Hardee’s, 6302 Lakeside Avenue – The following violations were reported during a routine inspection: employee entered kitchen from outside and put on gloves without washing hands first; there is no handwash sink in the front food prep/dispensing areas, preventing routine handwashing by food workers; dried lettuce found on vegetable slicer; inside hot water dispenser nozzle is dirty; temperatures of pulled pork and ham in lowboy above prep station are elevated; observed drain flies throughout facility; employee with long hair is handling food without wearing a hair restraint; ice scoop handle is stored touching ice; ice cream scoop is stored in still, room temperature water; cutting board is scratched and stained; fryer baskets have broken and/or bent wires; observed grease and food debris accumulation on all equipment and racks throughout kitchen; carpet in walk-in freezer is dirty; there is grease buildup on pipes around and behind fryer; observed grease accumulation and food debris buildup in all units (reach-in coolers, racks, warming units, etc.); milk shake mixer splash guard and walls of walk-in cooler are dirty; racks inside walk-in cooler have food debris and mildew on them; there is a leak at the mop sink; y-valve with spray nozzle attached to hose at mop sink doesn’t have the proper backflow prevention device installed; there is a direct plumbing connection at three-compartment sink; observed trash in outdoor enclosure; door on outdoor dumpster was left open; floors and walls throughout facility are covered in grease, dust and food debris; observed dust accumulation around ceiling vents.
DQ Grill & Chill, 3275 New Market Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; did not observe hot water or strong water pressure at front hand sink; there are torn gaskets on multiple units; racks are rusty; some units aren’t working; observed grease buildup on fryers, cooking equipment and hot holding units; there is buildup on racks inside units; lights are out in dry storage; some floor tiles are cracked; there are missing coving tiles near dish area.

Red Lobster, 8009 West Broad Street – The following violations were reported during a routine inspection: observed unsifted calamari breading at 74 degrees; mop isn’t hung or inverted to air dry.
Cheesecake Factory, 11800 West Broad Street – The following violations were reported during a follow-up inspection: soda nozzle closest to the entrance of the bar has black specks along the interior; temperatures of shredded cabbage and shredded cheese in first and fourth drawer units on the line are elevated; these two units are not holding items at the proper temperature.
McDonald's, 5106 Nine Mile Road – The following violations were reported during a routine inspection: dried food debris found inside microwave; observed several flies in kitchen and dining area; food service staff are wearing bracelets; spoons are being stored in room temperature water in between uses in drive-thru area; cutting board is heavily scored and stained; reach-in cooler and freezer door gaskets throughout kitchen are dirty and moldy; observed food debris/residue on walk-in cooler storage racks; trash cans in women’s restroom don’t have a covered lid; there is missing wall tile at mop sink in kitchen; observed grease buildup on French fry hood filters.

Melito’s, 8815 Three Chopt Road – The following violations were reported during a routine inspection: soda nozzle on right side of the bar is dirty; tuna in walk-in cooler is thawing while still in vacuum packaging; fryer baskets have broken wires; counter fan is dusty; observed buildup on gaskets of lowboy units; there is a direct plumbing connection at three-compartment sink in kitchen and bar; observed grease buildup on floor under fryer; mop isn’t hung or inverted to air dry.
Eastern Carry Out, 9024 West Broad Street – The following violations were reported during a routine inspection: hand sink bason is blocked by miscellaneous items; pathway to hand sink is blocked by large rice cooker and bucket; there are no paper towels at the hand sink; chopped cabbage is sitting out on counter without a time mark; glass cleaner is stored above bulk sugar and rice bins; white powders on spice cart are unlabeled; there is a gap under the screen door in back of kitchen; egg rolls are stored in a cardboard box in reach-in cooler; to-go containers are stored food contact side down on a dusty surface; single-service bowls are being used as scoops in bulk food bins; cutting boards are scratched and stained; fryer basket wires are bent and missing; basket used to store egg rolls and shelving racks in prep cooler are rusty; observed grease and food debris buildup on sides of wok stove and shelving racks in prep cooler and walk-in cooler; hood is greasy; floor fan behind prep cooler is dusty; bathroom door is propped open; dumpster lids and side doors were left open; there are cardboard boxes and miscellaneous items on ground at the dumpsters; paint on bathroom door is chipping.

Buffalo Wild Wings Grill & Bar, 1090 Virginia Center Parkway – The following violations were reported during a routine inspection: bar dish machine has 0 ppm chlorine; sanitizer bucket appears diluted; sanitizer bottle stored over clean dishes; employees handling food are wearing watches; an employee handling food with long hair doesn’t have it tied back; wet wiping cloth stored on cutting board instead of in sanitizer; ice bucket is wet and stored uninverted; cutting boards are scratched; ceiling in raw meat walk-in cooler is rusty; handle is loose and the faucet is leaking at hand sink next to mop sink; ceiling vent at three-compartment sink, shelving racks throughout kitchen and floor fan are dusty; beer lines in beer walk-in cooler are dirty; hose with spray nozzle attached at mop sink doesn’t have the proper backflow prevention device installed; observed caked on grease on grease barrel; dumpster side doors were left open; dumpsters are dirty and there is trash on the ground behind them; floors under fryers and under bar ice machine are dirty; light bulb in dry storage area doesn’t have a shield.

Domino’s Pizza, 5526 Lakeside Avenue – The following violations were reported during a routine inspection: temperature of tomatoes in reach-in unit is elevated; air temperature of this unit is elevated.
Little Caesars, 1547 North Parham Road – The following violations were reported during a routine inspection: person in charge couldn’t provide proof that a staff member is a certified food protection manager; food debris found in bottom of hand sink next to three-compartment sink; gasket of hot holding unit (order pick up cabinet) is torn; rack at Crazy Bread station is rusty; several items throughout kitchen are dusty (vent of air curtain at back door, electrical wires to wall-mounted water machine, vent of lowboy unit, top of hot holding cabinets, racks of rolling cart table at Crazy Bread station, and fan covers in walk-in cooler); several items throughout kitchen are dirty (bottom of hot holding cabinets, gaskets of hot holding cabinets, outside of flour bucket at dough station, all surfaces of scale at dough station, floor and baseboards of walk-in cooler, walls in oven, electrical outlets, bottom edges of somerset machine, and dish drying racks); observed excess food debris in catch tray of bottom oven; light shield above prep table is bowing and the food underneath is no longer protected from the light; there are cracked and missing baseboard tiles throughout the facility; there are cracked tiles throughout the facility; back hand sink is separating from caulking at wall; didn’t observe a “no smoking” sign anywhere around facility.
KFC, 3800 Mechanicsville Turnpike – The following violations were reported during a follow-up inspection: door gasket on chicken warmer box is torn; wire storage racks throughout kitchen and in walk-in coolers and freezer are rusty; door gaskets on reach-in freezer, walk-in coolers and walk-in freezer are dirty and moldy; there is grease residue on sides of hot line cooking equipment; walk-in cooler fan guards are moldy; floor rack in walk-in cooler is dirty; French fry warmer fans are dusty; observed heavy grease and food buildup on speed rack in walk-in cooler; walls and attached pipes/hoses behind hot line cooking equipment are greasy and coated in flour and dust; ceiling tile near ventilation is dusty throughout kitchen; floor drains throughout kitchen are very dirty; observed mildew on wall behind three-compartment sink; raw chicken walk-in cooler walls, ceiling and floor are dirty and moldy; there are cracked floor tiles throughout kitchen; there is missing floor tile in front of three-compartment sink; observed low grout on floor throughout kitchen.
