Twin Hickory Tavern, 1420 North Parham Road - During a risk factor inspection on Aug. 20, inspectors found that the person in charge could not provide verifiable proof of employee health training and food-handling staff questioned about the policy could not name the six major foodborne illnesses and their symptoms; found no hand soap at the bar hand sink; observed the person in charge rinse a contaminated knife in a cookline hand sink, a drink pitcher stored in the bar hand sink, and soda-dispenser lines draining directly into a hand sink in the upstairs kitchen; celery stored directly on racks in an upstairs bar reach-in cooler; raw fish and shrimp stored above cooked noodles in a lowboy cooler and raw fish stored behind sliced turkey in a drawer; chlorine sanitizer measuring at 0 ppm in the bar dish machine; three-dimensional brown buildup in the upstairs bar ice machine, three-dimensional pink and dark-colored buildup in the Coke machine's ice dispenser at server station, and dried food debris on a can opener blade; milk at 46 degrees in a serving-station lowboy cooler, sliced tomatoes measuring between 38 and 45 degrees and filled above the make table's fill line, and portioned shredded cheese at 59 degrees stored several inches above the fill line; chicken breading without a time stamp; blackened tuna on a specials menu without a consumer advisory statement and salmon that could be ordered undercooked without an asterisk linking it to the consumer advisory; sanitizer, glass cleaner and stainless steel cleaner stored next to food and food-contact surfaces; sanitizer spray bottles without identifying labels; and no no-smoking sign posted in public view.

Little Caesars, 4007 Mechanicsville Turnpike - During a routine inspection on Aug. 20, inspectors observed dried food debris on the can opener blade and base; staff working with food without hair restraints; a torn door gasket on the middle warming unit; dust, dirt, grease or dried food debris on point-of-sale cables and cords, wire storage racks throughout the kitchen, reach-in freezer door gaskets, a speed rack and door gaskets in the walk-in cooler, wire racks and interior bottoms of food warmers, the top of the pizza oven, hand sinks and faucet handles, and three-compartment sink faucet handles; and stained ceiling tiles in the dining area and kitchen, mildew on the wall behind the three-compartment sink and a dirty floor drain beneath the sink.

New Hunan Gourmet, 3107 Mechanicsville Turnpike - During a routine inspection on Aug. 20, inspectors observed a bucket stored in a kitchen hand sink; rusty wire storage racks in the walk-in cooler; food stored in and covered with plastic grocery bags; cardboard being reused throughout the kitchen, including cardboard egg liners; dirty and stained black walk-in cooler door gaskets; a staff bathroom hand sink draining very slowly; a direct plumbing connection beneath the three-compartment sink; a cracked floor tile in front of the hot line, damaged floor coving beneath the three-compartment sink and a heavily stained kitchen ceiling tile; and dried food stains on kitchen walls, grease on walls behind the reach-in cooler and hot line, and a dirty wall beneath the three-compartment sink.

The Pho Spot, 7801 West Broad Street - During a routine inspection on Aug. 20, inspectors found no employee health policy or written procedures for cleaning up vomit and diarrhea incidents; observed an employee return from a break, put on gloves and begin preparing food without first washing hands; found no hand soap available in the facility; observed a food tray and vegetable peeler on top of a hand sink; observed raw shrimp above cooked tofu, raw beef above ready-to-eat sliced onions, and raw pork belly above cooked rice noodles; dried food debris on a meat slicer and blade that the person in charge said had last been used and cleaned the previous day; cooked sliced beef at 98 degrees and cooked pork belly at 77 degrees held on a grill with the burners off after being cooked about 3½ hours earlier, as well as spring rolls at 127 degrees in a fryer basket about an hour after cooking; cut lettuce at 51 degrees and bean sprouts at 45 degrees in an ice bath without being fully submerged, as well as a large bag of bean sprouts at 55 degrees in a reach-in cooler that the person in charge said had been delivered from another location about two hours earlier; two pans of cooked sliced beef and cooked meatballs prepared the previous day without date marks; cooked shrimp, cooked pork, cooked rice noodles and cut lettuce intended for time control without written procedures or marked times; cooling rice noodles in a large pan covered with a lid; two employees without hair restraints; cups without handles and spoons with their handles submerged in bulk sugar, MSG and salt; multiple deeply cut and stained cutting boards; single-service to-go plastic bags being used to hold food in a freezer; dried food debris and grime on cookline equipment, the table supporting the grill, top shelves of make tables and refrigeration-unit gaskets; dried food debris and grime on floors beneath stationary equipment and in floor drains; and grease accumulation on hood vents and filters.

