Brazil Tropical Steakhouse, 9035 Quioccasin Road – The following violations were reported during a routine inspection: dried food debris found on deli slicer; facility isn’t using an approved sanitizer to sanitize all food contact surfaces; no date marks found on deli meats and cheeses; spray bottle of cleaner is unlabeled; knives are stored in between metal strip and wall; ice scoop is stored with handle touching ice; there is a direct plumbing connection at three-compartment sink in main kitchen; sliding door in front of storage closet is falling off the hinge and the edge is not smooth and easily cleanable.

Dunkin Donuts, 2300 Hungary Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; gray and pink buildup found in ice machine; quat sanitizer concentration in three-compartment sink is too high; y-connector on mop sink doesn’t have the proper backflow prevention device installed.

Daddio’s Grille, 12385 Gayton Road – The following violations were reported during a routine inspection: no managers or supervisors are a certified food protection manager; observed small dead flies and grime buildup inside soda gun holder at non-smoking side of bar; there is a small fly infestation at both bar areas; prep cutting boards are heavily stained and scratched; observed buildup of dust, food debris and grime on surfaces of wire storage racks, portable fans, blue dish rack, knife strip, rubber glass mats, inside keg coolers and on fan system in walk-in cooler; women’s restroom doesn’t have a covered trash can; floor in walk-in isn’t sealed; observed trash and grime on floors in bar areas; there is dust buildup on side wall near storage racks.

Deca’s Café, 391 Azalea Avenue – The following violations were reported during a routine inspection: there are no date marks on deli meat and cheese in reach-in cooler and walk-in cooler; quat sanitizer concentration at three-compartment sink is too high; cutting boards are scored; facility doesn’t have an irreversible temperature indicator for the dish machine; reach-in cooler and walk-in freezer door gaskets are dirty and stained black; dry storage wire racks have dried food debris/dust on them; walk-in cooler fan guards are dusty; outdoor dumpster doors were left open; there is damaged floor tile in dry storage area; observed dust on ceiling tiles near ventilation.

Popeyes Louisiana Kitchen, 4402 South Laburnum Avenue – The following violations were reported during a routine inspection: there is ice buildup on fans of freezer and along door causing water to pool at the door; observed buildup of grime along equipment of breading station, racks in units and fans, racks near drive-thru and on handles of scoops in flour/breading; observed grease on sides of fryers; floors around breading station, fryers and drive-thru are dirty; there is low grouting throughout kitchen.

CAVA, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: two cans of cucumbers in brine have dents along edge and side seams; walk-in cooler fan is dusty; observed food debris and grime on condensor and surrounding surfaces under front line; faucet is able to move and disconnect from base at hand sink behind front line; dumpster lid is broken and falling into the dumpster; wall behind two-door reach-in cooler is dusty.

McDonald’s, 5520 West Broad Street – The following violations were reported during a routine inspection: dried food debris found inside microwave; temperature of hamburgers in walk-in cooler is elevated (walk-in thermostat is elevated too); sliced cheese on time control doesn’t have the discard time indicated; staff are handling food without wearing hair restraints; sanibucket is stored directly on kitchen floor; there is food residue on wire storage racks in walk-in cooler; there is heavy grease residue on sides of burger grills; racks next to ice cream machine are dirty; speed racks have food debris on them; observed stained ceiling tile in dining room area; mops are not hung or inverted to air dry; observed grease residue on ventilation hoods throughout kitchen.

The Patron Mexican Restaurant & Cantina, 5990 Audubon Drive – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit and diarrhea; there is 0 ppm of sanitizer in dish machine; temperature of red and white salsa is elevated; multiple chemical spray bottles in kitchen and bar are unlabeled; sanitizer concentration in sanibucket is too high; meat is thawing at room temperature; bulk containers of white substances are unlabeled; there are gaps on the side and below patio door that leads into kitchen; there are multiple torn gaskets on units along the cook line; racks throughout are rusted; leg of dry storage rack is broken; observed ice buildup on freezer and salsa lowboy; door of ice machine is cracked; there is mold buildup on racks in drink walk-in and regular walk-in; some floor tiles are broken and cracked; observed low grouting throughout; ceiling vents are stained and rusted; observed grease on floors and coving tiles around fryers.

Tarrants West, 11129 Three Chopt Road – The following violations were reported during a routine inspection: hand sink near dish machine is blocked; water in women’s bathroom doesn’t reach the required temperature; raw chicken is stored above spinach in prep cooler; there is thick, black buildup inside two soda gun nozzles at bar; temperature of rice in warmer is inconsistent and should be stirred more often; two tubs of cooked wings in walk-in are past their expiration dates (person in charge states they were just removed from freezer); there are hunks of meat in raw walk-in without date marks; container of breadcrumbs and container of polenta in dry storage area are uncovered; lettuce storage bucket lid is heavily torn/melted and there is a large hole in the top; dumpster covers were left open; beer cooling unit and cables above cook line are coated in heavy dust buildup.

The Big Scoop, 200 Towne Center West Boulevard – The following violation was reported during a routine inspection: there is 0 ppm of chloride sanitizer being dispensed from the dishwasher.

Killa Dillas, 6114 Lakeside Avenue – The following violations were reported during a routine inspection: diced tomatoes are tightly covered during the cooling process in reach-in cooler; observed dried food debris on floors, front and back of establishment; there is food splatter on walls in back prep; floors are not sealed, smooth and easily cleanable (painted surface is worn down to cement floor).

Gogi bibimbap, 8902-A West Broad Street – The following violations were reported during a routine inspection: raw and cooked eggs are being stored out of temperature without time being tracked; sanitizer concentration is too high; facility is making its own kimchi by leaving it out for 24 hours (facility needs variance to keep TCS items out of temperature or for longer than seven days).


NOV. 17

Dunkin Donuts, 4869 Nine Mile Road – The following violations were reported during a routine inspection: there is no person in charge on staff right now; no handwash signage is posted in men’s restroom; there are torn door gaskets on walk-in cooler, walk-in freezer and reach-in cooler; observed dried food debris and black stains on cooler door gaskets throughout kitchen.

Buttermilk and Honey, 12246 West Broad Street – The following violations were reported during a follow-up inspection: hand sink in back of kitchen doesn’t have hand soap; take-out containers stored on prep line are still uninverted; some of the walk-in cooler racks are still covered in rust spots.

Panera Bread, 11350 Nuckols Road – The following violations were reported during a routine inspection: scooping utensil with handles is stored inside container of peppers; observed dried food on wall behind soup station and on floors under equipment; there is water pooling under proofing cabinet.

MOD Pizza, 5466 Wyndham Forest Drive – The following violations were reported during a routine inspection: facility doesn’t have a current certified food protection manager; facility doesn’t have a thermometer to measure temperature of water in high-heat dish machine; y-connector attached to nozzle at mop sink doesn’t have the proper backflow prevention device installed.

Chuck E Cheese, 9030 West Broad Street – The following violation was reported during a routine inspection: water in restroom doesn’t reach the required temperature.
