Eggs Up Grill, 2238 John Rolfe Parkway – The following violations were reported during a routine inspection: facility couldn’t provide a verifiable employee health policy; employee is drinking from an open cup in kitchen; hand sink at end of prep line doesn’t have hand soap; employee contaminated their gloves by using phone and touching their face and then began to work with clean dishes; temperature of sausage gravy in hot box on prep line is too low; temperature of pancake batter in reach-in refrigeration unit is elevated; dulce de leche in walk-in cooler is past its expiration date; spray bottle containing bleach solution is unlabeled; diced tomatoes are cooling while in a lidded container; several dry ingredients (garlic powder, brown sugar and cinnamon sugar) are stored unlabeled throughout the kitchen; an employee has long hair that is loosely restrained; top shelf above first prep unit on the line is covered in a layer of black debris; gasket liners are torn on left-side door of second prep unit on line, reach-in freezer on cookline, and on reach-in at bar; long cutting boards on prep units are scored and stained; wire racks in walk-in cooler, ingredient racks in dry storage area and drying racks in ware wash area are dirty; portable fan in kitchen is dusty; bread shelf is covered in old food debris; label residue found on several in-use containers; dumpster doors were left open; grease bin stored next to exit door in back of building is overflowing with debris; mop isn’t hung or inverted to air dry.

Bonefish Grill, 11251 West Broad Street – The following violations were reported during a routine inspection: temperatures of top layer of bagged pasta in prep unit and cut cabbage in salad prep unit are elevated; containers of pasta and cooked shrimp in reach-in cooler are past their expiration dates; thawed trout is still in reduced oxygen packages in cooler drawer; some plastic containers and lids are broken; metal wires are bent on fryer baskets; handles and faucets at hand sinks are damaged; observed buildup of dust and grease residue on surfaces of broiler, sides of grills, prep shelves, rubber mats at bar, monitors, cables, and racks inside reach-in cooler and walk-in cooler; observed several areas of low grout and broken floor tiles in kitchen.

Chez Max Restaurant, 10622 Patterson Avenue – The following violations were reported during a routine inspection: there is no hot water at hand sink in kitchen; there are gaps along the bottom of both doors facing outside; dead roaches found throughout the establishment (kitchen, dishwash areas and bathrooms); employees with long facial hair aren’t wearing beard guards; prep unit cutting board is heavily scratched and stained; observed dust buildup on wire racks and outside dish machine; outdoor dumpster doors were left open; there is a heavy buildup of dust on ceiling vents and panels; wall junction near dishwash area is damaged.

Frank’s West, 11238 Patterson Avenue – The following violations were reported during a routine inspection: two pans of portioned lasagna just removed from the freezer are not date marked; in-use knives are stored in dirty crevices between prep unit and tables; prep cutting board is heavily scratched and stained; floor mats are in disrepair; wire racks in walk-in cooler are rusty; observed dried food debris and dust on walk-in wire racks and top of pizza oven.

West Lake Chinese Restaurant, 11256 Patterson Avenue – The following violations were reported during a follow-up inspection: similar looking spices on cart are unlabeled; there is a leak at handles of front hand sink and the cold water handle doesn’t work; observed heavy buildup of food debris, dust and grease residue on walls and floors throughout kitchen; there is an accumulation of broken and unused equipment in kitchen that should be removed.

Kabab House, 3411 Old Parham Road – The following violations were reported during a follow-up inspection: dried food debris found on butcher blade; temperatures of butter chicken, Palok-alu, okra and eggs on buffet line are too low; tandoori chicken cooked yesterday in two-door reach-in cooler isn’t date marked; salad at buffet line is on time control but there isn’t a time stamp.

Olive Garden Italian Restaurant, 4467 South Laburnum Avenue – The following violations were reported during a routine inspection: observed black residue in top of ice machine; the HACCP logs are not being accurately and consistently filled out; multiple servers are handling exposed foods while wearing bracelets and are not wearing adequate hair restraints; food handler in charge of prep has long facial hair and isn’t wearing a beard guard; small white cutting board is scratched and stained; racks in dry storage are rusted; fryer baskets have broken wires; top of microwave is burnt; tea containers have cracked tops; lemon slicer blade cover is burnt and chipping; inside of bowl warmer and top of heat lamps are dirty; several items/areas are dusty (monitor wires, sprinkler cage in walk-in cooler, and top shelf above cookline storing single-service containers); hose with spray nozzle attached at mop sink doesn’t have the proper dual check valve installed; observed food splashes on walls.

Jimmy John’s Gourmet Sandwiches, 4420 South Laburnum Avenue – The following violations were reported during a routine inspection: there is 0 ppm of sanitizer at three-compartment sink; observed buildup on gaskets of units and racks of walk-in; there is a leak at front hand sink; under ice machine and three-compartment sink are in need of cleaning.

