Tres Machos, 2313 Westwood Avenue – The following violations were reported during a routine inspection: no handwash signage is posted at front of house hand sink; moldy jalapenos found in walk-in cooler; observed dried food debris on vegetable chopper; temperature of pan of rice is too low because the pan is filled above the fill line; multiple foods in reach-in cooler (beef and black beans) and walk-in cooler (tomato, cheese, pico) are elevated; containers of raw shrimp are stored uncovered in reach-in freezer and chest freezer; food is stored directly on walk-in cooler floor; employee is wearing a bracelet; deep fryer basket and ice bucket are in disrepair; facility doesn’t have quat sanitizer test strips; there is grease residue on sides of hot line cooking equipment; observed dried food debris on reach-in cooler door gaskets throughout kitchen; wire storage racks in walk-in cooler, chest freezer door gaskets and beer cooler door gaskets are dirty; there is a syrup spill on floor under bag in box machine; wall behind hot line is greasy; front of house ceiling vent is dusty; there are cracked floor tiles throughout kitchen; ceiling tile in front of house area is stained.

Wendy’s, 10177 Brook Road – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning up vomit and diarrhea; one hand sink in kitchen doesn’t have paper towels; temperatures of burgers, fries and queso in hot holding units are too low; temperatures of Swiss cheese and American cheese are elevated; employee with long hair isn’t wearing a hair restraint; employees are wearing bracelets; there is ice buildup in reach-in unit at drive-thru (not currently in use); gasket of walk-in freezer is torn and ice buildup is present; racks in walk-in are rusty; y-valve attached to mop sink faucet doesn’t have the proper backflow prevention device installed; there is heavy grease on the ground throughout dumpster pad; observed grease on floor under fryer; there is dust buildup on walk-in fan; there are missing and chipped floor tiles under fryer.

Qdoba, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: observed food debris on “clean” containers on drying racks; temperature of chicken on the line is too low; employees are wearing bracelets; racks in units and dish area are rusted; there is a leak at back hand sink.

Chipotle Mexican Grill, 4930 West Broad Street – The following violations were reported during a routine inspection: working container of salt near hot line is unlabeled; cutting boards are scored; observed dried food debris on reach-in cooler door gaskets on hot line; walk-in cooler fan guards are dusty; there are dried food stains on walk-in cooler storage racks; observed dust buildup on ceiling vent in kitchen.

Starbucks Coffee Company, 7000 Forest Avenue – The following violations were reported during a routine inspection: single-service cups are stored above the protection line; facility doesn’t have a temperature measuring device for the dish machine; lids and doors of outdoor dumpsters were left open (receptacles are shared with other tenants); wall behind ice machine is dusty.

Trevor’s Grill and Tavern, 4106 Williamsburg Road – The following violations were reported during a routine inspection: drinking cup without lid found on prep table; no hot water available at kitchen hand sink; raw ground beef patties are stored above ready-to-eat sauces in reach-in unit; slicer stored in back of facility is covered in debris; there is 0 ppm of chloride sanitizer in both main kitchen dishwasher and bar dishwasher; person in charge states that they are using non-continuous cooking techniques for chicken wings but a written procedure needs to be submitted to the inspection office to do this; temperatures of sliced tomatoes, sliced ham and raw chicken are elevated in prep unit in left-side of kitchen; raw meats are being thawed in standing water; person in charge with long facial hair isn’t wearing a beard guard; utensils are being stored in standing water next to grill; long cutting boards on prep units are heavily scored and stained; facility couldn’t provide chloride test strips; observed grease buildup along the cook line, under cooking equipment and behind cook line; used mop isn’t hung or inverted to air dry.

Pho Thien Phat 2, 9031 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; observed employee touch raw meat and then touch lettuce without washing hands; observed an employee touch dirty dishes and then touch clean dishes without washing hands; observed employees not using hand soap when washing hands; observed employees washing hands in sinks other than hand sinks; staff bathroom doesn’t have hand soap; there are no paper towels at any hand sinks; hand sink is blocked in bathroom outside kitchen; observed straw, food debris and a rag in hand sink near dish machine; there is 0 ppm chlorine in dish machine; tea pitchers are stained; there is food debris on two “clean” colanders; temperature of boba in warmer is too low; temperatures of shrimp, sausage and broth in prep cooler are elevated; lotion and cleaner are stored near food; observed improper cooling methods for cooked meats, tofu and noodles in walk-in cooler (cooling in deep, covered containers); beef broth is thawing in a bucket on kitchen floor; single-service containers are being used as scoops in bulk rice bins; prep cooler and walk-in cooler gaskets are torn; shelves in reach-in cooler are cracked; shelves in walk-in cooler are rusty; several items/areas are dirty (refrigerator gaskets, walk-in shelves, shelves in dry storage room, exterior of garbage bins in kitchen, walls around walk-in freezer and staff bathroom, floor and pipes behind cook equipment, hallway floor); walls and ceiling near grill are stained; there are cracks and gaps in floor on cook line, near dish machine and back door; screen door is torn; walk-in freezer door is bent; there are holes in walls in hallway; there is clutter in bathrooms outside kitchen; unused equipment in kitchen should be removed; two mops are not hung or inverted to air dry.

