MARCH 22

Chick-fil-A, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: sugar bin is unlabeled; several items/areas found soiled (handles to salad prep table, filter to hot and cold holding unit, area around front hand sink, pipes under three-compartment sink, fan grills and walls inside walk-in).

Subway, 73 South Airport Drive – The following violations were reported during a routine inspection: person in charge unable to provide written employee health policy; several dishes and food debris found in dish area hand sink; dish area hand sink doesn’t have hand soap or paper towels; box of food stored directly on walk-in freezer floor; wire racks in walk-in cooler are rusty; reach-in cooler door gasket is torn; observed food debris on walk-in cooler wire racks; walk-in cooler fan guard is moldy; walk-in freezer door gaskets are dirty; ceiling tile is missing in kitchen above mop sink; ceiling tiles throughout kitchen are stained; one ceiling tile in dining room is stained; ceiling tiles near ventilation are dusty; observed grease residue on walls and ceiling above oven.

Amanecer Latino Market, 8410 Staples Mill Road – The following violations were reported during a routine inspection: temperatures of chicken and container of beef under heat lamp are too low; temperatures of salsas in individual cups and slaw at front checkout counter are elevated; single cutting boards are heavily scratched; observed areas of low grout; tile around drain in dishwash area is broken.

Jersey Mike’s Subs, 7000 Forest Avenue – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; raw steak box stored on top of provolone cheese box; time isn’t being tracked for sliced tomatoes or lettuce on the front line; temperature of tomatoes on front line are elevated; walk-in cooler fan grills are dusty.

Saladworks, 7000 Forest Avenue – The following violation was reported during a routine inspection: hot water at front and back hand sinks doesn’t reach the required temperature.

Two Guys Pizza, 11636 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide verification that employees have been trained on their illness reporting responsibilities; temperature of marinara sauce hot holding is too low; temperatures of tomato slices and feta cheese in pizza make table are elevated; there isn’t a time policy/time tracking for pizza slices and portioned marinara; salad make table is non-functioning; pizza make table lids are in poor repair; facility doesn’t have test strips for chlorine sanitizer; lighting near bathrooms, where single-service items are stored, is very poor.

AHA Indian Kitchen, 3409 Old Parham Road – The following violations were reported during a routine inspection: facility couldn’t provide verification of an employee health policy; facility doesn’t have written procedures for cleaning up vomit/diarrhea; raw fish stored over ready-to-eat foods; raw lamb stored over veggies; can opener is dirty; there is no sanitizer in third sink; chai tea with milk and rice found at improper temperatures; no date marks found on any ready-to-eat foods in the walk-in; containers of soap and sanitizer are unlabeled; spices are unlabeled; clean dishes and a pot found on the floor; boxes of single-service items are stored directly on the floor; observed rust on low shelves; an airpot is being used for milky product; cardboard on shelves needs to be removed; drip trays, microwaves, lower work shelf, mop sink and kitchen hand sink are dirty.

Christian’s Pizza, 7003-A Three Chopt Road – The following violations were reported during a routine inspection: person in charge couldn’t verify that workers have been trained on employee health; interior of ice machine is dirty; temperatures of pepperoni, ground sausage, sauce in cups and ranch dressing are elevated; no date marks found on cooked sausage, fresh mozzarella or open ricotta; time isn’t being tracked for pizza by the slice (no employees could say what time they were made); containers of yeast, cinnamon and parmesan cheese are unlabeled; shelf in the walk-in is not six inches off the floor; walk-in shelves, floor of walk-in, exterior of pizza oven and flat top are dirty; fan grill in walk-in is dusty.

Sheetz, 7035 West Broad Street – The following violation was reported during a routine inspection: chlorine level in dish machine is too low.

Original Mexican Restaurant, 6406 Horsepen Road – The following violations were reported during a routine inspection: raw chorizo stored over ready-to-eat foods; no date marks found on flan, tamales, beef for fajitas, some sauces and cooked poblanos; the consumer advisory on the menu needs clarification; a spray bottle is unlabeled.

Deep Run Roadhouse, 12379 Gayton Road – The following violations were reported during a routine inspection: observed a food employee chewing gum; temperature of brisket on top portion of steam table is too low; observed food employees with long facial hair not wearing beard guards; single cutting boards are heavily scratched and stained.

Cupertino’s, 3621-A Cox Road – The following violations were reported during a routine inspection: food manager certification has expired; person in charge couldn’t provide written procedures; caramel color stored next to bleach; spices stored next to cleaner spray; several bins of bagel mix are unlabeled; plastic bins are stacked while still wet above three-compartment sink; fan in kitchen is dusty; ceiling panel above walk-in units is torn; ceiling panels throughout kitchen are stained.

MARCH 28

Dunkin Donuts, 3125 Lauderdale Drive – The following violations were reported during a routine inspection: observed grime and pink buildup inside ice machine; temperatures of creamer and milk in reach-in cooler at drive thru are elevated; there is a leaky pipe in dry storage area; light in walk-in freezer is flickering; mop isn’t hung to air dry.

Skrimp Shack, 4760 Finlay Street – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit/diarrhea; apron found inside hand sink near prep unit; dirty pans stacked in dish area hand sink; temperatures of lettuce and cheese in cold container on prep table are elevated; menu lists catfish but there was no catfish found in the establishment (boxes of swai found in walk-in freezer); observed an employee wearing bracelets while preparing food; greasy cardboard is lining prep counter in back area; gaskets on prep units are torn; wire racks in walk-in and back prep table are rusty; container lids are broken; towel dispenser isn’t working properly; single and prep cutting boards are heavily scratched and stained; observed food debris buildup on wire racks and prep tables; cables and monitors are dusty; observed mildew buildup on walls, ceiling and fan system inside walk-in cooler; there is a leak under hand sink in employee bathroom; observed food debris and grime on floors; ceiling vents and panels are dusty; floors throughout the facility are not smooth and easily cleanable.

Penn Station East Coast Subs, 9320-C West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have an employee health policy; employees are unaware of the “big six” reportable foodborne illnesses and the associated symptoms; hand sink near the office doesn’t have hot water; grime found inside ice machine; temperature of marinara in double broiler on grill is too low; temperatures of cheddar, swiss and provoline slices in prep unit are elevated; no date marks found on sliced meats wrapped in plastic in reach-in cooler; several bottles at the grill are unlabeled; long white cutting boards have deep grooves and brown stains; there is a leaky pipe at hand sink under water filtration system; floors throughout the kitchen are dirty.

Apollo’s Flame Baked Pizza & Grill, 9410-B West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures; temperature of heavy cream in reach-in cooler is elevated; sliced meats in walk-in cooler are past their expiration dates; several bottles in stovetop area are unlabeled; a gasket on Pepsi reach-in cooler is torn and held together with duct tape; fryer cabinet, racks throughout the facility and floor of Pepsi reach-in cooler are dirty; ceiling panels and floors throughout the kitchen need repair; wall near stand mixer has holes and insulation is coming out; panel is lifted off the wall near Pepsi reach-in cooler; hood vents need cleaning.
