Panera Bread, 1370 North Parham Road - During a risk factor inspection on Sept. 8, inspectors observed lettuce at 49 degrees and spring mix at 48 degrees in a salad drawer unit.

Richmond Marriott Short Pump, 4240 Dominion Boulevard - During a risk factor inspection on Sept. 8, inspectors observed a soda dispenser positioned above a hand sink in a way that made accessing the faucet difficult; dried food debris on a meat slicer blade and on pots and pans stored beneath prep tables; dried food on a can opener blade; three pans of pasta dated Aug. 29 in a prep unit and walk-in cooler, as well as several items with missing or incorrect dates; and ribeye steak, New York strip and a Marriott burger on the menu that could be cooked to order without an asterisk linking them to the consumer advisory.

Starbucks Coffee Company, 4101 Dominion Boulevard - During a routine inspection on Sept. 8, inspectors observed a Y-valve attached to the mop sink faucet without an appropriate backflow prevention device; and a dirty dumpster area surrounded by loose trash and debris.

Tarrants West, 11129 Three Chopt Road - During a risk factor inspection on Sept. 8, inspectors observed a server plate bread with bare hands; no parasite destruction letter for salmon; beans in a warmer at 121 to 130 degrees; opened hunks of meat without date marks and containers of spinach dated in early August that had not been redated after thawing; and a pan of sliced ham in the walk-in cooler dated Aug. 29.

Chopped Cheese and Pizza, 10209 Staples Mill Road - During a routine inspection on Sept. 9, inspectors found no employee health policy or employee health training; no written procedures for cleaning up vomit and diarrhea incidents; raw chicken stored above raw beef and ready-to-eat dressings and sauces; containers of salt, sugar and flour without identifying labels; a mesh curtain on the back door with a gap of several inches at the floor; no test strips for ammonium chloride sanitizer tablets; and cracked floor tiles and coving in the warewashing area.

Hibachi House, 3456 Lauderdale Drive - During a risk factor inspection on Sept. 9, inspectors found no written procedures for cleaning up vomit and diarrhea incidents; salad cups in the front make table at 45 degrees; cooked onions in a steam table at 120 degrees; a large container of rice prepared the previous day and stored in a three-door reach-in refrigeration unit without a preparation date; and an unlabeled spray bottle of degreaser in the warewashing area.

KickBack Jacks, 9318 West Broad Street - During a routine inspection on Sept. 9, inspectors found no written procedures for cleaning up vomit and diarrhea incidents; observed an employee use a double-gloved hand to place a raw beef patty on a grill, remove the extra glove and then place food liners in trays; heavy brown buildup inside an ice machine; sausage at 45 degrees, ham at 44 degrees, lettuce at 47 degrees, tomato sauce at 41 to 43 degrees and cheese at 47 degrees in a pizza oven cooler, milk at 43 degrees in a dressing cooler, and large bins of raw chicken at 43 to 44 degrees in a beer walk-in cooler after the door reportedly had been open for an extended period; several flies in the dry storage and dumpster areas; and plastic bins of chips stored beneath an electric fly trap in the dry storage area.

Marco's Pizza, 2284 John Rolfe Parkway - During an other inspection on Sept. 9, inspectors observed no paper towels in the dispenser at a hand sink.

Nuckols Farm Elementary School, 12351 Graham Meadows Drive - During a routine inspection on Sept. 9, inspectors observed turkey slices in the walk-in cooler with an Aug. 31 preparation date, which is two days beyond their storage expiration; and a direct plumbing connection beneath the three-compartment sink.

Short Pump Elementary School, 3425 Pump Road - During a routine inspection on Sept. 9, inspectors reported no violations.

Starbucks Coffee Company, 4511 South Laburnum Avenue - During a routine inspection on Sept. 9, inspectors observed heavy ice buildup on fans and the floor of a freezer; rust and peeling paint on floor drains; mildew buildup along sinks and caulking; debris in units and buildup on gaskets; and food buildup beneath units and around doors and corners of the facility.

Takara Ramen, 9468 West Broad Street - During a routine inspection on Sept. 9, inspectors observed a small bowl of rice at 59 degrees above the stove and a small bin of shrimp tempura at 64 degrees on a counter at the cookline, with the person in charge reporting that they were treated like other foods under the establishment's time control policy; cooked meats without date marks; a tube of scar gel stored directly next to food near the microwaves; torn gaskets in a reach-in cooler and two-door undercounter cooler on the cookline; and a leak at a prep sink on the cookline.

Target, 5401 West Broad Street - During a routine inspection on Sept. 9, inspectors observed pink and brown mildew inside an ice machine and a white substance along the ice guard; a food employee in the beverage area with long hair that was not restrained; a dish machine not reaching the required temperature during different cycles; the dish machine being operated with food contact equipment inside while the detergent bottle was completely empty; and dust along the wall behind the three-compartment sink, on and around a ceiling air vent and along the edges of ceiling tiles.

