Deep Run Roadhouse, 12379 Gayton Road – The following violations were reported during a routine inspection: gaskets are torn on hot box warmer on service line; there are direct plumbing connections at prep and three-compartment sinks; floor throughout kitchen is in disrepair (broken tiles).
APRIL 23
Hai Ky Mi Gia, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: did not observe active managerial oversight regarding hand washing, cold holding of TCS foods, cooling methods, reheating and more; facility doesn’t have a certified food protection manager; employee handled cell phone with gloved hand and then proceeded to prep food without first changing gloves and washing hands; employee handled raw meat with gloved hands and then immediately handle ready-to-eat produce; hand sink in back of kitchen is completely full of clutter rendering it unusable; can of quil eggs has a dent along bottom seam; there is light brown buildup on sides of ice machine near motor; temperature of rice in large rice cooker is too low; temperatures of half-and-half and cream in boba in make table are elevated; temperature of raw sprouts in water is elevated; pho and beef cooked yesterday in walk-in cooler aren’t date marked; various foods are improperly cooling (cooked chicken breast is covered in plastic wrap, and lettuce, cooked beef and cut tomato are prepped and stored above fill line in make table); there are food items stored directly on floor throughout kitchen; boba utensils and noodle baskets are stored in room temperature water; knives are stored in crevice between make table and adjacent table; single-service containers are being used as scoops in various dry storage ingredients; cutting board at make table is scratched and stained; racks throughout facility are rusted; reach-in cooler fan cover is falling down; there are torn gaskets on lowboy and three-door reach-in cooler; person in charge couldn’t provide test strips for three-compartment sink or dish machine; box fans are covered in grease and dust; pipes and wall behind cookline are covered in grease; all equipment on cookline are covered in grease and debris; floor under cookline equipment is covered in grease and food debris; mops are not hung or inverted to air dry; there are cracked/chipped tiles and low grout near three-compartment sink and dish machine; two light bulbs are burned out in vent hood; there is a hole between vent hood and wall into dining area; walls throughout kitchen are dirty; floor drain strainer is full of food debris; observed food debris on walk-in cooler floor under racks.

Dubai Girls Café, 1090 Virginia Center Parkway – The following violations were reported during a routine inspection: back hand sink doesn’t have paper towels; cup and sponge found at front handwash station; there is coffee residue in back handwash station; temperatures of milks and stuffed grape leaves in right display cooler are elevated; this cooler isn’t maintaining foods at the proper temperature and needs to be serviced; two containers of powdered foods are unlabeled; bottled water, oil, canned goods and an extra grill are stored directly on floors.
Chipotle Mexican Grill, 7000 Forest Avenue – The following violations were reported during a routine inspection: temperatures of cooked steak in mobile line and serving line are too low; spray bottle of sanitizer is hanging on water hose support beam at prep table next to prep sink; temperature of metal bins of pico in two-door reach-in cooler is elevated due to improper cooling methods; there is dust accumulation on walk-in cooler fan guards and air vents and the ceiling around dish machine area; two mops aren’t hung or inverted to air dry.
BB.Q Chicken, 7801 West Broad Street – The following violations were reported during a routine inspection: employee drink without lid found on prep table cutting board; employee cleaned but didn’t sanitize bowls; bowls to toss raw and cooked chicken in sauces are not being washed every four hours (employee states they are cleaned at the end of the day); kim chi and rice are on a time control policy but time isn’t being tracked; butane fuel is stored next to cooking oil and sauces; two containers of cooking utensils are stored in room temperature water; observed buildup of food debris on all cooling unit gaskets; observed buildup of grime and food debris on floors under ware washing and stationary equipment (cookline, tables, cooling units).
Ninja Hibachi, 10162 West Broad Street – The following violations were reported during a routine inspection: employee handled raw egg and then touched noodles without first washing hands; raw eggs are stored with noodles and next to bean sprouts; first aid kit and cleaners are stored with food and single-service items; raw shrimp is thawing in a bucket of water; duck and chicken are thawing inside sink; containers of spices, flour, sugar, etc. are unlabeled; containers of food are stored directly on floor inside kitchen and walk-in cooler; to-go containers are not stored inverted; observed heavy grease and grime buildup on exterior of cold holding units and gaskets, carts and shelves, rice cookers, door handles and in between cooking equipment; two lights are out in back hood; there is buildup on hand sink; hood and air vents are dusty; heavy grease found on filters of both hood systems.
Al-Modina Restaurant, 10188 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide an employee health policy or written procedures for cleaning up vomit and diarrhea; person in charge touched washed cilantro with bare hands; there is inconsistent date marking (old stickers left on containers and missing dates); observed grease buildup on hood and suppression system.
Pho Saigon, 10190 West Broad Street – The following violations were reported during a routine inspection: several employee drinks found stored in prep tables; there is dark residue on ice plate in ice maker; meat slicer and knives stored “clean” have dried food debris on them; temperature of cooked noodles in steam well is too low; temperatures of raw shrimp and pork in small Frigidaire refrigerator are elevated; several containers of spices, flour, sugar, etc. are unlabeled; some areas need cleaning (pan rack above prep, shelves, wire racks and plumbing under sinks); observed grime and food debris around perimeter of kitchen and under cookline.
