Richmond Psychiatric Associates, Inc. Ceasing Operations- Notice of Transfer of Patient Records

Effective immediately, Richmond Psychiatric Associates, Inc. is ceasing operations. Patient records will be transferred to Dominion Behavioral Healthcare, P.C. at 2301 North Parham Rd, Suite 5, Richmond VA 23229, who will serve as their custodian. Patients and their authorized representatives have the right to request a copy of their medical records or to have their records transferred directly to another psychiatrist of their choice. Upon receipt of written authorization, records will be provided within a reasonable time and the patient will only be charged the cost of copying and delivering their records.