Waffle House, 7109 West Broad Street - During a risk factor inspection on Aug. 20, inspectors observed employees crack shell eggs, change gloves without washing their hands and then handle hash browns and clean plates; shell eggs stored above ready-to-eat milk in a four-door cooler; gravy in a warmer measuring between 111 and 164 degrees; three chemical bottles without identifying labels; and a pallet of bagged bread stored directly next to a shelf holding jugs of chemicals.

Tay-Ho Restaurant, 7927 West Broad Street - During a routine inspection on Aug. 21, inspectors found no employee health policy or cleanup policy regarding vomit and diarrhea incidents; observed a hand sink next to the three-compartment sink blocked by several dishes; found catfish dishes listed on the menu while swai in the freezer was being used for those dishes; observed dirty soda guns at the bar and a dirty kitchen ice machine; sprouts, noodles, tomatoes and lettuce under time control without marked times; no consumer advisory on the menu for pho tai bowls; a bowl of lettuce covered with a damp cloth; two employees wearing bracelets while preparing food; a rice scoop stored in a cup of water at 75 degrees; and dust and grime on the sides of a prep cooler and fryer.

Topgolf Richmond, 2308 Westwood Avenue - During a routine inspection on Aug. 24, inspectors observed rust on a can opener blade; mold inside ice machines on the second and third floors; pimento cheese dated Aug. 23 and fresh herbs dated Aug. 19 held beyond their discard dates in the first floor walk-in cooler; a burned out light in the first floor kitchen hood; grease residue on the sides of hot-line cooking equipment on the first floor, dirty walk-in cooler door gaskets, dried food debris and blood on grill drawer gaskets, dirty bar area door gaskets and sticky or dirty liquor room reach-in cooler door gaskets; and grease residue and pooling on the walls and floors behind deep fryers in the second floor kitchen.

LIB True Taste, 1212 Concord Avenue - During a routine inspection on Aug. 24, inspectors reported no violations.

Courtyard by Marriott, 10077 Brook Road - During a follow-up inspection on Aug. 24, inspectors observed rust on walk-in cooler racks; and dust on walk-in cooler fan guards.

New Dragon City, 2220 John Rolfe Parkway - During a risk factor inspection on Aug. 24, inspectors observed dried flour and raw chicken juice buildup inside a breading container that was not being cleaned every four hours.

Quality Inn, 801 East Parham Road - During a follow-up inspection on Aug. 24, inspectors found that the employee health policy provided during the previous inspection had not been signed by food staff; the facility still did not have sanitizer on site; some single-use utensils again were stored with food contact surfaces facing upward; the right reach-in cooler is broken and not in-use; sanitizer test strips again were unavailable; dried food debris was observed around cereal dispensers and on kitchen countertops; a leak was observed at the three-compartment sink; and soiled ceiling tiles were observed above the three-compartment sink and dried food debris on kitchen floors, although some cleaning had been completed beneath the sink.

Target, 9001 Staples Mill Road - During a routine inspection on Aug. 24, inspectors observed no thermometer for measuring the surface temperature of dishes in the high-heat dish machine; and a Y-valve at the mop sink under continuous pressure.

