E-Loteria Botanas & Ice Cream, 9022 West Broad Street – The following violations were reported during a routine inspection: food protection manager certificate is expired; medicine bottles stored above drinks on drink shelf; gasket on lowboy in kitchen is torn.


JAN. 6

Mona Lounge, 2442 Old Brick Road – The following violations were reported during a routine inspection: there are no paper towels or hand soap at either bar hand sink; both bar hand sinks are being used as dump sinks; menu doesn’t contain an asterisk linking burger, which can be ordered undercooked, to the disclosure statement; several cleaning spray bottles are unlabeled; several pieces of salmon in the walk-in cooler are thawing while still in reduced oxygen packaging; prep and single cutting boards have deep scratches and staining; wooden flatbread boards are splitting and stained; metal wires on fryer baskets are bending; facility couldn’t provide sanitizer test kit; observed heavy dust and mildew buildup on surfaces of fan system, beer lines and walk-in cooler racks; observed grease and food buildup on prep shelves and sides and wheels of fryers and grill; there is dust buildup on ceiling vents and panels.

The Biryani Corner, 4821 Old Main Street – The following violations were reported during a routine inspection: there are items in back hand sink and equipment stored in front of it; there is no hand soap at either back or front hand sinks; temperatures of rice and chickpeas on stove are too low; temperatures of rice and heavy cream sitting out on counter are elevated; did not observe a date marking system for any prepped food; eye drops and body spray stored above open rice on prep shelf; multiple small containers of white substances, sauces and spices are not labeled; multiple live roaches found around prep sink; there is a buildup of grime on floor and ceiling.

R Town Wings, 4362 South Laburnum Avenue – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; there are no handwash signs posted in bathrooms; cooked chicken and sliced cheese are in boxes that raw chicken used to be in; raw beef stored over cheese and with veggies; there is heavy buildup on sides of breading containers; temperature of cooked wings in multiple fry baskets is too low; eggs, cooked chicken, fries and raw chicken are being held at room temperature; multiple bottles of over the counter pain meds stored on prep surfaces; raw fish is thawing on drain boards of three-compartment sink; food is stored directly on walk-in floor; food items are uncovered in walk-in; full fish are placed in units, not in a container; knives are stored between prep table and units; tongs are stored in water; fryer baskets are broken with fraying wires; racks are rusty; observed buildup on fryer, fryer baskets, cookline and racks throughout; nozzle attached on hose doesn’t have the proper backflow prevention device installed; there is heavy grease on hood filters and nozzles.

Pho Que Huong 1, 10073 Brook Road – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; person in charge was unable to verbally verify any reportable diseases; raw beef is stored above cooked pork and beef in lowboy unit; raw shrimp is stored above produce in walk-in cooler; dried food debris found on deli slicer and knives on knife strip; observed white, brown and green buildup in ice machine; temperature of chicken in make table is elevated because the metal bin is overfilled; the facility isn’t using a date marking system; pork on counter and prepared shrimp rolls in reach-in cooler are on time control but no time is marked; menu doesn’t contain an asterisk next to eye of round items; several bottles of chemicals are unlabeled; toxics are stored above food contact surfaces and single-service containers; broth is stored in old detergent buckets; concentration of bleach sanitizer in a bottle for deli slicer is too high; temperatures of cooked pork and chicken in lowboy unit are elevated because of improper cooling methods (covered in plastic wrap); frozen shrimp is thawing in an ice bath on counter; working containers of salt and sugar are unlabeled; observed harborage conditions in storage areas throughout facility (there are spiders and webs throughout storage areas); employee handling food is wearing bracelets; single-service containers are being used as scoops throughout kitchen; ice scoop handle is in contact with ice at the bar; single-service containers on cookline are not protected from contamination; siracha bottles are being used for oils and sauces on cooklines; make table and single cutting boards are scratched and stained; vent is hanging off of bar Pepsi cooler; tongs and plastic spatulas are broken and chipping; several items are dirty (inside and outside of all refrigeration units, inside of toasters, and all shelving, tops and handles of units); long hose with spray nozzle attached at mop sink doesn’t have a dual check valve installed; there is a direct plumbing connection at three-compartment sink; lid of dumpster was left open; observed food splashes on walls and ceiling tiles throughout the kitchen; one suppression nozzle is severely caked in grease; lines behind cookline are covered in grease; mop isn’t hung or inverted to air dry; there are some cracked and broken floor and baseboard tiles; observed low grouting; some ceiling tiles are water damaged; walls are damaged so that they are no longer smooth and easily cleanable; unused and unnecessary items throughout storage areas (TVs, DVD players, speakers, lawn care equipment, various tools and multiple refrigeration units) should be removed from the premises.

