Rico’s Mexican Grill, 11321-D Nuckols Road – The following violations were reported during a routine inspection: open employee beverage found next to clean dishes; observed black residue in nozzle of soda gun at bar; observed white buildup inside nozzle of sweat tea urn; there is no chlorine sanitizer detected at dish machine (sanitizer appears to not be moving through the line); food residue found on immersion blender food contact surface and on knife stored on magnetic strip; there is food debris on several plates at serving line; temperature of refried beans prepared last night in walk-in are elevated because of improper cooling methods; temperatures of raw chicken and sausage in grill drawer are elevated; ranchero sauce in walk-in is past its expiration date (observed mold inside the container); there are no date marks on carnitas or baked potatoes; ice scoop handle is in contact with ice at the bar; green cutting board at bar is heavily scratched; grease pan in hood system is full (observed grease dripping down onto the floor).

Dunkin’ Donuts, 10390 Ridgefield Parkway – No violations reported during a routine inspection.


JULY 9

Indian Aroma, 11636 West Broad Street – The following violations were reported during a routine inspection: temperature of rice pudding in reach-in refrigeration unit is elevated; gasket liner on left-side door of reach-in is torn; nonslip coverings are being used as shelf liners throughout the kitchen; mop isn’t hung or inverted to air dry.

Bell Greek, 7011 Three Chopt Road – The following violations were reported during a routine inspection: mixer attachment found stored in hand sink; back hand sink doesn’t have hand soap; dried food debris found on slicer; chlorine concentration in dish machine is 0 ppm; galaktobouriko in walk-in cooler was not date marked after being pulled from freezer; knife is stored in between prep unit lids; single-service containers by grill are not stored in a manner to protect from contamination; racks above three-compartment sink are rusty; fryer baskets have bent and broken wires; raw meat in cooling units are stored in non-food grade grocery bags; person in charge couldn’t provide test strips for three-compartment sink; hose with spray nozzle attached to mop sink without a dual check valve installed; outdoor dumpster receptacle is missing a lid; mop isn’t hung or inverted to air dry.

VFW Post 6364, 6502 Dickens Place – The following violations were reported during a routine inspection: dried food debris found inside microwave; temperature of roast beef in top portion of reach-in cooler is elevated; person in charge states that they will under cook steaks and hamburgers on request but there is no consumer advisory posted or on the monthly menu; several spray cleaner bottles are unlabeled; multiple stacks of to-go containers are stored uninverted and outside the original packaging; walk-in freezer door gasket is torn; reach-in cooler door gaskets and fan guards in walk-in cooler and freezer are dusty; spice rack and walk-in cooler door gaskets are dirty; observed grease residue on sides of kitchen equipment (cooler, stove, deep fryer); there is a direct plumbing connection underneath three-compartment sink; ceiling vent above kitchen entrance door is very dusty; hood filters in kitchen are greasy.

Hardee’s, 3815 Mechanicsville Turnpike – The following violations were reported during a routine inspection: observed mold on and around soda machine in drive-thru and dining area; dried food debris found inside microwaves; temperature of sliced ham in reach-in cooler is elevated; reach-in cooler underneath make line is not maintaining food at the proper temperature; several foods on time control were not discarded by the discard time; walk-in cooler door gasket is torn; walk-in cooler door and can opener base are rusty; all reach-in cooler and freezer doors have grease and food residue on them; all reach-in cooler and freezer door gaskets have dried food debris and mold on them; observed grease residue and food debris on sides and surfaces of cooking equipment (deep fryers, flat top grill, etc.); observed mold and food debris on wire storage racks in walk-in cooler; walk-in cooler fan guards are moldy; surfaces of all non-food prep equipment (including shelving under prep tables and wire storage racks) are dirty; hoses used in mop sink are touching mop sink floor; there is a direct plumbing connection underneath three-compartment sink; outdoor dumpster doors were left open; mop isn’t hung or inverted to air dry; there is black grime on floors throughout the kitchen, especially underneath equipment and storage racks; floor drains and the mop sink area are dirty; ceiling vent in kitchen is dusty and moldy; front of house ceiling tile is dusty near ventilation; floor underneath deep fryers is greasy.