Dunkin Donuts, 10260 West Broad Street – The following violations were reported during a routine inspection: person in charge ate from a snack cart in the middle of the kitchen next to active food prep; paper towel dispenser at front hand sink is broken; spray bottle of cleaner is touching clean containers; employee isn’t wearing a hair restraint; gaskets on walk-in cooler door are in disrepair; cold unit gaskets are dirty; dried food debris found in shelves below prep station; there is a leak at front hand sink and mop sink; observed buildup on floor along perimeter of kitchen and under stationary equipment.
Wendy’s, 2200 East Laburnum Avenue – The following violations were reported during a routine inspection: food stains and food debris found on white plastic panel in ice machine; there is dried food debris inside microwave; flat top grill is in disrepair and not in use; reach-in cooler doors underneath burger grill are in disrepair and not closing properly; large ice block is forming behind walk-in cooler fans; lights are out in hoods over oven; lights are very dim in hoods over deep fryers; walk-in freezer door gaskets are torn; wire storage racks in dry storage are rusty; paper towel dispenser by cash register hand sink is broken; walk-in cooler and freezer door gaskets are dirty and stained black; there is grease residue on sides of deep fryer and fry warmer; top of fry warmer is dirty and dusty; shelf under burger grill is greasy and dirty; reach-in cooler doors and door gaskets throughout kitchen have food debris and stains on them; dried food debris found in warming unit above burger grill drawers; outdoor walk-in freezer door gaskets are dirty and stained black; hand sink in men’s restroom is draining slowly; there is broken floor coving in men’s and women’s restrooms; cracked floor tile found in dry storage area; observed dust and dried food stains on kitchen ceiling tile; there is dust on drive-thru area wall; drain under dining area cabinet is dirty; there are dried food stains on walls and shelves underneath cash registers; observed grease residue dripping from ventilation hoods over burger grill and deep fryers.
Popeyes Chicken & Biscuits, 2318 East Laburnum Avenue – The following violations were reported during a routine inspection: dishes stored as “clean” on rack are still dirty; there is dried food debris inside microwave; observed small amount of pink mold on white plastic panel in ice machine; bulk container of flour is unlabeled; employees are wearing bracelets/jewelry on wrists; deep fryer baskets are in disrepair; door gasket on reach-in cooler near drive-thru is torn; storage racks throughout kitchen are greasy, encrusted with flour and there are dried food stains and debris on them; several items/areas are dirty (sugar and flour bulk bins, legs of storage shelf in walk-in cooler, space behind bun toaster, warmer box by drive-thru and hot water dispenser bases); observed stuck on food debris and crumbs on interior bottom shelf in reach-in freezer; there is grease residue on top of chicken warmer; side of hot box and reach-in freezer are dirty and greasy; there are food stains on drive-thru reach-in cooler door; observed dried food debris and stains on base of lemonade dispenser; crumbs found on front counter service shelves; observed grease buildup on sides of deep fryers; black rolling cart and walk-in cooler door have food debris on them; flour and food stains found on speed racks in walk-in cooler; outdoor dumpster doors were left open; observed stains and grease residue on walls near lemonade dispenser; there are dried food stains on wall behind soda machine; flour and food residue are caked on walls, cables and plugs behind drive-thru register; there are stained ceiling tiles throughout the kitchen; walls behind and underneath three-compartment sink are dirty and mildewed; observed dried food stains on walk-in cooler walls.
Mexico Restaurant, 5213 Williamsburg Road – The following violations were reported during a routine inspection: multiple dented cans found on racks; there is dried food debris on slicer; there is 0 ppm of sanitizer in dish machine; temperatures of salsa in cups and large pitcher are elevated; multiple prepped items (beans, cheese dip and beef) are lacking a date label; wet wiping cloths throughout kitchen aren’t stored in sanitizer between uses; there is a torn gasket on prep table and three-door reach-in; racks in dry storage and walk-in are rusty; lights are out under the hood; observed heavy ice buildup in freezer; floor tiles are cracked; there is low grouting; observed missing coving tiles and trim throughout.

Starbucks Coffee Co., 9002 West Broad Street – The following violations were reported during a routine inspection: hot water at hand sink nearest kitchen entrance doesn’t reach the required temperature; chemical spray bottles are stored over water bottles in dry storage; employee with long hair doesn’t have it effectively tied back.
APRIL 28
Fat Boy’s Bar + Grill, 2 West Williamsburg Road – The following violations were reported during a routine inspection: open drink found on top of prep table; temperature of cooked onions is elevated three hours after being cooked; spray bottles are unlabeled; dishes are stacked while still wet on clean dish rack; observed grease and debris on side of cooking equipment and inside of units; racks in units and in dry storage need cleaning; there is grease buildup on fryer baskets; fan above prep table is dusty; there is debris under equipment and dish area; observed food splatter on walls behind cookline and prep units; ceiling tiles are dirty and warped.
Sparrow’s Pizza, 6313 Rigsby Road – The following violations were reported during a routine inspection: there are no date marks on cut tomatoes, sausage, pepperoni and other TCS foods in make table; did not observe a time stamp on in-use pizza sauce stored at room temperature (person in charge states it is kept on a four-hour time control); bottle of rubbing alcohol and hand sanitizer is stored on a rack above liquid butter; several ceiling tiles are damaged; air returns are dusty; employee restroom doesn’t have a covered trash can; there is a direct plumbing connection at three-compartment sink.
