Acacia Midtown, 2363 Roux Street – The following violations were reported during a routine inspection: open staff beverage found on cold table cutting board; whisk is stored in hand sink next to deep fryers; staff are storing utensils in room temperature water in between use; some cutting boards are scored and stained; observed grease buildup on sides of hot line cooking equipment; there is food debris in door gaskets of reach-in freezer; walls underneath/behind dish machine and three-compartment sink are dirty.
Roy’s Big Burger, 5200 Lakeside Avenue – The following violations were reported during a routine inspection: observed dead pest in back area of kitchen next to deep freezers; reach-in cooler in kitchen has a torn door gasket; reach-in cooler and freezer door gaskets throughout kitchen are dirty; restroom doesn’t have a covered trash can; walls near ventilation are dusty; observed dried food stains and grease residue on floors and walls underneath three-compartment sink; there is heavy grease buildup on walls underneath steam table and soda machine; there is stained ceiling tile in back area of kitchen; hood filters are greasy; floor underneath reach-in cooler is no longer smooth and easily cleanable.
Panera Bread, 9960 Brook Road – The following violations were reported during a routine inspection: an employee took sip of personal drink and then put on gloves before washing hands; there is no handwash signage posted at hand sink by bread ovens; milk carafes in freezer are still dirty with milky residues after being cleaned; black mildewy substance found on ice machine plate in back of house and on ice spout for customer self-service; temperature of liquid eggs in cooling unit in prep area is elevated; this unit isn’t maintaining foods at the proper temperature; racks near ware washing and in walk-in cooler are rusted; there is a torn gasket on walk-in freezer; two cutting boards are stained with deep cuts; there is food debris buildup on wheeled food carts in kitchen and freezer; observed dust and dried food debris on racks near ware washing; gaskets are dirty on cooling units and walk-in coolers and freezer; several items/areas have buildup of dried food debris on them (floors under and walls behind stationary equipment throughout kitchen and cashier line, walls in ware washing, and floors of walk-in coolers and freezer); observed chipped tiles in floors by three-compartment sink and in front of bread ovens.

Chuy’s, 11229 West Broad Street – The following violations were reported during a routine inspection: observed small flies in kitchen and bar areas; prep and single cutting boards are heavily scratched and stained; rubber pad pieces are broken off on both dicers; observed dried food debris and dust on wire storage racks, prep shelves, mixer, fryers and tortilla press; there is old label residue on plastic containers; there are leaks at faucet handle and under spray sink in dish area; observed low grout on floors throughout kitchen and bar area.
Qin Dynasty Kitchen, 7801 West Broad Street – The following violations were reported during a routine inspection: cleaning supplies are stored in hand sink basin; raw beef is stored above peeled garlic in bottom of prep unit and above bok choy in walk-in cooler; raw squid is stored above cut vegetables in walk-in cooler; temperatures of bok choy and bean sprouts are elevated because they have been sitting out on the counter for about an hour; filled bao buns are thawing on the counter; bins of cooked and cooling meats are stored directly on walk-in cooler floor; frozen packages of food are stored directly on walk-in freezer floor; two knives are stored in crevices between equipment; cardboard is lining shelves where in-use cooking utensils are sitting; observed unsealed wood; raw meats are wrapped on wood sticks to be grilled; wooden butcher block for chopping meats is unsealed with deep splits in the surfaces.
9 Mile Grill, 309 West Nine Mile Road – The following violations were reported during a routine inspection: raw bacon is stored over produce in walk-in cooler; there is dried food debris inside microwave; items in walk-in cooler are not date marked; there is no food thermometer on site; food is stored directly on walk-in cooler and freezer floors; reach-in cooler interiors, exteriors and door gaskets are dirty; there is grease buildup on sides of hot line equipment and on hood ventilation system and filters; walk-in cooler wire racks are dirty and moldy; walk-in cooler fan guards are dusty; observed food debris and stains on kitchen shelves; there are food stains on walls throughout the kitchen; there is black grime accumulating on floor throughout kitchen, especially underneath equipment and in corners; there is grease residue on wall behind hot line cooking equipment; floor under hot line has food debris and grease residue on it.

Brick Road Coffee Co., 8055 Shrader Road – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certification; dried food debris found inside cold holding units; observed residue on floors under ware washing area and shelves.
Con Salsa Venezuelan Cuisine, 3016-A Mountain Road – The following violations were reported during a routine inspection: did not observe an employee health policy or written procedures for cleaning up vomit and diarrhea; raw chorizo is stored next to lettuce in bottom of prep unit; condensed milk in a squeeze bottle at the bar is at room temperature (person in charge states it was opened a week ago); fresh garlic in mayo-based sauce is sitting out on counter in a squeeze bottle and in portioned cups; Benadryl is stored above napkins; bottled juice made at the facility doesn’t have a label with the full warning about pasteurization; cutting boards have deep cuts and stains.
Papa John’s Pizza, 4744 South Laburnum Avenue – The following violations were reported during a routine inspection: back cutting board is scratched and stained; buildup of food debris found in units; observed dust and debris on racks and fans of walk-in; old sauce found on rolling rack with cutting board.

