Pho 1 Grill, 201 Towne Center West Boulevard – The following violations were reported during a follow-up inspection: hot water at hand sink next to three-compartment sink doesn’t reach the required temperature; knobs on this hand sink are leaking; interior shield of ice machine still has a buildup of black debris; gasket liner on right-side door of first prep unit is still torn; fryer baskets still have broken off metal pieces; there is still a direct plumbing connection underneath three-compartment sink.
Captain D’s Seafood, 4800 Williamsburg Road – The following violations were reported during a routine inspection: dried food debris found inside microwave; mold found inside ice machine; observed fruit/drain flies in kitchen prep area and near mop, dustpan and broom storage area; staff member with very long hair has it tied back but isn’t wearing a hair restraint; to-go boxes on hot line are not stored inverted; cutting boards are heavily stained and scored; one small deep fryer basket and four large fryer baskets are in disrepair; lowboy reach-in cooler door is not closing properly; door gaskets on reach-in cooler and freezer on hot line are torn; dining room soda machine has mold on its base; interior of cabinet beneath soda machine is stained and moldy/dusty; observed grease and food residue on sides of cooking equipment and coolers on hot line; door gaskets on reach-in cooler and freezer on hot line have food debris in them; wire storage racks are dusty and dirty; walk-in cooler fan guards and hanging wires/cords are dusty; walk-in cooler storage racks/shelves are dirty; there is a direct plumbing connection underneath three-compartment sink; there are cracked and missing wall tiles throughout the kitchen; there are dried food stains on ceiling throughout kitchen; there is dust on wall outlets and connected cords/wires and on ceiling vents; wall behind hot holding unit is greasy; floor drains are dirty.
Calypso’s LLC, 5717 South Laburnum Avenue – The following violations were reported during a routine inspection: wire storage racks in walk-in coolers are rusty and there is food residue on them; walk-in cooler fan guards are dusty; there is a direct plumbing connection underneath three-compartment sink; there are dried food stains on kitchen ceiling and walls; floor coving is missing under three-compartment sink; corner wall is damaged by walk-in cooler; walk-in cooler doesn’t have adequate light.
Ming’s Dynasty, 1200 North Concord Avenue – The following violations were reported during a routine inspection: menu lists “crabmeat” in fried wonton and seafood delight but imitation crab is being served; dried food splatter found inside microwave; dried food debris found on slicer; time is not marked for shredded cabbage sitting out on counter; observed grease buildup on pipes under equipment on cook line and under three-compartment sink; observed buildup on fan guards in walk-in cooler; there is a direct wastewater connection at three-compartment sink; both bathrooms were partially open because the doors are not self-closing.

Tay-Ho Restaurant, 7927 West Broad Street – The following violations were reported during a routine inspection: hand sink near kitchen entry doesn’t have paper towels; hand sink next to three-compartment sink is blocked; employee failed to sanitize tongs and cutting board at three-compartment sink; interior of ice machine is dirty; there is black buildup in soda gun nozzle; grilled beef wasn’t cooked to the proper temperature; there are no disclosure, asterisks or reminder statement on to-go menus for pho tai; dine-in menu has reminder but no asterisks for pho tai; many food boxes are stored directly on walk-in freezer floor; employee dried clean dishes with a towel after removing from the dish machine instead of allowing to air dry; one fryer basket is heavily bent; shelving racks in walk-in cooler are rusty; small block cutting boards are scratched; floor in dishwashing area is dirty; ceiling vents near office are dusty.
Torque Club at Richmond Raceway, 600 East Laburnum Avenue – The following violations were reported during a routine inspection: clean dishes are stored directly on kitchen floor; beer cooler door gasket and walk-in cooler door gasket (first floor kitchen) are torn; one light is out in hoods in first floor kitchen; observed dried food debris on sides of cooking equipment.
Pop’s Dogs & Ma’s Burgers, 7301 Brook Road – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certificate for the facility; tomatoes found stored inside hand sink; raw shell eggs are stored above hot dogs; temperatures of cut lettuce and cheese in small, left reach-in cooler are elevated; small reach-in cooler is not cold-holding properly; did not observe a date marking system for the facility; counter tops, cold units and storage shelves are dirty with food debris and grime; there is a small leak at three-compartment sink; there is dust buildup on walls and grime on floors.
Canastas Chicken, 5233-35 South Laburnum Avenue – The following violations were reported during a routine inspection: raw shell eggs stored above cilantro and raw beef stored next to cilantro in outside walk-in cooler; observed grease residue and food debris on containers and lids stored clean; sections of whole chickens didn’t reach the required cooking temperature after being removed from roaster; temperatures of top portions of rice and chicken in front service table are too low because the containers were overfilled; temperatures of coleslaw in front service cold table and cooked chicken in back prep unit are elevated; prep unit and single cutting boards are heavily scratched and stained; wire racks in both walk-in coolers are heavily rusted; handles on large prep unit are broken; observed buildup of grease and dust on surfaces of all wire storage racks, prep shelves, portable fans and reach-in freezer; there is old label residue on bins; there are direct plumbing connections at prep sink and three-compartment sink.

