Popeyes Louisiana Kitchen, 8121 Brook Road – The following violations were reported during a routine inspection: hand sink near breading station doesn’t have paper towels; there is flour residue on pans stored clean; single-service cups are stored on a dirty shelf; biscuit pans are dented and heavily pitted; observed buildup of grease and food residue on surfaces of reach-in coolers, fryers, warming cabinets, wire racks throughout, monitors, cables and pipes; observed heavy flour buildup in area above chicken fryers/holding cabinet; observed buildup of food debris and flour on floors, walls and ceiling vents/panels throughout the kitchen; there is trash inside drive-thru floor drain; there are areas of low grout throughout the kitchen.

Autumn’s Catering & Café, 6802 Paragon Place – The following violation was reported during a routine inspection: salmon taken from freezer is thawing in reduced oxygen packaging.

Saku Sushi, 9125-L West Broad Street – The following violations were reported during a routine inspection: temperatures of edamame and fried rice in warmers are too low because the warmers are unplugged; temperatures of chicken nuggets, raw tempura shrimp, raw tilapia, sushi rolls and imitation crab made this morning in the left prep cooler are elevated; temperatures of imitation crab and cream cheese in lowboy at sushi bar are elevated; dead roaches found on floor in self-service area in between freezer and counter; rice scoops are stored in ambient temperature water; cutting boards at sushi bar are scored; racks in walk-in cooler are rusted; gaskets are torn on walk-in cooler and prep cooler across from fryer; floor is uneven; there is a hole in the walk-in cooler floor; screen door is torn.

Tropical Smoothie Café, 11321-C Nuckols Road – The following violation was reported during a routine inspection: lids of two make tables are damaged (metal has split in the middle and two screws are missing on one of the lids).

The Back Porch BBQ, 1126 Westbriar Drive – The following violations were reported during a routine inspection: paper towels aren’t provided at back BBQ area outside; three cleaning spray bottles are unlabeled; observed improper cooling methods for brisket and collards (tightly covered while cooling) and pulled BBQ (cooling in deep pans) in walk-in cooler; screen door is damaged; small flies found around floor drain near ice maker; prep cutting boards are heavily scratched and stained; observed buildup of dust and grease on surfaces of hood systems, lamps, storage shelves and outside grease receptacle; there is heavy dust buildup around ceiling vents in kitchen.

Good Vices, 1321 East Nine Mile Road – The following violation was reported during a routine inspection: mop isn’t hung or inverted to air dry.

SB's Lakeside Loveshack, 6935-37 Lakeside Avenue – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit and diarrhea; bulk bin in dry storage area containing a white, powdery ingredient is unlabeled; gasket liners are torn on both doors of left-side two-door reach-in and on freezer chest in dry storage area; sink in “dish pit” is clogged; shared bathroom doesn’t have a covered trash receptacle; mop isn’t hung or inverted to air dry.

Capital Ale House, 4024-A Cox Road – The following violations were reported during a routine inspection: raw chicken is stored over ready-to-eat polenta cakes in first prep unit on the line; there is 0 ppm sanitizer being dispensed from the dishwasher and from the three-compartment sink; three out of the four soda nozzles between the two bars have dark debris buildup along the interior; andouille in walk-in cooler is past its expiration date; several bulk bins storing dry ingredients under prep tables are unlabeled; gasket liners are torn on all three doors of first prep unit on the line, left-side door of middle prep unit, middle and left-side door of last prep unit, both doors of lowboy freezer on cook line, and all three walk-in cooler doors; fryer baskets have broken off metal pieces; wire racks in dry storage area are covered in rust; drying rack in ware wash area is peeling; all long cutting boards on prep units are heavily scored and stained; vents on right-side walk-in cooler are covered in dust and dark buildup; y-valve attached to mop sink faucet doesn’t have the appropriate backflow prevention device installed; ceiling vents in between prep line and ware wash area are surrounded by dust; there is a hole in the wall exposing insulation next to the bar walk-in cooler; floor is cracked behind main bar; mop isn’t hung or inverted to air dry.

