in walk-in refrigerator and sushi refrigerator; bulk sugar bin is unlabeled; handle of ice scoop is touching ice inside ice bin; gasket of sushi refrigerator unit is soiled and badly torn; soiled foil is covering shelf and pipes at cookline; grease is dripping from hood filters.

Apollo’s Flame Baked Pizza & Grill, 9410-B West Broad Street – The following violations were reported during a routine inspection: temperature of carton of heavy cream in cooler next to grill is elevated; one spray bottle of grill cleaner is unlabeled; gasket in pizza cooler is torn.


JUNE 25

Red Robin Gourmet Burgers and Brews, 10067 Brook Road – The following violations were reported during a routine inspection: hot water at bar hand sink doesn’t reach the required temperature; food debris buildup found in three microwaves on cookline; two buckets and one spray bottle of sanitizer are stored beside customer utensils at bar and service station; many bags of sugar are unlabeled; there is ice buildup on walk-in cooler door making it unable to seal properly; observed small amount of dust on racks by food prep sink, walk-in cooler fan guards and on top shelves on cookline; observed buildup of food debris on exteriors and interiors of cooling equipment and dishwashers in kitchen and at the bar; there is a leak at the mop sink; there is a buildup of food debris on floors and wall under dish machine in kitchen and on walk-in cooler floor; mildewy substance found on mop sink basin; there is standing water by the mop sink/ice machine likely caused by ice machine pipe not draining into the floor drain.

Little Caesars, 1547 North Parham Road – The following violations were reported during a follow-up inspection: person in charge couldn’t provide food protection manager certificate; several items are dirty throughout kitchen (gaskets of hot holding cabinets, speed racks and shelves under prep table across from hot holding cabinets); light shield above make table is falling; there are several cracked floor tiles throughout kitchen; observed multiple cracked and missing coving tiles in front area.


JUNE 26

Taste of Brazil, 9722 Gayton Road – The following violations were reported during a routine inspection: raw bacon is stored over parsley and cilantro; temperatures of two containers of chicken and one container of beef cooked yesterday are elevated because of improper cooling methods (stored in deep, covered containers); temperatures of lettuce and tomato sauce in reach-in cooler are elevated because they were left outside of refrigeration too long; mingau de milho in display case and beef in reach-in cooler are past their expiration dates; glass cleaner is unlabeled; several chemicals are stored above food in utility storage room; various packaged breads are not labeled with all the required information; paintbrush with metal band is in condensed milk can (not made for food use); facility doesn’t have quat sanitizer test strips; y-connector on mop sink doesn’t have the proper backflow prevention device installed.

Starbucks Coffee Company, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: there is no hand soap at hand sink next to dish machine or at drive-thru hand sink; hand sink next to dish machine doesn’t have paper towels; can of iced energy drink is dented along top and bottom seam; temperatures of almond milk and strawberry mix in back right lowboy and milk in back left lowboy are elevated; single-service cups are stored past the protection line; gasket is torn on left, two-door reach-in freezer; walls are pitted throughout kitchen and back room; air vents are dusty above three-compartment sink.


JUNE 29

Starbucks Coffee Company, 11716 West Broad Street – The following violations were reported during a routine inspection: gasket liner is torn on last standing reach-in freezer unit in back of facility; there are two broken cabinets underneath the coffee machines behind the service counter; dumpster area is dirty and surrounded by loose trash and debris.

Wingstop, 11645 West Broad Street – The following violation was reported during a routine inspection: large can containing a white, powdery chemical is unlabeled.

China Wok, 11530-C West Broad Street – The following violations were reported during a routine inspection: temperatures of sprouts, shredded cabbage and tofu in second prep unit on the line are elevated again; spray bottle containing a chemical cleaner is unlabeled; gasket liner is still torn on right-side door of three-door prep unit; walk-in cooler vent is still dirty; spray nozzle attached to mop sink hose still doesn’t have the appropriate backflow prevention device installed.

Yummy Yummy Bowl, 7518 West Broad Street – The following violations were reported during a routine inspection: there are missing employee health signatures on Form 1B; two cans of soy sauce are heavily dented on seams; facility couldn’t provide parasite destruction letters from suppliers of saba, red snapper, salmon and yellow tail served at sushi bar; temperatures of bean sprouts, sausage, imitation crab and raw chicken at expo line are elevated because containers are overfilled; chemical sanitizer concentration in three-compartment sink is too high; rice scoops are stored in room temperature water; freezer isn’t maintaining the proper temperature (a work order is placed and no food is currently stored inside); food is stored in non-food grade plastic to-go bags.

Chili’s Grill & Bar, 9950 Brook Road – The following violations were reported during a routine inspection: pitcher found in hand sink at the bar; there is no handwash signage posted at bar hand sink; observed dried food debris in several microwaves on cookline and in server area; chlorine concentration in dish machine is too low; dishes are stacked while still wet; left lowboy cooler at bar has a marginal ambient temperature (no TCS foods are stored there); gasket is torn on bottom door of reach-in freezer; the interior top surfaces are burned/damaged in all microwaves; observed mildewy substance on surface behind nozzles of drink dispensing station at front of kitchen; there is a leak at hand sink across from chicken breading station; dumpster lid is dented, which is causing it to not seal properly; there is food debris on floors in walk-in cooler and walk-in freezer, under cookline and under sinks; observed food debris and grime on walls under and around sinks; ceiling vents are dusty; food debris found in mop sink.

Chanello’s Pizza, 5239 South Laburnum Avenue – The following violations were reported during a routine inspection: hand sink in employee restroom doesn’t have paper towels; temperature of wings made yesterday in plastic bags is elevated; metal container of a white substance is unlabeled; plastic bins are heavily scratched and pitted causing them to no longer be smooth; gasket is torn on walk-in door; pizza prep top has a broken door; walk-in temperature is slightly elevated; facility doesn’t have sanitizer test strips for three-compartment sink; observed dust and debris on racks, fans and on equipment around pizza oven.
