Stella’s Grocery, 5001 Huguenot Road – The following violations were reported during a routine inspection: black mildew found on sides of ice machine; temperature of carafe of milk on counter is elevated; spray bottle of sanitizer is stored next to bottles of food dyes; water cups and plastic spoons for self-service are not stored inverted to protect from contamination; white cutting board is scratched and stained; shelf under bar three-compartment sink is rusted; left door of sliding door reach-in cooler is held together with duct tape; shelves under cash register and floor under milk rack are dirty; side doors were left open on dumpster and recycling receptacles; there is trash on ground surrounding receptacles; observed dust accumulation on ceiling tiles surrounding air vent above ovens, back wall by back door and on walk-in cooler fan guard; food debris found along back baseboard of walk-in cooler.
Trevor’s Grill and Tavern, 4106 Williamsburg Road – No violations reported during a follow-up inspection.
Jamaica Jerk Pit, 8191-K Brook Road – The following violations were reported during a routine inspection: temperature of cooked cabbage on the stove is too low; in-use spoons are stored in ambient temperature water on top of grill.
PITSA / Trending Tikka Company, 11321-B Nuckols Road – The following violations were reported during a routine inspection: person in charge is not carrying out duties relating to food safety (observed inadequate cooking, improper cooling, bare hand contact with ready-to-eat food); employee cutting lettuce has one glove on and the other bare hand is holding lettuce; no paper towels are available at either hand sink in kitchen; raw lamb is stored over a container of spinach in reach-in refrigerator; raw eggs are stored on top of a container of cooked rice; food splatter found in microwave; observed slime inside ice machine (person in charge says it isn’t used anymore); temperature of cooked chicken is too low; chicken is being parcooked; temperatures of garlic in oil (bowl on counter and bowl in refrigerator) are elevated as it appears that it is heated and placed in refrigerator after being used (keep refrigerated or use time control); temperature of potatoes in reach-in refrigerator is elevated; there is no date mark on cooked potatoes; dates on some containers are not legible or had not been changed; vape and bottle of vape juice found on metal rack at prep refrigerator; open first aid kit found on dry storage shelf above open container of uncooked rice; packet of dish detergent is next to rice; there are candles stored throughout dry storage shelves; temperature of cooked lamb in bowl is elevated almost two hours after being cooked because it was left on the counter to cool; containers of white powders and spices are unlabeled; observed mouse droppings in pans containing spices in dry storage; spices in dry storage and on cookline are uncovered; boxes of food are stored directly on walk-in freezer floor; metal can previously containing powdered milk is being reused to store spices; cardboard is lining shelves in kitchen; observed food debris and grease on non-food contact surfaces throughout the kitchen; observed grease and food debris on floors throughout kitchen, especially at cookline and near mop sink; drain in dish area is soiled.

Fairuz Café, 11254 Patterson Avenue – The following violation was reported during a routine inspection: no handwash signage is posted at front hand sink.
Beirut Bites Restaurant, 12391 Gayton Road – No violations reported during a routine inspection.
Starbucks Coffee Company, 2363 Roux Street – The following violations were reported during a routine inspection: sani bucket is stored directly on kitchen floor; facility doesn’t have an irreversible temperature indicator stickers or thermos plate on site; there is dust accumulation on door gaskets of reach-in freezer in back of house kitchen area; there are coffee stains on drive-thru area wall; there are stains on dish area walls and ceiling tile; observed dust on wall behind drive-thru register.
Mell Jay’s Bar & Grill North, 7515 Brook Road – The following violations were reported during a follow-up inspection: there is no hand sink in downstairs bar area; several items are dirty (gaskets of reach-in freezer on cookline and interior/exterior of make table on cookline); observed cracks in floor tile in front of men’s restroom door; there is a hole in the wall next to mop sink area in kitchen.

