KanPai Japanese Steakhouse, 11964 West Broad Street – The following violations were reported during a routine inspection: there is a buildup of debris along the interior of the soda nozzle closest to bar entrance; temperature of three containers of white rice in walk-in cooler is elevated because of improper cooling methods (rice was prepared yesterday and is stored in large containers with lids); containers of white rice in walk-in cooler are not date marked; temperature of rice in a cooker in the sushi bar is too low; raw salmon found stored in reduced oxygen packaging in lowboy unit at sushi bar but the facility doesn’t have a HACCP plan in place; bulk bin of white powder in kitchen is unlabeled; boxes of raw shrimp are stored directly on walk-in freezer floor; gasket liners are torn on both doors of prep unit, walk-in cooler door and left-side door of reach-in unit on service line; long cutting board on prep unit is scored and stained; facility couldn’t provide chloride sanitizer test strips; observed an employee rinse hands in three-compartment sink instead of hand sink; ingredient wire racks in sushi bar area are covered in old food debris and dust; y-valve is connected to mop sink faucet without proper backflow prevention device installed; ceiling vents throughout kitchen are surrounded by a thick layer of dust; observed buildup of black debris on wall behind dishwasher.


JUNE 27

The Original Ronnie’s BBQ, 2097 New Market Road – The following violations were reported during a routine inspection: temperatures of potato salad and cole slaw in tall reach-in cooler near cook line are elevated; spray cleaner bottle is unlabeled; BBQ sauce is cooling in reach-in freezer in tightly covered containers; observed multiple flies in the restaurant; cutting boards are scored and stained; reach-in freezer thermostat next to deep fryers is reading 48 degrees and tall reach-in cooler thermostat is reading 58 degrees; wire storage racks throughout kitchen are rusty; one light is out in hood system; floor fans in kitchen are dusty; observed grease residue on sides of hot line equipment; wire storage racks throughout kitchen and in walk-in cooler are dirty; there is a direct plumbing connection under two-compartment prep sink and three-compartment sink; women’s restroom doesn’t have a covered trash can; observed grease, grime and trash on floor underneath hot line equipment.

Chick-fil-A, 10176 Brook Road – The following violations were reported during a routine inspection: observed staff in drive-thru area leave the restaurant with gloves on and return to handle food without washing hands and changing gloves; there are torn door gaskets on cooler underneath make line and thaw box; cutting board is scored and stained; reach-in cooler door gaskets throughout kitchen are dirty.

Mi Jalisco, 10482 Ridgefield Parkway – The following violations were reported during a routine inspection: temperature of pork in steam table is too low; several cleaning spray bottles are unlabeled; single-service cups and bowls are being used as scoops in dry ingredient bins; metal wires are broken and bent on fryer baskets; some rubber spatulas are broken; observed buildup of dust and mildew on racks and fans in walk-in cooler; there is a direct plumbing connection under prep sink.

Turning Point, 5320 Wyndham Forest Drive – The following violations were reported during a routine inspection: employee health policy was unavailable; menu doesn’t contain an asterisk linking “Seaside Toast” (that is served with two eggs cooked “your way”) to the consumer advisory statement; y-valve and spray nozzle connected to mop sink faucet doesn’t have the proper backflow prevention device installed.


JUNE 30

Wood and Iron—Short Pump, 12288 West Broad Street – The following violations were reported during a routine inspection: equipment is being stored in hand sink in bar area; first hand sink next to prep line doesn’t have paper towels; dishwasher is reading 0 ppm chloride sanitizer; there are black specks along the bottom of the interior shield of the left-side ice machine; temperatures of hush puppy breading, egg rolls and hot dogs in first prep unit on the line are elevated; temperatures of shredded lettuce, pico and shredded cheese in second prep unit on the line are elevated; observed old debris buildup in bottom of both beer chests behind bar; long cutting boards on all prep units are heavily stained and scored; gasket liners are torn on door of first prep unit on the line, both doors of lowboy freezer on the line, both doors of lowboy next to warmer, and on lowboy door underneath steam well; dumpster area is dirty and surrounded by loose trash and debris.

Cava Grill Short Pump, 11780 West Broad Street – The following violations were reported during a follow-up inspection: no hand sink has been installed in the back end of the facility where food prep is occurring; food preparation is still taking place in the back end of the facility where there are no finished floors, walls or ceiling panels; dry ingredient shelf above veggie wash prep sink is still covered in a layer of debris.

Supper Club, 417 North Ridge Road – The following violations were reported during a routine inspection: observed multiple employees touch fair or hair before handling food; dish machine has 0 ppm chlorine (sanitizer tubing was bent); dried food debris found on slicer; temperatures of grilled chicken, souffle and cooked peppers in walk-in cooler are elevated; temperature of steak still in reduced oxygen packaging in display case is elevated (person in charge states delivery was just received); pepper pickles, queso salsa, salsa and pizza sauce in walk-in cooler are past their expiration dates; time isn’t marked on chicken breading; spray bottle of degreaser is stored on food prep surface directly next to open food; spray bottle of degreaser is unlabeled; observed ground meat in reduced oxygen packaging in walk-in freezer with no label; person in charge couldn’t provide HACCP logs; employee with long facial hair isn’t wearing a beard guard; employees handling food are wearing watches; walk-in freezer racks are rusty; walk-in cooler gasket is torn; cutting boards are heavily scratched and stained; person in charge couldn’t provide test strips for three-compartment sink or dish machine; top of dish machine and walk-in cooler fan guards are dirty; there is a leak at three-compartment sink faucet; dumpster side doors were left open; observed carbon buildup on hood above smoker; walk-in cooler walls are dusty.

Yoshi Sushi, 1601 Willow Lawn Drive – The following violations were reported during a follow-up inspection: observed live and dead American cockroaches, German cockroaches, drain flies and house flies throughout kitchen, including in AccuTemp cooking machine; brown buildup found in soda nozzle at bar; sanitizer bucket at bar has 0 ppm chlorine; floor in walk-in cooler is bowing; doors of sushi display case are taped together; cutting boards at prep unit on left side of line and at sushi bar are scratched and stained; door gaskets are torn on beer fridge and mixers fridge at bar; gaskets of lowboy holding mixers at bar are dirty; there are splashes on ceiling and wall by drink station; caulking around three-compartment sink is moldy.

Panera Bread, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: cookie pans on clean rack still have food debris on them; employee handling food is wearing a bracelet; metal strainers are stacked while still wet from washing; cutting boards at several prep stations are deeply scratched; some caraffes have chipped lips; several items throughout facility are dusty (fan covers in drink walk-in cooler and walk-in freezer, air curtain, vents of lowboy drawer units, and cords to order display TVs); several items throughout facility are dirty (top of dish machine, walk-in cooler racks, wheels to racks in walk-in cooler, floor fan, and drain of ice box at coffee station); there is a leak coming from the NuCO2 unit; y-valve attached at mop sink doesn’t have the proper backflow prevention device installed; there is trash accumulation on ground around dumpster; outdoor dumpster lid was left open; floor throughout kitchen is in need of cleaning, especially under units and tables and in walk-in freezer; there are cracked floor tiles and baseboards throughout kitchen; there are missing baseboard tiles throughout kitchen; grout is low in places throughout kitchen, especially near dish machine and three-compartment sink.

Wellness Center Tepaz, 5000 Nine Mile Road – The following violations were reported during a routine inspection: hand sink doesn’t have hand soap; ice scoop has a broken handle; there is a direct plumbing connection under three-compartment sink; women’s restroom doesn’t have a covered trash can.