McDonald’s, 11218 West Broad Street – The following violations were reported during a routine inspection: gasket liners are torn on both standing reach-in freezer doors; y-valve connected to mop sink faucet doesn’t have the appropriate backflow prevention device installed.

The Daily Kitchen & Bar, 12201 West Broad Street – The following violations were reported during a routine inspection: verified employee health policies unavailable; out-of-order soda nozzle found stored in first hand sink at the bar; second hand sink at the bar doesn’t have paper towels or hand soap; second working soda nozzle in the bar has buildup of debris along the interior; there is a buildup of black debris along the top interior of the ice machine; temperature of chili beans prepared yesterday in walk-in cooler is elevated; food in all three make tables had elevated temperatures (shredded cheese, corn salsa, minced crab); temperature of four containers of cooked chicken date marked three days ago in the walk-in cooler is elevated; container of chicken sauce in walk-in cooler is past its expiration date; spray bottle of bleach and spray bottle of glass cleaner are unlabeled; large bulk storage containers of cooling food (pizza sauce and chickpea salad) have condensation built up on the lids; several squeeze bottles of sauce in prep units are unlabeled; cell phone found stored directly on prep unit cutting board; an employee prepping food has long loc hair falling over his shoulder; knife is stored in crevice of prep unit; observed buildup of dark debris along top of prep unit storing take-out containers; all three prep units have torn gasket liners; walk-in cooler wire racks and drying racks in warewash area are rusty; all three long cutting boards on prep units are stained and scored; quat sanitizer test strips are unavailable; interior of panini press is covered in a buildup of debris; bottom of beer chest in bar is dirty; fans along prep line are dusty; POS machine in kitchen is covered in dust; wire racks in walk-in cooler are covered in old food debris; dumpster area is dirty and surrounded by loose trash and debris; vents above prep line are surrounded by dust; there is a large area of peeling paint across from the expo line; light shield is cracked above prep line closest to the office.
Little Szechuan Restaurant, 10234 Staples Mill Road – The following violations were reported during a routine inspection: person in charge touched cooked noodles with bare hands; raw steak stored above uncovered tofu in small reach-in unit; raw shell eggs stored above ready-to-eat sauces, tofu and raw shrimp in walk-in cooler; temperature of large container of raw chicken is elevated because it is sitting out in three-compartment sink; observed inconsistent date marking in the facility; egg rolls, fried chicken, egg noodles and dumplings are missing date marks; observed small closed containers of gravy cooling on prep table; several items in cold holding units throughout the restaurant (tofu, dumplings, egg rolls and more) are uncovered; wet wiping cloths found in tray on counter instead of being stored in sanitizer; rice scoops are stored in water that isn’t hot enough; single-service containers without handles are being used as scoops in bulk flour and sugar containers; there are broken drawers in reach-in refrigerator; fryer baskets are heavily bent; torn and dirty cardboard and foil is being used to line shelves throughout kitchen and inside reach-in refrigerator; observed grime on faucet and rim of front hand sink; food residue found inside of ice machine; observed dried food debris and residue on wire racks in walk-in cooler, walk-in freezer and storage areas; dried food debris found inside reach-in refrigerators; floors are dirty with grime and buildup, especially in walk-in cooler and under three-compartment sink and stationary equipment.
J Five Catering, 3073 Mechanicsville Turnpike – The following violations were reported during a routine inspection: hand sink doesn’t have paper towels or hand soap; there is no handwash signage posted at hand sink; fry baskets are dirty; facility doesn’t have sanitizer test strips.

Park Lane Tavern, 2251 Old Brick Road – The following violations were reported during a follow-up inspection: observed small fly infestation throughout kitchen and bar; prep unit cutting boards are heavily scratched and stained; hose with spray nozzle attached at mop sink doesn’t have the proper backflow prevention device installed; observed buildup of food debris and grime on floors and walls throughout kitchen and bar.
El Amanecer Latino Market, 4806 Nine Mile Road – The following violations were reported during a routine inspection: observed dirty dishes in kitchen leftover from last night; fry baskets are dirty with food left in them; bags of raw chicken were left to thaw out at room temperature overnight; observed flies throughout restaurant; food is stored directly on walk-in cooler floor; foods in reach-in cooler are uncovered; ice scoop handle is touching ice in ice machine; there are torn gasket doors on reach-in cooler and freezer throughout kitchen; two lights are out in the hood system; cardboard is being reused to line walk-in cooler shelves and reach-in freezer bottoms; reach-in cooler and freezer door gaskets are dirty; walk-in cooler door gaskets are moldy; walk-in cooler fan guards are dusty; women’s restroom doesn’t have covered trash cans; outdoor dumpster doors and lids were left open; observed stained and damaged ceiling tile in store front area; back door frame in kitchen is damaged.
