Twin Hickory Tavern, 1420 North Parham Road – The following violations were reported during a routine inspection: bar hand sink doesn’t have hand soap; two cans on storage rack are dented along top and side seams; raw fish and shrimp are stored above cooked noodles and sauces in lowboy; there is brown buildup in soda nozzle at bar; sanitizer concentration in bar three-compartment sink is 0 ppm; menu doesn’t contain an asterisk next to Caesar dressing linking it to the advisory statement (dressing is made in-house with raw eggs); packaged meat is stored directly on walk-in freezer floor; single-service containers are being used as scoops for salt, pepper, sugar, etc.; single-service containers are not protected from contamination; fryer baskets have bent and broken wires; two lights in vent are burned out; right make table cutting board and small white cutting board are scratched and stained; observed dust accumulation around air vents in kitchen;.
Five Guys Burgers & Fries, 1071 Virginia Center Parkway – No violations reported during a routine inspection.
Marco’s Pizza, 9681 West Broad Street – The following violations were reported during a routine inspection: Tylenol, lotion and hand sanitizer are stored above pepper and parmesan packets at computer desk; observed a leak at three-compartment sink faucet; hand sink near walk-in cooler is slow to drain; y-connector attached to mop sink doesn’t have the proper backflow prevention device installed.

Bryan Park Bar & Grill, 5516 Lakeside Avenue – The following violations were reported during a routine inspection: observed brown and pink mildew on ice guard of ice machine; temperature of sliced cheese in make table is elevated; container of tomato paste in two-door reach-in is past its expiration date; chemicals (sanitizer tablet bottle, Tums and sanitizer spray bottle) are stored next to food and food contact surfaces; bottle of oil and shelf stable liquid butter in squeeze bottle are stored right next to each other; employee handling food with long facial hair isn’t wearing a beard guard; food handler is wearing a watch; grill spatulas and other utensils are stored in sanitizer water; single-service container is being used as a scoop in grits; make table and single cutting boards are scratched and stained; plastic spatula has chipped edges; beer storage racks and air vents are rusted; fryer basket wires are starting to bend; reach-in cooler is in disrepair; facility couldn’t provide quat sanitizer test strips; shelves above make table and table fan are dusty; grease accumulation found on sides of cookline equipment and under/behind cookline; observed grease and dust accumulation on vent hood filters; there are food splatters on top of microwave; interior and exterior joints of reach-in cooler are dirty; there are no dual check valves installed at spicket near floor drain or mop sink; there is a direct plumbing connection at three-compartment sink; observed dust and food splatters on walls and ceiling; mops are not hung or inverted to air dry; kick plate is coming off the back door; rust found on bottom of back door.
Robins Tea House at Lewis Ginter Botanical Garden, 1800 Lakeside Avenue – The following violations were reported during a routine inspection: one light is out above stove; spatula has chipped edges; observed dried food debris and grime accumulation in the freezer and hot holding cabinet; there is a direct plumbing connection at three-compartment sink; grease found on floor under stationary equipment.
Big Chile, 10184 West Broad Street – The following violations were reported during a routine inspection: raw chicken and chorizo are stored above raw fish; bulk container of sugar and squeeze bottles with oil and water are unlabeled; dried food debris found on gaskets of grill drawers.
Torchy’s Tacos, 11120 West Broad Street – The following violations were reported during a routine inspection: soda nozzle in the bar is encrusted with buildup along the interior; shelf above prep line storing take-out containers is covered in a thick layer of black buildup.
Dave’s Hot Chicken, 11525 West Broad Street – The following violation was reported during a routine inspection: there are broken off metal pieces on two fryer baskets.
Hardee’s, 1120 East Nine Mile Road – The following violations were reported during a routine inspection: dried food debris more than 24 hours old found in microwave; door gasket is torn on biscuit reach-in cooler; observed rust and corroded metal along tops and sides of self-service soda machines in dining room area; observed dried food debris, food stains, grease residue and mold on reach-in cooler and freezer interiors, exteriors and door gaskets; storage racks throughout kitchen are dirty and greasy; there is grease residue on fryers and hot holding unit; pipes along walls, ceilings and walk-in cooler fan guards, walls and ceiling are moldy; observed food debris and flour on tops and sides of ovens in kitchen; there are stained ceiling tiles throughout kitchen; ceiling tile and vents throughout kitchen are very dusty; there are dried food stains on wall behind three-compartment sink; grease residue found on floor underneath and wall behind deep fryers; there is syrup spilled on floor underneath bag and box machine; food stains found on walls throughout kitchen; mops are not hung or inverted to air dry.
Urban Taco, 5205 South Laburnum Avenue – The following violations were reported during a routine inspection: sani buckets and plastic buckets found in hand sink by the front; raw meat is stored over cut veggies in drawers under cookline; raw chicken is stored with mushrooms; temperature of queso in walk-in is elevated because it is stored in a covered deep container; temperatures of salsa, corn and chickpeas on prep line are elevated because the unit was turned off; bleach concentration in three-compartment sink is too high; chemical spray bottles are unlabeled; there are no sneeze guards or any other form of protection for taco toppings (salsa, corn, cut veggies) in self-serve unit; wooden paddle is fraying around where the handle meets the paddle; there are no test strips for bleach or quat sanitizer.
Portabella’s Restaurant, 2627 New Market Road – The following violations were reported during a routine inspection: there is 0 ppm of sanitizer in dish machine; temperatures of deli meat, lettuce and tomatoes in prep unit across from cookline are elevated; temperatures of pizza sauce and prepped salads in sliding door reach-in are elevated; observed grime and dust on dishes on shelf under microwave; there are torn gaskets on pizza prep unit and another prep unit on the cookline; walk-in racks are rusted.

