Sebastian & Co. Bistro, 1308 Gaskins Road – The following violations were reported during a routine inspection: person in charge isn’t ensuring that employees are following proper procedures as related to handwashing, stocking hand sinks, sanitizing and controlling cold holding temperatures within the kitchen; facility couldn’t provide employee health policy or written procedures for cleaning vomit and diarrhea; employee placed raw chicken on the grill and then changed gloves without washing hands first; bar hand sink doesn’t have hand soap; multiple hand sinks don’t have paper towels; ice has been dumped in hand sink at bar; there is 0 ppm of chlorine in dish machine; employee washed utensils and put away without sanitizing; observed buildup inside both soda gun nozzles; temperatures of several foods are elevated (gallon milk in mini fridge, cheddar cheese in right prep cooler, and pan of Caesar dressing and blue cheese crumbles in left prep cooler); chemical spray bottle above mini fridge is unlabeled; one chemical spray bottle and one can of bug spray is stored above mini fridge; rubbing alcohol is stored next to canned drinks and syrups; coloring cutting boards are scratched; rubber spatulas are chipped and burned; gaskets are torn in one-door prep cooler and left two-door prep cooler; spray hose at dish machine is submerged within sink basin; there is a leak from walk-in cooler ceiling that is draining into a bucket on the floor; there is a leak at mop sink near restroom; staff restroom doesn’t have a covered trash can; observed heavy dust buildup on walk-in cooler ceiling vent; wall behind stove is dirty; hood is greasy; floors are worn throughout kitchen.
Hangry Joe’s Hot Chicken, 1501 North Parham Road – The following violations were reported during a routine inspection: observed numerous handwashing violations (EHS was forced to stop service due to handwashing violations and lack of hand soap in kitchen; person in charge repeatedly ignored EHS instruction to stop service and retrieve soap but eventually left to retrieve it after the owner ordered them to via phone); person in charge states there is no certified food protection manager on staff; person in charge couldn’t provide verifiable employee health policy or written procedures for cleaning up vomit and diarrhea; employee drink without lid found next to toaster and above open spice bins; there is brown buildup in soda nozzle; observed green mildew buildup in ice dispenser of soda machine; blender cups are severely scratched with brown mildew buildup; temperature of waffle fries under hot holding lamp is too low; temperature of raw chicken wings in make table is elevated because they were stacked above fill line by about two inches; portioned mac and cheese and mashed potatoes are not date marked; several chemicals are not stored separate from foods and food contact surfaces; observed degreaser spilled on prep table; observed personal belongings stored on and next to foods and food contact surfaces; spice containers are open without any lid to protect them from contamination; food throughout kitchen is stored directly on the floor (garlic sauce, flour, sauces, etc.); rice paddle and ice cream scoop are stored in room temperature water; single-service container and bowl are being used as scoops in dry storage and sauce; various to-go containers are stored directly on floor throughout facility; a dirty ladder is touching exposed to-go containers; cutting board at kitchen make table is scratched and stained; mop sink hose is laying in bottom of mop sink; there is a direct plumbing connection at three-compartment sink; mops are not hung or inverted to air dry; observed dust accumulation on air vent and ceiling above make table; food splash found on wall behind lowboy freezer; facility doesn’t have a no smoking sign.

YaYa’s Cook Book, 11674 West Broad Street – The following violations were reported during a routine inspection: employee entered kitchen from outside facility and put on gloves without first washing hands; hand sink in kitchen doesn’t have hand soap; dishwasher sanitizer concentration is 0 ppm; there is debris buildup along the interior of soda nozzle at bar; temperatures of noodles on cookline and raw salmon in sushi cooler are elevated; large container of bagged noodles in walk-in cooler is not date marked; spray bottle of Windex isn’t labeled; raw tuna fillets are thawing while still in reduced oxygen packaging; box of noodles is stored directly on walk-in freezer floor; three knives are stored in crevices of both prep units in the kitchen; gasket liners are torn on all of the prep unit doors in kitchen; spray nozzle attached to mop sink hose doesn’t have the appropriate backflow prevention device installed; ceiling vent above prep line is surrounded by dust.
Kabuto Japanese House of Steaks, 8052 West Broad Street – The following violations were reported during a routine inspection: did not observe certified food protection manager verification, employee health policy or written procedures for cleaning up vomit and diarrhea; temperatures of tempura batter, rice, large yum yum sauce container and shell eggs are elevated; did not observe an ambient thermometer in glass reach-in cooler in server area; there are torn gaskets on server glass fridge; one cooler in bar area has excessive water pooling in bottom; one cooler in bar area has an elevated ambient temperature; there is food debris buildup on walk-in cooler shelf liners and racks; several items are dusty (walk-in cooler fan guards, ice machine vent and fan in the bar area); there is a leak at mop sink; hand sink at bar is slow to drain; hose at mop sink is resting below flood rim; observed buildup of food debris on walk-in cooler and freezer floors, walls in bar area, floors under sinks in bar area, and the perimeter of the kitchen; there is wall damage behind washer/dryer in dry storage; floors are damaged in dry storage; wall by mop sink needs resealing; there is a gap in cinderblock wall exterior of walk-in freezer; observed low grout in dish area.
Ballacino’s Pizza and Grinders, 8026-D West Broad Street – The following violations were reported during a routine inspection: observed dark buildup on ice plate in ice machine and on can opener blade; temperatures of cabbage and sliced tomatoes in prep unit are elevated; observed dried food debris on wire racks, roller carts, speed racks and walk-in cooler vents; there is a leak at prep sink; walk-in cooler floor is in disrepair; tiles under three-compartment sink are cracked or missing; hood lights are out.

