Las Palmas Mexican Restaurant, 8415 West Broad Street – The following violations were reported during a routine inspection: raw chicken is stored above raw beef in grill drawers; temperatures of raw chicken in grill drawer and salsa in reach-in are elevated; some foods are dated month/day/year and some foods are dated day/month/year; two spray bottles containing chemicals are unlabeled; can of air freshener is stored over clean cutting board; shrimp is thawing in empty sink; fly trap is stored above bags of dried peppers on shelf in prep room; cutting boards throughout kitchen are scratched and stained; observed litter on ground at dumpsters; side doors of dumpster were left open.

La Michoacana Crave & Munchies, 8129 Staples Mill Road – The following violations were reported during a routine inspection: facility couldn’t provide certified food protection manager documentation, employee health policy or written procedures for cleaning up vomit and diarrhea; employee’s personal drink found in food prep area; piece of metal equipment is covering the hand wash sink; dirty knives found on magnet strip on wall; there is food debris on fruit slicer; person in charge states the facility isn’t sanitizing dishes (observed quat dispenser but it wasn’t working and is in disrepair); several bottles and food containers with liquids and powders inside are unlabeled; observed many small flies throughout the kitchen; box of bananas, cans of food, vinegar and sauces are stored directly on the floor.

Five Guys Burgers & Fries, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: employee handling food has long facial hair and isn’t wearing a beard guard; spray hose is attached at faucet near mop sink but there is no backflow prevention device installed (person in charge states it is behind wall and will need to provide documentation proving this).

Waffle House, 8400 Brook Road – The following violations were reported during a routine inspection: dish container with personal items is set on top of hand wash sink; temperatures of sausage gravy, chili and two crocks of grits are too low; temperatures of raw chicken, chicken sausage patties and butter in walk-in are elevated; walk-in door is not able to fully shut because the handle knob inside is broken; eggs are being held past their discard time; observed dried food debris in utensil drawers, bottom of reach-in unit and on gaskets of cold units.

Lakeside Moose Lodge, 1207 Hilliard Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager, an employee health policy or written procedures for cleaning up vomit and diarrhea; package of raw sausage is stored above salad dressings in reach-in cooler; observed rust and black residue buildup on ice plate of ice machine; food residue found inside microwave; bottles of liquid, bulk container of white powder and a container of salt/pepper are unlabeled; fryer baskets are heavily bent; meat slicer and other unused equipment in the back storage room should be removed from the facility; there are no test strips for quat sanitizer; several sheet pans are encrusted with grease deposits; observed dried food debris and grease buildup on exterior of fryer and grill, gaskets of reach-in units, interior of reach-in units, racks inside walk-in cooler, racks in kitchen, exterior of ice machine and inside pizza oven; there is a direct plumbing connection at three-compartment sink; there are no covered trash cans in two women’s restrooms; dumpster door was left open; observed grime under cookline stationary equipment; observed dust and grime on floor, walls and ceiling in back storage area; floor tiles and ceiling tiles are in disrepair, especially in back storage room.

d’Annam, 7501 West Broad Street – The following violations were reported during a routine inspection: raw beef is stored behind cooked ready-to-eat beef in make table; temperature of raw seed sprouts is elevated because it is sitting out on the counter in an ice bath; eye of round served raw is listed on the menu but there is no asterisk leading to the consumer advisory statement at the bottom; frozen ice wands are stored directly on walk-in freezer floor; single-service containers above make table are not protected from contamination; floor tiles are cracked near dish machine and in the middle of the kitchen; mop isn’t hung or inverted to air dry.

Olive Garden Italian Restaurant, 11829 West Broad Street – The following violations were reported during a routine inspection: hand sink near kitchen entryway doesn’t have paper towels; observed HACCP records from Jan. 7 without the proper date marked on the verifiable records portion; three out of the four fryer baskets have broken off metal pieces; bottoms of salad plate chest and soup bowl chest are dirty; dumpster area is dirty and surrounded by loose trash and debris; two light shields above prep line are cracked.

Outback Steakhouse, 7917 West Broad Street – The following violations were reported during a follow-up inspection: gasket liner is torn in first reach-in cooler; wire racks in dishwashing area are rusty; fryer baskets are broken; stack of baking sheets are covered in dark black buildup encrusted all around them; floor tiles are cracked and missing throughout kitchen.

