JULY 14

Domino’s Pizza, 11210 Patterson Avenue – The following violations were reported during a routine inspection: person in charge couldn’t provide food protection manager certification; sanitizer concentration in three-compartment sink basin is at 0 ppm; observed dried food debris and dust on surfaces of keyboards, cables, monitors, walk-in cooler fans, prep table shelves and inside pizza make table; there is dust buildup in ceiling vents above make table and on floor behind oven.

Halal Maestro, 5401 Glenside Drive – The following violations were reported during a routine inspection: there is a direct plumbing connection under three-compartment sink; observed junk, trash and unnecessary items in outdoor dumpster area; dumpster doors were left open.

Little Caesars, 4007 Mechanicsville Turnpike – The following violations were reported during a routine inspection: bulk container of flour is unlabeled; staff are not wearing hair restraints while working with food; there is a torn door gasket on middle warming unit; top of pizza oven, point-of-sale monitors, cables and connected cords are dusty and dirty; wire storage racks throughout the kitchen have dried food debris and grease on them; exterior of pizza oven hoods and walk-in cooler fan guards are dusty; door gaskets on pizza make table and walk-in cooler are moldy; reach-in freezer door gaskets, wire racks and interior bottoms of food warmers are dirty; shelving under prep tables are dirty, dusty and covered in flour; observed dried food debris and mold on speed racks in kitchen and walk-in cooler; observed dried food debris on wire baskets holding sauce bottles; there are junk, trash and unnecessary items in outdoor dumpster area; there is stained ceiling tile in dining area and kitchen; ceiling tile near ventilation in dining area and kitchen is dusty; observed dried food stains on walls and floors; wall behind three-compartment sink has mildew; three-compartment sink floor drain is dirty; there are cracked floor tiles throughout kitchen; floor coving and ceiling light shields are missing in kitchen.


JULY 16

Poke Sushi Bowl, 6992 Forest Avenue – The following violations were reported during a routine inspection: observed employees exit front door with gloves on and return to serve food without changing gloves and washing hands; pot found stored in back hand sink; temperatures of krab and seaweed salad in right prep unit and krab in right lowboy unit are elevated; did not observe time stamps or time control policy for white rice that person in charge states is held on time control; medicine is stored on same shelf as condiments; tubs of sushi ginger and bags of rice are stored directly on floor; single-service containers are being used as scoops in sauce and raw rice; cutting board is scratched and stained; jalapenos are stored in a non-food grade grocery bag; middle rack above three-compartment sink is rusty; observed elevated air temperature in one-door reach-in cooler (no TCS foods in unit and a work order has been placed); several items are dirty (cup holder at tea lid sealer, broken blender, tea dispenser nozzles, inside and outside of toaster, and bottom of one-door reach-in cooler); side vent of tea dispenser, shelves above three-compartment sink, ceiling vent above cutting board and tabletop fan are dusty; mop isn’t hung or inverted to air dry.

Kabab House, 3411 Old Parham Road – The following violations were reported during a follow-up inspection: observed large flies in the kitchen; door handle to back door of kitchen is missing and there is a large hole exposed to the outside; plastic wicker drawers are being used for storage of loose spices; gasket is torn on two-door reach-in cooler; cup and utensil area for customers, gaskets of all refrigeration units, racks in all refrigeration units and microwaves are dirty; there is a direct plumbing connection at three-compartment sink; floors are dirty; baseboards and wall blanks are peeling away from the wall; there are stains on ceiling.

Little Caesars, 6040 Brook Road – No violations reported during a follow-up inspection.

Starbucks inside Publix, 4591 South Laburnum Avenue – The following violations were reported during a routine inspection: single-service cups are stored above protection line of storage cart; ceiling tile is missing above dry storage (there was a recent water leak and person in charge states that tile has been ordered).


JULY 17

Terra Nova Bar & Grille, 4700 South Laburnum Avenue – The following violations were reported during a routine inspection: salmon is thawed while still in reduced oxygen packaging; observed small flies in bar area and hall serving area; single-service items are stored open face up and on ground in closet; observed rust on racks and storage tables; dish machine at bar is in disrepair; observed heavy ice in freezer; there is a leak at the hand sink in the hall; observed significant damage to both cook line back walls due to a leak; tiles are coming off the wall behind dish machine; there are cracked floor tiles and low grouting on cook line and in dish area; observed leak at A/C unit.

Tambayan Grill, 5107-5109 Lakeside Avenue – No violations reported during a routine inspection.


