JUNE 4
Hardee’s, 6302 Lakeside Avenue – The following violations were reported during a routine inspection: front hand sink doesn’t have paper towels or a handwash sign posted; dried food debris found in two microwaves; there is greenish grime on ice dispenser in public-use drink machine; temperatures of creamer and milk in lowboy at front are elevated (door was ajar for unknown time and seal from gaskets isn’t very tight); there is a leak at drink machine in drive-thru area; observed food debris buildup and grime on sides, exterior and interior of all cooking equipment and cooling units/walk-ins; fan guards in walk-ins and cords at front are dusty; grime found on gaskets of all cooling equipment; y-connector attached to mop sink with spray nozzle under continuous pressure doesn’t have the proper backflow prevention device installed; observed food debris buildup and grime on floors under all equipment and racks, on walls around equipment, and at perimeter throughout kitchen; ceiling tiles and vents are soiled and dusty; observed food debris buildup and mildew at mop sink area.
The Beach House, 4040-B Cox Road – The following violations were reported during a routine inspection: open soda can found on first prep unit on the line; there is black debris buildup along interior of soda nozzle on far side of main bar; spray bottle containing a blue chemical is unlabeled; top prep shelf storing take-out containers is covered in dark buildup; gasket liner is torn on reach-in refrigeration unit storing sauces; dumpster area is dirty and surrounded by loose trash and debris.

Cava Grill, 11780 West Broad Street – The following violation was reported during a routine inspection: y-valve attached to mop sink faucet doesn’t have the proper backflow prevention device installed.
Starbucks inside Kroger, 11895 West Broad Street – The following violations were reported during a routine inspection: food protection manager certificate was unavailable; gasket liner is torn on lowboy refrigeration unit behind the register.
Peking Restaurant, 8904-F West Broad Street – The following violations were reported during a routine inspection: employee washed hands in prep sink instead of hand sink; bar hand sink doesn’t have paper towels; mildew found on sides and shields of ice machine; temperatures of cabbage, egg and garlic and oil are elevated because they are sitting out on the counter; there are no date marks on cut melon, dumplings, egg rolls, noodles, chicken, pork and shrimp; multiple spray bottles are unlabeled; most containers are scratched and discolored; observed food debris in refrigerator/freezer units and on gaskets and door handles; fans are dusty; dried food splatter found on non-food contact surfaces of mixing bowl; storage racks and shelving are soiled; y-connector attached to sink hose doesn’t have the proper backflow prevention device installed; observed damage to walls; ceiling tiles are stained; observed missing and cracked caulking at hand sink; floors and walls around cookline, prep areas and dish machine are in need of cleaning; floor is no longer smooth and easily cleanable.

Turning Point, 5320 Wyndham Forest Drive – The following violations were reported during a routine inspection: temperature of “wet batter” made with milk stored on ice is elevated; staff member is preparing drinks while wearing multiple bracelets; stickers were left on clean containers; a spray nozzle was left on hose at mop sink with a y-connection.
Wendy’s, 9116 West Broad Street – The following violations were reported during a routine inspection: temperatures of chicken nuggets in warmer and cheese sauce at grill are too low; temperatures of cubed chicken and butter in small cooler are elevated; two chemical spray bottles are stored above open boxes of gloves next to hand sink; two employees prepping food are wearing bracelets; employee at drive-thru is scooping ice without hair tied back effectively (two long braids hanging below waist).
Little Caesars, 6040 Brook Road – The following violations were reported during a routine inspection: did not observe employee health policy; hot water at front hand sink doesn’t reach the required temperature; temperature of sausage in prep unit is elevated; multiple containers of white powdery substances are unlabeled; food debris buildup found on racks throughout kitchen and walk-in cooler, bottom shelves and wheels of make tables, flour racks and inside reach-in freezer; there is a leak at food prep sink; observed food debris buildup on floors under stationary equipment.
Domino’s Pizza, 5526 Lakeside Avenue – The following violations were reported during a routine inspection: temperatures of packaged stewed tomatoes and slices of cheese in reach-in cooler are elevated; reach-in cooler doesn’t have an ambient thermometer; gasket is torn on reach-in cooler; observed buildup of dried food debris on oven, exterior of make table and computer/safe at front; there is a leak at three-compartment sink; food debris splash found on ceiling tiles; ceiling vents are dusty.
Sheetz, 550 South Airport Drive – The following violation was reported during a routine inspection: temperatures of almond milk and milk are elevated because they were left out on the counter.