Li's Garden, 1237 North Laburnum Avenue - During a routine inspection on Aug. 24, inspectors observed staff washing their hands at a kitchen prep sink; dried food debris inside a kitchen microwave; food stored directly on the kitchen and walk-in cooler floors; a rice scoop stored in room temperature water at 84 degrees; scored and stained cutting boards; food stored in plastic grocery bags; several deep fryer baskets in disrepair; black, moldy or dirty gaskets on reach-in coolers, freezers and the walk-in cooler, dirty dry storage wire racks covered with flour and grease buildup on the sides of hot-line cooking equipment; and grime beneath the hot line, residue on walls beneath the three-compartment sink and grease residue on walls behind the hot line.

China Taste, 2623 New Market Road - During a routine inspection on Aug. 24, inspectors found no hot water at the hand sink by the dish machine; shrimp, chicken and noodles at 48 degrees in a prep unit; a bottle of medication on a prep shelf next to cooked rice; multiple uncovered containers in the walk-in cooler; buildup on walk-in cooler racks, dust on dry storage racks, and grease buildup along the cookline and rice hot-holding units; and grease on and dripping from hood filters.

Bebop Korean Mexican Grill, 8310 Staples Mill Road - During a routine inspection on Aug. 24, inspectors observed employee health policies without signatures; an employee water bottle on a prep table with single-service items and sauces; a large container of raw chicken stored above cheese in a prep unit; a food employee wearing gloves cracked an egg with both hands and then touched cooking utensils and hamburger buns without first washing hands; breading for raw chicken and raw fish stored on a counter at 71 degrees, with raw foods placed directly into the breading and the container changed every four to five hours, as well as clumps of breading in the container; containers of pico, corn and bean sprouts prepared the previous day without date marks; large containers of flour and a squeeze bottle of oil without identifying labels; a bag and crate of onions stored on the floor; a nonfunctioning ice machine; an ice machine sitting on raw wood planks; grime and food debris on cold-unit handles, grill drawers, the exteriors of raw chicken and beef containers in grill drawers, and a fan grate in the walk-in freezer; and food debris and grime on floors.

The Daily Kitchen & Bar, 12201 West Broad Street - During a follow-up inspection on Aug. 26, inspectors observed pickled cabbage at 43 to 44 degrees and pickled onions at 43 degrees in the top right of a salad unit, as well as cheesecake at 45 degrees, tiramisu at 44 degrees and bread pudding at 43 to 44 degrees in a refrigerated drawer on the right side of the unit; and the right side and drawer of the salad unit were not maintaining proper temperatures, with a nearby hot well possibly affecting temperatures.

Taco Bell, 4388 South Laburnum Avenue - During a routine inspection on Aug. 26, inspectors observed mold on the chute of the ice machine and along soda nozzles by the drive-thru; pico at 47 degrees in a prep top across from the drive-thru; and dirt and debris on a unit that was not in use.

HogsHead Cafe, 8902 West Broad Street, Suite D - During a routine inspection on Aug. 26, inspectors observed an open employee drink stored above prep coolers; an employee place raw meat on a grill and then grab bread without first washing hands; raw chicken stored next to or above raw fish; chlorine sanitizer at 0 ppm in the dish machine; brisket at 94 degrees, chili at 121 degrees and chicken at 117 to 120 degrees in warmers after being moved from the walk-in cooler between 11 a.m. and noon; baked potatoes cooked the previous day at 41 to 44 degrees in a deep, covered container; blue cheese at 50 to 51 degrees near the top of a prep cooler and two pans of lettuce at 44 to 46 degrees in the prep cooler; many baby and adult cockroaches throughout the back kitchen, including on floors, walls and in traps; several buckets and boxes of food stored on the walk-in cooler floor; an employee drying dishes with a cloth after removing them from the dish machine; torn prep cooler gaskets, a worn shelf in the expo area and rusty walk-in cooler racks; gray or white buildup inside the top of the dish machine; open side doors on dumpsters; and worn grout in the main kitchen walkway near the dish machine.

