Taziki’s Mediterranean Café, 4024-C Cox Road – The following violations were reported during a routine inspection: hand sink on the cookline doesn’t have paper towels; there is no preparation date marked on large container of pasta in walk-in cooler; spray bottle containing a brownish chemical is unlabeled; gasket liner is torn on left-side door of prep unit at end of cookline; wire rack in dry storage/prep area is beginning to peel; drying shelf across from utility sink is heavily rusted; mop isn’t hung or inverted to air dry; light shield is missing in back prep area of kitchen.
McDonald's, 1778 North Parham Road – The following violations were reported during a routine inspection: temperature of gravy in walk-in cooler is elevated; temperatures of chicken filets and fish filets in hot holding unit are too low; spray bottle of degreaser is stored on top soda box; observed improper cooling methods for gravy and McGriddles in walk-in cooler (both are covered with plastic wrap with condensation on the top); fryer baskets have bent wires at corners; dumpster doors were left open; wet mops are not hung or inverted to air dry.
Sushi Masa, 8087 West Broad Street – The following violations were reported during a routine inspection: employee drinks are stored in prep units throughout the sushi bar and on prep tables in kitchen; several employees touched face, phones and hat with gloved hands and then continued with food prep without washing hands and changing gloves; utensil cleaning equipment found in sushi bar hand sink; raw shell eggs are stored above lettuce in walk-in cooler; employee failed to sanitize rice pots after washing and rinsing; large pots by back prep area are stored with dried food debris inside; temperatures of rice at sushi bar and shrimp tempura and sweet potato tempura under heat lamp are too low; did not observe a date marking system in the facility; cooked noodles and vegetables made the day prior are not date marked; several bottles of lye and other chemical cleaning solutions are stored on floors and prep shelves with utensils; sanitizer concentration of chlorine at dish machine is too high; fish in walk-in cooler is thawing while still in reduced oxygen packaging; observed mouse dropping behind water heater; food is stored directly on walk-in freezer floor; several employees engaged in food preparation are wearing watches; handle of ice scoop is stored inside ice bin; rice scoop is stored in water; utensils are not stored inverted at several stations; a single-service jug for scooping and mixing rice has been cut and reused; wood used inside back prep sink and under prewash station at dish machine is unsealed; ice bin in sushi bar is missing a lid; several cutting boards have heavy scratches and stains; observed grease on outside of cookline equipment; there is dried food debris on wire shelves in dry storage area; stickers are left on containers in walk-in cooler; there is a leak underneath three-compartment sink; all dumpster lids were left open and there is litter around the dumpster area; ceiling and floor tiles throughout the facility are cracked and in disrepair; observed low grouting; floor inside walk-in freezer is lifted and becoming unsealed; floor drains are dirty; there is grime underneath cookline; food debris found on floor in dry storage.

Taco Bell, 9420 West Broad Street – The following violations were reported during a routine inspection: observed dried food debris on three scoops and one pair of tongs hung up at three-compartment sink; y-connector attached to mop sink doesn’t have the proper backflow prevention device installed.
Weinstein Jewish Community Center, 5403 Monument Avenue – The following violations were reported during a routine inspection: raw salmon is stored above ready-to-eat foods in reach-in cooler; deli slicer is stored with dried food debris on it; various foods throughout kitchen (cut tomato, pasta, stew, etc.) are not date marked; stew stored in three-door reach-in cooler is uncovered; lights are burned out in vent hood and above two-door reach-in freezer; cutting boards on back of three-compartment sink are scratched and stained; observed grease accumulation on sides of grill and stove; observed accumulation of dried food splashes on stand mixers; food splatters found on walls and ceiling around stand mixer; back of hand sink is dirty; y-valve attached to mop sink doesn’t have a dual check valve installed; paint is peeling on wall behind spice rack; wet mop isn’t hung or inverted to air dry.