Pho 1 Grill, 201 Towne Center West Boulevard – The following violations were reported during a routine inspection: hand sinks in bar and next to cookline are blocked and inaccessible; hand sink next to three-compartment sink is only accessible by turning the valve on at the bottom of the basin; there is no date mark on container of cooked chicken in walk-in cooler; spray bottle containing a reddish cleaning chemical is unlabeled; scoop utensils are stored in standing water; knives are stored in crevices of prep units; long cutting boards on prep units are scored and stained; wire racks in walk-in cooler are peeling and rusty; gasket liner is torn on walk-in freezer door; five out of six fryer baskets have broken off metal pieces; unisex bathroom in kitchen doesn’t have a covered trash can.
Plaza Azteca Mexican Restaurant, 10099 Brook Road – The following violations were reported during a routine inspection: straws found in bar hand sink and a spoon and food debris were found in front kitchen hand sink; food debris found on meat slicer and can opener blade; temperature of pico de gallo in two prep units is elevated and it has exceeded the four-hour cooling time; temperatures of four large pans of black beans, lettuce, cheese, noodles and tomatoes in various refrigeration units are elevated; ice scoop handles are touching ice at bar and in the kitchen; seal is missing from walk-in cooler bottom edge; walk-in cooler has a large gap between door and floor where cold air can escape; lid of salsa cooler is in disrepair and is unable to close properly; two cutting boards on cookline and one cutting board at bar have deep scratches; spray nozzle attached to mop sink without the proper backflow prevention device installed; there are leaks at bar hand sink and hand sink by cookline.
Captain D’s Seafood, 4800 Williamsburg Road – The following violations were reported during a routine inspection: floors and walls are in need of cleaning along cookline, in dish area and around ice machine; there is low grouting; observed cracked or missing floor and wall tiles; drain grates are rusty; there are holes near drive-thru; several items need cleaning (outside and inside of equipment along cookline, especially around fryer, as well as racks throughout and fryer baskets); multiple containers and pitchers have holes, cracks and a worn surface; there are torn gaskets on units along cookline; fryer baskets in-use are broken; racks/legs of equipment are rusty; temperatures of multiple trays of fish by walk-in door and hush puppy mix on the line are elevated.

America’s Best Wings, 4730 Finlay Street – The following violations were reported during a routine inspection: hand sink near prep tables doesn’t have hand soap; raw burger is stored with vegetables and over cooked pasta; raw cabbage is stored with raw chicken; observed dirty and dead bugs in container over three-compartment sink; there is debris on scoop and other utensils over three-compartment sink; temperatures of cooked chicken and raw chicken in make table are elevated; containers are stacked without lids so the bottoms are in direct contact with the food below; prep shelves are lined with soiled cardboard; multiple containers are cracked; small pitchers have duct tape on them; gasket is torn on make table; racks are rusted; fryer baskets are in disrepair; there are no test strips for sanitizer; observed grease and other debris buildup along fryers, racks, and on shelves of prep tables; women’s restroom doesn’t have a covered trash can; observed grease on floor and wall of cookline; there is dirt and debris on mop room floor.

Burger King, 5217 Williamsburg Road – The following violations were reported during a routine inspection: hand sink by ice machine doesn’t have paper towels; there is 0 ppm of sanitizer at three-compartment sink; there are no time stickers on lettuce or tomatoes; observed buildup of grease on fryers along cookline; water and debris found in units; observed grease, debris and standing water under equipment and sinks.
Rapid Pizza, 1592 Mountain Road – The following violations were reported during a routine inspection: food protection manager certification is expired; there is no handwash signage posted at back hand sink; cleaning supplies are stored in front hand sink; there is dried flour debris on plastic food containers under pizza make table; gaskets are torn on front freezer; handle is broken on back glass reach-in cooler; outside digital thermometer on walk-in cooler isn’t working; observed dried food debris on wire racks above dishes and flour bins, the table the flour bins sit on, equipment on the rack by the back door, bottom shelves of tables in dish area, cooling unit gaskets, floor of walk-in, and exteriors/interiors of cooling units; there is a direct plumbing connection at three-compartment sink; observed garbage on the ground around dumpster; lids were left open on dumpster and grease barrel; buildup of food debris and grime found on perimeter edges; ceiling vent above back glass reach-in cooler is dusty.
