MARCH 20

IHOP, 1601 Willow Lawn Drive – The following violations were reported during a follow-up inspection: squeeze bottles full of yellow and clear liquids at cookline are unlabeled; produce boxes are stacked directly on walk-in cooler floor; temperature of water in which utensils are stored is too low; make table cutting boards on cookline and at server station are scratched and stained; fryer baskets have bent wires at corners; bottom of hand sink on left side of cookline is sagging and held together with duct tape; there is major ice accumulation in walk-in freezer (about two feet high along wall and on rack to the left); floor tiles in walk-in freezer are bowing and coming up; observed grime and dust accumulation on floor fan; waffle iron exterior (not plates) are heavily caked with grease; hand sink on serving line is out of order; two y-valves with spray hose connected at mop sink doesn’t have a dual check valve installed; there are areas of low grout throughout kitchen; cracked and chipped tiles found throughout kitchen; tiles are shifting in front of dish pit; observed large gash in concrete flooring of walk-in cooler; ceiling tiles are missing sporadically throughout kitchen; strip between wall and ceiling above three-compartment sink is falling; there is duct tape placed around corner edge between back of cookline and prep area.

Bartizan Events, 4035 Whittall Way – The following violations were reported during a routine inspection: raw chicken, fish and cheeses in reach-in cooler and freezers are being vacuum packaged by the facility without a HACCP plan in place; facility doesn’t have a test kit for quat sanitizer.

Dave & Buster’s, 4001 Brownstone Boulevard – The following violations were reported during a routine inspection: open employee drinks are stored on prep surfaces; employee handled cell phone and then continued food prep without first washing hands; hand sink is blocked by refrigeration units on cookline; dish machine isn’t reaching the required temperature; temperatures of cut tomatoes, bacon jam and bacon aioli double panned in prep unit are elevated; observed small flies at bar and in storage room; temperature of water in which utensils are stored is too low; double southbend ovens aren’t working properly; wire racks in coolers are rusty; hand sink across from beer cooler isn’t operational; observed buildup of dried food debris and grease on surfaces of ovens, fryers, prep units, wire storage racks and fan grates in walk-in coolers, prep shelves, suppression nozzles in hood, and inside microwaves; there is a leak under hand sink next to walk-in cooler; there is a leak at water line handle at bar; grease buildup found on floor under cookline; floor at bar is moldy and dusty and there is standing water.

Quarter Horse, 9018 West Broad Street – The following violation was reported during a routine inspection: two spray bottles of cleaner are unlabeled.

First Watch, 5310 West Broad Street – The following violations were reported during a routine inspection: staff beverage found without a lid; ice scoop handle is touching ice in ice machine; temperature of water in which utensils are stored is too low; multiple cutting boards in kitchen are scored and stained; observed dried food residue on wire storage racks in walk-in cooler; outdoor dumpster lid was left open; dumpster is overflowing with trash.

Papa John’s Pizza, 9038 West Broad Street – The following violations were reported during a routine inspection: did not observe the employee health policy; employee entered through front door and began prepping cheese without first washing hands; there is heavy black buildup on several pizza pans and on oven roller racks; temperatures of chicken nuggets, cheese stick, beef, spinach and chicken wings in bottom of prep cooler are elevated; observed dust buildup on oven fans; y-connector attached at mop sink doesn’t have the proper backflow prevention device installed.

Dunkin Donuts, 7300 Staples Mill Road – The following violations were reported during a routine inspection: observed buildup on hand sink faucet near expo prep; threshold at walk-in cooler/walk-in freezer is in disrepair; there is low grout in the dish room.

Sumo Sushi Express, 1090 Virginia Center Parkway – The following violations were reported during a routine inspection: several containers of prepared foods are being held longer than 24 hours without a date mark; employee medication is stored in reach-in cooler next to foods for service; salmon is thawing in reach-in cooler while still in reduced oxygen packaging; many personal items and equipment found throughout the facility.

