Mellow Mushroom, 12171 West Broad Street – The following violations were reported during a routine inspection: hand sink is being used as a dump sink at the bar (hand soap was located at the dump sink); temperature of top portion of ricotta on ice is elevated; temperature of garlic and oil sitting out on counter is elevated; three chemical spray bottles are unlabeled; observed small flies throughout kitchen (multiple flies seen in dish area, near ice machine, mop sink and prep sink); cutting boards in storage are heavily scratched.
Tasty Crab, 7801-A West Broad Street – The following violations were reported during a routine inspection: didn’t observe an employee health policy; noodles were dumped in the hand sink; there is no hand soap at hand sink in the back; observed black mildewy substance on tea nozzles and on walls of ice machine; food debris found on can opener blade and food processor blade; two utensils are stored in room temperature water; small cutting boards and prep unit cutting board have deep scratches and staining; soiled cardboard is lining shelves; wood isn’t sealed; there is torn equipment wrapping on freezers; several items/areas are dirty with grime and food debris (shelves and exterior of food prep surfaces throughout kitchen, sides of cookline equipment, food processor base, gaskets of cooling units, exterior of bulk flour bins, and racks throughout the kitchen, storage, walk-ins and dish area); there is a leak at the mop sink; observed buildup of grime on floors and walls around/under stationary equipment and shelving; walk-in cooler fans and ceiling vents are dusty; there is a black mold-like substance on walk-in cooler ceiling; floor drains and walls, floor and ceiling around mop sink are dirty; lights are not working above hood; one light is out in walk-in cooler; one light is out in walk-in freezer.

Shoreline Market, 10614 Patterson Avenue – The following violations were reported during a routine inspection: several packages of raw fish are vacuum packaged in the walk-in cooler; prep and single cutting boards are heavily scratched; paint is rubbing off the floor making it no longer smooth and easily cleanable.
The Brass Tap, 4901 Libbie Mill East Boulevard – The following violations were reported during a routine inspection: sanitizer bucket is stored in hand sink; there is no hand soap at hand sink next to three-compartment sink; there are six dented and damaged cans of roasted red peppers; raw shrimp is stored above cooked ready-to-eat pork and brisket in walk-in cooler; bar area dish machine sanitizer concentration is too low; food thermometer isn’t working; food staff are wearing jewelry on wrists; deep fryer in disrepair found stored in back kitchen should be repaired or removed from the facility; cutting boards are scored and stained; grease residue found on sides of flat top grill and wire storage racks used for clean dishes; observed food debris on reach-in cooler and freezer door gaskets; observed food debris and stains on bar area reach-in cooler door gaskets; two out of three sink basins in three-compartment sink are missing drainage pipes; wall underneath three-compartment sink has dried food stains and debris on it; there is mildew on wall behind dish machine; ceiling tile and vents are dusty; there are dried food stains on ceiling tile; wall behind cookline is greasy; bar area floor drain is dirty.

Potbelly Sandwich Shop, 10921-E West Broad Street – The following violations were reported during a routine inspection: container of sugar in back of facility is unlabeled; long cutting boards on prep units are heavily scored and stained; gasket liners are torn on first lowboy refrigeration unit on cookline, all prep units on the line and on the walk-in cooler door; wire racks in walk-in cooler are covered in dust and debris; y-valve attached to mop sink faucet doesn’t have the appropriate backflow prevention device installed; mop isn’t hung or inverted to air dry.

Westbury Deli, 8903 Three Chopt Road – The following violations were reported during a routine inspection: person in charge’s food protection manager certification is expired; person in charge couldn’t provide verification that staff have been trained on employee health or written procedures for cleaning up vomit and diarrhea; server grabbed lemons with bare hands to place in customer drink; temperature of sauteed onions in steam table is too low; there are no date marks on sliced into deli meats or chicken noodle soup; menu doesn’t contain an advisory statement for eggs that can be cooked to order; two bottles of pesticide labeled “for residential use only” found in kitchen; two bottles of cleaning solution are unlabeled; observed small flies and gnats in kitchen, primarily around the dishwashing area; ice cream scoop and spatula are stored in room temperature water; fryer baskets have bent wires; there are deep cracks in floor tiles throughout kitchen; some ceiling tiles in kitchen are stained; mop isn’t hung or inverted to air dry; observed dusty ceiling tiles in kitchen and surrounding areas; there is grease accumulation on outlet and power cord covers behind cookline.

Sportsman’s Restaurant & Lounge, 3306 Williamsburg Road – The following violations were reported during a routine inspection: pitcher and soda nozzle found in hand sink; temperatures of crab dip, bacon dip and pasta salad in two-door reach-in are elevated; there are no date marks on cut tomatoes, pasta salad or bacon dip; there is a leak at three-compartment sink; observed holes in the wall and chipping paint; there are missing coving tiles in bathroom; mold found on wall of side kitchen; light is in disrepair in back hallway.
Azzurro Restaurant, 6221 River Road – The following violations were reported during a routine inspection: person in charge couldn’t provide verification that staff have been trained on employee health or written procedures for cleaning up vomit and diarrhea; bottles of cleaner are hanging above bin of dry storage ingredients; chopper is stored on shelf directly next to chemicals; there is a significant leak from walk-in cooler fan; dish drying rack and dry storage racks are rusted; observed ice buildup in small homestyle fridge at drink station; person in charge couldn’t produce an irreversible registering temperature indicator for dish machine; there is a direct plumbing connection at three-compartment sink; air vents in bar area are dusty; observed areas of low grout throughout kitchen.
Domoishi / Alohahola, 1080 Virginia Center Parkway – The following violations were reported during a follow-up inspection: temperature of spam musubi prepared yesterday is elevated; whole fish are being thawed under not-running water.
Starbucks Coffee Company, 11800 West Broad Street – The following violation was reported during a routine inspection: shared employee bathroom doesn’t have a covered trash can.

Amanecer Latino Market, 8410 Staples Mill Road – The following violations were reported during a routine inspection: did not observe an employee health policy or written procedures for cleaning up vomit and diarrhea; food debris buildup found on can opener blade in kitchen and ice plate at front in market area; temperatures of cheese and salsa in small market cooler are elevated; this cooler isn’t maintaining foods at the proper temperature; salsa in the walk-in cooler is past its expiration date; clean knives are stored in dirty crevice between knife rack and wall; vinyl shelf liner on shelving in kitchen is torn and frayed; cardboard is lining the walk-in freezer floor; multiple small cutting boards have deep scratches; observed buildup of food debris on gaskets of coolers, sliding doors of market display cooler, shelving throughout kitchen and exterior/sides of cookline equipment; there are direct plumbing connections at prep sink in kitchen and three-compartment sink in market area; there is a buildup of grime and food debris in a few wall/floor areas (perimeter of kitchen and dry storage and under stationary equipment).

Graze Crave, 1318 Gaskins Road – The following violations were reported during a routine inspection: raw shell eggs are stored above carrots, pickles and sauce, and near raspberries and ready-to-eat meats; one opened package of uncured sopressata sausage and one opened package of uncured capacollo sauce are past their expiration dates.
Mi Mariachi Restaurant and Night Club, 3112-A Northside Avenue – The following violations were reported during a routine inspection: did not observe an employee health policy or written procedures for cleaning up vomit and diarrhea; raw shell eggs are stored above tomatoes; menu doesn’t contain a consumer advisory statement for raw oysters (they are new to the menu); there are direct plumbing connections at two-compartment sink in kitchen and three-compartment sink at bar; observed dried food debris and grime on floors under stationary equipment on the cookline and in dish area and grease trap; there are chipped and missing tiles around cookline and dish area.
