SEPT. 3

Shawarma Bistro, 12422 Gayton Road – The following violations were reported during a routine inspection: cook handled cell phone and continued food prep without washing hands; dried food debris and grime found on can opener blade; chicken hadn’t reached the required cook temperature when it was put into steam table; wire racks in walk-in cooler are rusted; observed food debris on surfaces of wire racks, ticket printers, portable fans and seasoning shakers.

Chanellos Pizza, 10166 West Broad Street – The following violations were reported during a routine inspection: hand sink doesn’t have paper towels; there are no date marks for any food stored in cold units; one bulb is out in hood system; observed buildup of food debris on pizza conveyor sheet; there is a buildup around grease trap and under dough mixer; AC ducts are dusty.

Starbucks Coffee Company, 4101 Dominion Boulevard – The following violations were reported during a routine inspection: both gasket liners are torn on second two-door reach-in refrigeration unit in back of facility; y-valve attached to mop sink faucet doesn’t have the proper backflow prevention device installed.

McDonald’s, 8975 Staples Mill Road – The following violation was reported during a routine inspection: observed light buildup on soda dispenser nozzles.

Starbucks Coffee Company, 4511 South Laburnum Avenue – The following violations were reported during a routine inspection: hand sinks in back kitchen and by drive-thru don’t have paper towels; there isn’t a way to test the high temp dish machine; observed some buildup in units and mildew along seals.


SEPT. 4

Breaker’s Sports Grille, 9127-V West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certification; person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; an employee handled dirty dishes and cell phone and then changed gloves without washing hands first; a lemon in the walk-in cooler is moldy; raw bacon and raw shell eggs stored over/next to ready-to-eat foods (potato salad and tortillas); quat sanitizer concentration in three-compartment sink in nonsmoking bar is too low; dried food debris found on slicer; temperatures of large pan of sliced lettuce and small pan of sliced tomato in prep cooler are elevated; temperatures of shredded cheese and sliced cheese in prep cooler are elevated; menu doesn’t contain a reminder statement; paint cans, sanitizer bottles and sealant are stored over food, gloves and plastic utensils in dry storage room; observed limescale-like buildup inside dish machine; there is heavy grease buildup on sides of fryers; y-connector on mop sink doesn’t have the proper backflow prevention device installed; there are direct plumbing connections at all three-compartment sinks; women’s bathroom doesn’t have a covered trash can; dumpster side cover was left open; mop isn’t hung or inverted to air dry; observed heavy grease buildup on walls and floors behind fryers; there is dust buildup on kitchen ceiling.

Arby’s, 8900 Quioccasin Road – The following violations were reported during a routine inspection: long cutting board on prep unit is stained and scored; top drying shelf above three-compartment sink is covered in a layer of heavy buildup; there is a leak coming from the left-side faucet on hand sink next to walk-in cooler; there is a direct plumbing connection underneath three-compartment sink.

Dunkin’ Donuts, 4000 Mechanicsville Turnpike – The following violations were reported during a routine inspection: there is no certified food protection manager on site; dish area hand sink doesn’t have paper towels; containers of sugar are unlabeled; staff are wearing bracelets; staff with long flowing hair are not wearing hair restraints; spoons are stored in room temperature water in between uses; walk-in freezer door still has manufacturer’s tape on both sides; tops of food equipment (coffee maker, oven, warmer, etc.) and reach-in cooler drawer gaskets are dirty.


SEPT. 5

Shagbark Restaurant, 4901 Libbie Mill East Boulevard – The following violations were reported during a routine inspection: observed black buildup in soda gun nozzles at bar area; shrimp is thawing in a pan of ambient temperature water in prep sink; to-go boxes are not stored inverted; cutting boards are heavily scored and stained; door gasket on reach-in cooler in bar area is torn; deep fryer baskets are in disrepair; one light is out in hood system; there is grease residue on sides of hot line cooking equipment; reach-in cooler and freezer door and drawer gaskets throughout kitchen and walk-in cooler fan guards are dirty and moldy; reach-in cooler and freezer exteriors have dried food debris and mold on them; walk-in cooler storage racks are dirty; bar area beer cooler wire racks are moldy; observed dried food residue on hot line walls; wall and floor under three-compartment sink are dirty; hood filters are dirty and greasy.

