APRIL 16

Mama Cucina, 4028-O Cox Road – The following violations were reported during a routine inspection: employee health policy wasn’t available; an employee worked with raw pork and then proceeded to cut bread with the same soiled gloves; only hand sink in the kitchen is blocked by a serving tray holding dirty pans; there is black debris along the interior of soda nozzle in the bar; chemical spray bottle is unlabeled; takeout containers are not stored inverted to protect from contamination; gasket liner is torn on two reach-in refrigeration units, prep unit door on the line, and on both doors of salad prep unit; long cutting boards on prep line are heavily scored and stained; no sanitizer test strips are available.


APRIL 17

Burger Haven, 8340 Brook Road – The following violations were reported during a routine inspection: raw turkey and raw beef are stored above sauces and opened canned goods in walk-in cooler; raw beef burgers are stored next to uncooked vegetable burgers in the grill drawer; temperatures of Swiss and American cheeses are elevated because they are out on the counter; multiple employees with long facial hair are not wearing beard guards.

Ben Wongs Tea, 1700 Willow Lawn Drive – The following violations were reported during a routine inspection: person in charge couldn’t produce the employee health policy; there are three dump sinks on the serving line but zero hand sinks (employees have to go through curtain to wash hands); kitchen hand sink doesn’t have paper towels; can with crimped dent found on dry storage shelf; single-service containers are being used as scoops in bins of brown sugar and flour; knives are stored in between drain board and prep table by three-compartment sink; spray hose at three-compartment sink is leaking.

Chick-fil-A at The Shops at Willow Lawn, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: hand sink next to three-compartment sink doesn’t have paper towels; hand sink in back of kitchen is blocked by hanging mops and brooms; boxes of food are stored directly on walk-in cooler floor; single-service cup is being used as a scoop in salad make table in back of kitchen; bins are stacked while still wet on dish drying rack; two-door reach-in cooler is leaking from somewhere in bottom; bottom of two-door reach-in cooler is dirty; mop sink faucet doesn’t have a dual check valve installed; hose attached to mop sink is laying in the bottom of the sink basin; wet mops aren’t hung or inverted to air dry.

The Smoothie Spot, 1366 Gaskins Road – No violations reported during a routine inspection.

Ironclad Coffee Roasters, 2100 John Rolfe Parkway – The following violations were reported during a routine inspection: sanitizer concentration in three-compartment sink is at 0 ppm; temperature of gallon jug of milk in front cooler is elevated because it is frequently taken out of the cooler to make orders; observed heavy buildup of flour and dust on surfaces of wire storage racks, prep shelves, ovens, fan system, walk-in cooler racks, ceiling panels and vents above prep tables, floor under prep tables and walk-in cooler floor; water is pooling on floor inside outdoor cooler storage.

Maple Street Biscuit Company, 11657 West Broad Street – The following violations were reported during a routine inspection: top prep shelf storing takeout containers is covered in a layer of dark buildup; wire racks in walk-in cooler are covered in old food debris; y-valve attached to mop sink faucet doesn’t have the appropriate backflow prevention device installed; there is dust surrounding the ceiling vents in back of facility.

Lazeez Modern Indian Flair, 2601 Tuckernuck Drive – The following violations were reported during a routine inspection: employee entered kitchen from outside and began handling food without first washing hands; there is 0 ppm chlorine at dish machine; dried lemon peel found on stored slicer; temperature of chicken curry made yesterday in walk-in cooler is elevated because of improper cooling methods (stored in a large covered bin); temperature of rice in warmer is too low; handle of scoop in rice warmer is in direct contact with the rice; gaskets are torn in two lowboy coolers; several single-use cutting boards and make table cutting boards are heavily scratched.

Franco’s Italian Restaurant, 9010 Staples Mill Road – The following violations were reported during a routine inspection: dried food debris found on meat slicer blade; observed buildup on dispenser inside ice machine; there are torn gaskets on prep unit; there is buildup on dish machine exterior and sides of cookline; leak found at three-compartment sink and back hand sink.

Panda Express, 9100 Staples Mill Road – No violations reported during a routine inspection.