Ukrop’s Market Hall, 7250 Patterson Avenue – The following violations were reported during a routine inspection: employee making pizza is wearing a bracelet; table top fans on pizza cookline are dusty; y-valve with 2 hoses is connected at mop sink without the proper dual check valve installed.
El Paso Mexican Restaurant, 3417 Cox Road – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy; bar hand sink doesn’t have paper towels; facility couldn’t provide parasite destruction letter from shrimp supplier; didn’t observe date written on old oyster tags; temperature of cooked cactus on steamer table is too low; pasta and chile relleno in walk-in cooler are past their expiration dates; menu doesn’t contain an asterisk linking steaks that can be served undercooked to the consumer advisory statement; chemical spray bottle is stored next to sugar bin; four chemical spray bottles are unlabeled; colored cutting boards are heavily scratched; floor throughout kitchen is rough and it is cracked near drains.
Jackie’s on Laburnum, 1241 North Laburnum Avenue – The following violations were reported during a routine inspection: pink mold found inside ice machine; bulk containers of seasoned flour and sugar are unlabeled; menu says catfish is being served but it’s swai; cutting boards are scored and stained; there is grease buildup on sides of hot line cooking equipment, particularly in between equipment; floor fans are dusty; observed dried food debris on bottom shelf of reach-in freezer, reach-in cooler door gaskets, and chest cooler for beer in bar area; there are leaks at prep sink faucet and underneath three-compartment sink; there are direct plumbing connections underneath prep sink and three-compartment sink; observed food stains on kitchen walls; ceiling tile is stained in kitchen; there is some mildew on wall behind three-compartment sink; bar area floor is no longer smooth and easily cleanable.

Jimmy John’s Gourmet Sandwiches, 11301 West Broad Street – The following violations were reported during a routine inspection: gasket liner on walk-in cooler door is torn; drying rack in back of kitchen is rusty; y-valve attached to mop sink faucet doesn’t have the proper backflow prevention device installed; mop isn’t hung or inverted to air dry.
Joey’s Hot Dogs, 4028-H Cox Road – The following violations were reported during a routine inspection: there is 0 ppm of quat sanitizer in three-compartment sink; gasket liner is torn on left-side door of lowboy unit on right of prep line; there is a direct plumbing connection underneath the three-compartment sink.
Taco Bell, 5811 West Broad Street – No violations reported during a routine inspection.
Sonic Drive-In Restaurant, 5805 West Broad Street – The following violation was reported during a routine inspection: temperature of ice cream mix in bottom of single-spout ice cream machine is elevated because the machine wasn’t turned on.
Touché Pies and Fries, 1221 North Laburnum Avenue – The following violations were reported during a routine inspection: observed dust and dried food debris on reach-in cooler door gaskets in kitchen; ceiling tile over oven in kitchen is stained.
