Habanero Mexican Grill, 9101-A Quioccasin Road – The following violations were reported during a routine inspection: person in charge couldn’t provide a verifiable employee health policy or written procedures for cleaning up vomit and diarrhea; raw steak is stored above cooked rice and shredded lettuce in reach-in cooler; temperature of shredded cheese in Coke cooler is elevated; the ambient temperature in this unit is elevated too; observed incorrect date mark on cooked lingua in lowboy cooler; temperature of water for utensil storage is too low; there is a direct plumbing connection at three-compartment sink; employee restroom doesn’t have a covered trash can.

Click, 409 North Ridge Road – The following violation was reported during a routine inspection: there is no date mark on cooked shrimp in make table lowboy.

Honest Indian Vegetarian Restaurant, 8900-A West Broad Street – The following violations were reported during a routine inspection: there is 0 ppm of sanitizer in dish machine; temperature of rice in warmer is too low; there are no date labels on curry, tofu, cooked cauliflower, rice and chickpeas; can of Raid found on cookline; cooked potatoes in a strainer found in mop sink; ice cream scoop is stored in room temperature water; women’s restroom doesn’t have a covered trash can.

Ay Mama, 9809 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; employee washed hands without soap at three-compartment sink; employee placed raw chicken on grill with bare hands and then put on gloves without washing hands first; heavy brown buildup found on rice scoop stored in bulk rice bin; temperature of milk in bar fridge is elevated; temperatures of several small plates of salsa in lobby are elevated; colored cutting boards are scratched; prep cooler gaskets are torn; fryer basket wires are bent; trash is accumulating outside of the dumpster.

Christian’s Pizza, 7003-A Three Chopt Road – The following violations were reported during a routine inspection: brown mildew buildup found on ice guard in ice machine; temperature of pizza sauce in walk-in cooler is elevated in the middle and on the sides; root beer syrup box is stored on chemical shelf below bathroom cleaner bottle; chemical spray bottles are stored on table next to serving trays; there are exposed gaps in back door in dining area; bottom of back door in kitchen is damaged; dead German cockroach found on floor in back room; boxes of fryer oil are stored directly on floor in back room; plastic spatulas have chipped edges; wall fan near ovens and ceiling vents throughout kitchen are dusty; observed food debris on rack in walk-in cooler; there is grease buildup on sides of fryers and grill; black grime buildup found on seal of reach-in freezer door; there is a leak at three-compartment sink faucet; doors of outdoor dumpsters were left open; there is dust on vent hood above fryers; observed grease on the floor below fryer and grill; two mops are not hung or inverted to air dry.

Lehja Restaurant, 11800 West Broad Street – The following violations were reported during a routine inspection: employee restroom in kitchen doesn’t have paper towels; spray bottle containing a red-ish colored chemical is unlabeled; top prep shelf storing takeout containers is covered in a layer of dark debris; gasket liner is torn on walk-in cooler door; wire racks in walk-in cooler are heavily rusted; long cutting board on prep unit is stained and scored; y-valve attached to mop sink faucet doesn’t have the appropriate backflow prevention device installed; there is a direct plumbing connection at three-compartment sink; employee restroom in kitchen doesn’t have a covered trash can.

Mariscos El Barco, 6409 Rigsby Road – The following violations were reported during a routine inspection: knife found in hand sink; temperature of top portion of sliced tomatoes in make table is elevated because tomatoes are filled above fill line; menu doesn’t contain an asterisk next to ceviche linking it to the consumer advisory statement; there is a torn gasket on lowboy of make table; there is a direct plumbing connection at three-compartment sink; observed low grout on floor on cookline.

Crafty Crab, 4995 Nine Mile Road – The following violations were reported during a routine inspection: hand sink near deep fryers doesn’t have paper towels; there are no handwash signs posted at hand sinks in bar area; observed small amounts of mold in ice machine; all sources of sanitizer at dish soap dispenser, automatic dispenser and dish machine need to be serviced; chlorine sanitizer level in sanibucket at bar is too high; there is food stored directly on floors in walk-in cooler, walk-in freezer and kitchen; knife is stored in gap between prep table and reach-in cooler; cutting boards are scored and stained; wire storage racks throughout kitchen are rusty; there are torn cooler door gaskets at bar reach-in cooler; interior of microwave is burnt and the plastic is bubbling up; reach-in coolers at bar area are in disrepair; several items/areas are dirty (speed rack used for cans, exteriors of bulk containers and walk-in cooler door gaskets); shelves above expo line are dusty; hood filters are greasy; shelf under bar area prep table is covered in sticky beverage residue; fan guards in walk-in cooler and bar area cooler are moldy; women’s restroom doesn’t have a covered trash can; there are dried food stains on walls throughout the kitchen; walk-in freezer floor is very dirty; observed black grime on floors in bar area underneath equipment.

