RVA Outlaw BBQ, 4040-B Cox Road – The following violations were reported during a routine inspection: food protection manager certificate is unavailable; facility couldn’t provide written procedures for cleaning up vomit or diarrhea; prep unit’s ambient air temperature is elevated.

Garden Café at Lewis Ginter Botanical Garden, 1800 Lakeside Avenue – The following violations were reported during a routine inspection: dried food debris found on slicer and in-use knife strip on grill line; temperature of cups of pasta in grab-and-go case is elevated; package of salmon vacuum sealed in-house found in the freezer (HACCP plan is required to conduct this method of vacuum packaging); observed ants crawling on soda machine grates and inside soda nozzles in salad bar area; single cutting boards are heavily scratched and stained; observed grease and food debris on surfaces of fryers, pizza oven, reach-in coolers, cooler gaskets, bottom prep shelves and black rolling carts; observed dried food debris and grease buildup on floors under grill cook line and three-compartment sinks; there is dust on ceiling panels and vents above prep table.

US Best Wings, 3716 Mechanicsville Turnpike – The following violations were reported during a routine inspection: food staff went outside, re-entered kitchen and changed gloves but did not wash hands; there are no paper towels at kitchen hand sink or men’s room hand sink; hand sink is blocked by trash can and door; items found in hand sink; observed mold inside ice machine; there is no form of date marking being used in the kitchen; chicken wings, fish fry flour and chicken breading are being held on time control but no discard times are marked; shrimp is thawing in a bowl of room temperature standing water; cutting boards are heavily stained and scored; wire storage racks in walk-in cooler are rusty; deep fryer baskets are in disrepair; door gasket on four-door reach-in cooler is torn; facility doesn’t have sanitizer test strips; there is grease buildup on sides of deep fryers; mold found on walk-in cooler fan guards; observed dried food debris and grease on wire storage racks throughout the kitchen; reach-in cooler door gaskets are moldy and dusty; there is a direct plumbing connection underneath three-compartment sink; observed missing/worn out floor tile in both kitchen and front of house area; floors in these areas are not smooth and easily cleanable; observed stained and dust ceiling tiles throughout, especially near ventilation.

Davinci, 4933 Nine Mile Road – The following violations were reported during a routine inspection: observed dried food debris and grease residue on slicer blade and base; dried food debris found inside microwave; there is no date marking system for items held longer than 24 hours; observed fruit/drain flies in kitchen; cutting boards are scored and stained; shelves underneath flat top grill and wire false bottoms for pans are rusty; observed grease residue on sides of deep fryers; chest freezer door gaskets are moldy; reach-in freezer door gaskets, top of oven and attached tubes are dirty; there is a direct plumbing connection underneath three-compartment sink; ceiling tile in kitchen is stained and dusty.

Burger King, 5217 Williamsburg Road – The following violations were reported during a routine inspection: hand sinks don’t have paper towels; there is ice buildup on small freezer unit and on freezer door; ice cream unit is in disrepair; observed debris and water buildup in multiple lowboy units; observed grease buildup on front and side of fryers; there is grease dripping down the outside of the burger oven hood; there is debris on floors of cooking/service area; dish and mop areas are dirty.

Subway inside Fas Mart, 2650 New Market Road – The following violations were reported during a routine inspection: cigarettes found on prep surface next to hand sink; back hand sink doesn’t have paper towels; front hand sink doesn’t have hot water; temperature of meatball in warmer is too low; handle of hand sink is leaking in back kitchen; observed debris along front service area.

Subway, 73 South Airport Drive – The following violations were reported during a routine inspection: dishes stored in hand sink by three-compartment sink; dishes are blocking hand sink on sandwich line; temperatures of tomatoes and chicken in walk-in cooler are elevated (walk-in cooler was left open for an extended period during lunch rush); food staff aren’t wearing hair restraints; cutting boards are scored and stained; oven in back of kitchen isn’t in use (unused equipment should be removed from the premises); wire storage racks in walk-in cooler are rusty; reach-in cooler and walk-in cooler door gaskets are dirty; walk-in cooler door handle and wire storage racks throughout kitchen are dirty; there is a direct plumbing connection underneath three-compartment sink; ceiling tile above oven is stained.