Tropical Smoothie Café, 1513 North Parham Road – The following violations were reported during a routine inspection: observed grime buildup on exterior of tortilla holders, floor under make tables and next to dish machine, under three-compartment sink by baseboard and on walk-in cooler and freezer floors; single-service cups are being used as scoops in protein powder buckets; person in charge says non-functioning dish machine in kitchen hasn’t worked for years (repair or remove); wet mop isn’t hung or inverted to air dry.

Pizza Hut, 9189-B Staples Mill Road – The following violations were reported during a routine inspection: scoop with handle is stored inside container of cheese in prep unit; several fryer baskets are in disrepair; observed dried food debris in bottom of rolling carts and cold holding units; there is mold-like buildup on floor drains.

Burger King, 5108 Nine Mile Road – The following violations were reported during a routine inspection: food staff aren’t wearing hair restraints; there is a torn door gasket on reach-in cooler by front door; reach-in cooler door gaskets are dirty; interior and door gaskets of reach-in freezer have food debris inside/on them; there is dried food debris on wire storage racks in walk-in cooler; y-valve on mop sink doesn’t have the proper backflow prevention device installed; ceiling tile near ventilation is dusty; there are dried food stains on drive-thru wall.

Rare Olde Times, 10602 Patterson Avenue – The following violations were reported during a routine inspection: sanitizer concentration in bar dish machine is 0 ppm after several runs; metal wires are breaking on fryer baskets; observed buildup of food debris and grease on surfaces of microwaves, wire cart, oven and broiler system, fire suppression nozzles, walk-in cooler fan system and inside keg coolers; observed food debris and staining on walls and floors under equipment; ceiling vent above prep line is dusty.

First Watch Restaurant, 11721 West Broad Street – The following violation was reported during a routine inspection: drying racks across from three-compartment sink are beginning to rust.

McDonald's, 8060 West Broad Street – The following violations were reported during a routine inspection: temperature of fries under heat lamp is too low; observed dried food debris and residue on gaskets of cold-holding units; y-valve connected to mop sink faucet doesn’t have the proper backflow prevention device installed.

McDonald’s, 7527 Staples Mill Road – The following violations were reported during a routine inspection: open bags of chips are stored with clean dishes next to three-compartment sink; observed dried food debris and residue on gaskets of cold holding units; hose connecting sanitizer at three-compartment sink has a gash which is causing a leak; there is heavy buildup on floor drain under three-compartment sink; wall behind three-compartment sink has black residue on it.

Potro’s Mexican Restaurant, 9074 Staples Mill Road – The following violations were reported during a routine inspection: facility doesn’t have parasite destruction statement from supplier for fish served raw and undercooked; facility is keeping oyster tags without recording date when the last oyster was sold; temperature of top half of rice in steam well is too low.

Pho New York, 6420 Horsepen Road – The following violations were reported during a follow-up inspection: person in charge is unable to demonstrate proof of food safety knowledge or produce proof of food protection manager certification; deli slicer was stored with dried food debris all over it, including on blade; several cleaning agents are stored next to dry ingredients; various raw meats are thawing at room temperature; observed rodent droppings on floor under hand sink at bar; frozen meat is stored in bowl and box directly on floor; there are bent wires on fryer baskets; make table and single cutting boards are heavily scratched and stained; facility couldn’t provide test strips for dish machine; top shelf of make table is dusty; inside of microwave is dirty with food splashes; observed interiors, racks and gaskets of reach-in units dirty with food debris, food spills and what appears to be mold; there is a direct plumbing connection at three-compartment sink; outdoor dumpster lid was left open; air vents above reach-in coolers and large pipe to vent hood are dusty; there are splashes on walls throughout kitchen; observed food debris and grime on floor under units; observed dust, grease and grime buildup on plumbing lines; there are cracked tiles and holes in the walls throughout facility; there is potential water damage in ceiling of storage room (ceiling is bulging and paint is torn); several mops are not hung or inverted to air dry.

Hardee’s, 4841 South Laburnum Avenue – The following violations were reported during a routine inspection: employee cleaned equipment and then put on gloves to prep food without washing hands first; there is no hot water at front hand sink; there is 0 ppm of sanitizer in three-compartment sink; temperature of chicken tenders is too low; several items/areas are dirty (racks, exteriors of units, and around fryers and grill top); observed grease and grime buildup on floors and walls; observed dust and mold on ceiling tiles.
