O’Charley’s Restaurant, 9990 Brook Road – The following violations were reported during a routine inspection: person in charge’s ServSafe certificate expired in February and there is no other certified food protection manager for the facility; person in charge couldn’t verify that employees have been trained on foodborne illnesses; paper towel dispenser at back hand sink isn’t functioning; drain tray with ice and limes found stored in bar hand sink; ready-to-eat cheese is stored next to raw salmon in grill drawer; an employee handled a raw burger patty and then fixed salad plate wearing the same gloves; dried food debris and grime found on microwave plates and doors; observed black residue inside ice machine; chemical dish machine isn’t sanitizing; temperature of fries on side of hot shelf at expo is too low; temperatures of raw chicken in grill drawer, cut tomatoes on prep line, alredo sauce in well and buttermilk at frying station are elevated; several working spray bottles of cleaners are unlabeled; thawed grouper is still in reduced oxygen packaging; bulk containers of flour and bottles of oil are unlabeled; observed numerous small flies throughout the kitchen, especially in dry storage area; observed pest droppings on floor in dry storage areas; several employees are wearing watches while preparing food; sanitizer bucket is stored on prep table; two knives are stored in crevice under microwave; ice scoop is stored on a dirty rack; cutting boards on prep line and throughout kitchen are heavily stained and scratched; gaskets on units are torn; caulking is cracked on hand sinks; salad reach-in unit door isn’t able to fully close; pans are dented; containers are chipped and cracked; trays holding portioned butter are broken; observed soiled foil on cook line equipment; there is a leak above three-compartment sink; there is a buildup of trash around the dumpster; observed heavy buildup of grime and food debris on floors throughout kitchen, especially under stationary equipment and cook line; floor drains are dirty; walls are stained with dried food debris and grime; there are cracked and missing tiles and low grouting throughout kitchen, causing water to pool; observed grease and soot buildup on vent above grill.

Arby’s, 5220 Williamsburg Road – The following violations were reported during a routine inspection: temperatures of lima beans, cooked spinach and baked potatoes cooked about three hours prior in reach-in are elevated; observed buildup and food debris on racks of units; there is buildup on gaskets and outside of units; there is a leak at three-compartment sink.

Roseshire Gaming Parlor, 4016 Glenside Drive – The following violations were reported during a routine inspection: bottles of water and oil are unlabeled; employee isn’t wearing a hair restraint; in-use wiping clothes are stored on counters at cook line and bar instead of in sanitizer buckets; there is a leak under sanitizer basin at three-compartment sink.

Crafted, 4900 Libbie Mill East Boulevard – The following violations were reported during a follow-up inspection: there is no certified food protection manager on staff; observed mold inside ice machine; observed fruit/drain flies throughout kitchen; various plates are stored uninverted on top shelf of hot line expo counter; wire storage racks in walk-in cooler, dry storage areas and at bar area are rusty; reach-in cooler in front of deep fryers has water pooling in the bottom; multiple lights are out in hood system; smoker on hot line, reach-in cooler in front of grill and multiple deep fryer baskets are in disrepair; cutting boards are heavily scored, stained and melted; observed mold, mildew and food debris on reach-in cooler/freezer door gaskets throughout the restaurant; observed mold and black stains on walk-in cooler door gaskets; wire storage racks throughout kitchen have food debris and dust on them; there is grease buildup on sides of hot line equipment, especially on spaces in between equipment; walk-in cooler fans and ceiling are dusty; shelf under hot holding steam table is greasy; bar area reach-in cooler door gaskets are dirty; there is cracked floor coving underneath dish machine; there is grease residue on walls behind hot line; floor drains throughout kitchen are dirty; observed grease residue, food debris and flies on wall behind three-compartment sink; dish machine area walls have mildew on them; hoods in kitchen are very greasy.

Dunkin Donuts, 7721 Brook Road – The following violations were reported during a routine inspection: food staff are wearing watches, bracelets and rings; there is food debris on glass door reach-in cooler door gaskets; wire storage racks near three-compartment sink and inside walk-in cooler are dirty and dusty; observed food debris on speed racks; walk-in cooler door gasket is dirty; there is a leak at the hand sink near ice machine and the sink is turned off.

Shorty’s Diner, 5625 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; an employee handled soiled dishes before putting away clean dishes; hand sink at dish area is blocked and there are no paper towels; marinara in walk-in cooler is past its expiration date and contains a mold-like substance; potato salad in walk-in cooler is past its expiration date; temperatures of sausage patties and links in steam well are too low; temperatures of cubed ham and raw liquid egg in ice bath are elevated; person in charge couldn’t provide time control policy; an employee’s cell phone is stored on a clean plate; cook with long facial hair isn’t wearing a beard guard; cooks are wearing a bracelet and watch; shelving racks in walk-in cooler and reach-in freezer are rusted; reach-in cooler on cook line has torn gaskets.