Tres Machos, 4120 Cox Road, Suite C - During a routine inspection on Sept. 9, inspectors observed an open drink on a prep table next to the line; a container of sponges stored in the bar hand sink, preventing proper access; debris inside two bar soda nozzles and pink slime along the bottom of the ice machine's interior shield; brown rice at 121 degrees in a steam well, with the person in charge reporting that leftover rice was reheated by adding warm water and placing it directly into the steam table without taking a final temperature; queso at 99 degrees in a steam well; an unlabeled spray bottle containing Clorox at the three-compartment sink; a large container of paprika without an identifying label in the dry storage area; a torn gasket on the walk-in cooler door; and food debris stains on drying racks above the three-compartment sink.

Chick-fil-A Willow Lawn, 4920 West Broad Street - During a risk factor inspection on Sept. 10, inspectors found no paper towels at the drive-thru hand sink.

District 804, 8004 West Broad Street - During a routine inspection on Sept. 10, inspectors found no employee health policy and no written procedures for cleaning up vomit and diarrhea incidents; no soap at two bar hand sinks; empty paper towel dispensers at the kitchen and hookah bar area; dried food on a can opener blade; quaternary sanitizer only reaching 100 ppm in the three-compartment sink; several cold unit gaskets in disrepair; no test strips for the quaternary sanitizer used at the three-compartment and bar sinks; and a small leak at the hand sink near the cookline.

Hermitage ACE Center, 8350 Hermitage High Boulevard - During a risk factor inspection on Sept. 10, inspectors found no paper towels at a hand sink.

Papa John's Pizza, 551 East Laburnum Avenue - During a risk factor inspection on Sept. 10, inspectors found sanitizer at 0 ppm in the three-compartment sink despite a full sanitizer container beneath the sink (dispenser failing to draw sanitizer or soap from their sources).

Sandston Elementary School, 7 Naglee Avenue - During a routine inspection on Sept. 10, inspectors reported no violations.

SpringHill Suites Richmond Northwest, 9960 Independence Park Drive - During a routine inspection on Sept. 10, inspectors found no food protection manager certificate; no written procedures for cleaning up vomit and diarrhea incidents; cream cheese cups at 62 degrees, salsa at 49 degrees, shredded cheese at 58 degrees and a carafe of Silk milk at 48 degrees on a service counter without time control; apples on a service counter without protection; and no quaternary or chlorine test strips for sanitizers.

The Westin, 6631 West Broad Street - During a risk factor inspection on Sept. 10, inspectors observed brown buildup inside an ice machine; ham at 61 degrees, feta at 57 degrees, tomato at 56 degrees, shredded cheese at 50 degrees and sausage patties at 46 to 63 degrees in a tabletop cooler; and rice dated Aug. 30 in a walk-in cooler.

Napoli Pizza, 3324 Pump Road - During a routine inspection on Sept. 11, inspectors observed an open drink can stored on a prep table with food; items stored in a kitchen hand sink; open packages of sliced turkey and pasta in reach-in coolers without date marks; brown staining and scratches on prep and individual cutting boards; no sanitizer test kit for the three-compartment sink; a direct plumbing connection beneath the three-compartment sink; and cracked floor tiles that were no longer smooth and easily cleanable in some areas.

Corner Bakery Cafe, 11000 West Broad Street - During a risk factor inspection on Sept. 14, inspectors found no soap at the hand sink behind the cookline; and sanitizer from the three-compartment sink dispenser at 100 ppm, which is below the required minimum of 150 ppm.

Dunkin, 3443 Cox Road - During a routine inspection on Sept. 14, inspectors observed flies on several donuts in a display case; some individual containers of milk creamer at 48 degrees in the drive-thru cooler; several flies throughout the establishment and on food; donuts in the display case that were not protected from flies or other forms of contamination; and a Y-connector with a sprayer and hose under continuous pressure attached to the mop sink without a backflow prevention device rated for continuous pressure.

Elephant Thai Restaurant, 11676 West Broad Street - During a follow-up inspection on Sept. 14, inspectors found no manager on duty who could satisfactorily answer questions when an employee was asked who was in charge; one of six fryer baskets with broken metal pieces that could become a source of physical contamination; old food debris buildup on a black wire rack in the walk-in cooler; heavy grease buildup beneath the cookline; and dead roach carcasses beneath the prep line.

Glen Lea Elementary School, 3909 Austin Avenue - During a routine inspection on Sept. 14, inspectors observed food staff wearing wristbands.

Henrico High School, 302 Azalea Avenue - During a routine inspection on Sept. 14, inspectors observed dust on walk-in cooler fan guards and dried food debris and mold on wire storage racks in the walk-in cooler.

Ratcliffe Elementary School, 2901 Thalen Street - During a routine inspection on Sept. 14, inspectors observed salad prepared that morning at 69 degrees in a reach-in cooler; a dead cockroach beneath a flat top grill on the hot line; a direct plumbing connection beneath the three-compartment sink; and dust on a ceiling tile and ceiling vents.
