The Daily Kitchen & Bar, 12201 West Broad Street – The following violations were reported during a follow-up inspection: hand soap is again unavailable at second hand sink in bar; container of chicken sauce in walk-in cooler is past its expiration date; squeeze bottle of dish soap next to dishwasher isn’t labeled; first wire rack on right-hand side of walk-in cooler is still rusty; light shield above prep line is still cracked (replacement on order).
Shake Shack, 12170 West Broad Street – No violations reported during a routine inspection.
Sakura Japanese Restaurant, 9008 West Broad Street – No violations reported during a follow-up inspection.
Plaza Azteca Mexican Restaurant, 10099 Brook Road – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy; raw shell eggs are stored above ready-to-eat salsa in walk-in cooler; bar soda nozzle is encrusted with debris along the interior; temperature of cooked chicken prepared yesterday in walk-in cooler is elevated because of improper cooling methods; temperature of white rice stored in left-side steam table is too low because it is double panned; salsa in walk-in cooler is past its expiration date; beef broth prepared two days ago in walk-in cooler isn’t date marked; several food items are being cooled from the previous day in large, deep, lidded containers; raw shrimp and raw beef are being thawed at room temperature in three-compartment sink; bulk bins storing spices are unlabeled; take-out containers on top shelf of prep unit are stored uninverted; gasket liners are torn on right-side door of first prep unit on the line, right-side door of second prep unit on the line and on walk-in freezer door; long cutting boards on both prep units are scored and stained; foil is being used as a liner on bottom shelf of prep table; spray nozzle connected to mop sink hose doesn’t have the appropriate backflow prevention device installed; two mops are not hung or inverted to air dry.
Starbucks Coffee Company, 8137 Brook Road – The following violations were reported during a routine inspection: observed mildew on racks in walk-in and reach-in units; fans of small prep units are dusty.

Hawk’s BBQ & Seafood, 8191-B Brook Road – The following violations were reported during a follow-up inspection: facility doesn’t have a certified food protection manager on staff; there is no handwash signage posted in restrooms; dried food debris found inside microwave; fish breader is being held on time control but no discard time is marked; bulk containers of flour are unlabeled; water is leaking from wall in kitchen leaving puddles of water on kitchen floor and causing damage to walls and floor as well as a bag of used linens held in dry storage area; food debris found in mop sink; observed many fruit/drain flies and other pests throughout kitchen; facility doesn’t have a sanitizer test kit; there are dried food debris, stains, liquid residue and mold on/inside reach-in cooler lids, doors, door gaskets, exteriors and interiors; wire storage racks throughout kitchen are dirty; there is a direct plumbing connection underneath three-compartment sink; there is heavily stained and damaged ceiling tile in kitchen; ceiling tile near ventilation is dusty; there is low light intensity on hot line; hood system lights are not working properly.
Chick-fil-A, 4808 Nine Mile Road – The following violations were reported during a routine inspection: there are no paper towels at dish area hand sink or raw chicken area hand sink; hand sink next to rethermalizer isn’t working (no water flow).
IHOP, 4840 South Laburnum Avenue – The following violations were reported during a routine inspection: didn’t observe sanitizer at three-compartment sink although the sink was set up to wash; temperature of sausage is too low; temperature of whipped butter out on counter is elevated; pancake mix is two hours past its discard time; there is 0 ppm sanitizer in sanibucket; racks in dry storage are rusty; prep unit by office is leaking and the doors are taped shut; observed grease buildup on cooking equipment.
Panera Bread, 11703 West Broad Street – The following violations were reported during a routine inspection: wire racks are peeling in both walk-in coolers; spray nozzle attached to mop sink hose doesn’t have the appropriate backflow prevention device installed.

Chick-fil-A, 3380 Pump Road – The following violations were reported during a routine inspection: knife stored in crevice of make table; y-valve attached to mop sink faucet without the appropriate backflow prevention device installed; dumpster area is dirty and surrounded by loose trash and debris.
China Wok, 11530-C West Broad Street – The following violations were reported during a follow-up inspection: open drinking glass again found on prep table across from cook line; raw shell eggs again stored over ready-to-eat, pasteurized commercial juices in walk-in cooler; found white rice again stored on counter with a temperature that is too low.
Wingstop, 4091-D Concord Creek Place – The following violations were reported during a routine inspection: observed small amounts of mold inside ice machine; employee is wearing a watch; a light is out in the walk-in freezer; wire storage racks throughout kitchen are dusty; outdoor dumpster doors were left open; there is grease residue on hot line walls; observed food residue on make table walls; there is grease on the floor underneath deep fryers.
Roma’s Italian Restaurant, 325 East Williamsburg Road – The following violations were reported during a routine inspection: oysters on the half shell don’t have the date of last sale marked on tag; didn’t observe soups or crab dip date marked or noted on sheet.

Original Gino’s Pizza, 10444 Ridgefield Parkway – The following violations were reported during a routine inspection: prep unit cutting boards are scratched and stained; observed dust and food debris on wire storage racks; there is food debris inside microwave; cooler gaskets are dirty.
District 804, 8004 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t verify employee health training; temperature of bags of wrapped pasta cooked yesterday in prep unit are elevated because of improper cooling methods (pasta is tightly wrapped, causing condensation).
China Taste, 2623 New Market Road – The following violations were reported during a routine inspection: temperature of garlic and oil in lowboy is elevated because it was kept out of refrigeration during lunch; chicken and egg rolls are past their expiration dates; medication found stored by cooked rice; multiple prepped items are uncovered in prep units and walk-ins; rice scoops are being stored in water; cut jugs are being used as scoops; observed buildup on racks in walk-in; dry storage racks are dusty; there is grease buildup along cook line and rice hot holding units.