Bojangles, 5207 Brook Road – The following violations were reported during a routine inspection: women’s restroom doesn’t have hand soap; observed grease and food debris on can opener blade and base; temperatures of chicken leg on hot line and chicken breast on serving line are too low; cutting board is scored and stained; deep fryer baskets are in disrepair; one light is out in hoods above biscuit ovens; raw chicken walk-in cooler fan guards and top of hot serving case are dusty; shelf under hot water dispenser, shelf above biscuit prep area and cabinets under ice machine in dining area are dirty; observed grease residue on sides of, surfaces under, and walls behind hot line equipment; observed flour and food debris on storage racks in dry storage and reach-in cooler doors and door gaskets; dried food residue found on storage racks in walk-in coolers; observed dried food stains on wall behind biscuit make table, three-compartment sink and prep sinks; there is stained ceiling tile in dining room area; grease buildup found on ventilation hood filters.

Wendy’s, 5212 Brook Road – The following violations were reported during a routine inspection: food staff are wearing multiple rings on hands; flat top grill and reach-in cooler combo unit in kitchen are in disrepair; one light is out in hood system above deep fryers; there are rotted out wood shelves in windowed seating area in front of house; walk-in freezer floors are warped; metal floors are separating from floor base; there is grease residue dripping down sides and undersides of deep fryers; shelf under both burger grills is covered in grease and grime; dried soda residue found on/inside cabinets in front of house area; observed dried food debris on reach-in cooler and freezer door gaskets; walk-in cooler door gaskets are dirty and stained black; outdoor dumpster doors were left open; wall under cash registers has dried food debris and stains; drive-thru area walls have mildew, food stains and dust on them; ceiling tile and ventilation throughout restaurant and hood filters are dusty; walk-in freezer floor is dirty and covered in ice; observed grease residue and food stains on hood systems throughout kitchen.


APRIL 21

Bella Luna, 1212 Concord Avenue – The following violations were reported during a routine inspection: observed heavy ice buildup in white freezer and two-door freezer; two-door freezer door is in disrepair; there is mildew on gaskets of units throughout; debris found in units; dish racks need cleaning.

Starbucks Coffee Company, 8800 Staples Mill Road – The following violations were reported during a routine inspection: back hand sink doesn’t have hand soap; observed small flies throughout facility; wet blenders are stored in cabinet without being fully dry; observed buildup around front hand sink faucet and inside floor drains; observed food debris and liquid under storage shelves and three-compartment sink.

Restaurante Las Flores, 9503-B West Broad Street – The following violations were reported during a routine inspection: employee put raw meats on the grill with gloved hands and then placed sliced limes in to-go containers without changing gloves and washing hands; raw shell eggs are stored above salsas in walk-in cooler; yuca is still cooling in a pot on the counter 1.5 hours after cooking; temperature of refried beans in warmer is too low; there are no date marks on any salsas or soups; can of cleaner is stored above ice machine and next to to-go cups; box of medicine is stored on clean cup shelf; multiple foods are stored directly on walk-in cooler floor; there is grease buildup between cooking equipment and on floors under/behind cooking equipment; floors throughout kitchen are no longer smooth and easily cleanable.


APRIL 22

Chick-fil-A, 4920 West Broad Street – The following violations were reported during a routine inspection: a staff beverage by the dish area doesn’t have a lid; there is still manufacturers’ tape on side of reach-in cooler storing mac and cheese; walk-in freezer floor, waffle fry freezer door and gaskets, and the interior of reach-in cooler under sandwich make station are dirty; observed dried food debris and crumbs on reach-in cooler door gaskets throughout the kitchen; there is dried food debris on wire storage rack above salad prep table; outdoor dumpster lids and doors were left open; there is grease residue on walls behind deep fryers; observed dried food debris and stains on wall behind three-compartment sink; wall next to bag in box has dried soda syrup on it; there are dried beverage stains on walls behind drive-thru hand sink and lemonade/tea dump sink; ceiling vent above drive-thru area door is dusty.

Tropical Smoothie Café, 8207 West Broad Street – The following violations were reported during a routine inspection: gasket is torn on walk-in cooler; facility doesn’t have test strips for quat sanitizer; observed heavy brown buildup on underside of several blenders between bottom of blender and blade attachment.

King’s Wok, 5223 South Laburnum Avenue – The following violations were reported during a routine inspection: rice lid and other utensils found in hand sink; there is debris on slicer and slicer blade; multiple ready-to-eat items are uncovered in walk-in and refrigerated units; multiple wet wiping cloths are out on prep table instead of being stored in bleach between use; knives are stored between prep surfaces; observed buildup of debris and grease on racks in units, dry storage racks, rolling racks, gaskets of units, and along cookline equipment; there is a leak at three-compartment sink and mop sink.
