Twin Hickory Tavern, 11351 Nuckols Road – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certification or verify that employees have received health training; several employee drinks found on prep counter; employee washed her hands with no soap for less than 20 seconds; observed staff cut, grab and place cooked chicken strips on wrap for customer without washing hands; bar hand sink is being used as a dump sink with strainer to catch ice and straws; raw pork chops and ribeye steak boxes are stored above onions and other ready-to-eat foods; observed dried food debris on slicer, peeler and can opener and person in charge states they haven’t been used today; crab meat and homemade blue cheese are improperly cold holding in prep unit; facility is using time control for onion rings and chicken breading without a written policy or time stamps; several containers of bulk sugar, salt, oil and water are unlabeled; observed small flies around bar area; two employees preparing food and handling clean silverware are wearing bracelets; person in charge states they are not washing any produce aside from precut greens; side of left prep unit is not cold holding properly and there is a lot of water pooling in the bottom; racks throughout kitchen are rusty; several cutting boards are heavily stained and scratched; facility doesn’t have test strips for the quaternary or chlorine based sanitizers; observed heavy dried food debris and dust on wire racks in dry storage; observed grime and food residue inside gaskets of multiple cold holding units and on floors throughout kitchen, especially under stationary racks and cook line equipment; y-nozzle connected to two hoses at mop sink doesn’t have the proper dual check valve installed; several ceiling and floor tiles are in disrepair, especially in front of walk-in cooler.
Wendy’s, 4231 Pouncey Tract Road – The following violations were reported during a routine inspection: wire racks in walk-in cooler are beginning to peel; y-valve attached to mop sink faucet doesn’t have the proper backflow prevention device installed; dumpster area is dirty and surrounded by loose trash and debris; there is dust surrounding the ceiling vent in back of kitchen.
Silver Diner, 10890 West Broad Street – The following violations were reported during a routine inspection: chemical spray bottle found in hand sink; temperature of sliced ham in first prep unit on the line is elevated (date marked on container is yesterday); temperatures of oatmeal and grits in steam well are too low; long cutting boards on prep units are scored and stained; there are broken off metal pieces on all four fryer baskets; drying rack in ware wash area is peeling; floor drain under three-compartment sink is clogged, causing water to drain slowly and begin to pool; spray nozzle attached to hose in mop sink doesn’t have the proper dual check valve installed.
Shawarma Inc., 6215 West Broad Street – The following violations were reported during a routine inspection: person in charge allowed customer to use restroom in food preparation area; raw chicken is stored above raw lamb in reach-in cooler; chicken prepped 1.5 hours ago is improperly cooling on counter; temperatures of pickled cucumber, hummus, salad and garlic sauce in front cooler are elevated; bag of spinach, several hunks of beef and several boxes of chicken are thawing at room temperature; sticky fly trap is hanging from the ceiling above clean dishes; hunks of beef and a bag of onions are stored directly on floor; employees are wearing bracelets; in-use utensils are stored in ambient temperature water; dumpster door was left open; mop isn’t hung or inverted to air dry.
Willard Commissary, 2607 Willard Road – No violations reported during a routine inspection.