JULY 10

Casa Grande Restaurant, 10120 Brook Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; bar hand sink doesn’t have hand soap; frying pan found in front hand sink; raw beef is stored next to veggies and raw eggs are stored over cooked items in lowboy unit; observed pink mildew on shield of ice machine and on lid of blender; temperature of rice in steam well is too low; observed roaches in dry storage area; several items are not covered to avoid contamination (shells on line, items in units and onions in walk-in).

Chick-fil-A, 1529 North Parham Road – The following violation was reported during a routine inspection: floor under ice machine by drive-thru is dirty with mildew and debris.

Mezeh Grill, 6927 Forest Avenue – The following violation was reported during a routine inspection: gaskets of lowboy units are torn.

Burger King, 3720 Mechanicsville Turnpike – The following violations were reported during a routine inspection: TCS foods held on time control aren’t marked with discard times; box of food is stored directly on walk-in freezer floor; y-valve at mop sink doesn’t have the proper backflow prevention device installed; bun toaster exterior is covered in grease and food residue; observed grease residue on top of deep fryer hoods, inside hood system, on French fry warmer and on sides of deep fryers; top and sides of oven are dirty; walk-in cooler fan guards are moldy and dusty; walk-in freezer fan guards are dusty; walk-in cooler door is moldy; observed food residue on storage racks in walk-in cooler; there is a direct plumbing connection underneath three-compartment sink; floor coving tile in front of broiler is cracked; floor and wall under and behind three-compartment sink are dirty; dry storage floors are greasy and walls have dried food stains on them; walk-in cooler ceiling and walls are moldy and dusty; ceiling tile in kitchen is stained and dusty near ventilation; there are some food stains on walls throughout kitchen; front of house ceiling tiles are stained; broiler fire suppression nozzles are covered in soot; lighting in walk-in cooler is too dark.

Maya Latin Kitchen, 353 East Williamsburg Road – The following violations were reported during a routine inspection: menu is missing asterisk next to huevos rancheros that are served with sunny side up eggs; mop isn’t hung or inverted to air dry; y-connector and spray nozzle attached at mop sink doesn’t have the proper backflow prevention device installed; fish is thawing while still in reduced oxygen packaging; chemical spray bottles are stored directly next to beer and wine at the bar.


JULY 11

Lulabelle's Cafe & Catering, 2012 Staples Mill Road – The following violations were reported during a routine inspection: there is no certified food protection manager on staff; dried food debris found on immersion blender blade; sliced deli meats, sliced cheeses and chicken salad are not date marked; working containers of flour, sugar and salt are unlabeled; sanibucket is stored directly on kitchen floor; cutting boards are scored; observed mold and mildew on dish machine door as well as limescale inside machine; reach-in cooler door gaskets, chest freezer door gaskets and deli style display cooler sliding door indentations are dirty; there is a direct plumbing connection underneath the kitchen and bar area three-compartment sinks; observed dust on ceiling vent in kitchen; there are food stains on dish room walls; there is missing floor tile in back area.

Boil Bay Cajun Seafood & Bar, 8161 Brook Road – The following violations were reported during a routine inspection: paper towel dispenser is empty at hand sink near prep line; raw oysters and raw chicken stored over noodles, cabbage and sauces in prep unit; chlorine sanitizer concentration at dish machine is 0 ppm; temperature of three bowls of chicken wings sitting under the prep table is elevated; temperature of raw scallops in prep unit is elevated because they are stacked too high in containers; several containers of cooked chicken wings in walk-in cooler are not date marked; employee medicines are stored on dry storage shelves near food; improper cooling methods observed for four containers of steamed crab legs in walk-in cooler (cooling in deep, covered bins); observed small fly infestation at bar under ice bin; in-use knife is stored in dirty crevice between prep table and unit; fryer baskets are heavily damaged (metal wires are bent and breaking off); single white cutting boards are heavily scratched and stained brown; hose with spray nozzle attached at mop sink faucet doesn’t have the proper backflow prevention device installed; doors were left open on outside dumpster; there is a hole in the wall in employee bathroom; bulb is out in the hood.

Sushi Hibachi Bistro, 10490 Ridgefield Parkway – The following violations were reported during a routine inspection: temperature of bin of rice cooked last night in the reach-in cooler is elevated; chlorine sanitizer level in bucket at sushi bar is above 150 ppm; sushi bar cutting board is heavily scratched and stained; observed food debris and grease buildup on pipes next to fryers, fryer baskets, cart next to fryers and inside coolers; there are food and grease stains on walls.