Napoli Pizza, 3324 Pump Road – The following violations were reported during a routine inspection: raw chicken and beef stored above lettuce and other raw vegetables in reach-in cooler; temperature of ham in pizza make table is elevated; sliced deli meats, cheeses and pizza sauce are not date marked; employees working with food are not wearing proper facial hair restraints; person in charge couldn’t provide test strips for chlorine sanitizer; observed dried food debris on surfaces of spice racks, under grill and racks inside prep units; observed food debris and grease buildup on floor under fryer and grill and under three-compartment sinks.
Caffespresso, 1127 Gaskins Road – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; yellow buildup found in ice machine; temperatures of heavy cream, oat milk, soy milk and almond milk in lowboy cooler are elevated; temperature of gallon of milk on top of lowboy cooler is elevated; ibuprofen and super glue are stored above tea on shelf.
Jimmy John’s Gourmet Sandwiches, 9018 Staples Mill Road – The following violations were reported during a routine inspection: employee wiped knife on apron before cutting a pickle; hand sink is dirty and the seal is in disrepair; vent in walk-in cooler is dusty.
Hunan Café, 9117 Staples Mill Road – The following violations were reported during a routine inspection: employee preparing food wiped nose and then washed hands for less than 10 seconds; kitchen hand sink doesn’t have hand soap; pink residue found inside ice machine; temperature of recently prepped raw chicken stacked on top of containers in prep unit is elevated; several containers of egg rolls, cooked noodles and fried chicken are not date marked; several bulk bins of flour, sugar, salt and other spices are unlabeled; cut raw broccoli in walk-in cooler was unwashed, per person in charge; observed grease and food debris on grill; there is grime on handles and exterior of cold holding units; wire racks are rusty with dried food debris on them; dried food debris found inside freezers; some floor tiles are cracked; hand sink needs to be resealed to the wall; observed grease and dried food debris on floors, especially behind cookline.
Kabob Place, 8906-G West Broad Street – The following violations were reported during a routine inspection: employee is plating naan with bare hands; buildup of food debris found on blade of slicer; temperature of rice in hot holding unit is too low; pot of chickpeas is stored directly on the floor; there are no coverings on items in the walk-in; large catering containers are uninverted and uncovered by the oven; observed grime and debris on racks throughout and along the cookline; observed chipping paint, low grouting and missing tiles around the three-compartment sink; floor, walls and ceilings need cleaning; there is grease on hood filters and hoses.
Heavenly Burgers, 608 East Nine Mile Road – The following violations were reported during a routine inspection: there is no certified food protection manager on staff; dried food debris found in microwave; there is rust on grease trap lid and sides underneath three-compartment sink (grease trap is leaking water onto floor when three-compartment sink drains); observed dead small flies/gnats inside reach-in freezers; there are dried food stains on kitchen ceiling and walls; floor tiles underneath grease trap in kitchen are damaged.

Taco Bell, 11242 Patterson Avenue – The following violations were reported during a routine inspection: unprotected single-service items are stored on greasy surfaces on prep line; observed grease residue and food debris buildup on surfaces of grill presses, tops of holding equipment, dry storage racks and prep storage shelves; there are leaks under three-compartment sink, at hot water handle on three-compartment sink and at water filtration system; two lights are out in hood.
West Lake Chinese Restaurant, 11256 Patterson Avenue – The following violations were reported during a routine inspection: trays of eggrolls prepared yesterday in walk-in cooler are not date marked; time isn’t marked for cabbage and sprouts held on counter; similar looking spices on cart are unlabeled; trays of eggrolls in walk-in cooler are stored uncovered under fan system; bags of meat are stored directly on walk-in freezer floor; cloth towel is covering/touching cooked rice in walk-in cooler; single-service cups are being used as scoops for uncooked and cooked rice; single cutting boards are heavily scratched and stained; observed buildup of dried food debris, grease and dust on surfaces of wire racks, soup well, prep shelves, rice warmers, and fans and shelf liners/racks in walk-in cooler; observed buildup of grease and food debris on wok backsplash, water faucets, broiler and grill top; there is a leak at handles on front hand sink and the cold water handle doesn’t work; there is an accumulation of broken and unused equipment in the kitchen that needs to be removed; observed heavy buildup of food debris, dust and grease residue on walls and floors throughout kitchen.
McDonald's, 11280 Patterson Avenue – The following violations were reported during a routine inspection: fries touched an employee’s hands as she was putting them in the carton; temperature of sausage gravy in walk-in cooler is elevated (container was covered while cooling); employee handling food isn’t wearing a hair restraint; blender container is scored inside and no longer smooth and easily cleanable.