Moe’s Southwest Grill, 10839 West Broad Street – The following violations were reported during a routine inspection: wire racks in walk-in cooler are peeling and rusty; observed old food debris buildup on ingredients rack in prep area.

La Rancherita, 3423 Cox Road – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; kitchen hand sink doesn’t have paper towels; jug of glass cleaner is stored directly next to napkins; three spray bottles of different cleaners are unlabeled; temperatures of small containers of salsa in salsa cart are elevated; walk-in cooler threshold is lifted from the floor and broken; observed a large hole in the wall at mop sink and a couple of small flies flying out from inside the hole; grout is low through the kitchen floor; tiles are cracked around walk-in cooler and three-compartment sink.


MARCH 25

Portabella’s Restaurant, 2627 New Market Road – No violations reported during a follow-up inspection.

Navarasa, 2610 Tuckernuck Drive – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; bins of raw meat and bags of sugar and rice are stored directly on the floor; fryer basket wires are bent; gaskets are torn in left prep cooler and mini fridge; observed grease buildup between fryers and stovetop; floor is worn.

Tavern at Triangle Park, 7110-F Patterson Avenue – The following violations were reported during a routine inspection: person in charge couldn’t produce employee health policy; an employee handled raw chicken with a gloved hand, removed glove and proceeded to cut ready-to-eat jalapeno peppers without first washing hands; food debris accumulation found on back of deli slicer; temperatures of salsa, coleslaw and homemade ranch in ice baths at expo station are elevated; salsa in walk-in cooler is past its expiration date; two spray bottles of sanitizer are unlabeled; bottle of hand soap is stored on rack next to juices; back door of kitchen was wide open throughout inspection; a fly trap is hanging over shelves of make tables/expo area; boxes of raw meat are stored directly on walk-in cooler floor; knives are stored in crevices of prep units; ice cream scoop is stored in a cup of water at room temperature; racks throughout kitchen are rusted, including in units; two lights are burned out in vent hood; gaskets are torn in clear door bar fridge; door of walk-in cooler is unable to completely close and seal; all make table and single cutting boards are scratched and stained; plastic spatulas have chipped edges; observed grease and food debris accumulation coating all units on cookline; there is dust accumulation on kitchen ceiling throughout and on counter fan above cookline; all gaskets are dirty; observed many areas of floor damage throughout kitchen; there are cracked tiles, deep cracks and a deep hole in floor at bar; wall is damaged above dish machine (wall is peeling) and in dry storage room (wall tiles falling and interior of wall exposed); observed grime and food debris buildup on floors under and around units; dust and food splashes found on walls; there is food debris on walk-in cooler floor under racks; mop isn’t hung or inverted to air dry.

Einstein Brothers Bagels, 3320 Pump Road – The following violations were reported during a routine inspection: front hand sink doesn’t have paper towels or hand soap, and it is being used as a dump sink; raw shell eggs are stored above ready-to-eat foods in prep unit; observed small flies near floor drain under espresso machine; sticky fly trap is installed above clean equipment in dishwash area; prep unit cutting board and slack boards are heavily stained and scored; baking sheets are splitting; there is food debris buildup on wire racks storing spices; several ceiling panels are heavily stained brown; ceiling panels are no longer smooth and easily cleanable.

THE LOCAL at Forest & Ridge, 403-B North Ridge Road – The following violations were reported during a routine inspection: temperature of top portion of tomatoes in right make table is elevated because they are stacked above fill line; temperature of cooked fettuccini noodles portioned in bags is elevated with condensation in left lowboy; quat sanitizer tablets are stored on shelf next to condiments at the bar; knives are stored in prep unit crevices; single-service containers are being used as scoops in bulk dry storage bins; left make table cutting board is scratched and stained; two lights are burned out in vent hoods; racks in indoor walk-in cooler are rusted; door flap on bottom of outdoor walk-in cooler is hanging down and no completely sealing the door; there is are leaks inside display unit and at back kitchen hand sink (hot water handle); observed food splashes on ceiling in back kitchen; there are areas of low grout in front of dish machine in kitchen and in front of beer taps in bar.