Va. House of Soul Cocktail Grill, 3406 Mechanicsville Turnpike – The following violations were reported during a routine inspection: there is no handwash signage posted in men’s restroom; there is no covered trash can in women’s restroom; observed mold and black stains inside soda gun nozzle at bar; reach-in cooler door gasket is cracked; beer cooler door gasket is torn/hanging; observed dried food debris and dust on reach-in cooler gaskets throughout kitchen and bar; wire mesh rolling cart with warming unit in kitchen is greasy; floor and floor tile in dining area and kitchen are cracked.

Sandano’s, 11389-B Nuckols Road – The following violations were reported during a routine inspection: raw shell eggs are stored over ready-to-eat foods in reach-in refrigerator; food residue found on slicer blade; temperature of shredded chicken in make table is elevated; quat sanitizer concentration in spray bottles is too high; unused equipment in back of kitchen should be removed from the facility; gaskets are torn and black with residue on make tables; fryer basket wire is poking out; racks in make table are rusty; facility couldn’t provide quat sanitizer test strips.

The Wine Whisperer, 11535 West Broad Street – The following violations were reported during a follow-up inspection: gasket liners are still torn on both doors of two-door refrigeration unit in kitchen; there is still a y-valve connected to mop sink faucet without the appropriate backflow prevention device installed.

Roslyn Conference & Retreat Center, 8727 River Road – The following violations were reported during a routine inspection: one can of pineapple juice and one can of garbanzo beans are dented; observed brown and pink mildew on ice guard of ice machine; dried food debris found on deli slicer and can opener blade; sanitizer wipes are stored above food in dry storage; racks in two-door reach-in cooler and walk-in cooler are rusted; cutting boards are scratched and stained; there is grease accumulation on sides of fryer and ovens.

Waffle House, 3202 East Parham Road – The following violations were reported during a routine inspection: there is no handwash signage posted in the men’s restroom; there are stains on clean coffee mugs; pans and utensils are stored with dried food debris and grease on them; temperature of closed container of gravy is elevated and has exceeded the two-hour cooling time; spray bottle of cleaner is stored next to clean utensils and single-service items; several employees preparing food are wearing watches and bracelets; observed grease and dried food debris on gaskets, interiors of drawers under grill, and on wire racks throughout the facility; water is pooling in right-side of reach-in refrigerator; floor drains are dirty; mop isn’t hung or inverted to air dry.

DQ Grill & Chill, 807 East Parham Road – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; there is no handwash signage in one restroom; temperatures of sliced tomatoes and ranch in prep unit are elevated because both items are overstacked; ice scoop held on wire rack isn’t sanitized; plastic containers are stacked while still wet above three-compartment sink; dried food debris found inside cabinets near to-go area; wire racks are rusty with food debris on them; there is buildup on the exterior of ice machine; there is a leak under left basin of three-compartment sink; there is excess trash and debris around the grease/garbage area; observed grime and dried food debris throughout facility, especially under stationary equipment; wall behind mop sink is in disrepair and coving is coming undone; hand sinks are coming off the wall.

Pho Thien Phat, 6403 Rigsby Road – The following violations were reported during a routine inspection: open drinks found on shelf above make table and on counter next to make table; raw eye of round and raw shrimp are stored over broth and cooked meats in two-door lowboy reach-in cooler and walk-in freezer; dried food debris found on deli slicer; temperature of vegetarian broth is too low; temperature of portioned cooked steak in right make table is elevated because it is in a bin stacked on top another bin; buckets of noodles and broth are stored directly on floor throughout kitchen and in walk-in freezer; rice paddle is stored in bin of still water; single-service containers are being used as scoops in rice and sauce buckets; bins are stacked while still wet; sriracha bottles are being reused for oils and sauces; wires on fryer basket are bent; dry storage shelves are rusted; walk-in freezer gasket is torn; water heater leaner and hose coming out of top are disconnected; observed brown/black mildew on seal around hand sink; there is grease accumulation on shelf under grill and legs of table; there is spilled liquid in bottom of two-door lowboy reach-in freezer; dried food debris found on walk-in freezer racks; fan guards in walk-in freezer are dusty; outside and handles of refrigeration units are dirty; there is a direct plumbing connection at three-compartment sink; food splashes found on walls; observed grease accumulation on pipes and floor behind cookline; floor in mop room and around water heater is dirty; mop isn’t hung or inverted to air dry; bottom of walk-in freezer door is rusted and lifted; there is a hole in the wall under mop sink faucet; observed condensation on mop sink room ceiling; there are ceiling tiles with water damage; wall next to mop sink is no longer smooth and easily cleanable.