The Lilly Pad, 9680 Osborne Turnpike – The following violations were reported during a routine inspection: hand sink doesn’t have paper towels; mildew found in soda nozzles; quat sanitizer level is too high in sani bucket; fish is thawing while still in reduced oxygen packaging in walk-in; utensils are stored in water on prep top; spray nozzle at three-compartment sink is hanging below flood line; women’s restroom doesn’t have a covered trash can.

Longhorn Steakhouse, 11311-A West Broad Street – The following violations were reported during a routine inspection: soda nozzles in the bar have dark debris along the interior; gasket liner is torn on salad reach-in refrigeration unit on prep line; one of out six fryer baskets has broken off metal pieces.

Corner Bakery Café, 11000 West Broad Street – The following violations were reported during a routine inspection: gasket liner is torn on walk-in cooler door; ceiling vents above cookline are surrounded by dust.

Zara Grill, 10865 West Broad Street – The following violations were reported during a routine inspection: temperature of cut tomatoes in mini refrigeration unit in front of facility is elevated; yogurt sauce prepared yesterday in three-door standing refrigeration unit has no preparation date marked; commercial spray bottle containing oven cleaner is unlabeled; facility doesn’t have a food thermometer; gasket liner is torn on standing reach-in refrigeration unit in front of facility; one of two fryer baskets has broken off metal pieces; racks inside three-door reach-in refrigeration unit are peeling and rusting.

Cafe Y Sabor Bakery and Restaurant, 7807 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy; dried food debris buildup found in microwave; temperatures of pastry mix with cheese and queso fresco in right reach-in cooler are elevated; temperature of cream cheese in left reach-in cooler is elevated; ambient temperature of this unit is too high, even after turning thermostat down; menu has the reminder but no disclosure regarding sunny side up eggs and cooked-to-order steaks; there is grease buildup on hood vents.

Boychik’s Deli, 4024-B Cox Road – The following violations were reported during a routine inspection: soap dispenser is attached to the wall above three-compartment sink; can of commercial Hot Spot pesticide found stored underneath three-compartment sink; long cutting boards on prep line and cookline are scored and stained; gasket liner is torn on first standing reach-in refrigeration unit on prep line, mini fridge storing milk in front of facility, and on doors of four other units; wire racks in walk-in cooler are peeling and rusty; spray nozzle attached to mop sink hose doesn’t have the appropriate backflow prevention device installed; there is a direct plumbing connection underneath three-compartment sink; ceiling vents throughout kitchen are surrounded by dust.

Chicken Fiesta, 8211 Brook Road – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit and diarrhea; hot water in women’s restroom doesn’t reach the required temperature; employee is washing dishes in front hand wash sink; back hand sink doesn’t have paper towels; raw shell eggs are stored above celery in walk-in cooler; there is a brownish residue on ice plate on ice machine in dish area; dried food debris found on clean knife on wall and on two clean vegetable slicers; temperature of cut plantains in glass reach-in cooler near front is elevated; ambient temperature of this unit is elevated; can of Raid found near mop sink; containers of cornstarch and baking soda are unlabeled; there is a gap under the back door; several items are stored directly on the floor (chicken and sauces in right walk-in cooler, oils in dry storage, and chicken and marinades in left walk-in cooler); cutting boards on the left two prep units have heavy scratches and stains; dry storage racks are rusted; fryer baskets have bent and broken metal pieces; there are torn gaskets on right chest freezer and middle reach-in freezer; observed food debris buildup on floors under and walls around cookline equipment, in dry storage, and in dish area.

New Asian Panda, 10430 Ridgefield Parkway – The following violations were reported during a routine inspection: open employee drink found on counter near soups; there is encrusted debris on containers storing in-use seasonings; temperatures of cooked noodles on counter and cut cabbage on top of prep unit are elevated; prep cutting board is heavily scratched and stained.

Naz’s Halal Food, 11426 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy; hand sink next to three-compartment sink is full of wash rags and sponges; two stacks of take-out containers are stored uninverted; facility couldn’t provide sanitizer test strips; mops are not hung or inverted to air dry.
