Shelly’s Food, 7304 Staples Mill Road – The following violations were reported during a routine inspection: several water bottles and coffee cups for employees are stored next to food and clean dishes throughout kitchen; dishes found in both hand sinks; raw shell eggs are stored above sliced tomatoes and cut peppers in front prep unit; raw shell eggs are stored above cut beef in walk-in cooler; papusa filling stored on counter does not have a time stamp; bucket of plantains and bucket of corn are stored directly on the floor; a cloth is being used to cover papusa dough; observed buildup around caulking of front hand sink.
Bosna Restaurant, 8030-F West Broad Street – No violations reported during a routine inspection.

Hardee’s, 6302 Lakeside Avenue – The following violations were reported during a follow-up inspection: there is no hand sink in front food prep/dispensing areas which is preventing routine handwashing by food workers; observed drain flies throughout facility; observed grease and food debris accumulation on all equipment and racks and inside all units (reach-ins, racks, warming units, etc.) throughout kitchen; there is grease buildup on pipes around and behind fryer; walk-in cooler walls and milk shake mixer splash guard are dirty; observed food debris and mildew in walk-in cooler and on racks within; there is a leak at hot water handle and from hose end attached to backflow prevention device; there is trash on the ground around dumpster; floors and walls throughout facility are covered in grease, dust and food debris; there is dust accumulation around ceiling vents.
TGI Friday’s, 4459 South Laburnum Avenue – The following violations were reported during a routine inspection: temperatures of chicken and shrimp in breading unit and pasta in prep top are elevated; quat sanitizer concentration at bar is too high; observed flies in back kitchen, dish area and bar; there are multiple containers of uncovered food in lowboy units and walk-in cooler; plates stored under prep areas are getting splashed with food debris; there is heavy ice buildup in multiple units, including walk-in freezer; racks are rusty and chipping; observed torn gaskets, loose seals and broken drawers; cutting boards on cook line are heavily stained; observed grease and debris on cooking equipment, fryers and inside/outside units; there is mildew buildup on gaskets and inside units; floor tiles are cracked; coving tiles are missing; floor is uneven and broken at dish area; there is low grouting throughout; light cover is missing on cook line; observed food debris on floors and walls throughout kitchen.
Leonardo’s Pizza & Pasta, 10883 West Broad Street – The following violations were reported during a routine inspection: temperature of meat sauce in steam well is too low; temperature of meat sauce in the walk-in cooler is elevated because of improper cooling procedures; gaskets are torn on all three doors of pizza prep unit, the middle and right-side door of dough prep unit, both doors of lowboy freezer, left-side walk-in cooler door, and on both doors of reach-in freezer across from canned ingredients shelf.

Capriccio Pizza, 9127-A West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certificate; an employee washed dirty dishes and then handled clean silverware without washing hands first; there is no handwash signage in employee bathroom or hand sink outside of employee bathroom; raw shell eggs are stored over bucket of tomato sauce in reach-in cooler; temperature of containers of tomato sauce in walk-in cooler is elevated; temperatures of shredded cheese, tomato sauce and sausage in pizza cooler and two containers of pasta in three-door reach-in cooler are elevated; there are no date marks on many containers of tomato sauce, cooked meats or shredded cheese; cleaner in spray bottle is unlabeled; stainless steel polish is stored next to spices; vitamins are stored above wines, forks and crackers; hand sink faucet in employee bathroom is leaking; dumpster side cover was left open.
Arthur Ashe Elementary School, 1001 Cedar Fork Road – The following violations were reported during a routine inspection: sanitizer concentration in three-compartment sink is too low; one light is out in the kitchen; light fixture in laundry closet is in disrepair; cutting boards are stained and scored; reach-in cooler and walk-in cooler door gaskets are moldy and stained black; milk cooler gaskets are moldy; walk-in cooler fan guard and ceiling are dusty; there is a direct plumbing connection underneath both three-compartment sinks; outdoor dumpster lid was left open because the dumpster is overflowing; ceiling tile near ventilation is dusty; ceiling tiles are stained.
Fair Oaks Elementary School, 201 Jennings Road – The following violations were reported during a routine inspection: there is a direct plumbing connection under three-compartment sink; outdoor dumpster door was left open; ceiling tile in dry storage room is stained; ceiling tile near ventilation in kitchen is dusty.
Harvie Elementary School, 3401 Harvie Road – The following violations were reported during a routine inspection: hand sink in staff bathroom doesn’t have paper towels; wire racks in walk-in cooler and wire rack between walk-in cooler and freezer are moldy; milk cooler door gaskets are dirty.

Fuzzy’s Lounge, 2709 Williamsburg Road – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; hand sink in bar area doesn’t have paper towels; raw steaks are stored over ready-to-eat greens in reach-in refrigeration unit; salmon is thawing in reach-in while still in vacuum sealed packaging; observed several large, black flies buzzing over food contact surfaces in kitchen; take-out containers are stored uninverted on top shelf of prep unit; long cutting board on prep unit is stained and scored; facility doesn’t have a bleach-based sanitizer available; wire racks storing dry ingredients and drying racks next to dishwasher are covered in dust; several ceiling lights do not have protective shields.
McDonald's, 8210 Brook Road – The following violations were reported during a routine inspection: fry freezer isn’t working; fryer baskets are bent.
Jing Sushi, 7019-G Three Chopt Road – The following violations were reported during a routine inspection: an employee rinsed his hands before heading back to work but did not use soap; raw shell eggs are stored on top of ready-to-eat sauces in prep unit; long cutting board on prep unit is scored; wire racks in walk-in cooler are covered in rust spots; mop isn’t hung or inverted to air dry.

Firebirds Wood Fired Grill, 11800 West Broad Street – The following violations were reported during a routine inspection: there is 0 ppm sanitizer in three-compartment sink; dry ingredients storage racks are peeling.
McDonald's, 7118 West Broad Street – The following violations were reported during a routine inspection: ice and a wet cloth found inside drive-thru hand sink; egg folds in cold drawer are past their discard time; quat sanitizer concentration is too high in three-compartment sink; observed dried food debris inside small reach-in freezer.