Hondo’s, 4120-B Cox Road – The following violation was reported during a follow-up inspection: diswasher isn’t reaching the required temperature (facility is in process of replacing current hot water heater).

Wingstop, 9048 Staples Mill Road – The following violations were reported during a routine inspection: observed grease and dried food residue on reach-in unit handles; there is dried food inside reach-in freezer; observed dried food and grime on storage racks; ice machine is leaking and not draining properly into the floor drain; observed grime and food residue on floors, hood vent and hand sinks.

White Tiger Indian Restaurant & Bar, 3991-K Glenside Drive – No violations reported during a follow-up inspection.


OCT. 21

Sticks Kebob Shop, 1700 Willow Lawn Drive – The following violations were reported during a routine inspection: fryer baskets have bent wires; both cutting boards at cook line are scratched and stained; observed grease accumulation on sides of fryer; the floor throughout kitchen is no longer smooth and easily cleanable.

Domino’s Pizza, 11442 West Broad Street – The following violations were reported during a routine inspection: pair of rubber gloves and two sanitizer buckets found in hand sink; no paper towels are available at hand sink next to three-compartment sink or in the restroom; pizza sauce container on time control policy doesn’t have a time stamp marked.


OCT. 22

Hyderabad Biryani House, 4040-H Cox Road – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit and diarrhea; dishwasher is reading 0 ppm of chloride sanitizer; large container of tandoori in walk-in cooler is past its expiration date; spray bottle containing a dark blue liquid chemical is unlabeled; facility doesn’t have a food thermometer; several bulk containers storing dry ingredients are unlabeled; boxes of samosas are stored directly on walk-in freezer floor; gasket liner is torn on left-side door of prep unit; wire racks in walk-in cooler and drying racks next to three-compartment sink are very rusty; long cutting board on prep unit is heavily stained and scored; foil is lining multiple shelves in kitchen; mop isn’t hung or inverted to air dry; ceiling light as soon as you enter the kitchen is missing a protective shield.

Mexico Restaurant, 11621 West Broad Street – The following violations were reported during a follow-up inspection: temperature of large bucket of queso cheese date marked the previous day is elevated; y-valve is still attached to mop sink faucet without the appropriate backflow prevention device installed; there is still a direct plumbing connection underneath the three-compartment sink.

BJ’s Restaurant & Brewhouse, 12010 West Broad Street – The following violations were reported during a routine inspection: sanitizer bucket found stored in hand sink next to three-compartment sink; soda nozzle at the entrance to the bar has debris buildup along the interior; dishwasher at bar isn’t reaching the required final surface sanitizing temperature; one fryer basket has a broken off metal piece; hand sink in bar has a leaking faucet.

Anna’s Pizza Pasta and Subs, 601 North Airport Drive – The following violations were reported during a routine inspection: women’s restroom doesn’t have paper towels; dried food debris found in microwave and on meat slicer blade and base; multiple foods in refrigeration (chicken wings, cooked pasta, red sauce, rice, etc.) are unlabled and without date marks; previously frozen raw chicken and beef are thawing at room temperature; bulk containers of flour and rice are unlabeled; cans of food are stored directly on kitchen floor; cloths used for wiping counters and equipment surfaces are stored hanging from a trash can instead of in a chemical sanitizer solution; cutting boards are scored and stained; wire storage racks in walk-in cooler are rusty; facility doesn’t have sanitizer test strips; Homestyle refrigerator top is dirty and dusty; reach-in cooler door gaskets throughout kitchen have dried food debris on them or are stained black; walk-in cooler and freezer door gaskets are stained black; two-door reach-in cooler fan guards are moldy and dusty; sides of hot line cooking equipment are greasy; there are stained and damaged ceiling tiles in dining area; there is water damage on mop sink wall; there is missing floor coving near back door to kitchen; observed stained and dusty ceiling tiles near ventilation in kitchen, as well as dusty tile and ventilation in dining room area; light is out above three-compartment sink; dishwashing area light intensity is too low.
