Desi Bites, 3621-C Cox Road – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; hand sink in front prep area is blocked with dirty dishes; women’s restroom doesn’t have paper towels or a covered trash can; raw lamb is stored above cooked rice and meat; dried food debris found on blender parts and knives; temperature of cooked rice sitting out on counter is too low (person in charge states it is on time control but no time is marked); temperatures of veggie slaw, noodles, grilled chicken, fried chicken and rice in prep cooler are elevated; there are no date marks on cooked potatoes, rice or chickpeas; flour bins in dry storage area are unlabeled; employees are wearing watches while prepping food; plastic forks are not stored inverted in self-service area; lowboy gasket in front prep area is torn; several items are dirty (cooler gaskets, exterior of food storage crates in veggie walk-in cooler, walls throughout kitchen, and bottom of lowboy in front prep area); there is a leak at front prep area hand sink; dumpsters and grease trap are uncovered; observed heavy grease buildup on grease trap; wall behind oven is dusty; there is low grout on tile flooring.
Hangry Joe’s Hot Chicken, 1501 North Parham Road – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certification; both paper towel dispensers in kitchen are not working; discarded gloves found in hand sink near fryers; an uncovered bin of salt has black debris in it; whole shell eggs are stored above uncovered buckets of sauces in walk-in cooler; observed dried food and grease debris in air fryer; observed what appears to be green mildew building up in ice dispenser at drink machine; there is white buildup inside hot water dispenser nozzle; tubs of mac and cheese in lowboy unit are not date marked; buckets of sauces, rice, flour and pickles are stored directly on floor throughout kitchen and in walk-in cooler; buckets of sauces in walk-in cooler are stored without lids; single-service containers and bowls without handles are being used as scoops in foods throughout kitchen; ice cream and rice scoops are stored in room temperature water; all fryer baskets have bent and broken wires; two lights are burned out in vent hood; bottom rack of chemical storage rack is rusted; knife on strip has a broken tip; styrofoam pad under deep freezer is coming apart and coated in black grime; cutting board at make table is scratched and stained; facility doesn’t have test strips for three-compartment sink or sanitizer buckets; several items in kitchen are covered in food debris (hinges of chicken make table, cart and casters of Ninja air fryer, and walk-in cooler racks); observed grease accumulation on sides, tops and backs of fryers and all surfaces of the burner; shelf above hand sink near fryers and shelf of make table are covered in dust; hose installed at mop sink is hanging below flood rim level; there is a direct plumbing connection at three-compartment sink; outdoor dumpster lid was left open; there is trash on the ground surrounding dumpster; two mops are not hung or inverted to air dry; observed dirt, dust and food debris on floor and wall in between walk-in cooler and wall; there is food debris on floor in walk-in cooler, especially under racks; observed black grime on floor under register counter and milkshake make table; floor drain next to mop sink is mostly clogged with food debris and grime; wall corner coving is missing throughout kitchen; wall cover is peeling away from the wall; baseboard tile is missing next to walk-in cooler; four drilled holes in wall next to walk-in cooler are uncovered.