La Cabaña, 4086 Crockett Street – The following violation was reported during a routine inspection: chlorine sanitizer concentration in spray bottle is too high.
Tokyo Sushi Japanese, 10274 Staples Mill Road – The following violations were reported during a routine inspection: employee preparing cucumber for sushi rolls isn’t wearing gloves; sushi rice on time control isn’t marked with a time stamp; fish is thawing under sushi counter while still in reduced oxygen packaging; rice scoops are stored in room temperature water; cardboard is lining shelves throughout kitchen; facility couldn’t provide test strips for chlorine sanitizer at dish machine.
IHOP, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: avocados on dry storage shelf are spoiled; sanitizer concentration is at 0 ppm in dish machine because no sanitizer is connected to it and person in charge states they don’t have any buckets of sanitizer on site; sanitizer spray bottle is stored next to syrup bottles and sugar packets near host stand; salmon is thawing in reach-in cooler while still in reduced oxygen packaging; squeeze bottle of clear liquid is unlabeled; observed several flying fruit flies in dry storage area; produce boxes are stacked directly on walk-in cooler floor; server is wearing a bracelet; to-go containers at serving station and on cookline are not protected from contamination; make table cutting boards on cookline and at server station are scratched and stained; knife has a chipped tip; fryer baskets have bent wires in corners; bottom of hand sink on left side of cookline is sagging and held together with duct tape; there is major ice accumulation in walk-in freezer; floor tiles in walk-in freezer are bowing and coming up; exterior of hot water nozzle at server station is caked with white buildup; observed grime and dust accumulation on floor fan; waffle iron exterior is heavily caked with grease; 2 y-valves with spray hose connected at mop sink doesn’t have a dual check valve rated for continuous pressure installed; there is a leak at mop sink; spray hose is laying in bottom of mop sink; there is low grouting in areas throughout the kitchen; tiles are shifting in front of dish pit; there is a large gash in concrete flooring of walk-in cooler; ceiling tiles are missing sporadically throughout kitchen (person in charge states they were damaged from snow storm); strip between wall and ceiling above three-compartment sink is falling; duct tape is placed around corner edge between back of cookline and prep area.
88 Street Food, 5905 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit and diarrhea; mold-like substance found on food in walk-in cooler; raw chicken is stored over raw beef; temperature of fried tofu in steamer is too low; temperatures of shredded lettuce, tofu, edamame, cabbage and egg roll are elevated; time isn’t marked on boba on counter; chicken and egg rolls are stored directly on newspaper; several hunks of raw meat are stored directly on walk-in freezer floor; observed buildup on walk-in cooler shelves, gasket and fan guards.
Einstein Brothers Bagels, 5607 West Broad Street – The following violations were reported during a routine inspection: lox is stored over ready-to-eat tomatoes and deli meats in walk-in cooler; spray bottle of clear liquid is unlabeled; single cutting boards by three-compartment sink and right make table cutting board are scratched and stained; observed several leaks from faucet and sanitizer hose to eco lab dispenser at three-compartment sink; hose connected to y-valve at mop sink is sitting in bottom of mop sink below flood rim; y-valve connected to mop sink faucet doesn’t have a dual check valve rated for continuous pressure installed.

Hong Kong Kitchen, 2907 Williamsburg Road – The following violations were reported during a routine inspection: open bowl of employee food found on counter over prep unit; cooked chicken is in front of hand sink blocking path and should be cooling in walk-in; raw beef is stored next to cooked egg rolls and noodles in walk-in; temperature of noodles in make table is elevated because they are stacked over fill line; temperatures of chicken wings left out on counter and chick in reach-in are elevated; there are no date marks on cooked chicken or egg rolls; multiple small and large containers holding a white substance are unlabeled; food items are being stored in non-food grade grocery bags; handle of knife is wrapped in tape and there is buildup of grime on it; facility couldn’t provide test strips for bleach sanitizer; observed buildup of grime and grease on cookline, racks throughout and inside units; there is grease on the floor and walls around cookline; observed heavy buildup of grease on hood filters and nozzles of hood.
Bowlero Richmond, 6540 West Broad Street – The following violations were reported during a routine inspection: neither kitchen or bar hand sinks have paper towels; dried food debris found inside microwave in kitchen; mold found inside ice machine in kitchen; observed fruit/drain flies in kitchen and especially the bar area; stained cutting boards are heavily scored and stained; there is grease buildup on sides of flat top grill; back room reach-in freezer door gaskets are moldy and stained black; pull out reach-in cooler draws, door gaskets on reach-in cooler in kitchen and door gaskets on walk-in cooler are dirty; walk-in cooler fan guards are moldy; observed dried food debris, crumbs and food stains on top shelf over hotline hoods; there is dried food debris on and in interior/exterior of cheese and chili dispensers; light shield is missing in main kitchen area; hood filters in hood system are greasy; observed dried food stains on walls throughout kitchen; dried concentrated soda syrup found on walls behind bag in box machine in kitchen.
Auntie Anne’s Pretzels, 11800 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy; spray bottle containing a blue chemical is unlabeled; gasket liner is torn on mini refrigeration unit in front of facility; drying racks above three-compartment sink are covered in old food buildup; center ceiling vent in front of facility is covered in a layer of dust buildup; mop isn’t hung or inverted to air dry.
