Mission BBQ, 5440 Glenside Drive – The following violations were reported during a routine inspection: dried food debris found on clean utensils over prep table; temperature of potato salad on ice is elevated; observed grime and dust on monitors and wires, shelves, drawers and around cookline equipment; observed water and grime on floors, especially under stationary equipment; there is low grout in back kitchen.

Casa Grande, 7818 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; person in charge couldn’t provide verification of employee health training and illness reporting or written procedures for cleaning up vomit and diarrhea; a staff member is eating at prep station; there is no hand soap at the only hand sink in kitchen; queso was taken from walk-in cooler and placed at hot well before being reheated; temperatures of raw pork and beef are elevated (had been thawing in prep sink); many bulk bins of food are unlabeled; many containers of food are uncovered in walk-in cooler and freezer; to-go containers are not stored inverted; food in walk-in cooler and freezer aren’t stored in food-safe packaging (stored in “thank you” bags); dried food debris found on wire racks in walk-in cooler; there is ice buildup on right side of walk-in freezer; tiles are in disrepair throughout kitchen; paint is peeling behind mop sink; there is low grout at floor drains, which is collecting moisture; light is missing above prep sink.

Zaxby’s, 9110 Staples Mill Road – The following violations were reported during a routine inspection: two containers of Roma tomatoes are past their expiration date; bucket of sanitizer is stored on prep table next to active prep and food; observed grease and dried food debris on exterior of grill drawers, gaskets and warmed drawers; there is grime on doors and handles of units; cooler and ice machine vents are dusty; there is a leak at mop sink and three-compartment sink faucet; prewash hose won’t shut off; observed dried food debris and grime around perimeter of kitchen, especially behind cookline; flooring is in disrepair by fryers, allowing moisture to collect.

Chipotle Mexican Grill, 8955 Staples Mill Road – The following violations were reported during a routine inspection: there is food debris on shelf storing portion cups with food contact surface stored face down; observed dried food debris inside prep unit at to-go station; buildup found on flat top.

Mi Jalisco, 10482 Ridgefield Parkway – The following violations were reported during a routine inspection: pink residue buildup found on shield inside ice maker; two food employees with long facial hair aren’t wearing beard guards; gaskets on prep unit door are torn; there is a direct plumbing connection under three-compartment sink.

Dunkin’ Donuts, 10390 Ridgefield Parkway – The following violations were reported during a routine inspection: observed dried food debris and dust on wire storage racks and fan system in walk-in cooler; y-connector with a spray nozzle and hose under pressure is attached to mop sink without the proper backflow prevention device installed.

Casa Grande, 10120 Brook Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; didn’t observe verification of employee health training and illness reporting or written procedures for cleaning up vomit and diarrhea; bar hand sink doesn’t have hot water; raw shell eggs are stored above sauces in lowboy cooler; raw chorizo is stored above onions in walk-in cooler; pink mildew-like substance found on ice machine in the bar; observed dried food debris on cleaned robot coupe and veggie slicer; meat slicer and coffee maker in dry storage are covered in flour (not in use and should be removed); temperatures of beef stew and beef hot holding on stove are too low; multiple foods cooked yesterday are not date marked (stuffed peppers, taquitos and shredded chicken in lowboy and shredded chicken and tamales in walk-in cooler); sugar at drink station and cinnamon sugar by ice machine are unlabeled; observed dead roach bodies in the corners of dry storage floor; foods are uncovered in lowboy cooler, freezers and in walk-in cooler; can opener blade and dry storage racks are rusted; gasket on right bar glass cooler is torn; raw wood at entrance to bar isn’t painted or sealed; women’s restroom doesn’t have a covered trash can; observed dried food debris buildup on floors under stationary equipment and under racks in dry storage, in floor drains, on wall in dry storage and ceiling vents; there is a broken ceiling vent above left reach-in freezer; there are broken ceiling tiles above mop sink; there is grease buildup on hood above stove.

Ciao Capri, 10478 Ridgefield Parkway – The following violations were reported during a routine inspection: no manager or supervisor has a food protection manager certification; pink residue buildup found inside ice maker; temperatures of crab cakes and tomato in oil in prep unit are elevated; prep and single cutting boards are scratched and stained; reach-in door gaskets are torn; observed food debris and grease residue on surfaces of ovens, coolers, handles, wire storage racks, utensil holders, fan grates in coolers and under grills; there is a direct plumbing connection under three-compartment sink; dust and grease buildup found on walls, ceiling vents and panels, and on floors under equipment.

European Taste, 10604 Patterson Avenue – The following violations were reported during a routine inspection: black residue buildup found inside ice maker; there is 0 ppm sanitizer in dish machine; cutting boards are heavily scratched and stained; observed dried food debris and grease residue on surfaces of spice carts and racks, storage shelves, inside coolers and door gaskets, and outside of ingredient bottles and jars; dried food spills and stains found on walls.

Subway, 4386 South Laburnum Avenue – The following violations were reported during a routine inspection: three-compartment sink has 0 ppm sanitizer; temperatures of spinach and tomatoes on make line are elevated; there is a cinnamon air freshener stored over slicer and Goo Gone stored on prep table; observed debris in units, around dish area, and on shelves; there is a leak at three-compartment sink faucet.

Wellness Center Tepaz, 5000 Nine Mile Road – The following violation was reported during a routine inspection: an ice scoop with broken handle is being used.


MAY 15

Domino’s Pizza, 11210 Patterson Avenue – The following violations were reported during a routine inspection: hand sink near three-compartment sinks doesn’t have paper towels; observed buildup of dust and food debris on surfaces of reach-in cooler, wire racks, fan covers in walk-in cooler, and behind oven; there is food debris buildup on floor and inside grout.

Subway, 11234 Patterson Avenue – The following violations were reported during a routine inspection: temperature of meatballs is too low; temperature of chicken portioned into boats is elevated because the container is overfilled; meats in the walk-in cooler are covered during the cooling process; food employee with long facial hair isn’t wearing a beard guard; prep cutting boards are heavily scratched and stained; there is a direct plumbing connection under three-compartment sink; ceiling panels are stained and no longer smooth and easily cleanable.

Jersey Mike’s Subs, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: employee had bare hands in pockets and touched hair and then proceeded to don gloves and handle food without washing hands first; there is no date mark on a bin of prepared tuna salad in walk-in cooler; cough drops are stored on top of an open box of sandwich paper; box of cold tablets are stored on rack next to sandwich paper; back door of establishment was propped open throughout inspection; box of chips is stored directly on floor; hose is laying in bottom of sanitizer basin of three-compartment sink; hose is laying in bottom of mop sink; outdoor dumpster doors were left open; mop isn’t hung or inverted to air dry.