Tropical Smoothie Café, 9972 Brook Road – The following violations were reported during a routine inspection: dried food debris found on can opener blade; there is sticker residue on clean plastic containers; observed food debris on clean scoops; feta and mozzarella opened yesterday aren’t date marked; handheld juicer and can opener are rusted; there is a buildup of food debris on walk-in cooler gasket.
Sushi Kingdom, 4300 Pouncey Tract Road – The following violations were reported during a routine inspection: neither hand sink in the kitchen has paper towels; two spray bottles of Windex are unlabeled; two large buckets of soy sauce are stored directly on walk-in freezer floor; cleaver stored in crevice of second prep unit on the line; long cutting boards on prep units are stained and scored; gasket liner is torn on right-side door of lowboy freezer and on freezer chest in kitchen.

Zero’s, 3073 Lauderdale Drive – The following violations were reported during a routine inspection: no hot water is available at the only hand sink in the kitchen; gasket liners are torn on three units; there a direct plumbing connection underneath three-compartment sink.
Papa John’s Pizza, 3067 Lauderdale Drive – The following violations were reported during a routine inspection: scrubber found in hand sink; string cheese is sitting out on the counter at room temperature without a time stamp indicating its discard time; gasket liners are torn on two doors of four-door prep unit on the line; there a direct plumbing connection underneath three-compartment sink; two mops are not hung or inverted to air dry.
Sheetz, 7500 Staples Mill Road – The following violation was reported during a routine inspection: area where soda lines drain is pitted.
El Paraiso Restaurante, 4034 Crockett Street – The following violations were reported during a routine inspection: raw shell eggs are stored above sliced tomatoes in reach-in cooler; dried food debris found on can opener blade that hasn’t been used today; temperatures of hot dog, tomato, lettuce and pastor in prep unit are elevated; prep unit isn’t holding foods at the proper temperature; observed lots of dried food and spills inside reach-in freezers; wire racks are dusty and rusty.
Me’kong Restaurant, 6004 West Broad Street – The following violations were reported during a routine inspection: bar hand sink doesn’t have hand soap; there is no handwash sign at beverage station hand sink; pot of raw shrimp is stored directly on kitchen floor; employee is wearing a watch in the kitchen; staff aren’t wearing hair restraints; wire storage racks in walk-in cooler are rusty; deep fryer baskets are in disrepair; cutting boards are scored and stained; facility doesn’t have sanitizer test strips; observed dust and food debris on wire storage racks; several items are dirty (reach-in cooler door gaskets throughout kitchen, walk-in cooler fan guards, bar area beer cooler door gaskets and interior, and three-door beer cooler door gaskets and door frame); there is grease buildup on sides of hot line cooking equipment; outdoor dumpster is missing a tight-fitting lid (appears to have broken off); outdoor dumpster doors were left open; observed dust on walls and ceiling in kitchen, especially in corners; there is grease residue on wall behind hot line.
Lalo’s Cocina & Tacos Bar & Grill, 8026 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide verification of employee health training or written procedures for cleaning up vomit and diarrhea; raw shell eggs are stored above ready-to-eat garnishments; temperature of top half of rice in steam well is too low because the container is floating above water line; observed inconsistent date marking and old stickers left on new food items; stirring utensils are stored in room temperature water; ice scoop handle is touching ice; fryer baskets are in disrepair; there is residue on exterior of chip buckets; sticker labels are left on clean containers; spray nozzle is attached at mop sink when not in use.
Popeyes Chicken & Biscuits, 8044 West Broad Street – The following violations were reported during a routine inspection: current ServSafe license is expired; temperature of egg batter in well is elevated; employee isn’t wearing a hair restraint; ice bin is flaking and the exterior is rusty; top of hot holding units is dusty; there is grease buildup on exterior of cookline; observed dried food debris on wire racks in walk-in cooler and walk-in freezer.
California Pizza Kitchen, 11800 West Broad Street – The following violations were reported during a routine inspection: soiled utensils found in both hand sinks on the cookline; utensils are stored in a dipper well that is turned off; long cutting board on cookline prep unit is heavily stained and scored; prep unit to the left of the grill is leaking along the inside; wire racks in dry storage room are peeling.