Hardee's, 3815 Mechanicsville Turnpike - During a routine inspection on Aug. 26, inspectors observed dried food debris more than 24 hours old inside both kitchen microwaves; corrosion along the top of the dining area soda machine with heavy condensation accumulating and dripping down its sides, as well as a burned out kitchen light in front of the walk-in cooler; grease and food residue on reach-in cooler and freezer doors, dried food debris or mold on their gaskets and food debris inside the units, moldy walk-in cooler and freezer door gaskets, grease residue and food debris on cooking equipment, dirty shelving and storage racks, mold and food debris on walk-in cooler wire racks, grime and pooled food debris around the mop sink, grease and flour residue on kitchen wire racks and mold on the splash panel and drip tray of the dining-area soda machine; a direct plumbing connection beneath the three-compartment sink; open outdoor dumpster doors; and black grime on kitchen floors, dusty ceiling tiles near vents, dirty floors and walls around the mop sink and greasy floors beneath deep fryers.

Roma Ristorante Italiano, 8330 Staples Mill Road - During a routine inspection on Aug. 27, inspectors found that the person in charge could not provide an employee health policy regarding foodborne illness reporting and training; the person in charge could not provide written procedures for cleaning up vomit and diarrhea incidents; an employee drink was stored with clean plates above a pizza prep table; a food employee handled a lemon with bare hands while garnishing a drink; the bar hand sink was being used to store drink mixers; raw shell eggs were stored above bread and cheese, and raw beef stored directly on top of raw salmon in a reach-in cooler; several knives stored as clean had dried food debris on them; meat sauce in a steam well measured 119 to 129 degrees; chicken breading was stored on the counter, with the person in charge reporting that raw chicken was placed directly into the container and the breading was changed every few hours; bags of flour were stacked against containers of cleaners and degreasers; bottles of cleaner and degreaser lacked identifying labels; salmon in reduced-oxygen packaging was thawing in a reach-in cooler; the establishment's only food thermometer was not operational; a container of salt and bottles of water and oil lacked identifying labels; a food employee was cooking while wearing a watch; an in-use knife was stored on the corner of a prep unit; to-go containers were stored food contact side up directly beneath dusty ceiling vents; the dish machine dispensed only 25 ppm chlorine after several priming cycles; foil and cardboard were used to line shelves and raw wood was used for chemical storage beneath the three-compartment sink; multiple knives had been repaired with duct tape or had chipped blades, and storage drawers in a reach-in freezer had cracks and chips; no test strips were available for the Steramine or chlorine sanitizers used in the establishment; dried food debris, residue or mold-like buildup was observed on cold unit gaskets, wire racks in the walk-in cooler and walk-in freezer, a salad prep unit with pooling water, the pizza prep station and prep table shelves; both dumpsters had their doors open; a wall near the mop sink had water damage and mold, tiles in the walk-in cooler were in disrepair, and a light in the walk-in freezer was out; and ceiling vents were dusty and floor drains beneath the three-compartment sink and dish machine had buildup.

Red Salt, 12221 West Broad Street - During a follow-up inspection on Aug. 27, inspectors observed sour cream at 43 to 45 degrees, sliced cheese at 51 degrees, greens at 43 degrees and raw chicken at 44 degrees in a grill unit; a sauté refrigerator that was out of use while awaiting service and a grill refrigerator that remained several degrees too warm; grease buildup on the floor beneath fryers; and low floor grout, damaged floor tiles and a hole in the wall near the back door.

Outback Steakhouse, 7917 West Broad Street - During a risk factor inspection on Aug. 27, inspectors observed shredded mozzarella in an ice bath at 50 degrees, shredded Parmesan in an ice bath at 65 degrees and pasta in a small cooler next to the flattop grill at 44 degrees, while a to-go cooler also had an elevated air temperature but contained no TCS foods; and four chemical spray bottles without identifying labels.