Baskin-Robbins, 5510 West Broad Street – The following violations were reported during a routine inspection: food is stored directly on walk-in freezer floor; utility sink basin, plastic containers behind three-compartment sink, and reach-in cooler interiors and door gaskets throughout kitchen are dirty; observed food debris, dust and crumbs on storage shelves throughout kitchen; there is debris on dipper well edges; there are dried food stains on sides of serving cases and backs of ice cream cake coolers; walk-in freezer door gaskets are moldy and stained black; observed crumbs, dried food stains and dust on/inside cabinet doors, drawers and interiors throughout kitchen; outdoor dumpster lid was left open; there are cracked and missing floor tile throughout kitchen; ceiling tile in dining area is stained; there is grime on kitchen floor underneath pipes; observed dried food stains on walls throughout kitchen; ceiling vents are dusty.
America’s Best Wings, 5650 Brook Road – The following violations were reported during a routine inspection: observed employee eating cereal on the prep line; employee washed hands in prep sink instead of hand sink; employee didn’t use hand soap when washing hands; employee washed dirty dishes and then put on gloves to handle food without washing hands first; there is no hand soap at the hand sink; raw chicken is stored above olives and jalapenos in prep cooler; observed food debris and a dead roach on slicer stored under prep counter; wings were placed directly into warmer from walk-in cooler without being reheated to the proper temperature first; temperatures of sliced cheesecake in display case and sauce cup in unit are elevated; medicine is stored next to food over the microwave; observed several flies and roaches on the floor and counters in kitchen; there is a gap in bottom of back door; employees are wearing bracelets; one fryer basket is heavily bent; there is sauce buildup on sauce bowl shelf; there is grease buildup on side of fryer; three-compartment sink is leaking; spray hose attached to mop sink doesn’t have the proper backflow prevention device installed; employee food is stored throughout kitchen on counters, shelves and coolers.
Best Western Richmond Airport Hotel, 5300 Airport Square Lane – The following violations were reported during a routine inspection: open drink found in kitchen; observed employees eating in the kitchen; hand sink doesn’t have paper towels; there is buildup of debris on can opener; temperature of milk in main fridge is elevated; chemical spray bottles are unlabeled; kitchen cabinets are in poor condition and one side is missing a door; coating on shelves is missing or chipping; facility couldn’t provide test strips for quat sanitizer; area under three-compartment sink needs cleaning; there are food splatters on wall.

DC Charcoal Chicken, 4909 Nine Mile Road – The following violations were reported during a follow-up inspection: food is stored directly on floors in walk-in cooler and walk-in freezer; multiple foods are stored uncovered/unprotected from contamination in walk-in freezer; one light is out in rotisserie chicken oven ventilation hood; there is grease residue on sides of hot line cooking equipment and on walls behind hot line in kitchen; wire racks storing clean dishes above three-compartment sink are dirty and greasy; outdoor dumpster doors and lids were left open.
Drury Plaza Hotel, 11049 West Broad Street – The following violations were reported during a follow-up inspection: temperatures of cream cheese and waffle batter on the service line are elevated; reach-in refrigeration unit is still in disrepair but no foods are stored within.

Cook Out Restaurant, 4300 South Laburnum Avenue – The following violations were reported during a routine inspection: lettuce, tomatoes and cheese time labels are over the four-hour mark and should be discarded; observed some buildup on racks in walk-in; there is grease along the cook line; floor tiles along cook line and near drains are cracked.
The Saucy Dragon, 1420 North Parham Road – No violations reported during a follow-up inspection.

Casa del Barco, 11800 West Broad Street – The following violations were reported during a routine inspection: open drinking mug found in kitchen across from expo line; employee began working with food with one bare hand and one gloved hand that had touched non-food contact surfaces without washing hands; a server is peeling fruit with bare hands; hand sink next to prep line doesn’t have paper towels; raw meatballs are stored above ready-to-eat food in last lowboy on the prep line; temperature of steamed potatoes cooked yesterday in prep unit is elevated; temperatures of shredded lettuce and enchiladas on prep line are elevated; enchiladas stored in last prep unit on the line are past their expiration date; two spray bottles containing liquid chemicals are unlabeled; observed improper cooling methods used for steamed potatoes (stored in deep pans covered in plastic wrap); small, stainless steel container of a white powdery substance on back prep table is unlabeled; observed small flying pests in the bar; chef with long facial hair isn’t wearing a beard guard; shelf above prep line storing take-out containers is covered in a thin layer of debris; long cutting board on prep line is scored and stained; wire racks in walk-in cooler are covered in old food debris; hand sink at kitchen entrance has a clogged drain; two mops in utility area are not hung or inverted to air dry.

More Than Sushi, 9199 Staples Mill Road – The following violations were reported during a follow-up inspection: there are still grease, dried food and cockroaches present on floors; interior and exterior of refrigerators and freezer and hood vents still need to be cleaned.
Holiday Inn & Suites Richmond West, 8008 West Broad Street – The following violations were reported during a routine inspection: observed brown residue on ice plate inside ice machine; queso is past its expiration date; wrong date marks found on pico and sliced tomatoes.
