Kabab House, 3411 Old Parham Road – The following violations were reported during a routine inspection: back hand sink is blocked by big pots on the floor; shell eggs are stored over uncovered ready-to-eat food; food debris found on butcher blade; can opener is dirty; temperatures of curried vegetables, curried chicken and goat in walk-in cooler are elevated; opened can of tomato paste is sitting on storage rack at room temperature; no cooked foods in the kitchen have date marks; medicine bottles are stored on shelf with spices; door handle to back door of kitchen is missing and there is a large hole exposed to the outside; observed large flies in kitchen; knives are stored under plastic wrap box; clean pots and pans are stored on the ground throughout kitchen; clean pots are stored in mop sink; single-service items are not stored in a manner to protect them from contamination, including on the floor; observed a knife with a broken tip; cutting boards are scratched and stained; plastic wicker drawers are being used for the storage of loose spices; racks throughout kitchen are rusty; gasket is torn on two-door reach-in cooler; dirty cardboard is being used to line shelves; rodent droppings found on ground in kitchen; several items are dirty and covered in grease (storage racks, lines running to burners and grill, shelf on grill, grill plates, burners, oven, drip trays and suppression nozzles); walk-in cooler fans and fan above walk-in cooler are dusty; several items/areas are dirty (walk-in cooler floor, cup and utensil area for customers, inside and outside all refrigeration units, gaskets and racks of all refrigeration units, inside and outside display cases and microwaves); observed leak at large pipe running behind cook line by hand sink; there is a direct plumbing connection at three-compartment sink; floors and walls throughout kitchen are dirty and covered in grease; baseboards and wall planks are peeling away from the wall; ceiling is stained.
Subway Sandwiches & Salads, 1216 North Concord Avenue – The following violation was reported during a routine inspection: observed an employee reach into lettuce to remove cheese with bare hands.
Regal Cinemas—Short Pump, 11650 West Broad Street – The following violations were reported during a routine inspection: food protection manager certificate wasn’t available; facility couldn’t provide employee health policy; gasket liners are torn on lowboy freezer under the concession line and standing reach-in refrigeration unit in back of kitchen; facility doesn’t have quat sanitizer test strips; y-valve connected to mop sink faucet doesn’t have the proper backflow prevention device installed; there is a direct plumbing connection under three-compartment sink; dumpster area is dirty and surrounded by loose trash and debris.

Sedona Taphouse, 5312 Wyndham Forest Drive – The following violations were reported during a routine inspection: several employee drinks are stored on prep surfaces; cleaning utensils found in bar hand sink; several handwashing signs are missing in bathrooms; temperatures of lettuce, tomatoes, mixed greens, slaw, batter and chicken in breading in ice baths are elevated; remoulade and jicama slaw are past their expiration dates; several chemical spray bottles are unlabeled; several knives and utensils are stored in crevices; utensils at salad station are not stored inverted; fryer baskets are heavily bent; y-valve connected at mop sink doesn’t have the proper backflow prevention device installed; chemical dispenser hoses are stored below flood rim of mop sink; there is pitting in floors throughout kitchen.
Clean Eatz Richmond, 11801 West Broad Street – The following violations were reported during a routine inspection: sanitizer in three-compartment sink tested at 0 ppm; temperature of cooked sweet potatoes is elevated and they are past the two-hour cooling timeframe; fish fillets in walk-in cooler are thawing while still in vacuum sealed packaging; vents in walk-in cooler are covered in a layer of dust; y-valve connected to mop sink faucet doesn’t have the proper backflow prevention device installed.
Domino’s Pizza, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: ServSafe certificate posted in office is expired; metal storage racks with sauces is rusty; vents on lowboy units are dirty; top of water heater and vents in bathroom are dusty; y-valve connected to mop sink doesn’t have the proper backflow prevention device installed; there is a leak from faucet at hand sink by three-compartment sink; there is a direct plumbing connection at three-compartment sink; doors of outdoor dumpsters were left open; floor by mop sink and behind prep line is dirty.
Applebee’s, Richmond International Airport – The following violations were reported during a routine inspection: there are torn gaskets on multiple units; walk-in racks are rusty; a missing kick plate on drawers is allowing dust; there is heavy ice buildup on prep units; observed heavy grease buildup on fryers and wheels of cooking equipment; debris found on racks throughout kitchen and bar; there is grease and food buildup on drawers for chips; observed mildew buildup on gaskets on units; observed low grouting at the bar; drain plates are rusty; observed grease buildup behind cook line; debris found under units and sinks.
Cross Grains, Richmond International Airport – The following violations were reported during a routine inspection: temperatures of quinoa, farro and tomatoes in bottom of small prep unit are elevated; knife is stored in between prep units; ice machine door is cracked; missing top plate on reach-in is allowing dust; observed low grouting in back area and under ice machine; several areas need cleaning (dish area, under ice machine and under cooking equipment).
Cheeburger Cheeburger / Sam Adams, Richmond International Airport, Concourse B – The following violations were reported during a routine inspection: open bag of deli meat is stored on top raw chicken bags; there is 0 ppm sanitizer at bar dish machine; there is a leak at bar hand sink; observed debris in back kitchen around reach-in and mop area; cleaning is needed under bar area.
Ironclad Coffee Roasters, Richmond International Airport, Concourse A – The following violations were reported during a routine inspection: temperatures of deli sandwich in display case and milk in lowboy are elevated; no time is marked on milk in self-service area; observed small flies throughout; there is debris buildup in drains and under units; observed food splatters on wall around syrups.