Zorba’s Greek-Italian Cuisine, 9068 West Broad Street – The following violations were reported during a routine inspection: temperature of large container of Greek dressing containing garlic and oil is elevated because it is sitting out on the kitchen counter; temperature of tomato sauces in steamer table is too low because it needs to be stirred; temperatures of gyro meat, diced tomato, tzatziki sauce and chicken broth in prep cooler are elevated; temperature of small container of feta cheese in ice bath is elevated.

Papa John’s Pizza, 551 East Laburnum Avenue – The following violations were reported during a routine inspection: hand sink next to pizza make table doesn’t have paper towels; dried food debris found on can opener and blade; containers of soap and sanitizer are unlabeled; sanitizer dispenser isn’t properly pulling sanitizer; sanitizer concentration at three-compartment sink isn’t at the correct level; observed food debris, stains and food residue on exterior, interior and door gaskets of pizza make table; tops of warming shelves, ceiling and walls near ceiling ventilation, and back, top and tubes behind pizza oven are dusty; walk-in cooler door gasket is moldy; walk-in cooler fan guards are dirty and dusty; there are dried food stains on walls throughout kitchen; staff personal food items are stored amongst items intended for customer use.

Farm and Oak, 3601-A Cox Road – The following violations were reported during a routine inspection: employee is drinking from an open beverage at cookline; opened beverage is stored on make table shelf; utensils are stored with dried food debris still on them; sanitizer concentration at bar dish machine is too low; temperature of grits in large bucket in walk-in cooler dated yesterday is elevated; buttermilk ranch is stored at room temperature; temperatures of several foods (cherry tomatoes, lettuce, buttermilk, salmon and pork chops) in three refrigeration units are elevated; several foods throughout the kitchen are missing date marks; various foods are marked with incorrect/old dates; onion jam, cooked lamb and short rib in walk-in cooler are past their expiration dates; brunch menu doesn’t contain a consumer advisory for eggs that can be served undercooked; cans of butane are stored in contact with utensils; temperature of large bucket of grits in walk-in cooler is elevated because of improper cooling methods (covered in plastic wrap in large container); large storage bins of similar-looking ingredients are unlabeled; there are dime-sized holes around back door knob; box of arugula is stored directly on the floor; bin of rice and bag of crumbs are stored uncovered; employee with facial hair is handling food without wearing a beard guard; knives are stored in crevices between table and unit; in-use utensils are stored in room temperature water; wet ice bucket isn’t inverted to air dry; single cutting boards throughout kitchen are scratched and stained; there is a leak in walk-in cooler; drawers in left make table are unable to close; fryer baskets have bent wires; observed damaged and chipped utensils hung above three-compartment sink; door to walk-in cooler is unable to fully close due to disrepair; top shelf of make table is dusty; sides of fryers are coated in grease; food debris found on rack in walk-in cooler; milk crates are dirty; y-valve with spray hose attached at mop sink doesn’t have a dual check valve installed; there are leaks from mop sink faucet, back hand sink, right-side hand sink, spray hose at dish machine and bar three-compartment sink; employee restroom door isn’t self-closing and the restroom doesn’t have a covered trash can; there is trash on the ground surrounding dumpsters and the doors to the dumpster were left open; lid of grease trap is open; ceiling tile is out of place above walk-in cooler; floors are severely damaged on cookline.

Sonic Drive-In Restaurant, 11250 West Broad Street – The following violations were reported during a routine inspection: chemical bottle is stored in hand sink in back of kitchen; prep unit on the line is leaking; dumpster area is dirty and surrounded by loose trash and debris; light shield above the prep line is cracked.

Los Panchos Mexican Restaurant, 3532 Pump Road – The following violations were reported during a follow-up inspection: temperatures of bulk containers of salsa and white sauce prepared yesterday in walk-in cooler are elevated; large container of shredded cheese in walk-in isn’t date marked; one wire rack in walk-in cooler is starting to peel and rust; drying shelf above dishwasher is still dirty and covered in a layer of debris; mop is again not hung or inverted to air dry.

Classic’s, 4280 Williamsburg Road – No violations reported during a follow-up inspection.

Tropical Smoothie Café, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: gaskets on units are torn; top door of unit is in disrepair; blender lids are torn/worn; observed food and grime buildup in gaskets of units, wheels and along tops of units (especially smoothie unit); there is buildup on racks throughout; spray nozzle is hanging into prep sink; floors, walls and ceiling are dirty.
