McDonald’s, 4708 Williamsburg Road – The following violations were reported during a routine inspection: boxes of cups are stored directly on the floor; observed buildup on gaskets, smoothie machine and inside of units; there is grease on sides of fryers and hanging on prep table legs; low grouting found in back kitchen near ice machine/mop sink.
Vietnam 1, 6215-B West Broad Street – The following violations were reported during a routine inspection: employee touched raw beef and then clean plates without first washing hands; seafood combination that uses imitation crab is listed on the menu as containing “crabmeat;” there is 0 ppm chlorine in dish machine; temperature of opened can of condensed milk on shelf is elevated; improper cooling methods observed for six large pots of broth in walk-in cooler (deep and covered pots, fat has solidified on surface of broth preventing air flow); in-use knives are stored in dirty crevice between prep counter and prep cooler; to-go containers are stored uninverted on dirty shelf; one long make table cutting board is stained yellow; one white square cutting board is heavily scratched; walk-in cooler racks are rusty; grease buildup found on sides and back of fryer; observed white buildup on walk-in cooler racks; hand sink faucet is leaking; observed heavy food debris and grease buildup on floor behind cookline units.
Firehouse Subs, 8191-A Brook Road – The following violations were reported during a routine inspection: observed two sick employees with person in charge stating they have an upper respiratory infection (EHS observed both coughing and one sneezing, employees were moved to non-food contact activities); observed employee sneeze into arm and then engage in food prep without changing gloves and washing hands; three bottles of cleaner are hanging on the back of hand sink; sliced beef thawed in sink without running water; sliced beef and meatballs are thawing on counter; observed small flies in kitchen; electric fly trap is stored above food prep table; red cutting boards are heavily scratched; there are torn gaskets on sandwich cooler and front two glass reach-in coolers; several items are rusted (wire shelves in front two glass reach-in coolers, wire rack next to front hand sink and prep table legs in front); observed dried food debris on interiors/exteriors and gaskets of cooling equipment; there are leaks at front hand sink and mop sink; faucet is spraying without steady stream at back hand sink.

KFC, 4821 Williamsburg Road – The following violations were reported during a routine inspection: floors throughout need cleaning; observed food and/or grease buildup on racks and inside of hot holding units, sides of fryers, behind cookline and under equipment; floor is cracked or missing in places which is causing standing water.
Lokmans Best Wings, 5308 Chamberlayne Road – The following violations were reported during a follow-up inspection: cutting boards are heavily scored and stained; top of pizza oven is very dusty; wire storage racks throughout kitchen are dirty and dusty; there is heavy grease residue on sides of hot line cooking equipment; door gaskets on reach-in freezer are dirty; there is a direct plumbing connection underneath three-compartment sink; two lights are out in hood system above pizza oven; there are food stains on walls throughout the kitchen, especially behind hot line, near hand sink and behind three-compartment sink; observed grease on floor underneath deep fryers; observed black grime on floors throughout kitchen, especially underneath three-compartment sink.
Yen Ching, 9031-9035 West Broad Street – The following violations were reported during a routine inspection: employee touched cell phone and then put on gloves to cut tofu without first washing hands; there is no hot water at hand sink and the sink leaks when hot water faucet is turned on; temperatures of several foods in two prep coolers are elevated (three large bins of egg rolls, noodles, sprouts, baby corn, raw shell eggs, large container of rice, fried chicken and ribs); glass cleaner is stored next to jars of chili garlic; burn gel is stored next to to-go containers; observed one live adult roach and one live baby roach crawling on counter near kitchen entryway, one dead adult roach in dry storage room, and several roaches in floor traps; shredded cabbage is covered with a linen cloth in reach-in cooler; rice scoops are stored in room temperature water; scoops without handles are being used in bulk food bins; ice scoop handles are in contact with the ice; several batches of wonton strips are stored in non-food grade black garbage bags; gasket is torn and loose in prep cooler nearest walk-in cooler; observed heavy grease buildup between cookline units and on floor under cooking equipment; shelving racks and fan guards in walk-in cooler are dirty; floor tiles are cracked; grout is worn in several places throughout the kitchen; floor panels in walk-in freezer are loose; there is a hole on bottom of interior side of walk-in cooler door.

Buz and Ned’s Real Barbecue, 8205 West Broad Street – The following violations were reported during a follow-up inspection: temperatures of raw chicken skewers, hot dogs and pulled chicken in four-drawer unit are elevated; this unit is not maintaining foods at the proper temperature and needs repair; both gaskets of two-door reach-in freezer are torn; observed grease and dust accumulation on back of order screen, wires and support beam next to cookline; employee restroom doesn’t have a covered trash can; there are areas of low grout throughout kitchen floor; baseboard tiles are cracked, chipped and missing around dish room; observed small areas of wall damage above hand sink next to three-compartment sink and above mop sink.
Tropical Smoothie Café, 11736 West Broad Street – The following violations were reported during a follow-up inspection: right-side handle to reach-in refrigeration unit in back of facility is still broken; mop isn’t hung or inverted to air dry.