Casa Italiana / Pizza Square & Italian Market, 8801 Three Chopt Road – The following violations were reported during a routine inspection: observed pink and brown build up on splash guard of ice machine; temperature of tomato sauce made the previous night in walk-in cooler is elevated because of improper cooling methods (stored in a large pot); opened pancetta in walk-in cooler isn’t date marked; single-service containers are not stored in a manner to protect from contamination; gaskets of reach-in cooler and beer tap cooler are torn; fryer baskets have bent wires at corners; cutting boards at prep station are scratched and stained; observed grease buildup on sides of fryers; walk-in cooler fan grates are dusty.
Dunya Kabob RVA, 9039 West Broad Street – The following violations were reported during a routine inspection: did not observe a person in charge of ensuring foods are properly cooled and dishes are properly sanitized; facility couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; an employee washed a knife in the hand sink, which is for handwashing only; no handwash sign is posted in bathroom; raw meat is stored directly over raw vegetables; employees failed to sanitize dishes after washing at three-compartment sink; temperature of sliced tomatoes cut this morning is elevated; burn gel and paint are stored above food prep surface and spices; flours and a spray bottle of water are unlabeled; bags of onions are stored directly on the floor; employee with long facial hair isn’t wearing a beard guard; to-go containers are stored uninverted above soup warmer; reach-in cooler has a torn gasket; racks in three-door lowboy cooler are rusted; facility doesn’t have chlorine sanitizer test strips; mop water is being dumped in public outside drain, which isn’t an approved sewage system; bathroom door found partially open because it is not self-closing; observed many cardboard boxes on the ground at the dumpster.

McDonald’s, 4708 Williamsburg Road – The following violations were reported during a routine inspection: two employees with long hair in ponytails are not wearing hair restraints; wires are bending on several fryer baskets; observed dried food debris and dust on walk-in cooler wire racks, drive-through cart, top of hot holding cabinets, coffee makers, shake machine, ice cream machine, portable fans and inside raw burger freezer; observed heavy buildup of grease and food under grills and in grease traps.
Sheetz, 550 South Airport Drive – The following violation was reported during a routine inspection: y-valve attached to mop sink with two hoses, one under constant pressure, doesn’t have the proper backflow prevention device installed.
Les Crepes, 1308 Gaskins Road – The following violations were reported during a routine inspection: observed a child walking around food storage areas unsupervised, sitting underneath food prep counter where blankets and a pillow were set up; person in charge couldn’t provide employee health policy, written procedures for cleaning up vomit and diarrhea, or the parasite destruction letter from salmon provider; strainer found in hand sink at verdant lady bar; there are no paper towels at bar hand sinks, verdant lady hand sinks or staff bathroom; no handwash signage is posted in staff bathroom; bar hand sinks don’t have hand soap; there is 0 ppm chlorine at large dish machine and glass dish machine; temperature of cooked chicken is too low; temperature of almond mik in mini fridge is elevated; two spray bottles of all-purpose cleaner are unlabeled; there are torn gaskets on walk-in cooler and lowboy cooler; rubber spatulas are chipping; knife handles are taped; public bathroom doors are not self-closing; dumpster side door was left open; floors in main kitchen and food storage area are unsealed; ceiling vent in walk-in cooler is leaking.
Genghis Grill, 11849 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide parasite destruction letter from salmon provider; there is 0 ppm chloride sanitizer in dishwasher; temperature of shredded cabbage in walk-in cooler is elevated; observed improper temperature of brown rice being stored in rice cooker on counter; temperature of noodles in lowboy unit is elevated; noodles in the walk-in cooler are not date marked; menu doesn’t contain disclosure next to meal items containing raw fish; can of Raid is stored on prep counter in back of kitchen; shelf storing take-out containers and entrée bowls is covered in a layer of dark buildup; observed large rust spots on wire racks in walk-in cooler; gasket is torn on left-side door of lowboy in cooking area; ingredient racks behind buffet line are covered in old food debris; y-valve connected to mop sink faucet doesn’t have the proper backflow prevention device installed; no toilet paper is available in right-side stall of women’s restroom.
