MARCH 5
Thai Top Ten, 9986 Brook Road – The following violations were reported during a routine inspection: employee seen dumping food into handwash sink; pink residue buildup found inside ice maker; temperatures of several foods are elevated (spring rolls on top of prep unit shelf, raw beef in prep unit, and half-and-half at bar cooler); prep unit is not holding foods at the proper temperature; chemical cleaning bottle is unlabeled; rice scoops are stored in ambient temperature water; prep unit cutting boards are heavily scratched and stained brown; wire storage racks in reach-in and walk-in coolers are rusted; observed dried food debris on surfaces of wire racks, prep shelves storing clean equipment, cords, floors throughout kitchen and inside coolers; observed dust on ceiling vents and panels above prep units.

Lakeside Grill, 6920 Lakeside Avenue – The following violations were reported during a follow-up inspection: prep unit and reach-in cooler are both not holding foods at the proper temperature; broken plastic containers and lids are being used; manufacturer wrapping is still on fryer and backsplash on cookline; prep unit cutting boards are heavily scratched and stained brown; facility couldn’t provide test kit for sanitizers; hose with sprayer attached to mop sink faucet doesn’t have the proper backflow prevention device installed.
Jersey Mike’s Subs, 7000 Forest Avenue – The following violations were reported during a routine inspection: employee didn’t wash hands upon immediately entering the kitchen; hand sink on cookline doesn’t have paper towels or hand soap; temperatures of sliced tomatoes and shredded lettuce in all make tables are elevated; single-service containers on top of deli case are not covered or inverted to protect them from contamination; knives are hanging on knife strip that is rusted; there is grease accumulation on hanging rack above prep sink; top rack in walk-in cooler is dusty; mop sink hose is laying in the bottom of the mop sink; the trash can in the women’s restroom has a broken lid and the receptacle is no longer covered; mop isn’t hung or inverted to air dry.

Glory Days Grill, 10466 Ridgefield Parkway – The following violations were reported during a follow-up inspection: there is a leak at the bar hand sink and at the mop sink faucet; sprayer attached to hose at the mop sink faucet doesn’t have the proper backflow prevention device installed.
Bistro 804, 443 North Ridge Road – The following violations were reported during a routine inspection: one can of clams has a dent affecting the top seam; temperature of cut and portioned turkey stacked in containers in walk-in cooler is elevated; temperature of chicken breast cooked this morning covered in reach-in cooler is elevated; rack in produce walk-in cooler and drain rack of drink machine are rusted; fryer baskets have bent and broken wires; gasket of lowboy reach-in freezer is torn; three cutting boards are scratched and stained; several items/areas are dusty (fan guards on meat walk-in cooler, electrical outlets and power cords on racks behind cookline, and tabletop fan above drink station); fan guards on produce walk-in cooler are covered in white grime; there is food debris on two-door lowboy cooler gaskets; there are direct plumbing connections at three-compartment sinks at bar and downstairs; outdoor dumpster doors were left open; a light is burned out upstairs and downstairs; one light shield upstairs is broken; ceiling above downstairs three-compartment sink is damaged from what appears to be water; concrete floors in upstairs and downstairs kitchen need to be repainted or resealed.
Classic Catering, 11609 Old Washington Highway – The following violations were reported during a routine inspection: didn’t observe a certified food protection manager certificate; several pieces of Tupperware are soiled; several chemical cleaners are stored above clean dishes; rodent droppings found on the floor in dish storage area (observed one trap); large bowl of potatoes are stored directly on floor under three-compartment sink; ice scoop is stored on top of ice machine which is soiled with dust; there is a leak at handwash sink; observed dust and cobwebs in dish storage area; wall above oven on right is soiled; ceiling tiles are soiled from leaks; several lights are out.

Plaza Azteca Mexican Restaurant, 12020 West Broad Street – The following violations were reported during a routine inspection: there is a buildup of pink debris on bottom of ice machine’s interior shield; temperatures of cooked ground beef and cooked shredded chicken prepared yesterday in prep unit are elevated; large knife is stored in crevice of first prep unit on the line; stack of take-out containers on shelf above prep unit are stored unprotected from contamination; small prep unit at beginning of conveyor belt oven is leaking and pooling water along the interior; one fryer basket has broken off metal pieces.
Chick-fil-A—Short Pump Town Center, 11800 West Broad Street – The following violation was reported during a routine inspection: two open drink cups found in back of kitchen.
Sheetz, 8711 Quioccasin Road – The following violations were reported during a routine inspection: cups in dispensers are stored past the fill line, allowing lip of the cup to be exposed to possible contamination; both make table cutting boards are scratched and stained; observed crumb and grease accumulation in crevices of freezer drawers.
Plentiful Foods, 1145 Gaskins Road – The following violation was reported during a routine inspection: did not observe an employee health policy.
Subway, 9127 Staples Mill Road – The following violation was reported during a routine inspection: observed dried food debris inside cold unit by serving line hand sink.

Sandston Bistro & Shake, 18 East Williamsburg Road – The following violations were reported during a routine inspection: open energy drink found near prep sink; racks in kitchen and units are rusted; freezer doors have a torn gasket; there is chipping paint on wood shelves near three-compartment sink.
Canaima Venezuelan Food, 2755 Hungary Spring Road – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; chemical cleaner is stored beside spices; lighters are stored with clean dishes; facility doesn’t have a food thermometer; two bulk containers of white powder are unlabeled; observed two dead cockroach bodies; there are gaps in the back door; reach-in cooler has a torn gasket and elevated temperatures; observed buildup of dried food debris and dust on racks throughout kitchen and storage area; there is a leak at three-compartment sink cold water handle; there is a direct plumbing connection at three-compartment sink; buildup of grease found on hood.
Pupuseria Ena, 9115 Quioccasin Road – The following violations were reported during a routine inspection: raw shell eggs are stored next to cucumbers in lowboy; dried vegetable debris found on back of deli slicer and back of blade.
West Coast Provisions, 301-A Maltby Boulevard – The following violations were reported during a follow-up inspection: observed grease and food debris on floor at cookline; observed black buildup and mold-like substance on floor drain at bar.