Yellow Umbrella Provisions, 1744 East Parham Road – The following violations were reported during a routine inspection: employee is chewing gum or food while preparing food; buildup of dried food debris found on can opener blade; there are incorrect date marks on beef bulgogi and shrimp sauce mixture; there are no date marks on hot pickled onions and carrots; corn marinade, sous vide garlic, hot pickled jalapenos and oil that had garlic cooked in it are past their expiration dates; several foods in walk-in are vacuum packaged and many additional vacuum packaged meats in the freezer are labeled for sale (the facility doesn’t have a HACCP plan, which means food must be removed from vacuum packaging within 48 hours); several foods in walk-in are cooling in large, plastic, covered tubs (bacon/cheese quiche filling, crab/cheese quiche filling, spinach/cheese quiche filling and pimento cheese); two bulk bins and multiple containers of powdered foods are unlabeled in dry storage area; there are insect control devices above vacuum packager and above a meat saw and seasonings; there is a direct plumbing connection at food prep sink; observed broken flooring and coving in dish area.
Papa John’s Pizza, 6100-D West Broad Street – The following violations were reported during a routine inspection: dried food debris and crumbs found in pizza make table door gaskets; there is mold and mildew on three-compartment sink; fan used in the kitchen, walk-in cooler fan guards and top of pizza oven are dusty; rolling wire cart is dirty; observed dried food debris and flour on pizza racks; there is a direct plumbing connection underneath three-compartment sink; mop isn’t hung or inverted to air dry; there are dried food stains on ceiling tile; observed dust on ceiling tiles near ventilation and ceiling vents.
Subway, 9621 West Broad Street – The following violations were reported during a routine inspection: did not observe a food protection manager certificate; temperatures of shaved steak and chopped chicken in walk-in cooler are elevated; walk-in cooler gasket is torn and dirty; walk-in cooler racks and floor are dirty; floor trim in walk-in cooler is loose; mop isn’t hung or inverted to air dry.
Mi Guerrero Grill, 61 South Laburnum Avenue – The following violations were reported during a routine inspection: there is no handwash signage posted in men’s restroom; there is mold inside the ice machine; food is stored in non-food grade plastic grocery bags throughout the kitchen; two lights are out in the hood system; reach-in cooler and freezer door gaskets are torn; cutting boards are heavily scored and stained; reach-in cooler door gaskets throughout kitchen and fan used in the kitchen are dirty; sides of hot line equipment and white wire racks for storing clean dishes are greasy; observed dried food debris and blood on bottom shelves of reach-in freezer; there is no covered trash can in women’s restroom; stained and damaged ceiling tiles found throughout the restaurant; there is missing ceiling tile in men’s restroom; the wrong material for ceiling tile is being used in kitchen area; observed dried food stains and grease residue on hot line wall.
Bojangles, 11 South Laburnum Avenue – The following violations were reported during a routine inspection: soap dispenser at deep fryer area hand sink doesn’t have hand soap and the dispenser is in disrepair; did not observe a discard time for fries; food staffer is wearing a watch; cutting boards are scored and stained; deep fryer baskets are in disrepair; four lights are out in deep fryer area hood system; one light is out in hood system over ovens; dried food debris and stains found on shelves under serving case; observed food debris on reach-in cooler and freezer door gaskets throughout kitchen; storage shelves in dry storage and walk-in coolers are caked with flour and dried food debris; drive-thru area warming unit is covered in grease and food residue; all kitchen hand sinks are dirty; there is grease residue on sides of hot line cooking equipment; top of front counter food warmer is dusty; cabinet interiors under front of house ice machine is dirty and covered with dried liquid residue; ceiling tile near ventilation and ceiling vents are dusty; walls behind drive-thru warming unit and walls/pipes behind deep fryers are greasy; walls, pipes and tubes behind biscuit ovens are covered in flour and dusty; wall behind/underneath three-compartment sink and floor drains throughout kitchen are dirty.
DQ Grill & Chill, 3275 New Market Road – The following violations were reported during a routine inspection: a can of queso on shelf is heavily dented; walk-in racks are rusted; gaskets are torn; fryer baskets are bent; ice machine is rusted causing holes and the unit to leak; walk-in cooler’s holding temperature is slightly elevated; observed grease buildup on all hot holding units; there is dust on fry hot holding unit in back; debris found inside of units; there is grime buildup in back hand sink; low grouting found in front kitchen; coving tiles by walk-in are missing; wall is stained with chipping paint by back walk-ins; mops are not hung or inverted to air dry.
Subway, 2650 New Market Road – The following violations were reported during a routine inspection: didn’t observe a food protection manager certificate; back hand sink doesn’t have paper towels; there are no date marks on deli meat in the walk-in cooler; there are no quat sanitizer test strips.
