Atlas 42, 4032-A Cox Road – The following violations were reported during a routine inspection: dishwasher is dispensing 0 ppm chloride sanitizer; container of chipotle mayo in walk-in cooler #2 is past its expiration date; wire racks in walk-in cooler #1 are starting to peel on edges; facility couldn’t provide chloride sanitizer test strips; there is a direct plumbing connection underneath three-compartment sink; dumpster area is dirty and surrounded by loose trash and debris.
Elephant Thai Restaurant, 11676 West Broad Street – The following violations were reported during a follow-up inspection: both hand sinks in kitchen are storing fans and large boxes; raw sushi is stored over ready-to-eat pickled radish in sushi bar lowboy; temperatures of shredded cabbage, fried chicken, noodles, cut melon and tofu in first prep unit on the line are elevated; can of Raid pesticide is stored behind sushi bar.
Subway, 9621 West Broad Street – The following violations were reported during a routine inspection: employee entered kitchen and began to prep food without first washing hands; large slicer hanging with clean dishes is dirty; observed food splatter in microwave and toaster oven below prep table; employees are wearing a bracelet and watch; gaskets on both walk-in units are torn; floor trim in walk-in cooler is peeling; ceiling is leaking in walk-in cooler; air curtains in walk-in cooler are broken; walk-in cooler shelving racks are dirty; there is a direct plumbing connection at three-compartment sink.

Burger King, 8081 Brook Road – The following violations were reported during a routine inspection: one large American roach found; observed small flies by milkshake machine; greasy soiled cardboard is used in multiple areas in kitchen; observed leak at hand sink by office; three-compartment sink nozzle is leaking; there is debris on floors and walls throughout facility; ceiling tiles are stained; there is low grouting and cracked floor tiles; mop sink floor is in disrepair.
Anant Cuisine of India, 4348 Pouncey Tract Road – The following violations were reported during a routine inspection: knife found stored in kitchen hand sink; container of flour and container of masala spice are unlabeled; long cutting boards are stained and scored; gasket liners on prep unit across from prep line are torn; walk-in cooler door gasket liner is torn; wire racks in walk-in cooler are peeling.
Subway, 11234 Patterson Avenue – The following violations were reported during a routine inspection: person in charge couldn’t provide a food manager protection certificate for a manager or supervisor; make table cutting boards are heavily scratched and stained brown; there is a direct plumbing connection underneath three-compartment sink; ceiling panels in kitchen are not smooth and easily cleanable.

Domino’s Pizza, 8703 West Broad Street – The following violations were reported during a routine inspection: temperatures of spinach, pasta, sausage, shredded mozzarella and ham in prep cooler are elevated; employee making pizzas is wearing a bracelet; walk-in cooler floor is not smooth and there is low grout at tile flooring.
Wing Zone, 8310 Staples Mill Road – The following violations were reported during a routine inspection: soap dispenser at hand sink next to prep table is broken and unusable; temperatures of cheese, cut tomato and cut lettuce in prep unit are elevated; observed breading with congealed pieces held on counter over night; two scoops with handles are resting inside bulk flour bin and there is heavy buildup on the handles; single-service to-go containers are stored uninverted; prep unit across from fryers is not maintaining foods at the proper temperature; several cutting boards are heavily stained and scratched; fryer baskets have heavy grease buildup; observed black residue in gaskets of reach-in freezer; there is buildup on cook line hand sink; outside of sauce nozzles is encrusted with dried sauce; shelves throughout the kitchen are dirty; observed heavy dust buildup on ceiling vents; grease and food residue found on floors under cook line equipment, shelves in walk-in cooler and three-compartment sink; there is black residue on wall behind three-compartment sink.
Subway, 4386 South Laburnum Avenue – The following violations were reported during a routine inspection: employee health policy is unavailable; three-compartment sink quat sanitizer level is at 0 ppm; drying rack next to walk-in cooler is peeling; long cutting boards on prep line are heavily scored and stained; there is a direct plumbing connection underneath three-compartment sink; mop isn’t hung or inverted to air dry.