Cook Out Store, 1501 Eastridge Road – The following violations were reported during a routine inspection: hand sink near milk shake machine doesn’t have paper towels; employee with long hair is handling food while only wearing a visor; ice scoop is stored with handle in contact with ice; single-service containers at cookline are not protected from contamination; rack in walk-in cooler is rusted; walk-in freezer door is unable to fully close because the gasket and bottom of door frame are damaged, causing ice buildup along the frame; observed dust accumulation on wall fans in dry storage; observed grease accumulation on back of monitor screens; floor throughout kitchen has low grout and cracked/missing tiles.

RVA Bakehouse, 5001 Libbie Mill East Boulevard – The following violations were reported during a routine inspection: molded strawberries found in walk-in cooler; observed dried food debris and stains on bottom shelf of dairy reach-in cooler; hose attached to mop sink doesn’t have the proper backflow prevention device installed; observed dried food debris more than 24 hours old on walls behind and next to three-compartment sink.

Spice Indian Cuisine, 1090 Virginia Center Parkway – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; employee is eating over the prep counter; employee handled cooked naan with bare hands; dried food debris found in small microwave; there are no date marks on cooked chicken, diced tomatoes, noodles and roasted cauliflower in walk-in; potato filling, hummus and cooked chicken dish made over 24 hours ago in the reach-in fridge are not date marked; facility doesn’t have a food thermometer; boxes of food are stacked directly on freezer floor; drinks and canned foods are stored directly on floor next to mop sink; pot of utensils are stored in water with a temperature that is too low; foil is lining several prep shelves throughout kitchen; a stained absorbent foam-like liner is on shelf at wait station; fryer baskets are in disrepair; facility doesn’t have test strips for quat or chlorine sanitizer; there is a leak at the hand wash sink in kitchen; dried food debris found along perimeter of kitchen throughout; observed buildup of grease on hood vents.

Zaxby’s, 4798 South Laburnum Avenue – The following violations were reported during a routine inspection: open can drink found on prep table; temperatures of eggs and cheese in prep line unit and chicken in drawers under grill are elevated; there is heavy ice buildup in standing freezer; a light is out under the hood; floors and walls in cook area and dish area are in need of cleaning; observed heavy food buildup inside units; there is grease along the cookline and prep line; observed heavy buildup on fryer baskets.

Skrimp Shack, 4760 Finlay Street – The following violations were reported during a routine inspection: employee used phone and then put on gloves to prep food items without first washing hands; observed food debris and a small dead fly on stored clean containers; temperatures of breading (with raw meat sitting in it), butter milk and hush puppy mix sitting out are elevated; multiple employees are prepping food while wearing jewelry; cutting boards are heavily stained and scratched; there are multiple heavily soiled pieces of cardboard on prep table shelves and on floor by freezer; several items need cleaning (rolling racks near dish, racks over dish and racks in walk-in); racks area is rusty; observed mildew along the outside of walk-in and freezer walls; hose with nozzle attached at mop sink doesn’t have the proper backflow prevention device installed; there is low grouting on floor near fryers; other parts of the floor are not in good condition; hood is greasy.

Krispy Delights, 3606 Nine Mile Road – The following violations were reported during a routine inspection: staff are washing hands in three-compartment sink instead of hand sink; raw shell eggs are stored over raw bacon, and raw chicken is stored over raw fish in reach-in cooler; there is dried food debris more than 24 hours old inside microwaves; breader/flour for chicken and fish is on time control but they aren’t labeled with discard times; raw animal proteins are thawing in three-compartment sink in stagnate, room temperature water; working bulk container of chicken/fish flour is stored directly on kitchen floor; food staff are not wearing hair restraints.

New Dragon City, 2220 John Rolfe Parkway – The following violations were reported during a routine inspection: temperature of garlic in oil in prep unit is elevated (person in charge states it had recently been taken out for orders and then returned); single cutting boards are heavily scratched.

Cupertino’s, 3621-A Cox Road – The following violations were reported during a routine inspection: facility doesn’t have verification of employee health training; facility couldn’t provide written procedures for cleaning up vomit and diarrhea; there is no handwash signage posted in women’s restroom; temperatures of almond milks in lowboy near display case and in self-service carafe are elevated; there are lubricants and cleaners stored above flour bins, and hand sanitizer is stored above front make table; facility doesn’t have a food thermometer; cutting board at prep cooler in back kitchen has a heavily scratched surface; broken containers are taped together; meat display gasket is torn; walk-in cooler racks and utensil wire holder are rusted; freezer chest found with heavy ice buildup; there are no test strips for chlorine or quat sanitizer; fryer wheel and gasket in meat display are dirty; there is no covered trash can in the women’s restroom; vent in walk-in cooler is dusty; observed crumbs on walk-in freezer floor; ceiling tiles are stained.