Noodles & Company, 4101 Dominion Boulevard – The following violation was reported during a routine inspection: drying racks above prep sink and prep table in back of kitchen are covered in old food debris.

Cook Out, 4245 Pouncey Tract Road – The following violations were reported during a routine inspection: raw shell eggs are stored over bucket of ready-to-eat pickles in walk-in cooler; temperature of chopped burgers prepped yesterday in walk-in cooler is elevated; sliced cheese and sliced tomatoes are at room temperature with an expired four-hour time stamp; gasket liners are torn on top left drawer of refrigeration unit next to fryer station and walk-in refrigerator door; shared gender employee bathroom doesn’t have a covered trash can.

Nash & Smashed, 10174 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t verify that employees have been trained in food safety relating to foodborne illnesses; person in charge couldn’t provide written procedures for cleaning up vomit and diarrhea; employee water bottles are stored next to batter and back prep station actively in use; no handwash signage is posted at front hand sink; cashier touched register and money and then grabbed sliced limes wearing the same gloves; temperature of mashed potatoes in hot holding warmer is too low; milk batter for chicken and waffle batter with unsalted butter are sitting out on prep table without time control; three working spray containers are unlabeled; soiled, in-use wiping cloths are stored on counters instead of a sanitizer solution; there is a small leak at front hand sink faucet.

Takara Ramen, 9468 West Broad Street – The following violations were reported during a routine inspection: microwaves are dirty; temperature of two large bowls of rice cooked last night in reach-in cooler is elevated because of improper cooling methods (covered in plastic wrap and stacked on top each other); bucket of sprouts in water and corn are not on a time control policy; there are no date marks on cooked eggs, cooked meats, rice or chopped cabbage; bulk bins of flour and sugar are unlabeled; employee cooking food is wearing a bracelet; kitchen floor is cracked; reach-in cooler gasket is torn; air temperature in two-door lowboy in prep room is elevated; there is heavy ice buildup in chest freezer; shelving racks in walk-in cooler and clean dish racks are rusty.

KickBack Jacks, 9318 West Broad Street – The following violations were reported during a routine inspection: temperature of milk in dressing cooler is elevated; observed numerous flies near dumpster (several open bags of trash had not been moved into the dumpster yet) and a few flies inside kitchen near dumpster door; mop sink next to dumpster is leaking; several single cutting boards are scratched and stained.


SEPT. 8

Chipotle Mexican Grill, 11728 West Broad Street – No violations reported during a routine inspection.

Plaza Azteca Restaurantes Mexicanos, 6623 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certificate or written procedures for cleaning up vomit and diarrhea; employee handled dirty dishes to load into dish machine and then put away clean dishes without washing hands first; facility couldn’t provide parasite destruction letter from tilapia or shrimp providers; soda gun and can opener blade are dirty; dried food debris found on slicer; temperature of cooked corn prepared yesterday in walk-in cooler is elevated because of improper cooling methods (deep container is wrapped in plastic and covered); temperature of rice in hot well is too low; temperature of spinach on top of pans in prep cooler is elevated; observed inconsistent date marking; four small pans of salsas in walk-in cooler are past their expiration dates; time isn’t being tracked for tomatoes at self-serve guacamole station; rubbing alcohol and degreaser are stored over bulk beans and rice; ice cream scoop is stored in ambient temperature water; ice buckets are not inverted to air dry; y-connector at mop sink doesn’t have the proper backflow prevention device installed; ceiling vents near dish area are rusty.

Tropical Smoothie Café, 1601 Willow Lawn Drive – The following violations were reported during a follow-up inspection: observed several live adult and baby roaches in kitchen; several employees preparing food aren’t wearing hair restraints; single cutting boards are heavily scratched and stained brown; observed heavy buildup of dust and grime on all wire storage racks; light fixtures above prep tables and units do not have protective shields.
