Marco’s Pizza, 1070 Virginia Center Parkway – The following violations were reported during a routine inspection: restroom doesn’t have paper towels; there are torn gaskets on multiple units; observed buildup of debris in units and on racks, especially in walk-in; there is mildew on gaskets and along seals on units.


MARCH 13

Shawarma Bistro, 1511 North Parham Road – The following violations were reported during a routine inspection: quat sanitizer concentration in three-compartment sink is too low; person in charge couldn’t provide test strips for quat sanitizer; squeeze bottles containing oil throughout the kitchen are unlabeled; one fryer basket has bent wires; left make table cutting board is scratched and stained; observed grease and food debris buildup on rice cooker handle.

Papa John’s Pizza, 602 East Laburnum Avenue – No violations reported during a routine inspection.


MARCH 17

Surrounding Counties, 7019 Three Chopt Road – The following violations were reported during a routine inspection: there is no handwash signage posted at hand sink; temperature of half-and-half at self-service bar is elevated and person in charge states there is no time control policy in place to monitor that; there are no date marks on open package of sliced ham or tomato salad; spray bottle of quat sanitizer is unlabeled; employee with facial hair is handling food without wearing a beard guard; coating is chipped on kneading attachment for mixer; air vent in back of kitchen is rusted; ceiling tiles in storage room are showing signs of water damage; mop isn’t hung or inverted to air dry.

Panera Bread, 1370 North Parham Road – The following violations were reported during a routine inspection: temperatures of cut cantaloupe and honeydew melon in fruit cup in drive-thru lowboy are elevated; ambient temperature of this unit is elevated; cutting boards throughout kitchen are scratched and stained; food debris buildup found on bars between salad units on serving line; there is dust accumulation on air vents and walk-in cooler fan guards; y-valve attached at mop sink doesn’t have a dual check valve installed; there are areas of low grout throughout the serving line; floor is dirty around baseboard in walk-in freezer.

Starbucks Coffee Company, 1302-B Gaskins Road – The following violations were reported during a routine inspection: quat sanitizer concentration at three-compartment sink is too high; y-valve attached at mop sink doesn’t have a dual check valve installed.

Urban Taco, 5205 South Laburnum Avenue – No violations reported during a follow-up inspection.


MARCH 18

Huang’s Express, 5462 West Broad Street – The following violations were reported during a routine inspection: boxes of food are stored directly on walk-in cooler floor; food is stored unprotected in walk-in cooler; there is grease residue on sides of wok cooking station; mold found on wire storage racks in walk-in cooler.

Hanabi, 7801 West Broad Street – No violations reported during a follow-up inspection.


MARCH 19

Perla Italian Restaurant, 10246 Staples Mill Road – The following violations were reported during a routine inspection: dishware is stored in bar hand sink to dry and there are no paper towels; observed residue on interior of ice machine, inside soda dispenser nozzle and on can opener blade; three containers of meat sauce are past their expiration dates; menu doesn’t contain a consumer advisory statement for steak and beef that can be served undercooked; several bottles of oil and water and bulk containers of flour, salt and sugar are unlabeled; gaskets of several cold unit doors are in disrepair; facility doesn’t have test strips for quat or chlorine sanitizer; old stickers found on outside of food storage containers; bathroom doesn’t have a covered trash can; observed dust and mold-like buildup on ceiling vents.

Chopped Cheese and Pizza, 10209 Staples Mill Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; facility couldn’t provide verified employee health training for new employees or written procedures for cleaning up vomit and diarrhea; observed staff touch clothing and come into the kitchen from outside and proceed to handle clean equipment without first washing hands; raw shell eggs are stored on top of bread; dried food debris found on meat slicer blade; staff with facial hair aren’t wearing a beard guard while preparing food; facility doesn’t have a test kit for sanitizer used at three-compartment sink; back of fryer has grease buildup on it; handles of cold storage are sticky; observed heavy buildup on hood vents; floor and ceiling tiles are in disrepair; observed dried food debris on floors under shelving; food splatter found on walls.

Sheetz, 7035 West Broad Street – The following violations were reported during a routine inspection: there are no paper towels at back hand sink in kitchen; quat sanitizer concentration in sanibucket is too high; temperature of marinara sauce in mini warmer is too low; temperature of ice cream shake mix carton in lowboy is elevated because it was left out of refrigeration for an extended period of time; ice scoop is chipped; person in charge states dish machine isn’t working; floor in walk-in freezer is no longer smooth and easily cleanable.

Waffle House, 5102 Williamsburg Road – The following violations were reported during a routine inspection: eggs are being held outside refrigeration for more than four hours; observed mildew on racks in units and walk-in; grease buildup found under cookline.

Famous Fried Chicken & Seafood, 3050 Williamsburg Road – The following violations were reported during a routine inspection: there are no paper towels at hand sink; there are no labeling of ingredients for cakes from a local bakery; temperature of collards on stove is too low because the burner is turned off; cutting board on prep table is heavily stained; observed buildup of breading on handles of breading make table; there is grease buildup on sides of cookline equipment and on hood filters.


MARCH 20

New Grand Mart, 9035 West Broad Street – The following violations were reported during a routine inspection: raw shell eggs are stored above kimchi buckets in walk-in cooler; dried food debris found on garlic grinder; temperature of cooked onions in warmer is too low; there are large, deep cracks in nine rice bowls; facility doesn’t have quat sanitizer test strips; several items/areas are dirty (sides of cookline equipment, front part of stove, and floor and pipes under cookline equipment); observed leaks at three-compartment sink faucet.

Zen Café, 9041 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; dried food debris found in microwave; there is no time marked on boba and person in charge couldn’t produce the time control policy; racks are rusty in two-door lowboy; there are torn gaskets on lowboy freezers; shelves and tops of food containers are dusty; red mini fan is dirty; observed wet black buildup in large crevice separating prep sink and wall; wallpaper in utility room isn’t flat against the wall; ceiling panels are torn in storage room.

Pizza Hut, 11385 Nuckols Road – The following violations were reported during a routine inspection: scummy buildup found inside dish machine, especially on the sides; observed dust and debris on racks in walk-in refrigerator and on fan guards in unit.

Charleys Cheesesteaks and Wings, 4101 Dominion Boulevard – The following violations were reported during a follow-up inspection: no food protection manager certificate is available; only one handwash station out of four has hand soap and paper towels; shredded cheese in the walk-in cooler is nine days past its expiration date; there is still a y-valve on the mop sink faucet without the appropriate backflow prevention device installed; wire racks in walk-in cooler are still dirty; stack of take-out containers are stored uninverted again.
