Waffle House, 5414 Williamsburg Road – The following violations were reported during a routine inspection: high temp dish machine isn’t reaching the required temperature; pink mildew found on sides of ice machine; there is no discard time on eggs at room temperature; racks in units and dry storage area are rusty; observed buildup on racks and gaskets in units.

HobNob, 6010 Hermitage Road – The following violations were reported during a routine inspection: facility couldn’t provide tags for previous bags of raw mussels that had been served; temperature of three bags of blanched fries is elevated because they are sitting out on table; a container of cooked brussels are not marked with time; two employees with long facial hair aren’t wearing beard guards; in-use knife is stored in crevice of prep unit; ice cream scoop is stored in ambient temperature water; prep unit cutting boards are heavily scratched and stained brown; observed dried food debris and dust on surfaces of cables, monitors, prep shelves, wire storage racks and fan system in walk-in cooler; y-valve attached to mop sink with spray hoses doesn’t have the proper backflow prevention device installed.

Charleys Cheesesteaks and Wings, 4101 Dominion Boulevard – The following violations were reported during a routine inspection: didn’t observe a food protection manager certificate; three hand sinks don’t have hand soap; hand sink in back of facility doesn’t have paper towels; there is 0 ppm of quat sanitizer in three-compartment sink; temperature of cheese sauce in heated dispenser is too low; temperature of shredded lettuce in walk-in cooler is elevated; there is no date mark on chicken wings in walk-in cooler; two cans of Raid found on chemical shelf in back of facility; boxes of food are stored directly on walk-in freezer floor; take-out containers are not stored inverted to prevent contamination; gasket liners are torn on both doors of first prep unit on prep line and on lowboy freezer on prep line; two out of six fryer baskets have broken-off metal pieces; wire racks in walk-in cooler are covered in old food debris; y-valve attached to mop sink faucet doesn’t have the proper backflow prevention device installed; mop isn’t hung or inverted to air dry.

Perry's Steakhouse & Grille, 11788 West Broad Street – The following violations were reported during a follow-up inspection: dishwasher in bar is dispensing 0 ppm of chloride sanitizer; menu doesn’t contain as asterisk linking Koji-Aged Lamb Lollipop that is able to be served undercooked to the consumer advisory statement; bulk bins storing sugar are still unlabeled; three knives are stored in prep unit crevices; small stack of take-out containers are not stored inverted to protect from contamination; drying rack in dishwasher area is still peeling; wire racks in walk-in cooler are still dirty.

TAVA Cuisine of India, 3406 Pump Road – The following violations were reported during a routine inspection: hand sink in the bar doesn’t have paper towels; there is 0 ppm of chloride sanitizer in three-compartment sink; container of meatballs in walk-in cooler is past its expiration date; spray bottle of degreaser is unlabeled; bucket of meat is stored directly on walk-in freezer floor; cooking utensils are stored in large bucket of standing water; two stacks of take-out containers are not stored inverted to protect from contamination; gasket liners are torn on both doors of prep unit; wire rack in walk-in cooler is covered in rust; wire rack in dry storage area is peeling on bottom shelf; long cutting board on prep unit is stained and scored; two bathrooms don’t have covered trash cans; ceiling vents above prep line are surrounded by dust; mop isn’t hung or inverted to air dry.

Dunkin Donuts, 3125 Lauderdale Drive – The following violations were reported during a routine inspection: none of the three employees present in the facility would identify themselves as being the person in charge; spoons are stored in cups of standing water; drying racks above three-compartment sink are peeling and rusty; women’s restroom doesn’t have a covered trash can.

Mosaic Restaurant, 6229-A River Road – The following violations were reported during a follow-up inspection: person in charge couldn’t produce shell stock tags prior to current working lot; temperatures of several foods in two make tables and lowboy are elevated (cooked sweet potatoes, cooked onions, pimento cheese, cooked rice, and cooked vegetables); all cutting boards on the cookline are scratched and stained; racks are rusted in both walk-in units and chemical storage; gasket is torn on small lowboy on cookline; fryer baskets have bent wires; observed food debris and grime on walk-in cooler racks; there is a leak at three-compartment sink at bar and it doesn’t seem to have cold water access; floor in back storage room is not smooth and easily cleanable.

Raising Cane’s, 10093 Brook Road – The following violations were reported during a routine inspection: one restroom doesn’t have hand soap; temperatures of single-serve containers of honey mustard and cole slaw in lowboy are elevated because they are stacked too high on top of each other; two employees with long facial hair aren’t wearing beard guards; walk-in cooler racks are rusted; several items/areas are dusty and dirty (walk-in cooler fans, racks in walk-in cooler and in single-service storage area, freezer floor and shelf in drive-thru area where drink lids are stored); y-valve attached to mop sink with hose doesn’t have the proper backflow prevention device installed; observed dried food debris and grease buildup on floors and walls around and behind the cookline and the ice machines at drive-thru; floor drains throughout establishment are dirty.