JULY 18

McCormack’s Big Whisky Grill, 1420 North Parham Road – The following violations were reported during a follow-up inspection: small cutting boards are scratched and stained; observed grease and charred food debris around grill; charred food debris found in range and bottom of ovens; there is grease accumulation on sides of fryer; floor fan is dusty; bottom of three-door reach-in cooler is dirty; there is trash on the ground in outdoor enclosure with smoker; floors are dirty by mop sink, under cook line and under cold holding units; four lights in vent hoods are burned out.

Royal Farms, 2401 Mechanicsville Turnpike – The following violations were reported during a routine inspection: tongs used for serving chicken are stored in crevice between warmer unit and shelf; clean dishes stored on rack are covered in dust and flour; one light is out in the hood system; cutting board is heavily scored and stained; observed flour, dried food debris and grease residue on interior and exterior of food warmer box; there is dried food debris on reach-in cooler drawer gaskets; chicken speed racks by deep fryers, walk-in cooler and freezer door gaskets and doors, and potato wedge cart are dirty; observed grease residue on sides of fryers; there is flour residue on storage racks throughout kitchen; women’s restroom door is not self-closing (broken); outdoor dumpster lid was left open; ceiling tile near ventilation is very dusty; walls behind chicken breading area and three-compartment sink have food stains and mildew on them, particularly underneath; floor drains throughout kitchen are dirty.


JULY 21

Bojangles, 11 South Laburnum Avenue – The following violations were reported during a routine inspection: hand sink near deep fryers and hand sink in back of kitchen don’t have paper towels; there is mold inside ice machine; reach-in cooler #10 has a torn door gasket; cutting boards are heavily scored and stained; raw chicken cooler door and wire storage racks in dry storage are rusty; dried food debris found on shelves under serving case; reach-in cooler and freezer door gaskets throughout kitchen have black stains, mold and food debris on them; storage shelves throughout kitchen, all kitchen hand sinks, and bulk containers holding sauce cups are dirty; drive-thru area warming unit is covered in grease and food debris; food debris found in reach-in cooler interiors; observed grease residue on sides of hot line cooking equipment; walk-in cooler fan guards are dusty and dirty; cabinet interiors under front of house ice machine is dirty and covered with dried liquid residue; ceiling tile is very dusty near ventilation; ceiling vents are dusty; wall behind drive-thru warming unit is greasy; wall, pipes and tubes behind biscuit ovens are covered in flour and dust; wall and pipes behind deep fryers are greasy; floor drains throughout kitchen and wall behind/underneath three-compartment sink are dirty.

Arby’s, 11298 Patterson Avenue – The following violations were reported during a routine inspection: temperature of gyro meat in paper boats in prep units is elevated; lima beans are covered and stacked during the cooling process in reach-in cooler; in-use knife is stored in dirty crevice between prep unit and table; sandwich unit cutting board is heavily scratched and stained; observed grease and food debris buildup on wheels of fryers, inside reach-in freezer, walk-in cooler fans and grease filter machine.

Little Caesars, 7332 Staples Mill Road – The following violations were reported during a routine inspection: facility doesn’t have a current certified food protection manager; bulk containers of white powders are unlabeled; observed dried food debris on wire racks near three-vat sink; there is residue buildup on outside of bulk containers; there is black grime on wall behind three-vat sink; ceiling vents are dusty.

Peking Restaurant, 8904-F West Broad Street – The following violations were reported during a follow-up inspection: there is no way to verify that employees have received training on staff health policy; hand sink in employee bathroom doesn’t have hand soap; there is black residue inside ice machine; food residue found on slicer; there is no date marking system so there is no way to determine how long ago foods were prepared/opened; menu states that grouper is being served but facility is using swai; salt and sugar are unlabeled; back door was left open; door to outside has gaps where pests can enter the kitchen; observed a large, live roach crawl across the floor by the three-compartment sink; observed roach droppings on walls in kitchen; linens are covering egg rolls and cooked chicken; box of broccoli is stored directly on the floor; food is stored on the floor of walk-in units; onions are stored on a rack over top an open trash can; halved pineapple in refrigerator is uncovered; observed employees cutting broccoli and placing in a storage container without washing first; food storage containers are stacked while still wet; unused fryer and slicer should be removed from the facility; torn gaskets found; there is water leaking from the side of the ice machine onto the floor; walk-in refrigerator racks are rusty; cutting boards are heavily scored; fans are dusty; observed food debris in refrigerator/freezer units; there is dried food splatter on non-food contact surfaces of mixing bowl; storage racks and shelving are soiled; hose with spray nozzle attached to faucet of mop sink doesn’t have the proper dual check valve installed; faucet of three-compartment sink is leaking; there is a direct plumbing connection at three-compartment sink; dumpster doors were left open; trash found on the ground in dumpster area; wall is damaged; ceiling tiles are stained; there is missing and cracked caulking at hand sink; floor is cracked; walk-in freezer floor has buckled.