Subway, 3135 Mechanicsville Turnpike – The following violations were reported during a routine inspection: dried food debris found inside microwave; observed mouse droppings on shelves under sandwich make line, in cabinet under hand sink, and on shelf under prep table in kitchen; employee handling food is wearing a bracelet; staff are not wearing a hair restraint; reach-in cooler door gaskets are dirty; there is a direct plumbing connection underneath three-compartment sink; there is a leak in pipe underneath three-compartment sink; ceiling vent over oven is dusty; there are dried food stains on wall behind prep table.

Pho So 1, 9135 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide verification that staff have been trained on employee health; an employee washed hands at three-compartment sink; an employee handled dirty dishes and a cell phone and then touched clean dishes without washing hands first; there isn’t a handwash sink in the dishwashing area; water in bathroom and kitchen doesn’t reach the required temperature; dried food debris found on slicer; temperatures of half-and-half and condensed milk in mini fridge are elevated; temperature of chicken in prep cooler pan is elevated because the pan is overfilled; there are no date marks on any cooked meats, rice or noodles in kitchen; times are not marked on sprouts, lettuce or cooked beef on counter; observed many dead roaches in kitchen and many dead baby roaches on floor in storage room; employee handling food is wearing a watch; single-service plastic containers are being used as scoops in bulk bins; reach-in cooler gasket is torn; staff bathroom and public bathroom doors were left open; dumpster side cover was left open; floor tile is cracked in kitchen; ceiling panels in kitchen are stained brown.

Market Café, 10950 Nuckols Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; person in charge couldn’t provide employee health policy; staff are washing dishes at three-compartment sink and storing without sanitizing first; temperature of rice/yogurt dish in prep unit is elevated; several food items prepared yesterday in both walk-in cooler and prep unit are not date marked; bulk containers of white powdery foods and a squeeze bottle with oil/water are unlabeled; several scoops without handles are stored in food bins; utensils used for cooking are stored in a container with water that isn’t hot enough; torn and soiled foil is being used to line shelves throughout kitchen; observed buildup of grease and dust on pipes behind cook line; dried food debris found on wire racks in storage area; observed dried food debris and residue on floors around perimeter of kitchen, especially in storage area.

Rico’s Mexican Grill, 11321-D Nuckols Road – The following violation was reported during a follow-up inspection: person in charge is filling bags of tortilla chips with bare hands.

Chen’s Chinese Restaurant, 11361 Nuckols Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit or diarrhea; uncovered employee drink found on shelf next to canned food; there is debris inside tea nozzle; food debris found on can opener blade; temperature of large pot of wonton soup stored on floor is too low; temperatures of raw chicken and bagged pork in make table are elevated; temperature of lo mein cooked 1.5 hours ago in walk-in is elevated because it is in covered, deep containers; chicken cooked 1.5 hours ago is cooling on speed rack at room temperature; chicken during the prep process and prepared wonton soup are stored directly on the floor; utensils for rice are stored in ambient temperature water; take-out containers are stored unprotected/uninverted; make table on end of cook line is not maintaining food at the proper temperature; observed dust and food debris on shelf above make tables; mop sink faucet doesn’t have a backflow prevention device installed; dumpster doors were left open; tiles are broken in front of three-compartment sink (standing water present); observed grease on pipes and floor at end of cook line; floor, pipes and floor drain under three-compartment sink need cleaning (scum and food residue present); there are heavy grease buildup and dripping grease on hood system vents.

El Rey International Supermarket, 6006 West Broad Street – The following violations were reported during a routine inspection: staff went outside, returned to kitchen and put on gloves without first washing hands; dried food debris found on meat slicer blade and base; did not observe any type of date marking system being used in kitchen (no foods are date marked); can of pesticide found in kitchen; bulk containers of flour, sugar and salt are unlabeled; food is stored directly on walk-in freezer floor; to-go boxes are not stored inverted at front counter self-service table; food is stored in non-food grade plastic grocery bags; cutting boards are scored and stained; wire storage racks in walk-in cooler are rusted; reach-in cooler door gaskets are torn; there is grease residue on sides of hot line cooking equipment; reach-in freezer door gaskets are dirty and dusty; reach-in cooler door gaskets are moldy; walk-in cooler fan guard is dusty; walk-in cooler door gaskets are dirty; there is a direct plumbing connection underneath all three-compartment sinks in kitchen and bar; there are cracked and missing floor tiles throughout the kitchen; ceiling tile in kitchen is stained; observed grease residue more than 24 hours old on hot line walls; hoods are greasy (filters and fire suppression system pipes).
