Azzurro Restaurant, 6221 River Road – The following violations were reported during a routine inspection: person in charge couldn’t provide employee health policy or written procedures for cleaning up vomit and diarrhea; raw chicken stored over ready-to-eat foods in lowboy; food debris found on slicer in storage area; bottle of degreaser is unlabeled; two medicine bottles are stored next to hot sauces; salt, sugar and flour throughout kitchen and bar are unlabeled; observed drain flies in bar and dish area; single-service containers are not stored inverted or protected to prevent contamination; gaskets are torn on white reach-in freezer and on wine and beer coolers at bar; small cutting boards in dishwash area and cutting board at pizza prep station are scratched and stained; fan blades in walk-in cooler are dusty; several items/areas are dirty (wine/beer cooler gaskets, reach-in cooler gaskets, soda lines at bar, suppression nozzles, sides of cook line equipment, grill shelf and wheel, hoses behind grill, power cords between prep table and lowboy cooler, top of salamander and u-bend at kitchen hand sink); floor and wall tiles throughout facility are cracked and chipped; observed low grout in between floor tiles throughout facility; there is grease buildup under fryer; light above coffee station doesn’t have a shield.
Starbucks Coffee Company, 6201 River Road – The following violations were reported during a routine inspection: there is 0 ppm quat sanitizer in three-compartment sink; temperature of whipped cream canister is elevated; single-service cups are stored above protection line; there are leaks at bottle cleaner connection and three-compartment sink sanitizer hose; there is trash on the ground near dumpsters; dumpster lids were left open; wet mop isn’t hung or inverted to air dry; observed debris in mop sink.
Tiki Tiki, 8917 Patterson Avenue – The following violations were reported during a routine inspection: ice machine is dirty; white powders on cart are unlabeled; soda drinks are stored in ice machine.
Rose Marie Inn, 8923 Patterson Avenue – The following violations were reported during a routine inspection: didn’t observe a food protection manager certificate; facility couldn’t provide employee health policy; both hand sinks are blocked with miscellaneous items; hand sinks don’t have hand soap or paper towels; raw meat is stored over ready-to-eat foods in beer fridge and single-door reach-in; microwave is dirty; dried food debris found on slicer; temperatures of tuna, potato salad and corn cobs in prep cooler are elevated; there are no date marks on deli meats, corned beef hash or corn cobs; potato salad in single-door reach-in is past its expiration date; carbon-off spray is stored next to vegetables in back room; quat sanitizer concentration is too high in sani wipe bin and three-compartment sink; dead mouse found on glue trap under single-door reach-in; wet wiping cloths are stored on counter instead of in a sanitizer solution; ice bucket is stored directly on floor next to ice machine; cutting boards are scratched and stained brown; bar reach-in cooler gasket is torn and is being used as storage; racks are rusted in single-door reach-in next to prep cooler; fans in back room and pipes under bar three-compartment sink are dusty; beer reach-in cooler tracks are dirty; observed leak at kitchen three-compartment sink faucet; dumpster lid was left open.
Chili’s Grill & Bar, 9950 Brook Road – The following violations were reported during a routine inspection: hand sink at end of cook line doesn’t have hand soap; hand sink in bar area doesn’t have paper towels; raw shrimp stored in same drawer unit as ready-to-eat sliced cheese; raw meats stored in same drawer unit as ready-to-eat sliced mushrooms; temperature of commercial tomato sauce stored on flat top grill is too low; temperature of cooked chicken stored in warmer is too low; temperature of pico in make table is elevated; temperature of pasta sitting out on counter is elevated; diced, cooked chicken is cooling in deep containers with lids in walk-in cooler; employee preparing frozen/thawing meats is wearing a bracelet; employee with long facial hair isn’t wearing a beard guard; plastic containers are stacked while still wet; take-out containers are not stored inverted or protected to prevent contamination; wire racks in walk-in cooler are peeling; long cutting boards on prep line are stained and scored; top shelf of wire racks in walk-in cooler are covered in food debris and powder; spray nozzle is connected to mop sink hose without the proper backflow prevention device installed; dumpster area is dirty and surrounded by loose trash and debris; ceiling vent above dishwasher area is surrounded by a layer of light dust; floor under fryer is soiled; there is glue on the wall in the corner of the prep line.
Papa John’s Pizza, 6100-D West Broad Street – The following violations were reported during a routine inspection: bathroom hand sink doesn’t have hand soap; dried food debris found on can opener blade; one light is out in pizza oven hood system; there are dried food debris and crumbs in pizza make table door gaskets; observed mold and mildew on three-compartment sink; hand sink splash guard is dirty and rusty; observed food debris on sides of pizza make table; point-of-sale screens, cables and keyboards are dirty and dusty; walk-in cooler fan guards are dusty; observed dried food debris and dust on walk-in cooler wire racks.

Teppanyaki Grill & Buffet, 9043 West Broad Street – The following violations were reported during a routine inspection: there are large gaps above and below screened exit door in back kitchen; employee working with food is wearing a bracelet; three fryer baskets are bent; racks in reach-in coolers in main kitchen are rusty; clean dish rack is dirty; there is a leaky pipe at dish area hand sink when the sink is turned on; dumpster side door was left open.
Yen Ching, 9031-9035 West Broad Street – The following violations were reported during a follow-up inspection: there was no hot water at the hand sink initially because the hot water had been turned off; rice in large strainer on cook line is at an improper temperature; temperatures of sprouts and noodles in cooler are elevated; walk-in cooler fan guards are dirty; observed low grouting at cook line; hood panels are loose; there is a loose floor panel at three-compartment sink; floors under wok stoves are dirty.
Firehouse Subs, 9069 Staples Mill Road – The following violations were reported during a routine inspection: temperature of top layer of chicken and roast in prep unit are elevated because the container was overfilled; y-connector at mop sink doesn’t have the proper backflow prevention device installed.
Starbucks Coffee Company, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: there isn’t a certified food protection manager on staff; hand sink by three-compartment sink doesn’t have paper towels or hand soap; single-service cups are stored past the protection line; syrup storage rack shelves are rusty and dusty; several items/areas are dirty (small rack next to three-compartment sink, gaskets of reach-in freezers, bottoms of reach-in coolers and freezers, side of reach-in freezer along back wall); dumpster door was left open; floors under equipment and racks in back room and backsplash on wall behind coffee rack are dirty; there are pitted walls throughout kitchen and back room; wall corner edge by fire extinguisher is missing.

Balkan Restaurant, 8905 Patterson Avenue – The following violations were reported during a routine inspection: an employee didn’t wash hands before touching food after washing dirty dishes; employee didn’t wash hands before touching ready-to-eat bread after touching raw meat; hand sink is blocked by a cart; temperature of ground meat in single-door reach-in cooler is elevated because it is cooling in a large, deep, covered container; various foods in double-door reach-in cooler are not date marked; menu doesn’t contain a consumer advisory or asterisks linking foods that can be ordered undercooked to the disclosure; dumpster side door was left open.
Papa John’s Pizza, 8903 Patterson Avenue – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy; there is dried food debris on can opener blade; string cheese sitting out on the counter wasn’t marked with the time; oven fans and walk-in cooler fan guards are dusty; observed food debris buildup on shelves under walk-in cooler fan guards; y-connector on mop sink doesn’t have the proper backflow prevention device installed; dumpster side door was left open.
Starbucks Coffee Company, 11332 Nuckols Road – The following violation was reported during a routine inspection: there is no handwash signage posted at drive-thru window hand sink.