Brunches at Wellesley, 3416 Lauderdale Drive – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; employee is chewing gum while working in the kitchen; dishwasher is reading 0 ppm sanitizer; no date mark found on container of chili in walk-in cooler; two containers of diced ham in walk-in cooler are past their expiration dates; squeeze bottle containing a thick, pink chemical is unlabeled; bulk bins in back of kitchen are unlabeled; employee working on the cook line has long facial hair and isn’t wearing a beard guard; gasket liners are torn on all three doors of first prep unit on the line, both doors of lowboy storing milk, and on both doors of lowboy unit in the bar area; fryer baskets have broken off metal pieces; long cutting boards on prep units are scored and stained; y-valve attached to mop sink faucet doesn’t have the appropriate backflow prevention device installed.

Applebee’s Neighborhood Grill & Bar, 9601 West Broad Street – The following violations were reported during a routine inspection: racks in refrigeration units on the cook line are rusted; lowboy units are not self-closing; observed grease buildup behind fryer and food debris/buildup under flat top grill; hand sink at bar is broken; there is a leak at three-compartment sink at bar.

Chicken Salad Chick, 9631 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit and diarrhea; temperatures of one pan of chicken salad and one pan of sliced tomato in cooler next to reach-in cooler are elevated; temperature of two pans of chicken salad in main cooler next to door is elevated; new time marks were given to lettuce, cheddar cheese and feta on time control even though they had elevated temperatures; two kitchen employees are wearing bracelets; y- connector on mop sink doesn’t have the proper backflow prevention device installed.

Tropical Smoothie Café, 1601 Willow Lawn Drive – The following violations were reported during a follow-up inspection: observed heavy black mildew buildup inside blender lids; can of Raid found on shelf above prep table; person in charge has long hair that is not restrained; single cutting board is heavily scratched and stained; observed heavy buildup of dust and grime on all wire storage racks; light fixtures above prep tables and units do not have protective shields.

Salaam Market LLC, 7103 Staples Mill Road – The following violations were reported during a routine inspection: facility couldn’t provide verification of employee health training; raw chicken, eggs and beef are stored over produce in reach-in refrigerator; two knives are stored with dried food debris on them; chickpeas, pasta salad and orzo salad prepared yesterday or earlier are not date marked; raw chicken is thawing on the counter instead of under running water; there is exposed wood under hand sink on restaurant side and under three-compartment sink on grocery side; observed dried food debris and grime on handles of reach-in units.

Tres Machos, 4120-C Cox Road – The following violations were reported during a routine inspection: bar hand sink doesn’t have hand soap; the dishwasher is dispensing 0 ppm sanitizer; temperature of beef fat in lowboy is elevated; large bucket of soapy water storing a kitchen utensil used to scrape food off plates is unlabeled; long cutting board on prep unit is stained and scored.

Wong’s Tacos, 201 Maltby Boulevard – The following violations were reported during a routine inspection: black residue found inside ice machine (around chute) and inside soda gun nozzle at bar; observed buildup of food debris and metal filings on can opener blade; temperature of queso dated 10/14 in reach-in is elevated because of improper cooling methods; temperatures of pork belly and chorizo in hot well are too low; temperatures of brussel sprouts and shrimp in bowls on flat top are too low; temperatures of pork belly, crema, pot stickers, lettuce, brussel sprouts and salsa in make tables are elevated; make table may not be functioning properly; time control is being used for fish, shrimp and chicken breading but the time isn’t written on the product (there is no written time control policy); menu doesn’t contain a disclosure statement for raw tuna and eggs that can be undercooked; several chemical spray bottles in dish area and server station are unlabeled; queso is cooling in ice bath in sink with very low levels of ice; bags of cooked brussel sprouts prepared 3.5 hours ago are stored in a deep, covered container in walk-in; rice scoop is stored in ambient temperature water; there is food residue on racks in walk-in; y-connector attached to mop sink faucet doesn’t have the proper backflow prevention device installed.

The Cocky Rooster, 12171-D West Broad Street – The following violations were reported during a routine inspection: temperatures of slurry and raw chicken at breading station are elevated; there is scummy buildup inside dish machine; y-connector attached to mop sink faucet doesn’t have the proper backflow prevention device installed; mop sink basin is soiled.

Kung Fu Tea, 10827 West Broad Street – The following violations were reported during a routine inspection: hand sink in back of kitchen doesn’t have hand soap; bottom of ice machine shield in back of kitchen is covered in black buildup; honey boba on time control doesn’t have the time marked; squeeze bottle containing a thick blue chemical is unlabeled; facility doesn’t have a food thermometer; dumpster area is dirty and surrounded by loose trash and debris; mop isn’t hung or inverted to air dry.

McDonald’s, 11311 Nuckols Road – The following violations were reported during a routine inspection: temperature of cream in dispenser is elevated (observed ice buildup inside dispenser); two employees with long facial hair aren’t wearing beard guards; stirring spoons are stored in ambient temperature water; there is ice buildup around walk-in freezer door; observed food debris in reach-in freezer and gasket of freezer; there is grease on the floor at cook line; there are missing and broken tiles at mop sink floor.
