JUNE 12

Pop’s Dogs & Ma’s Burgers, 7301 Brook Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager (person in charge states they are currently taking a class); did not observe employee health policy or written procedures for cleaning up vomit and diarrhea; eggs are stored above ready-to-eat items in bottom of prep unit; oven in disrepair/not in use should be removed from the facility; small fridge is in disrepair; observed buildup of food debris and grime on cooking equipment, cooling units, shelving, trash cans, table under grill, cash register, phone, counter on the kitchen side, floors at perimeters and walls around equipment; fan is dusty; there are leaks at faucet and under three-compartment sink; there is a direct plumbing connection at three-compartment sink; women’s restroom doesn’t have a covered trash can; floor in back storage room is raw wooden particle board; mop isn’t hung or inverted to air dry.

Pier 88, 4420 South Laburnum Avenue – The following violations were reported during a routine inspection: temperatures of fish, shrimp and chicken in prep unit and marinara in front prep unit are elevated; chemical spray bottle at bar and service area is unlabeled; rice scoop is stored in water.


JUNE 15

Papa John’s Pizza, 8903 Patterson Avenue – The following violations were reported during a routine inspection: did not observe written procedures for cleaning up vomit and diarrhea; quat sanitizer concentration in three-compartment sink is too low (bottle is empty); temperature of cheese sticks is elevated because they were left out on the counter; cleaner spray bottle and hand sanitizer are stored next to single-service cups; there is no sanitizer on site; y-connector attached to mop sink doesn’t have the proper backflow prevention device installed.

Firehouse Subs, 4028-K Cox Road – The following violations were reported during a routine inspection: no paper towels or hand soap are available at hand sink next to lowboy freezer; four bags of ready-to-eat food items are improperly thawing out on prep table at room temperature; gasket liner is torn on middle door of three-door prep unit; vents in standing reach-in refrigerator are dusty; y-valve attached to mop sink faucet doesn’t have the appropriate backflow prevention device installed; there is a direct plumbing connection at three-compartment sink; dumpster area is dirty and surrounded by loose trash and debris; ceiling vents above steamers and fryer are surrounded by heavy dust buildup.

Firehouse Subs, 9069 Staples Mill Road – The following violation was reported during a routine inspection: observed dried food debris inside bottom of cold units and gaskets.

Panera Bread, 9080 Staples Mill Road – The following violations were reported during a routine inspection: observed improper cooling methods for grilled chicken (covered in prep drawer); several cold units aren’t working properly; dried food debris found on wire racks, shelves and exterior/interior of cold units; observed dried food debris and grime on floors.


JUNE 16

Gino’s Ristorante Pizzeria, 10118 Brook Road – The following violations were reported during a routine inspection: did not observe an employee health policy; temperature of large container of cut lettuce is elevated and has exceeded the four-hour cooling time; temperatures of ranch (made in-house with buttermilk) in sliding glass door cooler and cheese slices in prep unit are elevated; ambient temperature of sliding glass door cooler is elevated even after person in charge lowered the thermostat; bucket of chlorine water is stored on top of bottled drinks; wooden table and shelving in kitchen and dish area need painting or resealing; there is missing coving tile under front hand sink and three-compartment sink; wall damage found under three-compartment sink; observed soiled and broken ceiling tiles in mop sink area; buildup of food debris found on perimeter of kitchen and floor under stationary equipment; two ceiling vents are dusty.

Poke Sushi Bowl, 6992 Forest Avenue – The following violations were reported during a routine inspection: working restroom doesn’t have hand soap; quat sanitizer concentration in three-compartment sink is too high; white cutting boards are scratched and stained; observed water damage on ceiling tile directly above single-service container storage.

Marco’s Pizza, 7000 Forest Avenue – The following violations were reported during a routine inspection: employee handling food is wearing a bracelet; two light bulbs are burned out in vent hood.

McDonald's, 5106 Nine Mile Road – The following violations were reported during a routine inspection: employee’s bare hands were in contact with fries when they filled a carton of fries for an order; pink and black residue found on ice deflector inside ice machine; temperature of milk is elevated at drive-thru window’s sliding glass door refrigerator, in refrigerated drawer, and coming out of dispenser at beverage station; these three units are not maintaining foods at the proper temperature and shouldn’t be used until repaired; unit holding lemonade may be running high as temperature of lemonade is slightly elevated.


JUNE 17

Revel, 6223 Lakeside Avenue – The following violations were reported during a follow-up inspection: employee drink without lid found on rack by hand sink; dishes are stored in hand sink at the bar; raw shell eggs are stored above lettuce in walk-in cooler; house-made pickles and pork belly are past their expiration dates; chest freezer needs defrosting; freezer to left of the cookline has a torn gasket; there are no quat sanitizer test strips; raw meat juice is leaking on the floor of walk-in cooler from a pan of meat hanging off bottom shelf; ceiling and vents are dusty; fryer that had previously been removed is back and not under the hood.

Balkan Restaurant, 8905 Patterson Avenue – The following violation was reported during a routine inspection: employee placed raw meats on grill with gloves, rinsed gloves, and then grabbed outside of customer bowls before sending them out.

Waffle House, 11900 Church Road – The following violations were reported during a routine inspection: chemical bottle and wiping cloths found in front hand sink; there is dried egg residue on omelet mixer on cookline; temperatures of chili and gravy in steam table are too low; temperatures of diced ham, chicken sausage and tomato in prep unit are elevated; shell eggs were not discarded at the end of four hours; several employees aren’t wearing hair restraints; butcher block cutting board on cookline is heavily scratched and stained; observed dried food debris and grease buildup on surfaces of shelves, under grills and inside reach-in coolers; observed food debris and grime buildup on floors under dishwash area and inside walk-in cooler and inside floor drains.


JUNE 18

JJ’s Grille, 10298 Staples Mill Road – The following violations were reported during a routine inspection: dried food debris found on blade of can opener that hasn’t been used today; temperatures of oysters, chicken and cod in prep drawers are elevated (likely because a gasket is loose); sanitizer concentrations are too high at bar three-compartment sink and in sanibucket; several chemical spray bottles are stored on hand sink; hood filters are heavily bent with gaps showing; observed dried food debris on cold unit gaskets; grime found on microwave; unit vents are dusty.

Best Wings & Tacos, 5956 Brook Road – The following violations were reported during a follow-up inspection: hot water at hand sink doesn’t reach the required temperature; temperature of French fries in fry basket is too low; temperature of milk for battering chicken stored in ice on the counter is elevated (ice is only touching the bottom of the milk container); two large bins of chicken are not date marked; sanitizer bottle is stored above rice and next to spices; there is missing coving tile in dish area; there is a hole in the wall under sink in the kitchen’s restroom.