Five Guys Burgers and Fries, 4450-A South Laburnum Avenue – No violations reported during a routine inspection.
Marco’s Pizza, 4450 South Laburnum Avenue – The following violations were reported during a routine inspection: gaskets are torn; racks in dry storage are rusted; small lowboy is not cold-holding food at the proper temperature; containers are cracked; nozzle attached to mop sink hose doesn’t have the proper backflow prevention device installed.
Crafted, 4900 Libbie Mill East Boulevard – The following violations were reported during a follow-up inspection: there is no certified food protection manager on staff; raw meats are stored above cooked and pre-portioned chicken wings in walk-in cooler; mold found inside ice machine; observed fruit/drain flies throughout kitchen, especially on three-compartment sink wall/corner; food staff in kitchen aren’t wearing hair restraints; various plates are stored uninverted on top shelf of hot line expo counter; cutting boards are heavily scored, stained and melted; wire storage racks in walk-in cooler, dry storage area and bar area are rusty; reach-in cooler in front of grill, smoker on hot line and multiple deep fryer baskets are in disrepair; there is water pooling in the bottom of the reach-in cooler in front of deep fryers; multiple lights are out in hood system; observed mold, mildew and food debris on reach-in cooler and freezer door gaskets throughout facility; there are mold and black stains on walk-in cooler door gaskets; observed food debris and dust on wire storage racks throughout kitchen; walk-in cooler storage racks are dirty and moldy; there is grease buildup on sides of hot line equipment, especially in between equipment; there is dust on walk-in cooler fans and ceiling; shelf under hot holding steam table is greasy; bar reach-in cooler door gaskets are dirty and the exterior is moldy; there is cracked floor coving underneath dish machine; wall and door frame next to mop sink are damaged; observed grease residue on walls behind hot line; floor drains throughout kitchen are dirty; wall behind three-compartment sink has grease residue, food debris and flies on it; observed mildew on walls around dish machine area.
Tous Les Jours Bakery Co., 7801 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy; there is no written policy for pastry held on time control in display case and no time stamp is written.
The Pho Spot, 7801 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit and diarrhea; hand sink is blocked and filled with dirty dishes; temperatures of bean sprouts and cut greens in ice bath are elevated; bean sprouts, cooked noodles and raw shrimp are being held on time control but there is no policy or time stamps; rice scoop handle is touching rice; several foods are stored in non-food grade plastic to-go bags throughout cold holding units; observed food debris on floors under units; there is grime inside hand sink.
Fogo de Chao, 11221 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide verification that employees have been trained on reporting requirements; bartender handled orange garnish with bare hands; hand sink in employee restroom doesn’t have hand soap; dish machine isn’t reaching the required plate surface temperature; temperature of beef ribs in warming unit on front low is too low; temperature of cooked lobster in prep unit is elevated; several containers of prepared foods are incorrectly date marked; salmon is thawing while still in reduced oxygen packaging in prep unit; employees working the grill are wearing watches; metal wires are broken on fryer baskets; handle on reach-in cooler is duct taped together; butcher block cutting boards on grill line are splitting apart; two hoses are attached to y-connector at mop sink; employee restroom doesn’t have a covered trash can; bulbs under hood system are missing protective shields.

Hungry Eyes Perlas, 201 Towne Center West Boulevard – The following violations were reported during a routine inspection: cup found in hand sink in bar; no hand soap or paper towels are available at bar hand sink; dishwasher is recording 0 ppm of sanitizer; temperatures of diced chicken, pasta, gyro meat and shredded cheese prepared yesterday in the walk-in cooler are elevated; temperature meat sauce in dry well is too low; temperatures of tomatoes, sliced salami and chopped lettuce in prep unit are elevated; spray bottle containing a blue chemical is unlabeled; gasket liners are torn on both doors of prep unit and on walk-in cooler door.
Bubba’s Bar & Grill, 4000 Williamsburg Road – No violations reported during a follow-up inspection.
Franco’s Italian Restaurant, 9010 Staples Mill Road – No violations reported during a follow-up inspection.
Lizzy’s African Restaurant, 5310 Chamberlayne Road – The following violations were reported during a routine inspection: raw shell eggs are stored above ready-to-eat foods in reach-in cooler; temperature of cole slaw on serving line is elevated because the ice packs keeping it cool were not in direct contact with the pan; can of pesticide found stored in kitchen; food is stored directly on the kitchen floor; food staff are wearing watch/bracelet; cutting boards are scored and stained; dried/frozen blood found on bottom shelf of reach-in freezer; there are dried food stains on kitchen walls; ceiling ventilation is dusty.

Sandston Moose Family Center, 4505 Oakleys Lane – The following violations were reported during a routine inspection: did not observe dates on items in walk-in or reach-in (burgers, mac and cheese, lettuce, tomatoes, etc.); observed buildup on standing fan and racks in dry storage area.

Cracker Barrel, 4435 South Laburnum Avenue – The following violations were reported during a routine inspection: person in charge couldn’t provide verification that employees have been trained on illness reporting requirements; dried food debris found on slicer not in use; temperatures of raw catfish and raw chicken in left grill drawer are elevated; observed small flies throughout kitchen; employee on cook line is wearing a watch; several employees preparing food are not wearing hair or beard guards; dessert cabinet door is unable to close; two small units are not working; fryer baskets are in disrepair; cutting boards are stained and scratched; observed dried food debris in most cold holding units; there is a buildup of grime on reach-in gaskets; monitors and cables have heavy dust buildup on them; observed grease and grime between cook line equipment; dried food found on racks throughout kitchen; ABV cap is missing from faucet near dish rack in dish room; general cleaning is needed throughout facility, especially underneath stationary cook line equipment (buildup of dried food debris and grease) and on walls and floors in dish room (buildup of food debris and water); floor drains are dirty with debris; ceiling tile is missing and there is mold-like residue on other ceiling tiles in dish room.
Rashida Bakes Café, 2810 North Parham Road – The following violations were reported during a follow-up inspection: person in charge couldn’t provide food protection manager certification; menu states eggs can be cooked to order but there is no consumer advisory posted; there is a direct plumbing connection at three-compartment sink; two lights are burned out behind vent hood.