Park Lane Tavern, 2251 Old Brick Road – The following violations were reported during a routine inspection: observed black residue buildup inside ice maker and inside soda guns and citrus juicer at bar; there is dried food debris on dicer blades and can opener blade; temperatures of cabbage and potatoes in water bath warmer are too low; container of chili in walk-in cooler is past its expiration date; observed small fly infestation throughout kitchen and bar; a server with painted artificial nails is handling exposed food and not wearing gloves; server handling exposed food is wearing several bracelets; prep unit cutting boards are heavily scratched and stained; observed buildup of food debris, grease and dust on surfaces of wire storage racks, broiler, oven suppression nozzles, outside of bulk flour bins, prep unit gaskets, and inside warmers and reach-in coolers; hose with spray nozzle attached at mop sink doesn’t have the proper backflow prevention device installed; backflow device at mop sink is leaking; observed buildup of food debris and grime on floors and walls throughout kitchen and bar;
Rise Biscuits & Donuts, 11561 West Broad Street – The following violations were reported during a routine inspection: employee health policy is unavailable; written procedures for cleaning up vomit or diarrhea are unavailable; pair of tongs is stored over the splash guard of the hand sink next to prep line; quat sanitizer concentration at three-compartment sink is 0 ppm; no food thermometer is available; gasket liners are torn on both prep unit doors on the line and both lowboy doors next to biscuit making station; one fryer basket has broken off metal pieces; long cutting boards on prep unit are stained and scored; y-valve attached to mop sink faucet doesn’t have the proper backflow prevention device installed.
Denny’s, 6598 West Broad Street – The following violations were reported during a routine inspection: there is no certified food protection manager on staff; mold found inside ice machine; dried food debris found in microwaves; foods are thawing in pans of room temperature water; food staff are wearing jewelry and watches on wrists; cutting boards are heavily stained and scored; deep fryer baskets and reach-in cooler underneath deep fryers are in disrepair; one light is out in hood system; several items/areas are dirty (reach-in cooler door gaskets throughout kitchen, wire storage racks throughout kitchen, floor drains throughout kitchen, and walk-in freezer and cooler doors); sides of hot line cooking equipment, interiors of reach-in coolers under hot line cooking equipment and counter surfaces on hot line have dried food debris and grease residue on them; hand sinks throughout kitchen are dirty and mildewed; cabinet interiors under service line are dirty and moldy; walk-in cooler fan guards are moldy; there is a direct plumbing connection underneath three-compartment sink; hot line floor (and behind hot line) has black grime and food debris on it; there are dried food stains/debris on walls throughout kitchen; there is missing floor tile in staff break room; hoods are greasy.
Krispy Kreme Doughnuts, 4906 West Broad Street – The following violations were reported during a routine inspection: there is mold inside the ice machine; temperature of milk in walk-in cooler is elevated; walk-in cooler is not holding at the proper temperature (person in charge states that it has had elevated temperatures for several days); spray cleaner bottle is unlabeled; there is no drain pipe at dish area hand sink; outdoor dumpster lid was left open; there are missing and cracked floor tiles in middle of kitchen; dry storage/back area floor is not smooth and easily cleanable.

Little Caesars, 5101 South Laburnum Avenue – The following violations were reported during a routine inspection: pitchers are stored in hand sink; time has not started for pizzas on speed rack; several employees are wearing watches; observed grease and food residue on wire racks throughout kitchen and on drawer handles in office area; there is a leak at hand sink nearest to walk-in cooler; there is grime on floors (perimeter of kitchen); observed buildup on floor behind prep sink and wall; several ceiling tiles are stained; vents are dusty.

McDonald’s, 4423 South Laburnum Avenue – The following violations were reported during a routine inspection: tongs stored in hand sink; temperature of half-and-half in reach-in unit is elevated because the door was stuck ajar; quat sanitizer solution in three-compartment sink is at 500 ppm; observed grime inside gaskets of reach-in units throughout kitchen; there are ice buildup and food pieces in bottom of reach-in freezer.
KFC, 4821 Williamsburg Road – The following violations were reported during a routine inspection: temperatures of fries and fried pickles in reach-in unit are elevated because the unit is broken; coleslaw labeled with incorrect discard date; observed grease and dried food debris on shelves throughout kitchen; there is heavy grease buildup inside warmer unit and on fryers; observed dried food and residue on carts; general cleaning is needed throughout kitchen on floors and walls, especially behind fryers; observed black mold-like residue on ceiling tiles; water is pooling by drive-thru hand sink; there is pitting on floor near breading station; coving is coming apart from the wall above expo line.