Sakura Japanese Restaurant, 9008 West Broad Street – The following violations were reported during a routine inspection: menu says crabmeat salad, uzumaki, kani salad, spicy kani roll and fuji roll contain “crabmeat” but the facility is using imitation crab; there is brown staining on tea pitchers; dish machine has 0 ppm chlorine because the sanitizer bucket appears to be empty; cement mixer and paint primer are stored above bulk food bins; two cleaner spray bottles are unlabeled; rice scoops are stored in room temperature water; ice scoop handle is touching ice at tea station; several cutting boards are scored and stained; hand sink outside bathroom leaks when turned on; staff bathroom door was left open; observed grease buildup on pipes and floor behind cookline equipment.
Yogi Tea, 7801 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide food protection manager certification for anyone and the person in charge’s certificate is expired; didn’t observe signatures on employee health policy; raw shell eggs are stored above portioned sauce cups; temperature of rice in small bowl on the counter is elevated; temperature of milk in reach-in cooler is elevated because it had been out for preparing drinks for an unknown length of time; two containers of powdered food substances are unlabeled; cardboard is lining bottom shelves of tables.
AHA Indian Kitchen, 3409 Old Parham Road – The following violations were reported during a routine inspection: raw chicken and whole shell eggs are stored above curries in reach-in cooler; temperatures of soup and chicken in hot holding table are too low (person in charge states items are on time control but couldn’t produce time control policy or time stamps); there are no date marks on cooked rice or potatoes in reach-in cooler; spray bottle of sanitizer is unlabeled; bottle of spray paint is stored next to open spice bottles; propane tank and can of paint is stored next to oils; sanitizer spray bottle is sitting on prep table next to cutting board and knife; back door of kitchen was propped open during the duration of inspection; observed a large fly in bin of cooked rice in walk-in cooler; immersion blender, box of potatoes, ginger, oils and a tub of raw chicken are stored directly on kitchen floor; food ingredients throughout kitchen are not protected from contamination (bins without lids, bags of ingredients wide open, spice jars without lids); employee handling food is wearing a bracelet; employees with facial hair aren’t wearing beard guards; rice paddle is stored in bin of room temperature water; single-service containers are being used as a scoop in flour; single-service utensils are not stored inverted; cutting board at make table is heavily scratched and stained; fryer baskets have bent wires; spice storage rack is rusted; three-door reach-in cooler has a torn gasket; observed dried food debris splatter all over interior of microwaves; observed food debris and grime on gaskets of all refrigeration units; rack in between lowboy and bottom of sliding door reach-in freezer are dirty; y-valve with hoses are attached at mop sink without the proper dual check valve installed; there is a leak from cold water faucet at mop sink; there is a direct plumbing connection at three-compartment sink; one light is out in vent hood; hand sink is dirty and slow to drain; walls throughout kitchen are dirty with food splashes; observed grease accumulation behind cookline on ground and on walls, pipes and vent hood; food debris found on walk-in cooler floor.
Tropical Smoothie Café, 1513 North Parham Road – The following violations were reported during a routine inspection: temperatures of sausage, eggs, chicken, rice and sliced turkey in make table are elevated; bottle of drain cleaner is stored on top of ice machine; sanitizer bucket is stored next to exposed single-service cups; single-service cups are being used as scoops in chocolate and vanilla protein powder bins; small purple cutting board is scratched and stained; dry storage racks across from three-compartment sink have rusted legs; dish machine that has been out of order for years should be removed from the facility; tortilla holders are heavily scratched; bottom of smoothie make table, wall under three-compartment sink and exterior of sugar bucket are dirty; mop isn’t hung or inverted to air dry; two ceiling tiles are missing and one with water damage above make tables; there is food debris accumulation along baseboard of walk-in cooler; there are splashes on walls throughout kitchen; observed what appears to be black mold on caulking behind sinks and adjacent tables; white wall wrapped around make tables and in back of kitchen are no longer smooth and easily cleanable.
Pizza Hut, 5100 Williamsburg Road – The following violations were reported during a routine inspection: temperatures of cheese, sausage and ham in top of pizza prep unit are elevated; there are torn gaskets on two-door refrigerator; ice buildup found on freezer fans; observed heavy grease and debris buildup on fryers, racks, gaskets, inside of units including walk-in, and on/around dish machine; there are cracked floor tiles and missing coving files; observed low grouting in dish area; floors and walls under equipment and around corners of the kitchen are in need of cleaning.
APRIL 9
Liu’s Kitchen, 11408 West Broad Street – The following violations were reported during a routine inspection: an employee rinsed their hands in three-compartment sink instead of using hand sink; hand sink in bar doesn’t have paper towels; there is a buildup of black debris on interior shield of ice machine; observed debris buildup on soda nozzle in the bar; dishwasher isn’t reaching the required sanitizing temperature; container of fried chicken pieces in walk-in cooler has no preparation date marked on it; gasket liners are torn on five refrigeration units; wire racks in walk-in cooler and dry storage area are peeling and rusty; facility couldn’t provide sanitizer test strips; there is a direct plumbing connection underneath three-compartment sink.
Pizza Hut, 3113 Mechanicsville Turnpike – The following violations were reported during a routine inspection: restroom doesn’t have paper towels or a covered trash can; food is stored directly on walk-in freezer floor; deep fryer baskets are in disrepair; observed dried food debris and limescale buildup on dish machine exterior and limescale buildup on interior; wire storage racks throughout kitchen are greasy, dusty and dirty; grease residue and grime found on exterior and interior of pizza make table; observed grease and dried food debris on POS monitors and buttons and on wall behind dish machine; walk-in cooler and freezer door gaskets and wall behind pizza oven are dirty; observed grease residue on walk-in cooler glass doors; hand sink by dish machine isn’t draining water; there are food stains on walls near pizza make table; ceiling tile is dusty especially near ventilation; hood system exterior walls are dusty; back room floor is unsealed making it no longer smooth and easily cleanable.