Breakers Sports Bar & Grill, 9127-V West Broad Street – The following violations were reported during a routine inspection: ice was dumped into a hand sink at the non-smoking bar and there is no hand soap there; menu doesn’t contain a consumer advisory statement or asterisks linking all foods cooked to order to it; paint, rust remover and an unknown chemical are stored on shelves above beer; quat sanitizer level is too high in three-compartment sink at non-smoking bar; beverage tubing is in direct contact with ice in ice bin at waiter station; there are no test strips for quat or chlorine sanitizers; there is a limescale-like buildup inside dish machine; side of oven is greasy; dust buildup found on cords at waiter station; shelf holder in main kitchen is dirty; y-valve connected at mop sink doesn’t have the proper dual check valve installed; floors under fryers and dish machine are dirty.

Jersey Mike’s Sub Shop, 11444 West Broad Street – The following violations were reported during a routine inspection: ingredients rack storing condiments and sauces in front of facility is covered in dust; there is a direct plumbing connection underneath three-compartment sink.

The Tin Pan, 8982 Quioccasin Road – The following violations were reported during a routine inspection: dried food grime found on can opener blade; quat sanitizer tablets, medications and personal items are stored above clean dishes in dish room; multiple employees with facial hair are handling food without wearing beard guards; cutting boards at make tables are scratched and stained; walk-in cooler floor is bowing and disconnecting from each other; make table lid cover is being held together with duct tape; food debris is splattered on top of microwave; observed dust accumulation on most ceiling vents throughout kitchen, wall behind ice machine, filter of ice machine, box fan, black storage rack next to office door, shelf above make tables, receipt printer and on top of heat lamps; there is grime on handles of make tables and lowboy units; observed grease accumulation on sides of fryers, right side of grill, on legs and shelf under grill and on vent hood filters; dried food debris found on power strip above hand sink and walk-in freezer floor; hose at mop sink is laying in the bottom of the sink; there are direct plumbing connections at prep sink and three-compartment sink at bar and in kitchen; there are splashes on walls throughout kitchen; two mops are not hung or inverted to air dry; tiles around drain cover in front of walk-in cooler are cracked and broken; base board tile is missing in doorway of dry storage.

Bernie’s Sub & Pizza Shop, 415 North Ridge Road – The following violations were reported during a routine inspection: dried food debris found on back of deli slicer blade and on rim around blade; single-service items are not stored inverted to protect from contamination; racks in reach-in units are rusted; bottoms of reach-in units are dirty with food debris; grime buildup found on oven handles; floor is dirty under and around equipment and tables throughout kitchen; observed dust accumulation on ceiling near three-compartment sink; floor in center of kitchen is in disrepair with large deep pitting in tiles; light is burned out above deli slicer.


MARCH 4

Noodles & Company, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: temperatures of portioned and loose noodles in various units are elevated; single green cutting board is scratched and stained; observed grime on handles and exteriors of all units on cook lines; hanging rack above fryer is dusty; there is a leak at hand sink in front of kitchen; there are areas of low grout in floor throughout kitchen; observed debris and grime on floor under racks and units throughout kitchen.

Papa John’s Pizza, 9853 Brook Road – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; open can of soda is stored on top of pizza boxes; there is food debris on pizza slicers; two employees with facial hair are handling food without wearing beard guards; gaskets are torn at make table, walk-in cooler and sauce cups cooler; there are no test strips for quat sanitizer; light above pizza oven doesn’t have a shield; observed dried food debris on wall by mop sink, floor in front of walk-in cooler and walk-in cooler floor; there is dust buildup on walk-in cooler fans; there are leaks at hand sink next to make table and at mop sink; y-valve attached to mop sink with spray nozzle under continuous pressure doesn’t have the proper dual check valve installed.

Family Secrets Restaurant, 7103 Brook Road – The following violations were reported during a follow-up inspection: knife hanging on wall magnet is rusted; there is food splatter in two microwaves; observed fish and chicken breading on counter at an ambient temperature; menu doesn’t contain an adequate consumer advisory for steak and burgers that may be served undercooked; menu states catfish but swai is being served; observed several dead cockroach bodies and harborage conditions; dirty mats made of textured foam-like material are lining the drink glass shelves in bar area; broken microwave and broken dish machine should be repaired/replaced or removed from the facility; there are no test strips for chlorine-based sanitizers; food debris found inside make table; walk-in racks are soiled; sides of equipment on cookline need cleaning; mop sink doesn’t have the proper dual check valve installed; there is a direct plumbing connection at three-compartment sink; mops are not hung or inverted to air dry.
