Emmy Squared Pizza, 2225 Old Brick Road – The following violations were reported during a routine inspection: hand sink at bar is being used as a dump sink; raw beef is stored over ready-to-eat food in prep unit; dried food debris found on “clean” containers and lids; temperature of cooked bacon in prep unit is elevated and has exceeded the six-hour cooling time; bulk bins of similar looking ingredients are not labeled; an employee is cloth drying glasses immediately out of the dish machine; there is a leak on cold water handle at three-compartment sink; y-connector and hoses attached at mop sink faucet don’t have the proper backflow prevention device installed; there is a leak at the y-connector and hoses at mop sink faucet; observed buildup of food and grime inside floor drains under prep sink and dish machine.

Panera Bread, 10301 West Broad Street – The following violations were reported during a routine inspection: prep line hand sink doesn’t have paper towels; temperature of salad in top drawer of salad crisper is elevated.

Muse Paintbar, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: hand soap and dish soap are stored in handwash sink in kitchen; a dented can of sun dried tomatoes is stored with the good cans; hand soap and dish soap are stored in squeeze bottles without labels; wine cooler has a torn gasket; dumpster doors were left open; dumpster lid is broken and hanging in the dumpster; facility doesn’t have a “no smoking” sign posted.

Minilandia Ice Cream & Candy, 4062 Crockett Street – No violations reported during a routine inspection.

McDonald’s, 2310 Mechanicsville Turnpike – The following violations were reported during a routine inspection: water at hand sink next to fry warmer doesn’t reach the required temperature; sanitizer concentration in sanibuckets is at 0 ppm; staff are working with food without wearing hair restraints; there are torn door gaskets on French fry reach-in freezer; y-valve on mop sink doesn’t have the proper backflow prevention device installed; outdoor dumpster doors were left open.

JUNE 10

Eat 33, 6901 Staples Mill Road – The following violations were reported during a routine inspection: person in charge’s food protection manager certificate is expired; relocate employee drinks stored above food, food prep and clean dishes to minimize risk of contamination; foil and cardboard are being used to line prep shelves and cold holding units; observed heavy ice buildup at bottom of two-door reach-in freezer.

Rapid Pizza, 1592 Mountain Road – The following violations were reported during a routine inspection: temperature of marinara sauce in water bath warmer is too low; cooked chicken wings are not date marked; package of deli ham in walk-in cooler is past its expiration date; observed buildup of dried food debris and dust on surfaces of all wire storage racks, blue dish rack, and inside prep units and gaskets; the three-compartment sink basins don’t have an indirect plumbing connection; there is a buildup of food debris and grime on floors and under and behind equipment.

Bowlero West End, 8037 Shrader Road – The following violations were reported during a routine inspection: paper towel dispenser is broken; observed grease and dried food debris buildup on sides of cook line equipment, prep shelves, beverage mats, unit vents, pizza ovens, prep unit gaskets and utensil containers; there is a leak at three-compartment sink; there is grease and dried food on the floor and walls behind cook line equipment; perimeter of kitchen is dirty.

America’s Best Wings, 7536 West Broad Street – The following violations were reported during a routine inspection: relocate staff water bottles stored on prep shelves to minimize the risk of contamination; staff washed hands without soap; cooked pasta and fried chicken are not date marked; several spray bottles of cleaning chemicals are unlabeled; facility doesn’t have a food thermometer; observed several live insects inside mop sink; several containers of fried chicken in the walk-in cooler are uncovered; observed grease and dried food debris on sides of fryers and wire storage racks.

Nineveh Bakery & Special Food, 8026-C West Broad Street – The following violation was reported during a routine inspection: observed grease and food residue behind cook line.

Wendy’s, 9116 West Broad Street – The following violations were reported during a routine inspection: there are hand soap and a handwashing sign at the mop sink (wash hands only at hand sinks); temperature of cheese sauce in steam well is too low; did not observe a written time control policy for sandwich lettuce, tomatoes or raw beef patties; there is no time sticker for raw beef patties; dumpster lid was left open.

Angela’s Ristorante, 425 North Ridge Road – The following violations were reported during a routine inspection: person in charge unable to provide proof of food protection manager certificate; can of red peppers is dented along the the top seam; food debris found on deli slicer; TCS foods in walk-in cooler and prep units are not date marked; single-service containers not stored in a manner to protect from contamination; rack in back kitchen by three-compartment sink is rusty; four plastic spatulas are chipped on side edges; cutting board at prep unit is scratched and stained; several items in kitchen are dusty (top shelf above prep unit on cook line, lines running to AC unit, walk-in cooler fans, fans in two-door reach-in freezer, and hood and vent system above pizza oven); racks surrounding the cook line are greasy; there is grime buildup in gaskets of lowboy units; there is a direct plumbing connection under three-compartment sink; observed grease buildup under fryer; light in kitchen by AC unit doesn’t have a protective cover.

Domino’s Pizza, 70 South Airport Drive – The following violations were reported during a routine inspection: temperatures of cheese and ham in pizza make table top are elevated because the lids were left open during downtime; several food items are marked to be discarded eight days after prep date (expiration date should be seven days after prep); self-closing door at restaurant entrance is broken and several flies were observed in the facility; three-door reach-in cooler has a torn door gasket; observed leaks at three-compartment sink faucet and hanging spray nozzle; there are food stains on the ceiling; ceiling tile is dusty near ventilation; hoods and hood filters above pizza oven are greasy.

Antonio’s Pizza, 63 South Airport Drive – The following violations were reported during a routine inspection: hot water at hand sink and three-compartment sink doesn’t reach the required temperature; observed dried food debris on meat slicer blade and base; there is excessive unnecessary items/equipment stored in rooms behind dining room; cutting boards are scored and stained; soda machine in dining room, microwave in kitchen and homestyle reach-in cooler in kitchen are in disrepair; wire storage racks in walk-in cooler are rusty; observed food debris and black stains on reach-in cooler door gaskets; dried food residue found on wire racks in walk-in cooler; there is a direct plumbing connection underneath the three-compartment sink; there are dried food stains on walls throughout kitchen; floors and walls underneath/behind oven are dirty and dusty; ceiling tile above pizza oven is stained.

JUNE 11

Starbucks Coffee Company, 11716 West Broad Street – The following violations were reported during a routine inspection: food protection manager certificate and employee health policy are unavailable; dishwasher’s recorded final sanitizing temperature is too low; gasket liner is torn on lowboy in front of facility underneath drink dispensers; dumpster area is dirty and surrounded by loose trash and debris; mop isn’t hung or inverted to air dry.
